  • As the ensemble averaged dz/dt signal during exercise gets smoothed, it is difficult to find the distinctive marks for estimation of stroke volume. The cross correlation function was made use of estmating these marks for automatic calculation by computer from the ensemble averaged dz/dt signal. LVET(Left Ventricular Ejection Time) and stroke volume were estimated based on the calculated parameters from the characteristic points. LVET, stroke volume calculated by hand, by the ensemble average and the cross correlation were compared for accuracy validation.

  • This paper proposes a new algorithm for sleep apnea detection based on stroke volume. It is very important to detect sleep apnea since it is a common and serious sleep-disordered breathing (SDB). In the previous studies, methods for sleep apnea detection using heart rate variability, airflow and blood oxygen saturation, tracheal sound have been proposed, but a method using stroke volume has not been studied. The proposed algorithm consists of detection of characteristic points in continuous blood pressure signal, estimation of stroke volume and detection of sleep apnea. To evaluate the performance of algorithm, the MIT-BIH Polysomnographic Database provided by Phsio- Net was used. As a result, the sensitivity of 85.99%, the specificity of 72.69%, and the accuracy of 84.34%, on the average were obtained. The proposed method showed comparable or higher performance compared with previous methods.

Estimation of Cardiac Pumping Performance according to the Ventricular Electrical Activation Time Distribution by Using Physiome Model (피지옴 모델을 이용한 심실의 전기활성시간 분포에 따른 심박출 성능평가)

  • The purpose of the study is to examine the effects of pacemaker location on cardiac pumping efficacy theoretically. We used a three-dimensional finite element cardiac electromechanical model of canine ventricles with models of the circulatory system. Electrical activation time for normal sinus rhythm and artificial pacing in apex, left ventricular free wall, and right ventricular free wall were obtained from electrophysiological model. We applied the electrical activation time maps to the mechanical contraction model and obtained cardiac mechanical responses such as myocardial contractile ATP consumption, stroke work, stroke volume, ejection fraction, and etc. Among three artificial pacing methods, left ventricle pacing showed best performance in ventricular pumping efficacy.

  • A ventricular assist device (VAD) is used for bridge to heart transplantation and heart diseases. Knowing the status of a pneumatic pulsatile VAD when implanting it into the body is important: when the velocity of blood flow through the VAD is slow, a thrombus may occur, and thrombosis can be fatal to a patient. In order to determine the state of a VAD, various sensors need to be implanted. Because this introduces the risk of infection and difficulties with sensor management, we developed a method for estimating the state of a VAD indirectly via the pressure in an air tube that can be measured in vitro. We compared the measured values to in vitro experimental results. The estimated and measured values showed some errors, but the accuracy can be improved by refining the estimation process to minimize the risk of infection.

Evaluation of Cerebrovascular Reactivity According to Age Using Transcranial Doppler (Transcranial Doppler를 이용한 연령에 따른 뇌혈관 반응성 평가)

  • Cerebral vasoreactivity is an index of autoregulation of cerebral perfusion, and can be measured using functional images such as Xe CT, SPECT and PET in reponse to hypercapneic stimulus. In order to measure cerebral $CO_2$ vasoreactivity in routine TCD study conveniently and reliably, we devised a method of rebreathing into closed volume of reservoir bag as a hypercapneic stimulus, and applied it to 44 healthy volunteers. As a hypercapneic stimulus, we applied fitting mask connected with closed reservoir bag for about 90 seconds, and mean blood flow velocity(MBFV) and pulsatility index(PI) were evaluated at proximal middle cerebral arteries(MCA) of 50-55 mm depth, before and after the hypercapneic stimulus. Age affected the MFV and PI value showed significant and the MFV was 56.45(SD=9.75)cm/sec, while PI was 0.406(SD=0.089). As age increases the flow velocity decreased significantly whereas PI value increased(P<0.05). The vasoreactvity significantly decreased with age(P<0.05). The decrease of cerebral blood flow quantity and cerebral blood flow velocity is not only because of increase of diameter of cerebrovascular resulting from aging, but the resistance increase of small blood vessel resulting from the increase of PI & RI value is regarded. We suppose that the rebreathing method is a reliable and convenient technique as a hypercapneic stimulus in determining cerebral $CO_2$ vasoreactivity. The rebreathing method could be non-invasive and useful methods in estimation of the cerebrovascular reactivity and could be applied to the basal and follow-up evaluation of the cerebrovascular reserve of the ischemic stroke patients.
