병원과 종합병원의 환자중심 의료서비스 제공 수준 평가- 환자경험평가를 중심으로 (Evaluation of Patient-Centered Healthcare Provision in Hospitals and General Hospitals- Based on Patient Experience Assessment)

  • Objectives: The purpose of this study is to evaluate patient experience assessment of inpatients, and to prepare measures to improve the quality level of medical services and guarante patient rights. Methods: The study was conducted among 199 patients admitted to hospitals and general hospitals in the metropolitan area. The analysis method used was crossover analysis, including a comparison of means, and logistic regression analysis. Results: The overall average score of satisfaction with healthcare service was 3.39 for nurses, 3.35 for hospitals and 3.42 for general hospitals. Age at the time of hospitalization affected satisfaction. The overall average score of healthcare service satisfaction was 3.09 for doctors, 3.14 for hospitals, and 3.04 for general hospitals. The factors affecting hospital satisfaction were gender and subjective health status. The factors affecting satisfaction in general hospitals were education, medical department, and hospitalization route. Conclusions: Hospitals should also introduce a systematic management system of general hospitals and strengthen the guarantee of the rights of patients who can improve the quality of medical care through positive communication between medical personnel and patients.

Nurses' Safety in the Hospital Environment: Evolutionary Concept Analysis

  • Purpose: The purpose of this study was to analyze the concept of nurses' safety within the context of the hospital environment. Methods: We used Rodgers' method of evolutionary concept analysis and reviewed the relevant literature and noted and categorized characteristics that appeared frequently. Results: Nurses' safety was defined as safe status and safe activity among nurses. Three key defining attributes were identified as: (a) the minimization of actual or perceived risk, (b) personal duties and rights, and (c) ensuring within a safe working environment. Antecedents of nurses' safety were categorized into three dimensions: (a) individual, including vigilance and knowledge gained through education and training; (b) institutional, including safety provision in the organizations; and (c) national, including legislation. The outcomes of nurses' safety included the following: (a) continued competence in nurses' work; (b) enhancement of the quality of patient care; and (c) reductions in nurses' turnover rates. Conclusion: Nurses' safety ensures that qualified nurses are able to continue to perform their duties and provide good patient care. The findings of this study could contribute to future research examining nurses' safety. In addition, appropriate tools must be developed to measure the concept.

국내외 뇌사, 존엄사와 안락사에 대한 인식의 변화와 윤리 - 소아를 중심으로 (The change of perspective on brain death, euthanasia and withdrawal of the life supporting medical treatments in Korea for pediatric patients)

  • A recent High Court's decision regarding the withdrawal of life supporting medical treatment (artificial ventilator) from an elderly female patient in the terminal stage has opened up a new era of the "euthanasia dispute" in Korea. With this decision, the legitimate withdrawal of life supporting treatment became possible under certain conditions and the Korean Medical Association is working toward the establishment of practical guidelines for the terminal-stage patients. However, there are still very few debates on the cases of pediatric patients in the terminal stage or suffering from fatal diseases. For pediatric patients, the core principle of autonomy and following procedure of "advance directives" are hardly kept due to the immaturity of the patients themselves. Decisions for their lives usually are in the hands of the parents, which may often bring out tragic disputes around "child abuse", especially in Korea where parents have exclusive control of the destiny of their children. Some developed countries such as the U.S.A., the U.K. and Canada have already established guidelines or a legal framework for ensuring the rights of the healthcare system regarding children suffering from severe illness, permitting the withdrawal of Life supporting medical treatment (LSMT) in very specific conditions when the quality of life of the children is severely threatened. For the protection of the welfare and interest of the children, we should discuss this issue and develop guidelines for the daily practice of pediatricians.

진료지원인력의 확대된 업무 수행을 위한 합법적이고 합리적인 해결 방안 (Legal and Practical Solutions for the Expanding the Roles of Medical Support Staff Nurses)

  • Purpose: Medical support staff nurses have traditionally performed various supportive tasks for physicians, often extending beyond standard nursing roles. Despite these long-standing practices, there is a notable lack of official recognition and legal protection for these expanded responsibilities, leading to increasing legal concerns. Therefore, there is a need for proposing a rational solution to address these issues. Methods: The number of medical support staff nurses is rising, particularly as they fill gaps left by the 2024 resident physician strike. The study focuses on identifying potential challenges arising from this shift and developing strategic improvements to address these challenges effectively. Results: This study proposed legally expanding the scope of nursing duties and creating a robust system for training and certifying nurses to handle these responsibilities effectively, by integrating these roles within the advanced practice nurse (APN) framework. Conclusion: Integrating these roles within the framework of APN can offer a sustainable and legally sound solution to the ongoing healthcare crisis, ensuring patient safety and safeguarding healthcare workers' legal rights.

의료행위 기준에 따른 치과위생사 직무 타당도 평가 (Evaluation of Dental Hygienist Job Validity according to Judgment Standard of Medical Practice in Medical Law)

  • 본 연구는 치과위생사 직무를 다양한 각도에서 분석하고 검토하여 향후 치과 팀 내 치과진료업무를 효율적으로 분담하고 직무에 따른 교육 과정을 개발하는 데 기초 자료로 활용하기 위하여, 의료법에서 의료행위를 판단하는 기준에 따라 치과위생사 직무의 타당도를 평가하고자 하였다. 본 연구는 2017년 11월 10일부터 20일까지 강릉원주대학교 치과대학의 12명 교수를 대상으로 치과위생사 직무의 타당성을 평가하기 위해 자기기입 설문조사를 실시하였다. 치과위생사 직무가 질병예방과 치료, 환자요양지도, 보건 위생상 위해 발생 여부의 의료행위 판단기준에 해당하는지에 대한 전문가 일치율을 산출하였고, 각 행위를 의료행위 타당성 평가기준에 따라 점수화하여 level 1~4로 최종 분류하였다. 본 연구 결과에 따르면, 응답자의 50% 이상이 치위생 관리에 포함하는 치은출혈, 치주낭, 임상적 부착수준 측정 및 기록과 전문가 치면세균막 관리, 스케일링, 칫솔질 및 구강 관리용품 처방, 교육을 포함한 대상자별 구강보건교육과 치료 후 주의사항에 대한 상담이 세 가지의 의료행위 판단기준에 모두 해당된다고 응답하였다. 치과위생사가 치과임상에서 수행하는 행위는 의료행위 판단 기준에 따라 크게 4가지 범주로 분류하였고, 범주의 수준이 높을수록 수행난이도가 높고, 전문지식과 기술이 요구되는 직무로 판단할 수 있다. 치은출혈, 임상적 부착수준, 치주낭 측정 및 기록과 치면 연마, 전문가 치면세균막 관리, 스케일링, 치근활택술, 국소적 항균제 적용의 항목은 최종 점수 4.3으로 수행난이도와 전문성이 요구되는 Level 4 그룹으로 분류되었다. 우리나라 치과진료현장에서 환자의 안전과 건강권을 보장하면서 효율적으로 진료를 분담하기 위해서는 수행 행위에 따라 필요한 지식의 수준과 적절한 교육, 자격 기준 등에 대한 표준화된 지침이 개발되어 활용될 필요가 있다.
