Reconstruction of the pharynx and cervical esophagus presents a tremendous surgical challenge to the Head and Neck Surgeon. Because life expectancy of patients with advanced carcinoma of the hypopharynx, and cervical esophagus is limited, treatment must be aimed at palliation. A variety of techiques have been proposed over the years with none proving entirely satisfactory. These techiques include prosthesis; skin graft; cervical flaps; tubed cutaneous and myocutaneous chest flaps; visceral reconstruction with stomach, colon. and jejunum; and jejunal free autografts. Many factors dictate the best method of reconstruction in any given clinical situation. The goal of the surgery is a one-stage reconstruction of swallowing function with minimal morbidity to allow as short a hospital stay as posible. Nine patients underwent the free jejunal autograft reconstruction of the pharyngoesophagus after the ablative surgery for the advanced hypopharyngeal cancer. Postoperative complications included one perioperative death, two abdominal wound dehiscences, two neck hematomas, one carotid rupture, one funtional dysphagia, one late strictures. There were no graft failure, no immediate stenosis and no fistula. An oral diet was started between days 8 and 16, with an average of 9 days and median of 8 days. Patients left the hospital between days 9 and days 38, with an average of 23.4 days and median of 23 days. This method of reconstruction is advocated as reliable palliative procedure with short-term follow-up. In conclusion, we at Korea Cancer Center Hospital are of the opinion that the free jejunal autograft offers an excellent, safe and relative easy method of the pharyngeal and cervical esophageal reconstruction with significant advantages over other techiques.
Background. Ever since the first report of deltopectoral flap in pharyngo-esophageal reconstruction in 1965, various methods of flap reconstruction have been introduced, allowing surgical eradication of tumors that were once thought to be inoperable. Despite these advancement, many literatures emphasize distant metastasis and second primary malignancies as the most important factors that contribute to the low 5-year survival rate of the patients. Specific consensus about defining second primary cancer is still debatable, due to small number of reports regarding second primary tumors arising in flaps used for reconstruction of defects in the head and neck region. Case. We report a case of a 72-year-old male patient who, under the diagnosis of hypopharyngeal cancer, underwent total laryngectomy with partial pharyngectomy, extended right radical neck dissection with extended left lateral neck dissection, right hemithyroidectomy and radial forearm free flap reconstruction on June 16, 2003. After 37 cycles of radiation therapy, the patient exhibited no sign of recurrence. The patient revisited our department on June 14, 2016 with chief complaint of dysphagia that started two months before the visit. Radiologic studies and histology revealed squamous cell carcinoma in neopharynx, one that had been reconstructed with forearm free flap. Conclusion. Until now, only a handful of reports regarding patients with second primary cancer in reconstructed flaps have been described. Despite its rarity, diagnostic criteria for second primary cancer should always kept in consideration for patients with recurred tumor.
배경: 위내시경 검사나 기관삽관 시 식도 손상에 의한 식도 천공례는 대단히 드물다. 그러나 식도 천공의 발생 시 신속한 처치가 이루어지지 않는다면 예후는 매우 불량하다. 저자들은 기구에 의한 식도 천공례에서 기존 치료에 대한 효과를 알아보기 위하여 후향적 분석을 시행하였다. 대상 및 방법:1999년 1월부터 2005년 3월까지 저자들이 치험하였던 기구에 의한 식도 천공환자 12예를 대상으로 하였다. 천공의 원인과 부위, 내원까지의 지연 시간, 그리고 치료방법 등에 따른 예후 등을 분석하였다. 결과: 위내시경 검사에 의한 천공이 6예로 가장 많았으며(50.0%), 식도확장술에 의한 경우가 4예(33.3%), 그리고 내시경 포트삽입술과 기관삽관에 의한 경우가 각각 1예(8.3%)씩 있었다. 이 중 7예가 흉부 식도의 천공이었으며, 5예가 경부 식도의 천공이었다. 치료 방법으로는 식도절제 및 재건술이 5예, 절개 및 배농술이 4예, 폐쇄성 흉강 삽관술이 1예, 그리고 내과적 치료가 2예였다. 수술에 의한 위중한 합병증은 없었으며, 경미한 폐렴과 창상 감염이 각각 1예에서 발생되었다. 수술을 거부하여 내과적 치료를 시행하였던 1예에서 사망하여 사망률은 8.3%이었다. 결론: 기구에 의한 식도 천공에서 부위와 지연 시간에 상관없이 외과적 수술은 안전하며 또한 효과적인 치료방법이라고 생각되었다.
방사선 치료 후 발생한 악성 섬유성 조직구종은 아주 드물며 일반적으로 예후가 나쁘다. 52세 남자환자가 6개월 전부터 발견된 흉골부위에 발생한 동통성의 종괴을 주소로 입원하였다. 환자는 5년전 식도암으로 방사선 치료를 받은 병력이 있었다. 종괴는 절개생검상 흥골육종으로 진단되어 항암약물요법을 5회 시행하였으나, 종괴가 점점 커져 수술을 시행하였다. 수술은 흥골을 덮고있는 피부와 늑연골을 포함하여 흥골 전절제술을 시행하였고 이로 인한 결손부위는 자가늑골 이식, 양측 대흥근 근육판과 피부이식으로 흥벽재건술을 시행하였다. 수술후 조직소견은 악성 섬유성 조직구종에 일치하였다. 수술후 경과는 순조로왔으며 36일째에 퇴원하였다.
배경: 식도 천공의 초기 증상은 명확하지 않은 반면, 조기 진단과 아울러 적절한 치료가 즉각적으로 이뤄지지 않을 때는 매우 치명적인 결과가 나온다. 이에 본 연구에서는 식도 천공의 예후에 영향을 미치는 인자들을 파악하여 치료 성적의 향상에 기여하고자 한다. 대상 및 방법: 1984년 10월부터 2000년 6월가지 식도천공으로 내원한 32명을 대상으로 하여 환자의 성별, 연령, 천공 원인, 천공 부위, 치료 시작까지의 소요시간, 천공에 의해 발생한 증상과 합병증 그리고 치료 방법 등의 임상 관찰 항목을 조사하였으며, 각 항목에 따른 식도 천공 환자 생존과의 연관성을 알아보았다. 결과: 환자는 남자가 24명, 여자가 8명이었고 평균연령은 49.7$\pm$16.4세였으며, 천공의 원인은 기구조작과 수술손상등으로 인한 의인성인 경우가 14례(43%)로 가장 많았다. 천공부위는 흉부식도에서 가장 많았으며(26례, 81.2%), 증상은 흉통이 가장 많았다. 식도천공에 의한 합병증으로는 종격동염, 농흉, 전신패혈증, 복막염의 순이었으며 치료결과 생존 23례 사망 9례로 전체 사망률은 28.1%였으며 주요 사망원으로는 전신패혈증과 호흡부전증이었다. 치료로는 8례(25.0%)에서 보존적 치료만으로 치유가 가능하였고 수술적 치료로 경부배농술이 5례(15.6%), 일차봉합술이 7례(21.8%), 식도격리-우회로 시행후 식도 재건술을 시행한 경우가 12771(37.5%) 시행되었다. 초기치료에 식도천공이 완치된 경우는 18711(56.2%)였고, 초기치료에 실패한 14례(43.8%)의 경우에서는 다음 단계 치료과정후 완치되거나 도중 사망하였다.
배경: 식도-위 접합부에 존재하는 고압력대는 위-식도 역류를 예방하는 중요한 역할을 하는데 주로 하부 식도 괄약근이 관여한다. 그러나 하부 식도괄약근이 제거된 환자에서 횡격막 crura가 고압력대로 나타나 괄약양의 역할을 하는 지를 알아보기 위하여 식도 내압을 측정하였다. 대상 및 방법: 식도-위 절제술을 시행받은 환자 22명과 대조군으로 정상 성인 3명을 대상으로 식도 내압을 측정하였다 결과: 대조군은 3명 전원에서, 환자는 22명중 12명에서 고압력대가 나타났는데 Ivor-Lewis법 수술을 시행받은 환자보다는 좌측 개흉을 시행받은 환자에서 많이 나타났다. 고압력대의 위치는 환자군에서 비강으로부터 42.5$\pm$0.9cm로 대조군과 비슷한 위치에서 나타났으나, 길이는 2.13$\pm$0.6cm로 대조군 보다 짧았다. 고압력대의 압력은 station pull-through technique(SPT), rapid pull-through technique(RH)에 의해 측정한 결과 각각 13.78$\pm$1.63mmHg, 28.58$\pm$6.06 mmHg로 대조군 보다낮았으며, 부분적으로 연하에 의해 이완되고(84.4%), 복압증가에 의해 수축하는 양상을 보였다. 결론: 하부 식도 괄약근이 제거된 경우에도 횡격막 crura에 의해 흉복부 접합부에 고압력대가 나타나며, 이 횡격막 crura가 괄약양 역할을 하기 때문에 식도-위 절제술후 손상된 횡격막 crura의 복구를 시행함으로써 위-식도 역류를 감소시킬 수 있을 것으로 사료된다.
We present a rare case of critically compromised airway secondary to a massively dilated sequestered colon conduit after several revision surgeries. A 71-year-old male patient had several operations after the diagnosis of gastric cancer. After initial treatment of pneumonia in the pulmonology department, he was transferred to the surgery department for feeding jejunostomy because of recurrent aspiration. However, he had respiratory failure requiring mechanical ventilation. The chest computed tomography (CT) scan showed pneumonic consolidation at both lower lungs and massive dilatation of the substernal interposed colon compressing the trachea. The dilated interposed colon was originated from the right colon, which was sequestered after the recent esophageal reconstruction with left colon interposition resulting blind pouch at both ends. It was treated with CT-guided pigtail catheter drainage via right supraclavicular route, which was left in place for 2 weeks, and then removed. The patient remained well clinically, and was discharged home.
Tracheoesophageal fistula[TEF] is a rare but life-threatening lesion that may occur from ventilation with a cuffed tube. It occurs most frequently when an inlying esophageal tube is also being used-usually for feeding purposes. The mechanism of injury appears to be pressure experted on the tracheal wall by the cuff, which then compresses the "party wall" of the trachea and esophagus against the foreign body that lies in the esophagus. The patient was 32 years old female who had been receiving a treatment of respiratory failure induced by postoperative sepsis with assist ventilator and nasogastric tubal feeding. Sudden attack of abdominal gas distention and massive drainage of gas through N-G tube were developed during assist ventilation in that patient, so we diagnosed as tracheal stenosis with a tracheoesophageal fistula induced by prolonged endotracheal intubation We performed tracheal reconstruction and primary closure of perforated esophagus after weaning ventilator. The postoperative course was uneventiful.eventiful.
Acquired, nonmalignant tracheoesophageal fistula is an uncommom and difficult problem to manage. The most commom cause is a complication of endotracheal or tracheostomy tubes. Most are diagnosed while patients still require mechanical ventilation. The principle of treatment is two stage operation. First, new tracheostomy tube is placed so that the baloon is below the fistula, and gastrostomy and feeding jejunostomy are made for the drainage and feeding. Finally after weaning from the mechanical ventilation, tracheal resection and reconstruction are made, and the esophageal defect is closed in two layers and a viable strap muscle interposed into the two suture site to prevent recurrence. Recently, we experienced a case of acquired nonmalignant tracheoesophageal fistula which was developed during mechanical ventilation. She was successfully treated with the above two stage operation.
For most surgeons, stomach and colon are the first choice for reconstruction of the esophagus, as well as for bypass. When the esophagogastric or esophagocolonic anastomosis is made in the neck, cervical anastomosis site leakage is the main complication. In our most recent four patients who underwent a transhiatal & posterior mediastinal esophagogastric or esophagocolonic anastomoses following esophageal resection, we performed the cervical anastomoses with a circular EEA stapler. No leaks have developed at the anastomosis site. In these four patients the cancer was tiny and was located on the upper or middle third of the thoracic esophagus. A total esophagectomy was performed by blunt resection without thoracotomy. Surgical staplers have been used previously for esophagogastric anastomosis through a right thoracotomy with a very low rate of leakage. When the esophagogastric or esophagocolonic anastomosis is performed in the neck, the prevalence of leakage does not increase the postoperative mortality, but it can increase significantly the duration of hospitalization and morbidity. The use of the circular stapler allowed us to perform four consecutive cervical esophagogastric & esophagocolonic anastomoses without any leakage and to shorten the operating time.
