골반고정장치에 의한 감압치료가 임산부 요통환자의 통증, 일상생활 제한 및 요추만곡에 미 치는 영향에 관한 연구 (The Effects of Decompression Therapy by Fixed Pelvic System on Low Back Pain During Pregnancy)

  • Background and Purpose : This study serves the purpose to present the effective way on back pain therapy for women during their pregnancy by comparing and analyzing the effects influencing on pain index, limits on daily life, and diminution of lumbar curve, when having treated for 6 weeks, dividing into therapy group, applied by decompression therapy on spine without unnecessary effects on the pregnant women and their fetus and the existent general physiotherapy group, applied on diminution of their back pain. Subject & methods : We made impregnate women of 30(25${\sim}$34years), as the subject of this study, who understood, and agreed with, this study, among pregnant women who were diagnosed as back pain. Pain intensity, limit index on daily life, and lumbar curve test, prior to experiment was implemented as paired t-test, comparison of the difference of average by each group before and after experiment was analysed as independent t-test, and statistical examination level was set up as 0.05. Results : The difference of average of pain and limit on daily life between the group, SDTG(spinal decompression therapy group), and the group, GPTG(general physical therapy group), before and after experiment was diminished as much as 44.4% and 16.79% each after completed 6 weeks' experiment, limit on daily life in both groups showed as much as 37.2% and 21.5% each, in the end, they statistically showed conspicuous difference(p<.05). Comparison of lumbar curves between both groups, SDTG and GPTG, before and after experiment, were diminished from -23.13${\pm}$4.85 to -15.66${\pm}$3.71, and there was no remarkable difference(p<.05). Conclusion : As, in this study, decompression therapy is more effective than general physical therapy with pregnancy low back pain in pain, limitation of daily life, lumbar curve.

스트레스-유도 열충격단백질 27(Heat Shock Protein 27)의 활성과 물리치료의 상관성 (The Activation of Stress-induced Heat Shock Protein 27 and the Relationship of Physical Therapy)

  • Purpose: Heat shock proteins (HSPs) are a group of proteins that are activated when cells are exposed to a variety of environmental stresses, such as infection, inflammation, exposure to toxins, starvation, hypoxia, brain injury, or water deprivation. The activation of HSPs by environmental stress plays a key role in signal transduction, including cytoprotection, molecular chaperone, anti-apoptotic effect, and anti-aging effects. However, the precise mechanism for the action of small HSPs, such as HSP27 and mitogen-activated protein kinases (MAPKs: extracellular-regulated protein kinase 1/2 (ERK1/2), p38MAPK, stress-activated protein kinase/c-Jun N-terminal kinase (SAPK/JNK), is not completely understood, particularly in application of cell stimulators including platelet-derived growth factor (PDGF), angiotensin II (AngII), tumor necrosis factor $\alpha$ (TNF$\alpha$), and $H_2O_2$. This study examined the relationship between stimulators-induced enzymatic activity of HSP27 and MAPKs from rat smooth and skeletal muscles. Methods: 2-dimensional electrophoresis (2DE) and matrix assisted laser desorption ionizationtime-of-flight/time-of-flight (MALDI-TOF/TOF) analysis were used to identify HSP27 from the intact vascular smooth and skeletal muscles. Three isoforms of HSP27 were detected on silver-stained gels of the whole protein extracts from the rat aortic smooth and skeletal muscle strips. Results: The expression of PDGF, AngII, TNF$\alpha$, and $H_2O_2$-induced activation of HSP27, p38MAPK, ERK1/2, and SAPK/JNK was higher in the smooth muscle cells than the control. SB203580 (30${\mu}$M), a p38MAPK inhibitor, increased the level of HSP27 phosphorylation induced by stimulators in smooth muscle cells. Furthermore, the age-related and starvation-induced activation of HSP27 was higher in skeletal muscle cells (L6 myoblast cell lines) and muscle strips than the control. Conclusion: These results suggest, in part, that the activity of HSP27 and MAPKs affect stressors, such as PDGF, AngII, TNF$\alpha$, $H_2O_2$, and starvation in rat smooth and skeletal muscles. However, more systemic research will be needed into physical therapy, including thermotherapy, electrotherapy, radiotherapy and others.

텍스트마이닝을 활용한 국외 노인작업치료의 연구동향 분석 (Study on the Research Trend of Overseas Elderly Occupational Therapy Using Text Mining)

  • 목적 : 본 연구는 텍스트마이닝을 활용하여 국외 노인 작업치료의 양적 변화와 현황 파악을 정량적으로 분석하고자 하였다. 연구 방법 : 2009~2019년까지 PubMed를 활용하여 Elderly, Health, Occupational therapy가 들어간 제목과 초록이 존재하는 연구논문을 분석대상으로 선정하여 초록의 단어들을 수집하였다. 수집된 정보를 통해 연도별 발생 논문 편수, 핵심어 분석, 연도별 핵심어 분석, 단어들 간의 관계성을 분석하였다. 결과 : 2009-2019년에 발표된 논문은 9,941편이며, 2009년부터 점차적으로 증가하여 2017년 또는 2018년에 최다 빈도를 보인 후 2019년에 감소하는 추이를 보였다. 최근 5년간 빈도수가 많은 단어는 Care, Group, Intervention, Pain, Treatment, Work이었다. 최근 11년간 평균적인 빈도수를 기준으로 단어 간의 관계성을 파악한 결과, Function, Health, Intervention, Participation이 단어들 간의 높은 관계성을 보였다. 결론 : 본 연구는 텍스트마이닝이라는 새로운 연구방법을 적용하여 작업치료의 동향을 실증적, 체계적으로 분석하여 거시적이고 종합적인 결과를 제시했다는 점에 의의가 있다. 이를 토대로 노인과 관련된 작업치료 임상 및 연구 현장에서 새로운 연구방향을 설정하는데 도움이 될 것으로 기대한다.

환경적용이 수면장애 모델 쥐의 혈중 멜라토닌 농도에 미치는 영향 (Effect of Different Environmental Application on Blood Melatonin Density in Sleep Disordered Rats)

  • 목적 : 본 연구는 수면장애 동물모델 쥐에게 환경변화를 적용하였을 때, 혈 중 멜라토닌의 변화를 통하여 수면장애 개선 효과를 확인하는데 그 목적이 있다. 연구방법 : 본 연구는 경북 소재의 동물실험실에서 실험을 진행하였고 수면장애 모델의 흰쥐 26마리에게 적용하였다. 3일간 환경변화를 적용한 실험군과 일반 케이지에서 환경변화를 적용받지 못한 대조군의 수면장애 모델 쥐 각각 13마리의 혈 중 멜라토닌의 농도를 확인하였다. 결과 : 수면장애 모델 쥐에게 환경변화를 적용한 결과, 환경 적용 후 실험군에서는 3일간 일별 멜라토닌 농도의 변화는 유의하게 증가하였고(p=.000), 대조군은 3일간 일별 멜라토닌 농도의 변화는 약간의 증가는 있었지만 유의한 변화는 없었다(p=.212). 환경 적용 전, 후 멜라토닌 농도의 변화는 실험군, 대조군 모두 유의한 차이를 보였지만 실험군에서는 멜라토닌 농도의 유의미한 증가가 있었고, 대조군에서는 멜라토닌 농도의 유의미한 감소가 있었다. 결론 : 수면장애 환자가 증가하고 있는 현대사회에서 환자들에게 적용되어지는 환경적용이 수면장애에 어떠한 영향을 주는지 확인하기 위해 동물모델을 제작하였고 인간의 환경변화와 유사한 환경변화를 적용하였으며 이를 통해 수면장애의 개선효과를 확인할 수 있었다.

도시 치유숲공원 조성을 위한 이용선호도 분석 연구 (A Study on the Analysis User Preference for Urban Healing Forest Park Planning Methods)

  • A survey was performed to present a scheme for building a healing forest park inside a city in consideration of the fact that healing forests are located in non-urban areas because of their nature, and are therefore not conveniently accessible to urban population (n=196). As a result, among 196 respondents to the survey, 74.5% indicated that it was necessary to build an urban healing forest park. In the case of the expected usage pattern, were an urban forest park to be built, the largest proportion responded that it would be used to "walk", followed by "rest and meditation" and then "meeting/talk/date". An analysis showed that the largest proportion responded that they factor in "Accessibility" (48%), followed by "Park condition" (16.8%), and "Vegetation type" (12.2%). According to the analysis of usage preference for healing therapy, it was shown that the largest proportion preferred "Terrain therapy" (37.8%), followed by "Exercise therapy" (19.9%), and "Naturopathy" (18.9%). The factor anlaysis of usage preference regarding urban healing forest park facilities showed that the components were divided into "Operation facilities" and "Natural healing space", and the largest proportion preferred "Trails for healing", followed by "Space or forest for meditation" and "Health promotion center", and then "Facilities for a exercise therapy". In building an urban healing forest park, we think that it is effective to introduce zones and facilities in consideration of park characteristics that are centered on everyday behaviors, such as taking a walk, which differ from those of healing forests located in non-urban areas.

추간원판의 퇴행성 변화에 관한 고찰 (A review of degenerative changes in the intervertebral disc)

  • The intervertebral disc is a cartilaginous structure that resembles articular cartilage in its biochemistry, but morphologically it is clearly different. It shows degenerative and ageing changes earlier than does any other connective tissue in the body, It is believed to be important clinically because there is an association of disc degeneration with back pain. Degenerative changes in the intervertebral disc are thought to develop as aging, mechanical stress and metabolic factors. Genetic factors may also play a part in the onset or progress of the degenerative process. They, together with environmental factors, may act as determinants of the structural characteristics of the intervertebral disc and produce a tendency to generation, In this short review we outline the morphplogy and biochemistry of normal intervertebral disc and the changes that arise during degeneration. Therefore this study will review degeneration of intervertebral disc, so we will have knowledge about low back pain associated with degenerative change in the intervertebral disc.

노인 낙상예방을 위한 운동중재에 관한 고찰 (Prevention of Falls in the Elderly: A review of exercise interventions)

  • Falls are a leading cause of injury, disability and death among people older than 65. Even without injury, falls cause a loss of confidence that results in reduced physical activity, increased dependency and social withdrawal. Fall prevention in the eldery people is a multifaced task that includes both the identification of risk factors and their modification strategies. Fall risk factors have been identified as both intrinsic and extrinsic. Intrinsic factors are those related to the physical and cognitive function of the person. Extrinsic factors are those which are included in the environmental hazards of everyday life. Combinations of interventions aimed at modifying both intrinsic and extrinsic risk factors can result in significant fall prevention in the geriatric population. This article selectively reviews the literature reporting exercise intervention to improve strength, balance and mobility.
