식도천공의 임상적 고찰 -16례 보고- (Clinical Results of Esophageal Perforation)

  • Prompt recognition and proper treatment of esophageal perforation or rupture may ax~ert death or minimize complications. We have experienced sixteen patients of esophageal perforation at the department of thoracic and cardiovascular surgery, Hallym Medical college during the period fromJan. 1986 to Sep. 1993. The ratio between male and female patient was 13:3 and their age ranged from 23 years to 67 years old. The major cause of esophageal perforations was spontaneous rupture in 7 cases[45%], surgical trauma in 2 cases[12%], instrumental trauma in 2 cases[12%], and others in 5 cases[31%]. The common site of esophageal perforation was in the lower third portion of the esophagus[10 cases, 62%]. The most consistent symptom of esophageal perforation was chest pain in 11 cases, temperature elevation within a few hours was 9 cases.Contrast roentgenographic studies demonstrated the perforation in all but 2 of the 16 patients. The frequent complications of esophageal perforation were empyema in 7 cases[45 %] and mediastinitis in 2 cases[12%]. fourteen patients had suture closure and drainage with 2 deaths, and 2 patient received only drainage procedures. The mortality rate was 12%[2 cases] and cause of death was sepsis and aortic rupture.

  • Esophageal perforation is uncommon, however, due to the lack of serosa layer inflammation spread is rapid and common to neighboring structures, leading to significant mortality and morbidity. With an advancement of endoscopic diagnosis there is an increase of esophageal injury and perforation due to esophageal endoscopic procedure. The authors have recently experienced three cases of iatrogenic esophageal perforation following diagnostic endoscopic procedure of the esophagus. We summarize the pervention, diagnostic and therapeutic strategies for iatrogenic perforation.

  • Perforation of the esophagus is an uncommon problem with significant morbidity and mortality. In children undergoing endoscopy, the risk of perforation is higher when interventional endoscopy is performed. The clinical features depend upon the site of esophageal perforation. Opinions vary regarding the optimal treatment protocol, and the role of conservative management in this context is not well established. Esophageal perforation that occurs as a consequence of endoscopy in children requires careful evaluation and management, as outlined in this article.

식도천공 9례 보고 (Esophageal Perforation: 9 Cases)

  • H.S. Lee, M.D., H.S. Yu, M.D. Esophageal perforation occurred rarely but often lead to a high mortality and morbidity. In the past, the main cause of esophageal perforation in Korea were instrumental perforation in patient with lye stricture of the esophagus. We experienced 9 cases of other forms of esophageal perforation from 1972 through 1977 and obtained the following results. 1. These 9 patients ranged from 10 months to 40 years in age at the time of admission. Six were women. 2. Causes of perforation are instrumental perforation in 3, foreign body perforation in 3, spontaneous perforation in 3 and one pneumatic esophageal perforation. 3. Perforation developed in a variety of locations in the esophagus. Three occurred in cervical part, two in the upper thoracic part, two in the lower thoracic part. 4. The main clinical symptoms and signs were dyspnea, subcutaneous emphysema, chest pain and fever. 5. Thoracic rentgenogram disclosed subcutaneous and mediastinal emphysema, widening of mediastinum and pleural effusion at the time of admission. 6. Complications of esophageal perforation were mediastinitis [7 cases], empyema [4 case], respiratory distress [4 cases] and sepsis [3 cases]. 7. In 3 deaths of the nine patients who sustained perforation of the esophagus, one was due to transfusion of infected blood and two of them were due to sepsis following empyema and mediastinitis. Early treatment [less than 24 hr] gave no hospital death, and good results obtained in the perforations of cervical and upper thoracic esophagus.

  • Perforation of the esophagus, with any of its possible consequences, consetitutes an emergeny. We are reporting one case of transhiatal esophagectomy with esophagogastrostomy. The cause of esophageal perforation was baloon dilatation with underlying stricture. We recognized immediately intrathoracic perforation through routin check of Chest PA and confrormed dye leakage through esophagogram. She underwent emergency operation.

  • Background: The esophageal perforation is related to high morbidity and mortality rates if the diagnosis and treatment are delayed. The aim of this study is to evaluate the affecting factors on outcome and prognosis of the patients who suffer from esophageal perforation. Material and Methods: Twenty-six patients were treated with esophageal perforation in our institute from Jan. 1992 to Feb. 2010. We retrospectively reviewed the results of treatment for esophageal perforation to understand the risk factors affecting survival in patients. Results: The subjects are 26 patients suffered with esophageal perforation and the mean age is $52{\pm}17.8$ years old. 16 esophageal perforations were caused spontaneously, 25 cases were treated surgically. The mortality rate is higher in the cases o pre-operative mediastinitis. Conclusion: The shortness time interval until the beginning of treatment should be achieved to reduce the incidence of fatal post-treatment morbidity and mortality.

식도천공 및 후천성 식도기관(지)루 (Esophageal Perforation and Acquired Esophagorespiratory Fistula)

  • Esophageal perforation is one of the most grave prognostic problems among thoracic and general surgical emergencies which necessitate urgent operative measures. In Korea,there are still many persons ingesting lye for suicidal attempt and thoracic surgeons in Korea have more chances to deal with lye burned esophagus with or without instrumental perforation than those in Western countries. Main cause of esophageal perforation in Korea is instrumental perforation in patients with lye stricture of the esophagus during diagnostic endoscopy or therapeutic bouginage. Other causes are corrosion of the esophagus due to ingestion of caustic agents, pathologic perforation, surgical trauma, stab wound and spontaneous rupture of the esophagus in our series. Therapeutic measures are various,and depend on duration of perforation, severity of its complications, pathology of perforated portion of the esophagus and degrees of inflammation at the point of perforation. The most important therapeutic measures are prevention of this grave condition during esophagoscopy, bouginage and surgical procedures on lungs and mediastinal structures and to make early diagnosis with prompt therapeutic measures. During the period of January, 1959, to December, 1971, the authors experienced 65 cases ofesophageal perforation including acquired esophagorespiratory fistula at Dept. of Chest Surgery, the National Medical Center in Seoul, and obtained following results in the series. 1. Female were 35 cases, and peak age incidence was 2nd and 3rd decades of life. 2. Among 65 cases, 43 were corrosive esophagitis or benign stricture of the esophagus due to caustic agents, 7 were patients with esophageal cancer. and there were 5 cases of esophageal perforation developed after pneumonectomy or pleuropneumonectomy. 3. Causes of perforation are instrumental perforation in 45, acute corrosion in 7, pathologic perforation in 7, surgical trauma in 3, stab wound in 2 cases, and one spontaneous rupture of the esophagus. 4. Most frequent sites of esophageal perforation were upper and mid thoracic esophagus, and 8 were cases with cervical esophageal perforation. 5. Complications of esophageal perforation were mediastinitis in 42, empyema or pneumothorax in 35, esophagorespiratory fistula in 12, retroperitoneal fistula or abscess in 5,pneumoperitoneum in 3, and localized peritonitis in 1 case. 6. Cases with malignant esophagorespiratory fistula were only 3 in the series which is predominant cause of acquired esophagorespiratory fistula in Western countries. 7. Various therapeutic measures were applied with mortality rate of 27.7% in the series. 8. In usual cases early treatment gave better prognosis, and least mortality rate in cases with perforation in mid thoracic esophagus. 9. Main causes of death were respiratory complications,acute hemorrhage with asphyxia, and septic complications. 10. Esophageal perforation developed after pneumonectomy gave more difficult therapeutic problems which were solved in only 1 among 5 cases.

식도천공의 치료 및 임상고찰 (Clinical Analysis and Treatment of Esophageal Perforation)

  • 배경: 식도천공은 즉시 진단되어 치료하지 않으면 높은 이병률과 사망률을 보인다. 수술 방법의 진보에도 불구하고 식도천공의 치료의 시기와 방법은 아직 논쟁의 여지가 많은 상태이다. 대상 및 방법: 1990년 1월에서 2004년 6월까지 계명대학교 동산의료원에서 식도 천공을 진단받은 38명의 환자에서 차트를 이용하여 후향적 조사를 하였다. 결과: 환자는 남자가 28명, 여자가 10명이었고 평균 연령은 43.84$\pm$18.89세($1{\~}73$세)였다. 천공은 경부에서 8예, 흉부가 28예 그리고 복부가 1예 발생하였고 자연성 식도 천공이 $34\%$, 의인성이 $32\%$ 그리고 외상성이 $34\%$로 조사되었다. 치료는 경부식도 천공에서는 일차봉합 6예, 배농만 시행한 경우가 2예였으며, 흥부식도 천공에서는 일차봉합 14예, 식도 절제술 3예 그리고 보존적 치료가 12예였다. 사망률은 경부 식도 파열이 $25\%$, 흉부식도 파열이 $34\%$였다. 환자의 생존에 유의한 위험인자는 술 전의 패혈증 여부로 나타났다. 결론: 식도파열에서 가장 중요한 위험인자가 술 전의 패혈증 상태로 나타났다 술 전에 상태를 호전시킬 수 있는 적극적인 치료가 예후를 향상시킬 수 있을 것이다.

  • 진단이나 치료 목적으로 내시경술이 빈번하게 사용됨으로써 오늘날 식도천공의 위험은 증가하는 경향을 보인다. 식도벽에 경미한 상처를 주더라도 치명적인 경우까지 야기시킬 수 있으며 조기진단과 신속한 치료를 하지 않으면 늪은 사망률 병3변(病變)을 일으킨다. 본 연자들은 식도 이물 제거시 식도경술로 인한 식도천공 및 식도주위농양을 동반한 2례에 대한 치료를 하여 좋은 결과를 얻었기에 문헌고찰과 함께 보고 하는 바이다.

의인성 하인두-식도천공에 대한 외과적 고찰 (Surgical Evaluation of Iatrogenic Hypopharyngo-esophageal Perforation)

  • Background : Esophageal perforation due to a traumatic endoscopy or intubation is exceedingly rare. If riot noticed immediately or treated promptly, however, the morbidity and mortality is significant. We performed a retrospective review of patients with iatrogenic hypopharyngo-esophageal perforation to assess the outcome of current management techniques. Material and Methods : We retrospectively analyzed all cases iatrogenic hypopharyngo-esophageal perforation diagnosed at our hospital from January, 1999, through April, 2004. The study group consisted of 11 patients (4 men) with a mean age of 47.6 years (range, 21-83 yr). We reviewed the 11 patients with perforated injuries of the hypopharynx or esophagus during the diagnostic or therapeutic procedures. Result: Perforations were due to diagnostic gastroscopy ($54.5\%$, 6/11), esophageal dilation ($27.3\%$, 3/11), endoscopic port insertion ($9.1\%$, l/11), and tracheal intrathoracic ($9.1\%$, 1/11). Seven patients had intrathoracic and 4 had cervical perforations. Treatment included incision and drainage (5), resection and reconstruction (4), drainage only (1), and observation (2). Nonfatal complications included transient pneumonia (1), and wound infection (1). They occurred in advanced mediastinal abscess ]patients. Mortality was $9.1\%$ (1/11) in old patient who managed medically in cervical esophageal perforation. Conclusions : Current mortality rates in iatrogenic esophageal perforation were improved compared to previous published rates of $19\%\;to\;66\%$ for all patients with this condition. We concluded that aggressive and definitive surgery for thoracic esophageal perforations improving the survival rate, whether diagnosed early or late.
