  • We experienced a case of acquired benign bronchoesophageal fistula associated esophageal diverticulum which was treated successfully by division of ~stulous tract and esophageal diverticulectomy.Benign bronchoesophageal fistulas associated with esophageal diverticulum are very rare. This presentation is characterized by paroxysmal cough especially after drinking liquids and is easily diagnosed by esophagogram. We report a case with review of literatures.

  • Recently we experienced one, case of epiphrenic esophageal diverticulum of a 50 - year - old female patient who had complained heaviness in her chest after meals and occasional dysphagia for one year. Preoperative barium study showed a large epiphrenic esophageal diverticulum at about 7cm above the diaphragm which protruded to the right side of the mid thorax. On the operation field, epiphrenic esophageal diverticulum, measuring 5x 6x3cm in size, was noted. Diverticulectomy and extended myotomy was performed and the postoperative course was uneventful.

  • Eleven patients with esophageal diverticulum were operated on between August 1982 and August 1992 at the Department of Thoracic and Cardiovascular Surgery of Chonnam National University Hospital have been studied. 9 patients were male and 2 were famale and the age distribution was between 20 and 55 years. Various subjective symptoms were noticed preoperatively. Diagnosis was confirmed by esophagography. The types of esophageal diverticulum were traction type in 6 cases and pulsion type in 5 cases. There were nine cases of midesophageal diverticula, 1 cases of upper-thoracic esophageal diverticulum and 1 cases of epiphrenic diverticulum. Diverticulectomy alone in 10 cases and diverticulopexy was performed in 1 case. Fistulectomy in 5 cases, lobectomy in 1 case, segmentectomy in 1 case, and Eloesser`s procedure in 1 case were performed with associated procedures. There were no death or morbidity and all patients have achieved marked improvement of symptoms except three patients who had a concomitant mucoepidermoid tumor, had a complicated postoperative pyothorax, and had a postoperative recurrence of midesophageal diverticulum. Recurrence of symptoms were not noticed during follow up except 1 recurrence of diverticulum confirmed by esophagogram.

  • Diffuse esophageal spasm[DES] is a rare disease, and its surgical management is controversial. We experienced one case of diffuse esophageal spasm with a large epiphrenic diverticulum. We resected the diverticulum with right side approach and underwent extended esophageal myotomy from the apex of the chest to 3 cm above the diaphragm. And then we preserved the LES without antireflux procedure.

  • We'd performed the upper gastrointestinal study for a total of 1,033 insureds-male 630, female 403 persons-who were examined at medical dept. of Dae Han Kyoyuk Insurance Co., Ltd., from August, 1986 to January, 1987. The results on diverticulum were as follows; 1. The incidence rate of duodenal diverticulum is exceptionally higher than esophageal diverticulum. 2. In all of 53 insured who have duodenal diverticulum and esophageal diverticulum, there was little difference between male and female in the incidence rate of diverticulum; 5.08% in male, 5.21% in female. 3. The possessing rate of both diverticulums increased by age regardless of sex. ; 0.71% in 20yrs, 2.12% in 30yrs, 11.11% in 40yrs, 12.75% in 50yrs, 30.43% in 60yrs more. 4. The possessing rate of both diverticulums in male is 0% in 20yrs, 1.97% in 30yrs, 7.21% in 40yrs, 15.09% in 50yrs, 27.27% in 60yrs more and in female, 1.16% in 20yrs, 2.40% in 30yrs, 7.87% in 40yrs, 10.20% in 50yrs, 33.33% in 60yrs more. 5. Those who have duodenal diverticulum 47 insureds felt the following subjective symptoms; uncomfortable 8.51%, heartburn and tingling each 4.26%, sore throat 2.13%and esophageal diverticulum's heartburn 16.67%. 6. There occurred the following complications in 47 insureds with duodenal diverticulum-gastric polyp, erosive gastritis, antral gastritis, cascade stomach, fatty liver, polyp in GB and choledocholithiasis; each 2.13% and cholelithiasis 6.38% and cascade stomach. 7. All duodenal diverticulum occurred in duodenal inlet. 8. The number of lesion was single in all esophageal diverticulum, but there was each one case with 2 lesions and 3 lesions in duodenal diverticulum.

  • Esophageal diverticulum is very rare disease. Since it usually has no symptoms, it is hard to find and diagnose. Moreover, if the diverticulum is located nearly to thyroid gland, surgeons sometimes misdiagnose as a thyroid nodule. Here, we represent our case which has papillary thyroid cancer and esophageal diverticulum together. In this case, we experienced the surgical procedure and postoperative complications of the esophageal diverticulum. Even though esophageal diverticulum was in preoperative computed tomography and ultrasound, it was neglected before the surgery so that found during the operation. The purpose of this study was to know the preoperative radiological findings of the esophageal diverticulum and to find out what to be aware of during surgery and how to manage the complications after surgery.

  • Esophageal diverticulum is a rare entity, the true incidence of which is unknown, mainly because there are usually no clinical symptoms. Most esophageal diverticulum are found incidentally during an endoscopic or radiologic examination. Their classification is based on anatomical location; namely, upper third(Zenker or pharyngoesophageal), middle third(thoracic), or lower third(epiphrenic). Here we report a 52-years-old female presenting with dysphagia and regurgitation. Esophagogram showed esophageal diveticulum at lower cervical esophagus. Its positional aspect, it is different from Zenker's diveticulum. Treatment is surgical via an endoscopic or external approach. In view of the patient's age and anatomical location, various surgical approaches were considered as a therapeutic option for the management. This paper presents our experience in the management of esophageal diverticulum which was unusual location and stapled open resection without sternotomy.

  • Vascular ring, caused by Kommerell's diverticulum and ligamentum arteriosum, in a patient with right aortic arch and mirror image branching is extremely rare. A 10-month-old boy with coughing and stridor was diagnosed as having tracheo-esophageal stenosis, which is caused by a vascular ring with Kommerell's diverticulum, ligamentum arteriosum, right aortic arch, and mirror image branching. Kommerell's diverticulum was successfully resected via a left thoracotomy. The patient has been free from tracheo-esophageal stenosis for a year after the surgery.
