Improvement of orthognathic surgical techniques make it possible to design esthetic surgical correction for total esthetic face. In order to find the esthetic line which guide esthetic surgical correction in patients of orthognathic surgery, cephalometric soft tissue analysis of esthetic faces were performed. In esthetic Korean young adults, 25 males and 25 females who were within 1 S.D. of E-line, ANB, P/A facial height ratio, were analyzed in natural position keeping their face eye level. 1. Sn position is constant in males and females. The Sn-N'-N' Vertical plane angle is $5.3^{\circ}$ in both sexes. Sn is positioned in front of 5 mm in female 7 mm in male from the N' vertical plane. 2. The Sn-Ls line make constant angle to horizontal plane with $72.5^{\circ}$ in both sexes, which is called "upper esthetic line". The Ls-Pg' line makes constant angle to $72.4^{\circ}$ (range $72.2^{\circ}$ in female to $72.6^{\circ}$ in male), which is called "lower esthetic line". 3. When inter-esthetic line angle (the Sn-Ls line to Ls-Pg' line) has $144.9^{\circ}$, lower third face has esthetic upper and lower lip. 4. In treatment planning, Sn is first corrected in proper position, and then upper and lower esthetic line are established with the angle of 144.9. The maxilla is moved to tangent Ls to the upper esthetic line, and mandible is moved to tangent Li and Pg' to the lower esthetic line, according to the "y"-shaped esthetic lines, then lower third face showes esthetics.
Outcome of esthetic ceramic restorations are affected by tooth size, gingival contour, occlusal relationship, etc. For this reason, demand of orthodontic treatment before esthetic ceramic restoration is increasing. If a Bolton ratio discrepancy, a problem of the maxillary incisor's vertical position, a problem of inclination of anterior teeth, a pathogenic occlusion is existed, a pre-prosthodontic orthodontics should be accomplished. These problems can be satisfactory only after the prosthetic treatment is performed after orthodontic treatment. When orthodontic treatment is given, it should be treated with the following principle. 1. Treat it in the direction of functioning occlusion. 2. Keep the patient's stable occlusal scheme. 3. Treat the teeth by considering the average tooth size and Bolton ratio. Ortho-Prostho combined treatment with optimal treatment plan can lead a patient's function, esthetics, and long-term stability.
Purpose: This research is to investigate factors that affect dental aesthetics and analyze whether individual perception affects and difference of genders affects the esthetic factors. Materials and methods: Seventy dental students of Chonnam University aged from 25 to 35 years old without periodontal problems were included. Maxillary dental cast for participants were made, and standardized photo was taken with a digital camera. Maximum visual width and the position of gingival zenith of maxillary anterior teeth and maximum height of the maxillary central incisors were measured by a measurement program and the shape of central incisor was categorized. Questionnaire was conducted to evaluate esthetic perception. SPSS program was used to perform statistical analysis. Results: The average visual width ratio of right maxillary anteriors was 1.38:1:0.78 and 1.41:1:0.81 for the left which differ from the golden ratio. The width to height ratio for right and left central incisor was 0.84 and 0.83 respectively. The gingival zenith position was determined to be: mostly, central incisors and canines were located in the distal side, lateral incisors were located in the center. The visual width ratio of right maxillary anterior teeth, ratio of width to height of central incisor, gingival zenith position, crown morphology and amounts of gingival exposure upon smiling were not significantly different between genders, and facial patterns and these dental esthetic factors were not related to aesthetic perception. Conclusion: Even though participants had the visual width ratio of maxillary anteriors or ideal width to ideal height of central incisors that did not agree with the golden ratio, they had high satisfaction for dental esthetics. Esthetic perception depends more on subjective judgements of participants than objective indices.
The purpose of this review was to suggest practical criteria for the clinical translucency evaluation of direct esthetic restorative materials, and to review the translucency with these criteria. For the evaluation of reported translucency values, measuring instrument and method, specimen thickness, background color, and illumination should be scrutinized. Translucency parameter (TP) of 15 to 19 could be regarded as the translucency of 1 mm thick human enamel. Visual perceptibility threshold for translucency difference in contrast ratio (${\Delta}CR$) of 0.07 could be transformed into ${\Delta}TP$ value of 2. Translucency differences between direct and indirect resin composites were perceivable (${\Delta}TP>2$). Universal and corresponding flowable resin composites did not show perceivable translucency differences in most products. Translucency differed significantly by the product within each shade group, and by the shade group within each product. Translucency of human enamel and perceptibility threshold for translucency difference may be used as criteria for the clinical evaluation of translucency of esthetic restorative materials.
Esthetic is one of the prime goal of the orthodontic treatment. It is represented by the word of harmony or balance. There is no absolute esthetic norm but it is important to study about it in orthodontic and orthognatic surgical treatment planning. The purpose of this study is to obtain the norm of Korean female adolescents who were candidates of 1986, 1987 Miss Korea Beauty Contest. The 77 individual samples were selected and studied by cephalometric analysis. Obtained results were as follows: 1 Korean esthetic norm was obtained. 2. Simple correlation coefficient was obtained between each measurements. 3. Holdaway ratio of Korean beauty was 3.3 : 1. 4. To compare with caucasian, profile of Korean beauty had less prominant nose, more prominant lip and deeper sulcus depth.

Background The most common cutaneous malignant masses are basal cell carcinoma (BCC) and squamous cell carcinoma. The predominant site of a malignant mass is the face, which has many opportunities to be exposed to ultraviolet radiation. However the predilection sites of malignant masses have been equivocal due to the use of general regions, rather than anatomical landmarks, in surveys. A facial esthetic unit is an anatomical site classified as an area of similar facial contour characteristics that can be distinguished from other areas. The purpose of this study is to determine widely accepted anatomical landmarks using the esthetic unit. Methods We retrospectively analyzed 118 cases of malignant masses in our clinic from January 2005 to October 2012. We evaluated the patients' age, gender, and predilection site of the malignant mass by esthetic unit through pathology, medical records and patient photographs. We mapped the occurrence site of the malignant mass on schematic drawings of the esthetic units. Results Most of the malignant masses were BCC. The ratio of males to females was 1:1.41. The frequent predilection site of a malignant mass was on the nasal unit (33.1%), followed by the buccal unit (11.0%). Primary closure was the most common method of repairing a surgical defect (38.9%), followed by a local flap (35.5%). Conclusions This review described the relationship between clinical characteristics and esthetic units by proposing objective predilection sites for malignant masses, which can be used commonly as a framework in the study of malignant masses by unifying equivocal occurrence sites.
Purpose: This study classified alveolar arch forms and evaluated differences in alveolar bone thickness among arch forms in the anterior esthetic region using cone-beam computed tomography (CBCT) images. Materials and Methods: Axial views of 113 CBCT images were assessed at the level of 3 mm below the cementoenamel junction (CEJ) of the right and left canines. The root center points of teeth in the anterior esthetic region were used as reference points. Arch forms were classified according to their transverse dimensions and the intercanine width-to-depth ratio. The buccolingual alveolar bone thickness of each tooth was measured at 3 mm below the CEJ and at the mid-root level. Differences in the mean thicknesses among arch forms were analyzed. Results: Anterior maxillary arches could be classified as long narrow, short medium, long medium, and long wide arches. Significant differences in buccolingual alveolar bone thickness among the arch groups were found at both levels. The long wide arches presented the greatest bone thickness, followed by the long medium arches, while the long narrow and short medium arches were the thinnest. Conclusion: Arch forms were classified as long narrow, short medium, long medium, and long wide. The buccolingual alveolar bone thickness exhibited significant differences among the arch forms.

Purpose: This study was designed to help to provide the criteria of the esthetic smile in oromaxillary region on twenties, fifties and sixties. Materials and methods: The facial straight photographs of 33 adults (male 15, female 18, mean age 27.2 years old) and 20 seniors (male 7, female 13, mean age 55.6 years old) in resting and smile position were taken. The measurements and proportion of lip-teeth relation during smile were statistically analyzed, and survey of freshmen and seniors who belong to School of Dentistry, Chonnam National University were taken to select the best esthetic smile among the photographs. Results: In the relationship between the upper lip and the teeth, average smile was the most frequent, and mean of smile line ratio was 1.090 in young group and 0.90 in old group. The correlation between the buccal corridor ratio and exposed teeth count was inversed. At smile, most frequently exposed tooth was the second premolar (63.64%) in young gourp and the first premolar (35.00%) in old group. The correlation between the upper lip change ratio and exposed clinical crown length of maxillary central incisor was inverse, but there was no correlation between the lower lip change ratio and exposed clinical crown length of maxillarycentral incisor. Conclusion: In the result of survey, the students selected the case exposed to the first premolar as the most esthetic smile. The most esthetic smile between full facial photograph and the lip only photograph by the freshmen was different, but that by seniors was identical.
The treatment of areas demanding esthetic requirements, such as maxillary anterior teeth, should take into account the achievement of a healthy, harmonious to the surrounding tissue, and an attractive smile line. In this case report, smile line, soft tissue and hard tissue morphology, and the anatomy and proportion of the tooth, must be considered. In patients with unesthetic maxillary anterior ratio due to inadequate gingival contour and diastema caused by peg lateralis, it would be challenging to achieve an esthetic restoration by orthodontic treatment alone. In such case, after orthodontic treatment, dento-gingivo-facial compositional diagnosis and analysis, followed by gingivectomy and prosthodontic restoration is needed to improve the interdental mesial/distal, width/length ratio to achieve a satisfactory esthetic result. In addition, when improving the tooth proportion of peg lateralis by prosthodontic treatment, Porcelain laminate veneer (PLV), which results in less tooth structure loss, reproduction of similar shade to that of the proximal tooth and high transparency, is recommended. This case report demonstrates esthetic improvements by prosthodontic restoration through systematic diagnosis and treatment procedure in patients with unesthetic maxillary anterior proportion after orthodontic treatment due to peg lateralis by means of two female patients aged twenty years old.
Objective: This study aimed to quantitatively assess the relationship of smile esthetic variables with various types of malocclusion, and identify the cephalometric factors affecting smile measurements. Methods: This retrospective study included 106 patients who were treated with retention at the orthodontic department of Taipei Medical University Hospital. Hard-tissue variables were measured using lateral cephalographic tracings, and nine smile esthetic variables were measured using facial photographs. The patients were divided into three groups according to their overjet (< 0, 0-4, and > 4 mm). An analysis of variance was conducted to compare the pretreatment cephalometric variables and smile esthetic variables among the three groups. Multiple linear regression analysis was performed to identify the cephalometric factors affecting the smile measurements in each group. Results: Except the upper midline and buccal corridor ratio, all of the smile measurements differed significantly among the three groups before orthodontic treatment. Some of the smile characteristics were correlated with the cephalometric measurements in different types of malocclusion. The overjet was the major factor influencing the smile pattern in all three types of malocclusion. Conclusions: Smile characteristics differ between different types of malocclusion; the smile may be influenced by skeletal pattern, dental procumbency, or facial type. These findings indicate that establishment of an optimal horizontal anterior teeth relationship is the key to improving the smile characteristics in different types of malocclusion.
