  • A flare from an Air Force fighter was abnormally dispensed during the landing. To determine the cause of the abnormal dispensing, fracture analysis, signal analysis and reproduction experiments based on physics of failures were performed. The primary cause of the failure was analyzed to be due to an intermittent fault of an internal circuit card in the AN/ALE-40 chaff/flare dispenser by a broken lid of a capacitor, and the root cause which had derived the primary cause was considered to be an improper handling during the domestic maintenance which were changed from the overseas maintenance due to the DMSMS problem. Therefore, the overall process of the maintenance capability development system was reviewed and alternative ways that considers maintenance error decision aid(MEDA) for system improvement were suggested to prevent further failures.

  • The wind turbine gearbox is important rotating part to transmit torque from turbine blade to generator. Generally, gear shaft which rotates causes vibration by influence of stiffness and mass with gear shaft. Root cause of this vibration source is well known to gear transmission error that is decided from gear tooth property. Transmission error excites a gear, and makes excitation force that is vibrated shaft. This vibration of shaft is transmitted to gearbox housing through gearbox bearing. If the resonance about which the natural frequency of the gearbox accords with shaft exciting frequency occurs, a wind turbine can lead to failure. The gearbox for wind turbine should be considered influence of vibration as well as the fatigue life and its performance by such reason. The cause to vibration should be closely examined to reduce influence of such vibration. In this paper, the cause of the vibration which occurs by a gearbox is closely examined and the method which can reduce the vibration which occurred is shown. It is compared with vibration test outcome of a 3MW gearbox for verification of the method shown by this paper.

  • Method: We reviewed the papers of communication analysis studies in various fields and summarized communication error researches. Results: We proposed perspectives for communication error analysis and explained the need for extended analysis framework using advanced human-machine interfaces, which will cause the change of communication between operation team, including shifted abstraction phenomena. And we describe main concepts and the case study of H-H-S framework and finally introduce current studies of communication analysis in nuclear domain. Conclusion: The goal of communication analysis is common in all industries. We have to share and co-work for event prevention. Application: The results of the communication analysis might help to facilitate the understanding for interested researchers.

  • This paper proposes a method to improve and analysis error cause of FI(Fault Indicator) information to be used for detecting fault section in distribution automation system. FI error cause is made by consideration fault current magnitude and time. So, a new method to prevent FI error is proposed to include fault current magnitude, time and direction. Therefore, it's considered network environments that grounded and ungrounded network in distribution automation system. The proposed method is proved by Matlap Simulink. By the result in this research, it's possible to quickly restoration, supplying stability and reliability power to customer.

  • The Oil Coolers is very important unit for the stable thermal performance in machine tools, semiconductor equipments and high precision measuring systems. To select a proper oil cooler for the temperature control of core unit in a machine, not only cooling ability but also static and dynamic sensitivity of temperature sensors are considered. In this paper, the relationship between cooling ability and inflow oil temperature is identified. The cooling ability is increased with the increase of inflow oil temperature. The oil temperature control errors of a cooler are influenced by mainly sensitivity of temperature sensors and heating velocity in a machine. The validity of error cause analysis for temperature control is proved by real cooling experiments with inflow and outflow temperature sensors.

  • 한국에서는 석유화학 산업의 역사가 30년 이상이 되어 시설이 노후화되기 시작하여 잠재적인 사고의 위험 가능성이 높아지고 있다. 지금까지의 석유화학 산업에서의 전통적인 위험성 평가와 시스템의 제어는 기계적인 결함에만 중점을 두었기 때문에 인간의 행동을 제어하는 것은 간과하여 왔다. 자동화 기술과 제어기술의 발전도 필요하지만 인간의 의사 결정 요소가 석유화학산업에서 사고를 예방하는데 필수적이다. 거의 모든 심각한 사고는 인간 행동과 안전 장비의 기계적인 결함이 동시에 부적당할 때 발생한다. 진보적인 인간의 신뢰성 분석 소프트웨어는 실패 데이터를 수집하고, 한국의 화학 산업에서 인간의 오류 확률을 분석하기 위해 개발되었다. 이 논문에서는 Root cause Analysis를 통한 결과와 PIF(Performance Influencing Factor) 평가 결과를 보여준다.

  • The timing chain system, which is a typical power transmission technology applied to a vehicle, has been widely used by the automotive industry because it is normally designed to last a car's lifetime. However, the timing chain system may cause some problems due to the shape of the chains and the polygonal behavior on contact between the chain and the sprocket. In addition, noise and vibration caused by transmission error are the most typical problems encountered by major automotive manufacturers and they are considered as the main source of customer complaint. The initial tension of the chain-sprocket system is thought to be the main cause of transmission error, and it is regarded as the source of engine vibration and noise. The initial tension of the chain system should be controlled carefully since a low initial tension can cause twisting, which may lead to a system malfunction, while a high initial tension can reduce the service life due to a worn down contact surface. In this paper, the kinematic analysis model is generated with various initial tensions, which are controlled by changing the shape of the fixed guide with the largest contact surface with chain. The results showed that the transmission error was minimized on a particular range of initial tension, and the tendency showed that the error changed with a higher sensitivity at a lower initial tension.

Development of a Multiple Linear Regression Model to Analyze Traffic Volume Error Factors in Radar Detectors

  • Traffic data collected using advanced equipment are highly valuable for traffic planning and efficient road operation. However, there is a problem regarding the reliability of the analysis results due to equipment defects, errors in the data aggregation process, and missing data. Unlike other detectors installed for each vehicle lane, radar detectors can yield different error types because they detect all traffic volume in multilane two-way roads via a single installation external to the roadway. For the traffic data of a radar detector to be representative of reliable data, the error factors of the radar detector must be analyzed. This study presents a field survey of variables that may cause errors in traffic volume collection by targeting the points where radar detectors are installed. Video traffic data are used to determine the errors in traffic measured by a radar detector. This study establishes three types of radar detector traffic errors, i.e., artificial, mechanical, and complex errors. Among these types, it is difficult to determine the cause of the errors due to several complex factors. To solve this problem, this study developed a radar detector traffic volume error analysis model using a multiple linear regression model. The results indicate that the characteristics of the detector, road facilities, geometry, and other traffic environment factors affect errors in traffic volume detection.

  • The study on semiconductor industrial accident in korea has been focused on frequencies of each type, employee, characteristics, cause and un-safety condition, behaviour and so on. Those attributes of semiconductor industrial accidents were usually analyzed independently, so that it was hard to provides a well-process and systematic guide lines for efficient safety management. There fore, there were a few studies based on comprehensive survey in terms of sharp-type of safe management. The questionnaire survey carried out for the workers(284) who were responsible for safety management in to center with corporate company with semiconductor industry the factor analysis showed that there were three factor of safety management. They were 1) Investment and operation and management for accident prevention, 2) Unsafe, safety management 3) General human error and behavior the industries of respondents were correlative with three group. Three Groups showed a statistically significant differences on the number of cases. Actually, the group with the larger investment and the better unsafe cause, human error a of accident prevention had a smaller cause of accident cases.
