  • H type의 기관식도루 특히 성인에서 발견된 경우는 아주 드물다. 최근 영남대학교 의과대학 흉부외과학교실에서는 식도폐쇄를 동반하지 않은 선천성 기관식도루 1예를 수술치험하여 양호한 성적을 얻었기에 문헌고찰과 함께 보고하는 바이다.

식도질환의 외과적 치료 75례 (Surgical experience of esophageal disease: report of 75 cases)

  • A clinical study was performed on 75 cases of the esophageal cancer and benign esophageal diseases experienced at Department of thoracic & cardiovascular surgery, School of Medicine, Keimyung University during 3 year period from 1978 to 1982. Of 75 cases of the surgical esophageal diseases, there were 35 patients of the esophageal cancer. 17 patients of benign esophageal stenosis, 10 patients of esophageal perforation, 4 patients of diverticulum. 3 patients of achalasia, 2 patients of congenital T-E fistula, one of upper esophageal web, one of esophageal foreign body, one of leiomyoma and patient of hemangioma. First, esophageal carcinoma was more frequent in men than in women by a ratio of five to one, and the peak incidence occurred in the 5th to 6th decade. Dysphagia was the most common symptom in 88.6 percent of our cases. The tumor was located mostly in the middle & the lower one third [91.4%]. The histological diagnosis was made in 35 cases. The squamous cell carcinoma was the most common [82.9%] and the rest was the adenocarcinoma in the lower one third [17.1%]. Thirty-five cases were operated and resection was feasible in the twenty-five patients [71.4%] with 2 cases of hospital mortality [5.7%]. All but two of the esophageal stenosis were caused by corrosive esophagitis and ages ranged from 7 to 70 years with average age of 32 years. Corrective operations were performed on 17 patients of esophageal stenosis of whom 12 patients had esophagocologastrostomy, 3 patients esophagogastrostomy and in non-corrosive esophageal stenosis one case and esophagoplasty and another case had release of external compression. There was one complication of stenosis of the esophageal perforation were traumatic in five cases, empyema in three cases, caustics in one case and postemetic in one case. 10 patients of the esophageal perforation underwent operation: primary closure in 5 cases, two staged colon interposition in 2, esophagogastrostomy in 1 and closed thoracotomy in 2 cases There were 2 complications of leakage of anastomosis sites in postoperative period. 4 patients of traction type of diverticulum underwent diverticulectomy & 3 patients of achalasia underwent modified Heller`s operation. 2 patients of congenital esophageal atresia had distal tracheoesophageal fistula & underwent one staged operation with the results of one death caused by pneumonia. Upper esophageal web had divulsion through the esophagoscope and foreign body in upper esophagus was removed through cervical esophagotomy. One case of leiomyoma in esophagus had esophagectomy and reconstruction with right colon. And one case of hemangioma in esophagus had esophagectomy & esophagogastrostomy.

  • Congenital bronchoesophageal fistula is a rare anomaly that can appear in adult life uncommonly. There was a largest review that include 100 cases in the literature. In our case, he was 48 years old male patient who admitted for chronic coughing and recurrent lobar pneumonia on the right lower lobe since 10 years old. We could confirmed the fistular preoperatively by barium swallow examination and performed division of the fistula. The patient’s postoperative course was unremarkable. According Braimbridge’s classification, it was belonged to the type II.

성인 양성 식도기관지루 -수술치험 1례- (Esophagobronchial Fistula in Adult(a case report))

  • Benign esophagobronchial fistula without esophageal atresia is a relatively rare disease. Fistula between the esophagus and bronchi may be congenital, traumatic, inflammatory or neoplastic. In our country, several case reports have been presented and the causes were either congenital, spontaneous, or inflammatory, such as, tuberculosis and diverticulum of esophagus. A 36 year old man experienced cough secondary to swallowing a Korean soup, frequent URIs and dyspnea. Esophagobronchial fistula was diagnosis by the esophagogram. Treatment was by resection with mid-lobectomy of the Rt. lung. After surgery, the patient`s general condition was stable.

H-type 기관식도루 1례 (A Case of H-type Tracheoesophageal Fistula)

  • Isolated(H-type) tracheoesophageal fistula without esophageal atresia occurs in approximately 4% of esophageal anomalies, and represents the least form of abnormal laryngotracheo-esophageal communication. Its symptoms such as coughing and choking The during the feeding, abdominal distension and recurrent pneumonitis usually start from birth. Diagnosis is made between 4 days to 4 years using the contrast esophagography and/or tracheoscopy. In case of diagnostic delay the postoperative mortality is not negligible and the most common cause of mortality is respitatory problems(infection, respiratory distress). So early diagnosis is essential in the newborn period with high index of suspicion. We report a case of H-type TEF in which operative repair was successful with references to recent literature.

  • Slide tracheoplasty. as a treatment for congenital tracheal stenosis, has been recently reported to have good results and quite a number of advantages as compared with conventional tracheoplasties. The aim of this study is to report a new surgical technique modified from the slide tracheoplasty. "the slide cricotracheoplasty" for the congenital cricotracheal stenosis. A girl was born by Cesarean section and the diagnosis of esophageal atresia (Gross type C) and cricotracheal stenosis (30 % of total length of trachea) was established. Esophageal atresia was successfully corrected at the 8th day of life. At the 31st day of life, corrective surgery for congenital cricotracheal stenosis. the slide cricotracheoplasty. was performed with success. Slide cricotracheoplasty is almost the same procedure as slide tracheoplasty except for two technical features. First the cricoid cartilage was split on its anterior surface. Second the split cricoid cartilage was fixed to pre vertebral fascia to maintain enough space to accommodate the sliding caudal segment of trachea because of the stiffness of the cricoid cartilage. We believe that the sliding cricotracheoplasty is a new surgical technique for congenital cricotracheal stenosis that has similar results and advantages as the sliding tracheoplasty.

신생아 외과환아에서 말초혈관을 통한 전비경구적 영양요법에 대한 고찰 (Total Parenteral Nutrition(TPN) via Peripheral Veins in Neonatal Surgical Patients)

  • Parenteral nutrition has been an essential part of postoperative care of neonates requiring major surgery who are unable to tolerate enteral feeding for long periods during the postoperative period. However, TPN via central venous catheters(central TPN), used in increasing trend, still presents significant morbidity. To find out whether TPN via peripheral veins(peripheral TPN) could be used as a viable alternative for postoperative parenteral nutrition in neonates, a clinical study was carried out by a retrospective analysis of 53 neonates subjected to peripheral TPN for more than 7 days after surgery. Operations consisted of procedures for esophageal atresia with tracheoesophageal fistula, gastroschisis and omphalocele. Surgery was performed at the Division of Pediatric Surgery, Department of Surgery, Hanyang University Hospitall, from 1983 to 1994. The mean total duration of TPN was 13.3 days (range; 7-58 days), the average daily total fluid intake was 117.6 ml/kg during TPN and 158.6 ml/kg during subsequent oral feeding. The average daily total calorie intake was 57.7 kcal/kg during full strength TPN and 101.3 kcal/kg during subsequent oral feeding. The mean urine output was maintained at 3.5 ml/kg/ hour during TPN and at 3.6 ml/kg/hour during subsequent oral feeding. The increment of body weight observed during TPN was 132 g in TEF, 53 g in gastroschisis and 3 g in omphalocele patients, while loss of body weight was not observed. The mortality rate was 5.7 %(3/53) and was related to the underlying congenital anomalies, not the TPN. The most common complication of peripheral TPN observed was laboratory findings suggestive of liver dysfunction in 23 cases(43.4 %) with no significant clinical symptom or signs in any case, transient pulmonary edema in one case, and generalized edema in one case. None of the major complications usually expected associated with central TPN were observed. The result of this study suggest that peripheral TPN can be used for adeguate postoperative nutritional support in neonates requiring 2 to 3 weeks of TPN.

소아의 선천성 및 후천성 식도 질환에서 내시경적 풍선 확장술 (Endoscopic Balloon Dilatation in Children with Congenital and Acquired Esophageal Anomalies)

  • 목 적: 소아의 선천성 및 후천성 식도 질환의 치료에 있어 내시경적 풍선 확장술의 안전성, 효과 그리고 기술적인 문제들을 평가하였다. 방 법: 1993년부터 10년 동안 선천성 및 후천성 식도 질환의 치료로 부산대학교병원 소아과에서 내시경적 풍선 확장술을 시행한 8명을 대상으로 이들의 의무 기록을 후향적으로 분석하였다. 시술은 풍선 카테터를 내시경 겸자구를 통해 삽입하여 내시경 직시 하에서 시행되었으며 최초로 사용되는 풍선의 크기는 식도의 내경과 협착 부위의 직경에 기초하여 결정하였다. 협착이 아주 심한 경우에는 먼저 유도 철선을 통과시킨 후, 유도 철선을 따라 풍선 카테터를 삽입시켰다. 풍선을 협착 부위에 위치시키고 서서히 확장하면서 목표하는 크기로 풍선이 팽창되면 1분 동안 확장된 상태를 유지하였다가 감압하여 1분 이상의 휴식을 가졌다. 확장 후 합병증이 없으면 풍선의 직경을 1 mm 간격으로 늘리면서 추가 확장하였다. 결 과: 남아가 3명, 여아가 5명이었고, 평균 연령은 4.2세였다. 8명의 환자에서 27회(환자 당 평균 3.2회)의 확장이 시행되었다. 기저 질환의 종류는 선천 성 식도 폐쇄에 대한 수술 후 발생한 식도 협착이 3예, 식도윤이 2예, 그 외 식도 무이완증, 빙초산에의한 부식성 식도염 그리고 고압성 식도 하부 괄약근이 각각 1예였다. 6~15 mm 직경의 풍선을 사용하였고, 최초 풍선의 크기는 협착의 형태와 정도에 따라 결정되었다. 합병증으로는 식도 천공과 심한 호흡 곤란이 각각 1예 있었다. 6예(75%)에서 시술 후 정상적인 음식 섭취를 보였다. 결 론: 내시경적 풍선 확장술은 소아의 선천성 및 후천성 식도 질환을 치료하는데 있어 안전하고 효과적인 방법이다.

Impact of Intravenous Omega-3-Enriched Lipid Emulsion on Liver Enzyme and Triglyceride Serum Levels of Children Undergoing Gastrointestinal Surgery

  • Purpose: To investigate the impact of omega-3-enriched lipid emulsion (LE) on liver enzyme (aspartate transaminase [AST] and alanine aminotransferase [ALT]) and triglyceride (TG) levels of children undergoing gastrointestinal surgery. Methods: This experimental randomized controlled group pretest-posttest design study included 14 children who underwent gastrointestinal surgery due to duodenal atresia, jejunal atresia, esophageal atresia, and need for parenteral nutrition for a minimum of 3 days at RSUD Dr. Soetomo Surabaya between August 2018 and January 2019. These children were divided into two groups, those who received standard intravenous LE (medium-chain triglyceride [MCT]/long-chain triglyceride [LCT]) and those who received intravenous omega-3-enriched LE. Differences in AST, ALT, and TG levels were measured before surgery and 3 days after the administration of parenteral nutrition. Results: Liver enzyme and TG levels in each group did not differ significantly before versus 3 days after surgery. However, TG levels were significantly lower in the omega-3-enriched intravenous LE group (p=0.041) at 3 days after surgery, and statistically significant difference in changes in TG levels was noted at 3 days after surgery between MCT/LCT intravenous LE group and the omega-3-enriched intravenous LE group (p=0.008). Conclusion: The intravenous omega-3-enriched LE had a better TG-lowering effect than the MCT/LCT intravenous LE in children undergoing gastrointestinal surgery.

Experiences with Endoscopic Interventions for Variceal Bleeding in Children with Portal Hypertension: A Single Center Study

  • Purpose: The aim of this study was to compare the efficacy and safety of band ligation and injection sclerotherapy in the endoscopic treatment of children with variceal bleeding. Methods: The study population included 55 children, all of whom were treated at the time of endoscopic diagnosis of esophageal varices at Asan Medical Center, Seoul, Korea, between January 1994 and January 2011. The primary outcomes included initial success rates and duration of hemostasis after endoscopic management (band ligation vs. injectionsclerotherapy). Results: The mean age was $6.7{\pm}5.2$ years and the mean follow-up time was $5.4{\pm}3.7$ years. The most common cause of esophageal varices was biliary atresia. Of 55 children with acute variceal bleeding, 39 had band ligation and 16 had injection sclerotherapy. No differences between groups were observed in terms of the size, location, and presence of red color sign. The success rates of band ligation and sclerotherapy in the control of acute bleeding episodes were 89.7% and 87.5%. The mean duration of hemostasis after endoscopic intervention was $13.2{\pm}25.1$ months. After one year, 19 of 39 patients (48.7%) treated with band ligation and 7 of 16 patients (43.8%) with injection sclerotherapy had experienced rebleeding episodes. Complications after the procedures were observed in 10.3% and 18.8% of children treated with band ligation and injection sclerotherapy. Conclusion: The results of our current study suggest that band ligation and injection sclerotherapy are equally efficient treatments for the control of acute variceal bleeding and prevention of rebleeding.
