Current scintigraphic tests of gastrointestinal motor function provides relevant pathophysiologic information, but their clinical utility is controversial. Many scintigraphic methods are developed to investigate gastrointestinal motility from oral cavity to colon. These are esophageal transit scintigraphy, oropharyngeal transit study, gastric emptying test, small bowel transit time measurement, colon transit study and gastroesopahgeal reflux scintigraphy. Scintigraphy of gastrointestinal tract is the most physiologic and noninvasive method to evaluate gastrointestinal motility disorders. Stomach emptying test is regarded as a gold standard in motility study. Gastrointestinal transit scintigraphy also has a certain role in assessment of drug effect to GI motility and changes alter therapy of motility disorders. Scintigraphy provides noninvasive and quantitative assessment of physiological transit throughout the gastrointestinal tract, and it is extremely useful for diagnosing gastrointestinal motor dysfunction. This article reviews the current procedures, indications, significance and guidelines for gastrointestinal motility measurements by scintigraphy.
The etiology of intractable hiccups is most commonly idiopathic. However, they are occasionally associated with some underlying disorders including gastro-esophageal reflux disease. There are a few previous reports describing the association of intractable hiccups with high dose corticosteroid. We experienced an unusual case of intractable hiccups following a high dose intravenous methylprednisolone therapy in a patient with right third nerve palsy. Since methylprednisolone is commonly used in various neurological problems, physicians should be aware of its possible side effect including intractable hiccups.

This paper deals with mechanism design, which makes it possible to apply multiple suturing without the repeat of inserting and removing the endoscope. This successive suture mechanism includes gripper, linkage, rotational part, and articulation part. Also, the mechanism is operated through 4mm one-side channel attached to the endoscope by means of pulling wires.
Esophageal hiatal hernia is common disorder in western sociey, and mainly affects mid-aged women, There are two types of hiatal gernia; common and more benign type is sliding [type I], and more severe type is paraesophageal [type II], and Skinner subdivided type II as true II, IIA, III, and paraesophageal group, As Skinner`s nomination, this case could be belong to IIA, which imply the state that natural hiatus is occupied with gastric antrum or duodenum instead of normal esophago-gastric junction, Main problem of paraesophageal hernia comes from its complication; intestinal obstruction, volvulus, strangulation, and incarceration, as well as pulmonary aspiration. So, as soon as confirm diagnosis, it should be corrected surgically to prevent above complications, and sometimes it could result in serious condition. We have experienced paraesophageal hiatal hernia in 3-day newborn infant and have repaired it successfully. We used transabdominal approach to repair and to prevent reflux Nissen`s fundoplication was performed. We would report that with reference study.
Six cases of achalasia were treated by modified Hellers operation at the department of thoracic % cardiovascular surgery of Kosin medical college from April 1984 to July 1988. Among 6 cases, 4 were male and 2 were female. Preoperative main symptoms were dysphagia in 5 cases and regurgitation in 1 case, its duration was variable from 1.5 years to 40 years. Esophagocardiomyotomy was perfomed in all cases, and the results were excellent to good, but reoperation was needed in one case due to incomplete myotomy. One case of advanced achalasia was also experienced and the result was satisfactory. Antireflux procedures were not performed in all cases and there was no reflux esophagitis on follow-up study.
Functional gastrointestinal disorders (FGIDs) such as infantile colic, constipation and colic occur in almost half of the infants. The aim of this paper is to provide a critical and updated review on the management of FGIDs and their impact on the health of the infant and family to health care physicians. Guidelines and expert recommendations were reviewed. FGIDs are a frequent cause of parental concern, impairment in quality of life of infants and relatives, and impose a financial burden to families, health care, and insurance. Therefore, primary management of the FGIDs should be focused on improving the infants' symptoms and quality of life of the family. If more than parental reassurance is needed, available evidence recommends nutritional advice as it is an effective strategy and most of the time devoid of adverse effects. The role of healthcare providers in reassuring parents and proposing the correct behavior and nutritional intervention by avoiding inappropriate use of medication, is essential in the management of FGIDs.
Background and Objectives : Laryngopharyngeal reflux disease (LPRD) originates from regurgitation of gastric contents to the laryngopharynx, and it is relatively common disease in otolaryngology. Proton-pump inhibitor (PPI) medication is frequently using treatment method in present time, but controversies exist regarding treatment for it. High-resolution manometry (HRM) system is a newly developed device that using 36 channels and pressure topography plotting program, checking the function and status of esophagus. The aim of this study was to evaluate clinical usefulness of HRM in LPRD patients who do not response to PPI medication. Materials and Method : From July 2009 to July 2010, a total of 99 patients who were suspected LPRD according to symptoms (Reflux Symptom Index >13) and laryngoscopic findings (Reflux Finding Score >7) were retrospectively enrolled in this study. Patients were consisted of 31 men and 68 women, the mean age was 51.8 years, the mean BMI was 22.46. Patients have taken PPI medication for 2 months, after medication, they performed HRM. Comparative analysis was performed with results of the difference in symptoms. Results : In 99 patients, 37 patients (37.4%) show not improvement of LPRD symptoms after PPI medication. Among them, 18 patients (48.6%) showed abnormal findings that were classified as Peristaltic dysfunction (38.9%), Diffuse esophageal spasm (11.1%), Relaxation impairment of LES (11.1%), Achalasia (5.6%), Hypotensive LES (11.1%), Relaxation impairment of UES (11.1%), Nutcracker esophagus (5.6%), Decreased resting pressure (5.6%). Other 62 patients (62.6%) show improvement of LPRD symptoms, 23 patients (37.1%) showed abnormal findings. Most common finding was Peristaltic dysfunction (43.4%) More prevalent abnormal findings of HRM were found in patients who revealed no response to PPI medication (48.6% vs. 37.1%), but it does not reach the statistical significance. Conclusion : Although statistical significances is not showed, the difference observed in the frequency of HRM abnormal finding between response and no response for PPI medication. It has been estimated that HRM may be used to determine differential diagnosis in patients with LPRD. Further studies in lager population containing normal controls will be needed to prove clinical usefulness.

식도의 운동장애에 대한 분류는 새로 기술되는 질환도 있고, 분류가 바뀌는 경우도 있으며 제외되는 경우도 있어 아직 확립이 되지 않고 계속 진행 중인 상태이다. 방사선학적으로 일차연동운동은 없으며 식도의 하부 끝은 특징적으로 새 부리모양(Smooth, Tapered, Beak-like appearance)을 보인다. 대부분의 식도운동장애는 평활근 부위에서 발생되며 여러 질환에 의해 식도의 정상 연동이 감소되거나 소실되고(Hypomotility), 비정상적으로 증가되는 경우(Hypermotility)도 있다. 적절한 방사선학적 검사와 진단을 위하여 식도의 해부학과 생리학을 이해하는 것이 중요하다. 또 방사선학적 진단 시에는 환자의 증상이나 Manometry 소견을 참고해야 한다. 하부식도괄약근의 기능은 정상일 수 있으며 음식이 내려갈 때 완전히 열린다. Sperandio M 등은 diffuse esophageal spasm의 대부분이 평활근으로 되어 있는 distal esophagus에서 일어나기 때문에 diffuse 대신 distal로 바꾸어 DES로 표현해야 한다고 주장하고 있다. 식도 운동 장애의 진단에 있어 바륨 검사의 유용성은 Ott 등에 의하면 Achalasia 95%, DES 71%, NEMD 46%로 Overall sensitivity는 56%이지만 방사선 검사로 진단하지 못하는 Nutcracker esophagus나 Nonspecific disorder를 제외하면 89%의 sensitivity를 보인다. Purpose: Acid neutralization during chemical clearance is significantly prolonged in children with cystic fibrosis, compared to symptomatic children without cystic fibrosis. The absence of available reference values impeded identification of abnormal findings within individual patients with and without cystic fibrosis. The present study aimed to test the hypothesis that significantly more children with cystic fibrosis have acid neutralization durations during chemical clearance that fall outside the physiological range. Methods: Published reference value for acid neutralization duration during chemical clearance (determined using combined impedance/pH monitoring) was used to assess esophageal acid neutralization efficiency during chemical clearance in 16 children with cystic fibrosis (3 to < 18 years) and 16 age-matched children without cystic fibrosis. Results: Duration of acid neutralization during chemical clearance exceeded the upper end of the physiological range in 9 of 16 (56.3%) children with and in 3 of 16 (18.8%) children without cystic fibrosis (p=0.0412). The likelihood ratio for duration indicated that children with cystic fibrosis are 2.1-times more likely to have abnormal acid neutralization during chemical clearance, and children with abnormal acid neutralization during chemical clearance are 1.5-times more likely to have cystic fibrosis. Conclusion: Significantly more (but not all) children with cystic fibrosis have abnormally prolonged esophageal clearance of acid. Children with cystic fibrosis are more likely to have abnormal acid neutralization during chemical clearance. Additional studies involving larger sample sizes are needed to address the importance of genotype, esophageal motility, composition and volume of saliva, and gastric acidity on acid neutralization efficiency in cystic fibrosis children.
