Evaluation of the CNESTEN's TRIGA Mark II research reactor physical parameters with TRIPOLI-4® and MCNP

  • This paper focuses on the development of a new computational model of the CNESTEN's TRIGA Mark II research reactor using the 3D continuous energy Monte-Carlo code TRIPOLI-4 (T4). This new model was developed to assess neutronic simulations and determine quantities of interest such as kinetic parameters of the reactor, control rods worth, power peaking factors and neutron flux distributions. This model is also a key tool used to accurately design new experiments in the TRIGA reactor, to analyze these experiments and to carry out sensitivity and uncertainty studies. The geometry and materials data, as part of the MCNP reference model, were used to build the T4 model. In this regard, the differences between the two models are mainly due to mathematical approaches of both codes. Indeed, the study presented in this article is divided into two parts: the first part deals with the development and the validation of the T4 model. The results obtained with the T4 model were compared to the existing MCNP reference model and to the experimental results from the Final Safety Analysis Report (FSAR). Different core configurations were investigated via simulations to test the computational model reliability in predicting the physical parameters of the reactor. As a fairly good agreement among the results was deduced, it seems reasonable to assume that the T4 model can accurately reproduce the MCNP calculated values. The second part of this study is devoted to the sensitivity and uncertainty (S/U) studies that were carried out to quantify the nuclear data uncertainty in the multiplication factor keff. For that purpose, the T4 model was used to calculate the sensitivity profiles of the keff to the nuclear data. The integrated-sensitivities were compared to the results obtained from the previous works that were carried out with MCNP and SCALE-6.2 simulation tools and differences of less than 5% were obtained for most of these quantities except for the C-graphite sensitivities. Moreover, the nuclear data uncertainties in the keff were derived using the COMAC-V2.1 covariance matrices library and the calculated sensitivities. The results have shown that the total nuclear data uncertainty in the keff is around 585 pcm using the COMAC-V2.1. This study also demonstrates that the contribution of zirconium isotopes to the nuclear data uncertainty in the keff is not negligible and should be taken into account when performing S/U analysis.

생태계서비스 가치평가를 활용한 예비타당성조사 편익분석 개선 방향 연구 - 장항습지복원사업 사례를 중심으로 - (A Study on Improving the Estimation of Social Benefits Using the Economic Valuation of Ecosystem Services in Preliminary Feasibility Analyses for Ecological Restoration Projects - Focused on the Case of Janghang Wetland Restoration Project -)

  • This study is to propose the ecosystem service valuation method as a complementary or alternative tool to overcome the limitations of the contingent valuation method(CVM), typically used to assess social benefits in preliminary feasibility studies. With an increasing interest in natural and environmental restoration projects, we assessed social benefits with theses CVM and ecosystem service valuation method from a case of Janghang wetland restoration project and compared the extent of the two social benefits. For quantitative evaluation of ecosystem services, the biophysical quantity for each ecosystem service indicator was calculated and then converted into currency (KRW) units to estimate the economic value of ecosystem services. The four ecosystem regulating service indicators were selected including greenhouse gas capture/storage, air pollution, water quantity and quality regulation. The amounts of CO2 sequestration and storage as a ecosystem's greenhouse gas regulating service in the study area were 73.04 tCO2/yr and 5,867.53 tCO2/yr respectively. The reduction of SO2, one of air pollutant gases by ecosystems was calculated to be 180.27 kg/yr, the reduction of NO2 to be 378.90 kg/yr, and the reduction of fine dust (PM10) to be 9,713.92 kg/yr. The amount of freshwater regulating service by the ecosystem was estimated to be 459,394,319ℓ/yr, and the amount of nitrogen in freshwater removed by the ecosystem was 78.00kg/yr. Study results show that the benefits derived from the CVM were KRW 227.8 billion over the 30-year analysis period and those from the ecosystem service valuation method were KRW 41.4 billion for regulatory services and KRW 148.8 billion for cultural services, totaling KRW 189.5 billion. With KRW 184.8 billion of the total costs, the benefit/cost ratio using the CVM was 1.23 and that with the ecosystem service valuation method was 1.03. This study implications include that the CVM and ecosystem service valuation method can be applied together to assess and compare social benefits for natural and environmental restoration projects.

Textbook Outcome of Delta-Shaped Anastomosis in Minimally Invasive Distal Gastrectomy for Gastric Cancer in 4,505 Consecutive Patients

  • Purpose: Textbook outcome is a comprehensive measure used to assess surgical quality and is increasingly being recognized as a valuable evaluation tool. Delta-shaped anastomosis (DA), an intracorporeal gastroduodenostomy, is a viable option for minimally invasive distal gastrectomy in patients with gastric cancer. This study aims to evaluate the surgical outcomes and calculate the textbook outcome of DA. Materials and Methods: In this retrospective study, the records of 4,902 patients who underwent minimally invasive distal gastrectomy for DA between 2009 and 2020 were reviewed. The data were categorized into three phases to analyze the trends over time. Surgical outcomes, including the operation time, length of post-operative hospital stay, and complication rates, were assessed, and the textbook outcome was calculated. Results: Among 4,505 patients, the textbook outcome is achieved in 3,736 (82.9%). Post-operative complications affect the textbook outcome the most significantly (91.9%). The highest textbook outcome is achieved in phase 2 (85.0%), which surpasses the rates of in phase 1 (81.7%) and phase 3 (82.3%). The post-operative complication rate within 30 d after surgery is 8.7%, and the rate of major complications exceeding the Clavien-Dindo classification grade 3 is 2.4%. Conclusions: Based on the outcomes of a large dataset, DA can be considered safe and feasible for gastric cancer.

아동·청소년 자살고위험군 자살예방평가 및 프로그램 (Development of Suicide Prevention Programs for Mental Health Professionals Working with Children and Adolescens at High Risk of Suicide)

  • 우리나라 자살률은 Organization for Economic Cooperation and Development 국가 중 압도적 1위이며, 특히 아동·청소년들은 삶의 만족도가 낮고, 자살률 또한 매년 증가하고 있다. 아동·청소년 시기는 어느 발달단계보다 격동적이고 충동적인 시기로 자살 그 자체가 치명적이고, 전염성이 높다. 또한 의미 있는 작업을 잃어버렸을 경우에도 자살률이 높은데, 이는 자살고위험군 아동·청소년도 마찬가지이다. 정신건강복지법이 개정되고, 정신건강전문요원에 작업치료사가 포함되기 시작하면서 정신건강서비스에 있어 작업치료를 포함한 다학제적 접근이 강조되고 있다. 아동·청소년 자살을 막기 위한 선별도구로 자살사고척도(Suicidal Ideation Questionnaire), Beck의 자살사고척도(Beck Scale for Suicide Ideation), 콜롬비아 대학 자살 심각성 평가 척도(Columbia-Suicide Severity Rating Scale), 청소년용 삶의 이유척도(The Reasons for Living Inventory for Adolescents), 학생 정서·행동 특성검사(Student Emotional and Behavioral Screening Test)가 사용되었고, 작업치료평가도구인 Canadian Occupational Performance Measure도 사용되었다. 또한 다양한 학문과 기관에서 다양한 자살예방프로그램이 제시되고 있다. 하지만 자살고위험군 현실적 특성상 신속하고 적기에 서비스를 제공해야 하기 때문에 다른 자살예방프로그램보다 상대적으로 그 수가 적으며 이 또한 대부분 인지행동적 측면에 초점이 맞춰져 있다. 따라서 본 연구는 아동·청소년 자살고위험군을 대상으로 정신건강서비스를 제공하는 전문가들에게 다양한 자살예방프로그램을 소개하고, 각기 처해 있는 실정에 맞게 현장에서 활용할 수 있도록 하는 데 목적이 있다.

eBPF를 활용한 컨테이너 활동 분석 시스템 (eBPF-based Container Activity Analysis System)

  • 클라우드 환경의 도입은 애플리케이션 배포와 관리 방식을 혁신적으로 변화시켰으며, 마이크로서비스 아키텍처와 컨테이너 기술이 그 중심에 자리 잡고 있다. 그러나 이러한 변화는 마이크로서비스와 같은 복잡한 서비스 환경에서 서비스 간 상호작용을 정확히 파악하고 내부 프로세스를 세밀하게 분석해야 하는 새로운 과제를 제기했다. 기존의 모니터링 기법은 이와 같은 복잡한 환경을 충분히 분석하는 데 한계를 보였으며, 이를 해결하기 위해 최근 eBPF(extended Berkeley Packet Filter) 기술이 주목받고 있다. eBPF는 리눅스 커널에서 발생하는 이벤트를 실시간으로 수집하고 분석할 수 있는 강력한 기술로, 커널 영역에서 발생하는 파일 시스템 활동을 비롯한 다양한 이벤트를 모니터링할 수 있다. 본 논문에서는 이러한 eBPF를 기반으로 한 컨테이너 활동 분석 시스템을 제안하며, 이 시스템이 컨테이너와 호스트 시스템의 커널 영역에서 발생하는 이벤트를 실시간으로 모니터링하고, 수집된 데이터를 분석하는 방법을 제시한다. 또한, eBPF를 활용한 대표적인 컨테이너 모니터링 시스템인 Tetragon, Falco, Tracee를 이벤트 탐지 방식, 기본 정책 적용 여부, 이벤트 유형 식별, 시스템 콜 차단 및 경고 생성 기능 측면에서 비교 분석하여 각 시스템의 장단점을 평가하고, 컨테이너 프로세스 감시 및 제한에 필요한 기능을 파악하였다. 나아가, 본 논문에서는 제안된 시스템을 컨테이너 메타데이터 수집, 내부 활동 모니터링, 시스템 메타데이터 연계의 세 가지 측면에서 설명하고 평가하며, 관련 연구와의 비교를 통해 eBPF 기반 모니터링 시스템의 효용성과 향후 발전 가능성에 대해 논의한다.

북미지역주민(北美地域住民)의 사상체질(四象體質) 분포(分布)에 관(關)한 연구(硏究) (A Study on the Sasang Constitutional Distribution Among the People in the United States of America)

  • 동(東) 서양의학(西洋醫學)이 여러 방면(方面)으로 눈부시게 발전(發展)해 왔음에도 불구(不拘)하고 동일질병(同一疾病)에 대한 약(藥) 효과(效果)의 개인별(個人別) 차이(差異)나 질병(疾病)에 대한 개인별(個人別) 감수성(感受性)의 차이(差異)에 따른 여러 가지 면역관계(免疫關係) 질환(疾患)의 다양성(多樣性)이나 난치병(難治病)의 다양(多樣)한 예후(豫後) 등(等)의 이유(理由)를 정확(正確)히 이해(理解)하지 못하고 따라서 적절(適切)한 대처(對處)를 하지 못하고 있는 것이 현실(現實)이다. 그런데 사상의학(四象醫學)의 네 체질(體質)에 따른 질병(疾病) 관리(管理), 치료(治療) 및 예방법(豫防法)은 현대(現代)의 난치병(難治病)이라고 할 수 있는 성인병(成人病), 면역계(免疫係) 질환(疾患), 스트레스성(性) 질환(疾患)의 관리(管理)에 효과적(效果的)으로 적용(適用)할 수 있으므로 현재(現在) 한방임상의학(韓方臨床醫學)에서 많이 응용(應用)되고 있다. 이러한 사상의학(四象醫學)을 세계(世界)에 폭넓게 적용하기 위해서는 국제적(國際的)으로 응용(應用)할 수 있는 체질진단(體質診斷)의 기준(基準)이 마련되어야 한다. 우선(于先) 본(本) 연구(硏究)는 외국인(外國人)에게도 과연(果然) 체질(體質)이 존재(存在)할까 하는 의문점(疑問点)을 해결(解決)하기 위(爲)하여 미국인(美國人)을 대상(對象)으로 체질(體質) 분류(分類)를 시도하여 체질(體質) 존재(存在) 여부(與否)를 확인(確認)하는 작업(作業)부터 시작(始作)하였다. 또 체질(體質)이 존재(存在)한다면 체질(體質) 진단(診斷) 도구(道具)로는 어떤 것이 좋을까를 알아보기 위(爲)한 연구(硏究)를 병행(竝行)하였다. 선택(選擇)된 체질(體質) 진단(診斷) 도구(道具)로는 경희대학교(慶熙大學校) 사상의학(四象醫學) 교실(敎室)에서 개발(開發)되어 학회(學會)에서 공인후(公認後) 임상(臨床)에서 널리 사용(使用)되는 체질(體質) 진단(診斷) 도구(道具)인 QSCCII를 바탕으로 이를 영문(英文)으로 번역(飜譯)하고 채점(採點) 방법(方法)을 보완(補完)하여 새롭게 제작(製作)된 new QSCCII + 사용(使用)하였다. 본(本) 연구(硏究)는 국내(國內)에서 표준화(標準化)되어 사용(使用)하고 있는 체질진단진단도구(體質診斷診斷道具)인 QSCCII를 보완하여 미국(美國)에서 응용(應用)할 수 있는 새로운 진단(診斷) 도구(道具)를 마련하고자 처음으로 시도(試圖)된 연구(硏究)이다. 조사(調査) 대상(對象)은 University of Bridgeport. Connecticut. U.S.A의 학생(學生), 교직원(敎職員)그리고 Health Science Center의 Clinic을 방문(訪問)한 사람중(中) 본(本) 조사(調査)에 협력(協力)한 사람이 주(主)로 그 대상(對象)이 되었으며 기타(其他) 주변(周邊)의 현지인(現地人)들이 대상(對象)이 되었다. 년(年) 조사대상인원(調査對象人員) 344명(名)이었고 전체(全體) 조사(調査) 대상(對象)에서 재검사(再檢査)를 할 수 있었던 인원(人員)은 240명(名)이었다. 연구기간(硏究期間)은 1998년(年) 9월(月)부터 1999년(年) 8월(月)까지 약(約) 1년(年) 여(餘)에 걸쳐 실시(實施)되었다. 이러한 연구결과(硏究結果)를 고찰(考察)해 볼 때 아래와 같은 결론(結論)을 얻었다. 1. 미국(美國) 사람에게도 사상체질(四象體質)은 존재(存在)한다. 추론(推論)컨데 미국(美國)에는 다양(多樣)한 인종(人種)이 섞여 살고 있으므로 외국인(外國人) 모두에게 역시(亦是) 체질(體質)이 존재(存在)한다고 볼 수 있다. 2. 미국인(美國人)에게 특(特)히 백인(白人)에게선 소양인(少陽人)으로 진단(診斷)되는 경향(傾向)이 높았다. 이 결과(結果)는 미국(美國)사람에게도 사상체질(四象體質)은 존재(存在)한다는 가설(假說)과 다소(多少) 부합(附合)된다 사료(思料)된다. 3. 검사재검사(檢査再檢査)를 통하여 분석(分析)된 결과(結果)를 볼 때 그 결과(結果)가 일관(一貫)되게 나오는 것으로 보아 new QSCCII +가 외국인(外國人)의 체질(體質)을 진단(診斷)할 때 일관(一貫)된 결과(結果)를 얻을 수 있는 진단방법(診斷方法)일 가능성(可能性)을 시사(示唆)한다. 4. 표준(標準) 집단(集團)의 체질(體質) 분류(分類)에서는 인종(人種)에 관계(關係)없이 체질(體質)이 존재(存在)하고 있었다. 5. 반응(反應) 빈도(頻度)가 낮은 문항(問項)은 미국인(美國人)을 위(爲)한 표준화(標準化) 연구(硏究)를 할 때 미국인(美國人)에게 이해(理解)가 될 수 있는 또다른 표현(表現)으로 바꾸어 적용(適用)해 볼 필요(必要)가 있을 것으로 추정(推定)된다. 6. 미국인(美國人)의 체질(體質)을 정확(正確)하게 하기 위(爲)해서는 표준화(標準化) 작업(作業)을 함으로써 QSCCII라는 진단도구(診斷道具)를 이용(利用)하여 측정(測定) 진단(診斷) 데이터를 평가(評價)할 수 있는 진단(診斷) 기준(基準)이 만들어져야 한다. 7. 체질(體質)이 불투명(不透明)하다고 나온 71명(名) 중(中)에는 잠재적(潛在的) 태양인체질(太陽人體質)이 포함(包含)되어 있을 것으로 추정(推程)되나 태양인(太陽人)의 희소성(稀少性)에서 기인(起因)하는 new QSCCII+의 변별력저하(辨別力低下)에 대한 해결방안(解決方案)에 대(對)한 연구(硏究)가 추후(追後)에 진행(進行)되어야 할 것으로 사료(思料)된다. 8. 연구결과(硏究結果) 북미지역(北美地域)의 체질분포(體質分布)는 다음과 같다. 연인원(年人員)을 대상(對象)으로한 분포(分布)에서는 소양인(少陽人) 36.25 %(87명), 태음인(太陰人) 13.75 %(33명(名)), 소음인(少陰人) 20.41%(49 명(名)), 분류(分類)가 안되거나 태양인(太陽人)인 경우(境遇)가 29.58%(71 명(名))이었다.

소비자 중심의 간호서비스 질 결정요인에 관한 연구;SERVQUAL모형을 중심으로 (A Study on the Determinants of Consumer-Oriented Nursing Service Quality;SERVQUAL Model based)

  • As the tendency of the society is centralized into consumers and services, patients are getting to ask better medical services. The consumers influenced from various social surroundings became to have some expectation of nursing service. Compared with their expectation, the quality of the services which they virtually get may be recognized and evaluated. So it is necessary to know exactly what the consumers want in nursing services. The purpose of this study is to examine the determinants which can evaluate the quality of nursing services by researching into consumers' expectation and perception of the nursing services depending on the consumer-oriented attributions on the basis of the model of SERVQUAL. 1,144 of outpatients were selected as the subjects for this research. They used to continuously visit the same hospital after being hospitalized and nursed in 9 hospitals randomly selected among the second-level medical organizations in Seoul from January to February, 2001. The collected data are analyzed into the Descriptive Statistics, t-test, GLM and Multiple Regression through the SAS program. Delphi was used for the research tool and the results of the research are as follows; the determinants in evaluating the quality of nursing services consist of 5 categories such as Tangibility, Reliability, Responsiveness, Assurance, Empathy. The values of Cronbach' $\alpha$ appeared to be 0.96 in the expectation of nursing services, 0.94 in the perception of nursing services and 0.96 in the importance of nursing services. The determinants in the expectation of nursing services were ranged in the order of Assurance, Empathy, Reliability, Responsiveness and Tangibility. And those in the perception of nursing services were in the order of Assurance, Empathy, Reliability, Tangibility and Responsiveness. Those in the importance of nursing service were in the order of Empathy, Assurance, Reliability, Tangibility and Responsiveness. Finally, those in the quality of nursing service were in the order of Tangibility, Responsiveness, Empathy, Reliability and Assurance. Each expectation of nursing services appeared different depending on the subjects' age, gender, clinical department and reason for hospitalization. The hypothesis examined in this research shows that the group having higher personal needs shows meaningful differences in the expectation of nursing services, and the subjects who have had external communication show higher perception of nursing service than uncommunicative ones. After all, we can see that the statistical differences in the perception of nursing services depend on whether the subjects have external communications or not. The determinants in the expectation of nursing services can explain the quality of nursing service up to 14.96%. The statistically meaningful determinants in the expectation could be arranged in the order of Reliability, Assurance and Tangibility. And the more expectation brings about the lower evaluation of the quality of nursing services. The determinants in the perception of nursing service can explain the quality of nursing services up to 29.85%. The statistically meaningful determinants in the perception could be arranged in the order of Responsiveness, Reliability, Tangibility, Empathy, and Assurance. And the more perception brings about the higher evaluation of the quality of nursing services. According to the result of the above research, I would like to propose as follows. As long as this research is oriented to get knowledge of the consumer-oriented nursing services, it should be continued to draw the other elements determining the quality of the nursing services. Furthermore, this research is based upon the Parasuraman, A., et al.'s SERVQUAL Model(1991), which deals only expectation, perception and quality of consumer-oriented nursing services, so it will be necessary to inspect and verify it through the other models containing the offerers of nursing services in the future. On the other hand, as this research evaluates the actual quality of nursing services based on the expectation and perception of nursing services, it can be utilized as fundamental data to develop the marketing strategies and to estimate the qualities as well. I hope this research will be periodically estimated to be the useful data to develop the marketing strategies in the nursing service area.

일간 빔 출력 확인을 위한 평가도구인 Machine Performance Check의 유용성 평가 (Assessment of the usefulness of the Machine Performance Check system that is an evaluation tools for the determination of daily beam output)

  • 목 적: Machine Performance Check (MPC)는 Electronic Portal Imaging Device(EPID)를 기반으로 빔 출력을 별도의 설치 없이 측정할 수 있는 장점을 지닌 자체 검사 소프트웨어이다. 본원에서는 MPC와 QA Beamchecker PLUS 간의 일간 빔 출력을 비교 및 상관관계를 분석하여 MPC의 유용성을 확인하고자 하였다. 대상 및 방법: 본 실험을 진행하기 위해 선형가속기(Truebeam 2.5)를 이용하였고, 광자선(6 MV, 10 MV, 15 MV, 6 MV-FFF, 10 MV-FFF), 전자선(6 MeV, 9 MeV, 12 MeV, 16 MeV, 20 MeV) 총 10개의 에너지를 대상으로 5 개월간 치료 전 빔 출력을 MPC와 QA Beamchecker PLUS로 측정하여, 총 80 회의 데이터를 획득하였다. Pearson 상관계수를 사용하여 MPC와 QA Beamchecker PLUS 간의 빔 출력을 비교 및 상관관계를 평가하였다. Pearson 상관계수는 0.8 이상은 아주 강함, 0.6 이상 0.8 미만 강함, 0.4 이상 0.6 미만 보통, 0.2 이상 0.4 미만 약함, 0.2 미만 아주 약함을 의미한다. 결 과: MPC와 QA Beamchecker PLUS 모두 일간 빔 출력 일치도는 2 % 이내로 나타났다. MPC의 빔 출력은 광자선이 $0.29{\pm}0.26%$, 전자선이 $0.30{\pm}0.26%$로 나타났고, QA Beamchecker PLUS의 빔 출력은 광자선이 $0.31{\pm}0.24%$, 전자선이 $0.33{\pm}0.24%$로 나타났다. MPC와 QA Beamchecker PLUS 사이의 Pearson 상관계수는 광자선의 경우 15 MV에서는 아주강함, 6 MV, 10 MV, 6 MV-FFF 그리고 10 MV-FFF에서는 강함으로 나타났고, 전자선의 경우 16 MeV, 20 MeV에서 강함, 9 MeV, 12 MeV에서 보통, 6 MeV에서 아주 약함으로 나타났다. 결 론: MPC는 일간 빔 출력 평가 면에서 광자선과 고에너지 전자선에서는 QA Beamchecker PLUS와 강한 상관관계로 보임을 확인할 수 있었다. 다만, 저에너지 전자선(6 MeV)에서는 낮은 상관관계를 보였지만, 관찰기간동안 MPC, QA Beamchecker PLUS 모두 빔 출력 일치도는 2 % 이내로 일간 빔 출력 확인 용도로는 적절할 것으로 판단된다. 의료서비스 마케팅을 위한 품질지각과 만족에 관한 연구 (A Study on Qulity Perceptions and Satisfaction for Medical Service Marketing)

  • INSTRODUCTION Service quality is, unlike goods quality, an abstract and elusive constuct. Service quality and its requirements are not easily understood by consumers, and also present some critical research problems. However, quality is very important to marketers and consumers in that it has many strategic benefits in contributing to profitability of marketing activities and consumers' problem-solving activities. Moreover, despite the phenomenal growth of medical service sector, few researchers have attempted to define and model medical service quality. Especially, little research has focused on the evaluation of medical service quality and patient satisfaction from the perspectives of both the provider and the patient. As competition intensifies and patients are demanding higher quality of medical service, medical service quality and patient satisfaction has emerged as a critical research topic. The major purpose of this article is to explore the concept of medical service quality and its evaluation from both nurse and patient perspectives. This article attempts to achieve its purpose by (1)classfying critical service attibutes into threecategories(satisfiers, hygiene factors, and performance factors). (2)measuring the relative importance of need criteria, (3)evaluating SERVPERF model and SERVQUAL model in medical service sector, and (4)identifying the relationship between perceived quality and overall patient satisfaction. METHOD Data were gathered from a sample of 217 patients and 179 nurses in Seoul-area general hospitals. From the review of previous literature, 50 survey items representing various facets of the medical service quality were developed to form a questionnaire. A five-point scale ranging from "Strongly Agree"(5) to "Strongly Disagree"(1) accompanied each statement(expectation statements, perception statements, and importance statements). To measure overall satisfaction, a seven-point scale was used, ranging from "Very Satisfied"(7) to "Very Dissatisfied"(1) with no verbal labels for scale points 2 through 6 RESULTS In explaining the relationship between perceived performance and overall satisfaction, only 31 variables out of original 50 survey items were proven to be statistically significant. Hence, a penalty-reward analysis was performed on theses 31 critical attributes to find out 17 satisfiers, 8 hygiene factors, and 4 performance factors in patient perspective. The role(category) of each service quality attribute in relation to patient satisfaction was com pared across two groups, that is, patients and nurses. They were little overlapped, suggesting that two groups had different sets of 'perceived quality' attributes. Principal components factor analyses of the patients' and nurses' responses were performed to identify the underlying dimensions for the set of performance(experience) statements. 28 variables were analyzed by using a varimax rotation after deleting three obscure variables. The number of factors to be extracted was determined by evaluating the eigenvalue scores. Six factors wereextracted, accounting for 57.1% of the total variance. Reliability analysis was performed to refine the factors further. Using coefficient alpha, scores of .84 to .65 were obtained. Individual-item analysis indicated that all statements in each of the factors should remain. On 26 attributes of 31 critical service quality attributes, there were gaps between actual patient's importance of need criteria and nurse perceptions of them. Those critical attributes could be classified into four categories based on the relative importance of need criteria and perceived performance from the perspective of patient. This analysis is useful in developing strategic plans for performance improvement. (1) top priorities(high importance and low performance) (in this study)- more health-related information -accuracy in billing - quality of food - appointments at my convenience - information about tests and treatments - prompt service of business office -adequacy of accommodations(elevators, etc) (2) current strengths(high importance and high performance) (3)unnecessary strengths(low importance and high performance) (4) low priorities(low importance and low performance) While 26 service quality attributes of SERPERF model were significantly related to patient satisfation, only 13 attributes of SERVQUAL model were significantly related. This result suggested that only experience-based norms(SERVPERF model) were more appropriate than expectations to serve as a benchmark against which service experiences were compared(SERVQUAL model). However, it must be noted that the degree of association to overall satisfaction was not consistent. There were some gaps between nurse percetions and patient perception of medical service performance. From the patient's viewpoint, "personal likability", "technical skill/trust", and "cares about me" were most significant positioning factors that contributed patient satisfaction. DISCUSSION This study shows that there are inconsistencies between nurse perceptions and patient perceptions of medical service attributes. Also, for service quality improvement, it is most important for nurses to understand what satisfiers, hygiene factors, and performance factors are through two-way communications. Patient satisfaction should be measured, and problems identified should be resolved for survival in intense competitive market conditions. Hence, patient satisfaction monitoring is now becoming a standard marketing tool for healthcare providers and its role is expected to increase.

대학생의 감각처리 유형과 대인관계문제 및 성인애착의 관계 (The Correlation between Sensory Process Type and Interpersonal Problems, Adult Attachment on University Students)

  • 목적 : 본 연구는 대학생의 감각처리 유형과 대인관계문제 및 성인애착의 정도를 알아보고 이들의 관계를 파악하고자 한다. 연구방법 : 국내 K지역의 대학생 169명을 대상으로 감각처리 유형은 청소년/성인 감각프로파일(Adolescents/Adults Sensory Profile; AASP)을, 대인관계문제는 한국판 대인관계문제검사 원형척도의 단축형(Short Form of the Korean Inventory of Interpersonal Problems Circumplex Scale; KIIP-SC)을, 성인애착은 친밀관계경험척도(Experiences in Close Relationships Scale-Revised; ECR-R)를 사용하여 측정을 실시하였다. 감각처리 유형과 대인관계문제 및 성인애착의 관계는 피어슨 상관분석을 사용하여 분석하였다. 결과 : 대학생의 감각처리 유형은 감각찾기만 "일반인보다 덜함"으로 나타났고, 나머지 등록저하, 감각예민, 감각회피는 "일반인과 유사함"의 결과를 보였다. 대인관계문제는 자기희생과 과순응성에서 어려움이 많았고, 자기중심성과 통제지배가 어려움이 적었으며, 성인애착은 애착회피가 애착불안보다 더 안정적이었다. 감각처리 유형 중 등록저하와 감각예민은 모든 대인관계문제 및 성인애착과 양의 상관관계를 보였고, 대인관계문제 중 자기희생과 과관여는 감각찾기와, 냉담, 사회적 억제, 비주장성, 과순응성은 감각회피와 양의 상관관계를 보여 감각찾기와 감각회피는 서로 다른 결과를 나타내었다(p<.05). 결론 : 작업치료 임상에서 장애인뿐만 아니라 대인관계와 애착의 문제를 경험하는 비장애인에게 감각처리의 유형에 따른 중재를 실시한다면 그들이 질 높은 사회활동을 영위하는데 도움이 될 것이다.
