Cyclic fatigue resistance of M-Pro and RaCe Ni-Ti rotary endodontic instruments in artificial curved canals: a comparative in vitro study

  • Objectives: To compare the flexural cyclic fatigue resistance and the length of the fractured segments (FLs) of recently introduced M-Pro rotary files with that of RaCe rotary files in curved canals and to evaluate the fracture surface by scanning electron microscopy (SEM). Materials and Methods: Thirty-six endodontic files with the same tip size and taper (size 25, 0.06 taper) were used. The samples were classified into 2 groups (n = 18): the M-Pro group (M-Pro IMD) and the RaCe group (FKG). A custom-made simulated canal model was fabricated to evaluate the total number of cycles to failure and the FL. SEM was used to examine the fracture surfaces of the fragmented segments. The data were statistically analyzed and comparisons between the 2 groups for normally distributed numerical variables were carried out using the independent Student's t-test. A p value less than 0.05 was considered to indicate statistical significance. Results: The M-Pro group showed significantly higher resistance to flexural cyclic fatigue than the RaCe group (p < 0.05), but there was no significant difference in the FLs between the 2 groups (p ≥ 0.05). Conclusions: Thermal treatment of nickel-titanium instruments can improve the flexural cyclic fatigue resistance of rotary endodontic files, and the M-Pro rotary system seems to be a promising rotary endodontic file.

근관치료용 전기모터의 회전 안정성 평가 (AN EVALUATION OF ROTATIONAL STABILITY IN ENDODONTIC ELECTRONIC MOTORS)

  • 수종의 근관치료용 전기모터들의 반복사용과 시간에 따른 회전속도의 변화를 비교하여 근관치료용 전기모터의 회전 안정성을 평가하고자 하였다. 시간에 따라 변화하는 실제회전속도 및 전압과 전류를 측정하고, Student t-test를 통한 실제회전속도와, 반복측정분석을 통한 상관관계를 비교분석 하였다. 동일 종류의 전기모터들 사이의 모든 측정 결과들은 차이를 보이지 않았다. 모든 전기모터에서 반복사용과 시간의 흐름에 따른 회전속도의 변화는 차이를 보이지 않았다. 이상의 결과로, 본 실험에 사용된 모든 전기모터는 일정한 회전속도를 유지해야 하는 회전 안정성의 조건에 충족됨을 알 수 있으며, 반복사용과 시간에 따른 신뢰성 있는 회전속도와 안정적인 전압 및 전류의 유지가 가능하리라 생각된다.

중합효소연쇄반응법을 이용한 급성 치수 및 치근단 질환의 병원성 세균의 동정 (IDENTIFICATION OF PUTATIVE PATHOGENS IN ACUTE ENDODONTIC INFECTIONS BY PCR BASED ON 16S rDNA)

  • The purpose of this study was to investigate the frequency of 7 putative pathogens in endodontic infections. The specimens were collected from infected pulpal tissue of patients who were referred for root canal treatment to the department of conservative dentistry, Chosun University Samples were collected aseptically using a barbed broach and a paper point. The cut barbed broaches and paper points were transferred to an eppendorf tube containing 500 ml of 1 X PBS. DNAs were extracted from the samples by direct DNA extraction method using lysis buffer (0.5% EDTA, 1% Triton X-100). Identification of 7 putative pathogens was performed by PCR based on 16S rDNA. The target species were as follows : Porphyromonas endodontalis, Porphyromonas gingivalis, Prevotella intermedia, Prevotella nigrescens, Bacteroides forsythus, Actinobacillus actinomycetemcomitans, and Treponema denticola. Our data revealed that the prevalence of P. endodontalis was found in 88.6% (39/54), P. ginivalis 52.3% (23/44), P. nigrescens 18.2% (8/44), P intermedia 15.9% (7/44) B. forsythus 18.2% (8/44), A. actinomycetemcomitans 3.3% (1/44), T. denticola 25% (l1/44) of the samples. The high prevalence of P. endodontalis and P. ginivalis suggests that they may play an important role in the etiology of endodontic infections.

성견 하악의 치주-치근단 실험적 복합병소에서 골조직 재생에 관한 연구 (BONE REGENERATION OF THE EXPERIMENTAL ENDODONTIC-PERIODONTIC COMBINED DEFECTS IN THE MANDIBLES OF THE DOGS)

  • The endodontic-periodontic combined lesions have been difficult to get correct diagnosis and predictable treatment. This study was to make the experimental endodontic-periodontic combined defects in dogs for the study of the periodontal regeneration and to evaluate the efficacy of the enamel matrix protein and e-PTFE membrane in the experimental endodontic-periodontic combined defects. 5 mongrel dogs were used. The pulp chambers were opened and the plaque was inserted into the chambers to induce the periapical lesions on the mandibular second, third and fourth premolars of the dogs. 1 month later, the root canal treatments were done with gutta perch a and ZOE sealer. On the day of surgery, the periapical defects were standardized by trephine bur. The buccal dehiscence defects were made by the dental bur and bone chisels. The apicoectomy with retrofilling was done. The prepared roots were randomly selected for test and control groups. In the experimental groups, the enamel matrix derivative and e-PTFE membrane were used. Nothing was placed on the control group. Fluroscent labelling was used to evaluate the bone formation. After 4 and 12 weeks, the dogs were sacrificed and undecalcified sections were prepared and stained with toluidine blue. Those histologic sections were examined by fluorescent microscopy and light microscopy. The results were as follows. 1. In the control group, new bone was formed in the periapical defects and scarcely in the buccal dehiscence defects. New cementum was not detected at 4 and 12 weeks. 2. In the experimental groups, new bone, new cementum and periodontal ligament were found in the periapical and buccal dehiscence defects. The relative amount and the quality of the new bone, new cementum and periodontal ligament tissue that had formed on the experimental groups were superior to those of the control group. 3. The current observation implicated that e-PTFE membrane and enamel matrix protein could be the effective tools for the guided tissue regeneration of the endo-perio combined defects.

치근단절제술에 대한 환자의 인식과 만족도 조사 (Patients' perception and satisfaction with apicoectomy)

  • 연구목적: 본 연구는 치근단절제술과 역충전을 시행한 환자를 대상으로 치근단미세수술의 결과에 대한 환자의 인식과 만족도를 조사하는 것을 목적으로 하였다. 연구 재료 및 방법: 연세대학교 치과보존과에서 치근단 수술 후 최소 3개월 이후 정기검진을 위해 내원한 환자를 대상으로 설문조사를 시행하였다. 응답자의 나이/성별간 응답의 차이가 있는지는 contingency table을 이용해서 분석하였다 (p=0.05). 결과: 응답자의 60% 정도가 이전에 치근단 수술에 대해서 들어본 적이 없었으며 치근단 수술을 결정한 이유는 63.3%에서 자연치를 보존하기 위해서라고 응답하였다. 이후 비슷한 상황이 생긴다면 대부분의 환자가 다시 치근단수술을 선택할 것이라고 하였으며 수술의 만족도는 전반적으로 높은 편이었다. 결론: 치근단수술은 대부분의 환자에서 만족스러운 치료로 인식되고 있다.

Surgical management of a failed internal root resorption treatment: a histological and clinical

  • This article presents the successful surgical management of a failed mineral trioxide aggregate (MTA) orthograde obturation of a tooth with a history of impact trauma and perforated internal root resorption. A symptomatic maxillary lateral incisor with a history of perforation due to internal root resorption and nonsurgical repair using MTA was referred. Unintentional overfill of the defect with MTA had occurred 4 yr before the initial visit. The excess MTA had since disappeared, and a radiolucent lesion adjacent to the perforation site was evident radiographically. Surgical endodontic retreatment was performed using calcium enriched mixture (CEM) cement as a repair material. Histological examination of the lesion revealed granulation tissue with chronic inflammation, and small fragments of MTA encapsulated within fibroconnective tissue. At the one and two year follow up exams, all signs and symptoms of disease had resolved and the tooth was functional. Complete radiographic healing of the lesion was observed two years after the initial visit. This case report illustrates how the selection of an appropriate approach to treatment of a perforation can affect the long term prognosis of a tooth. In addition, extrusion of MTA into a periradicular lesion should be avoided.

상악 측절치 구개치은발육구의 치료 (Treatment of a lateral incisor anatomically complicated with palatogingival groove)

  • 구개치은발육구는 상악 전치에 나타나는 이상 치근 형태의 하나로 구개측 결절부에서 구개치은발육구를 관찰하였으며, 이 구는 치근단을 향하여 여러 깊이와 길이로 연장될 수 있다. 구개치은발육구는 세정되기 어렵고 청결이 불가능하여, 치태와 치석 등이 축적되어 접합상피의 파괴를 일으키고 빠르게 골연하 결손부가 발생할 수 있다. 또한 이차적으로 치수 조직을 이환시켜 근관-치주 병소가 발생할 수 있다. 본 증례 보고는 구개치은발육구로인해 근관 문제까지 야기된 임상증례를 보고하고 근관치료와 함께 여러가지 재료를 이용한 구개치은발육구의 치료결과를 관찰함으로써 이러한 치료방법의 효용성을 평가하고자 하였다.

치근단 병소에서 면역적격세포의 분포에 관한 연구 (A QUANTITATIVE ANALYSIS OF THE IMMUNOCOMPETENT CELLS IN PERIAPICAL LESIONS OF THE HUMAN TEETH)

  • Periapical lesions are developed as a result of inflammatory response to irritants from root canal system. Clinicians remove these irritants from root canal system and seal the root canal space to induce healing of the periapical lesions. Immunopathologic responses may play an important role in development and progression of periapical lesions and periapical lesions contain immunocompetent cells. The purposes of the present study were to analys and to compare the distribution of the immunocompetent cells in the human periapical lesions according to the stage of endodontic treatment using indirect immunoperoxdase technique. Obtained 94 human periapical lesions were devided into four groups: Group 1 : no endodontic treatment(28 samples) Group 2 : root canal enlarged and irrigated(28 samples) Group 3 : root canal filled(29 samples) Group 4: unknown(9 samples) Monoclonal antibodies to examine target cells were UCHL-1 for T lymphocytes(1 : 200, Dakopatt, Denmark), L26 for B lymphocytes(1 : 200, Dakopatt, Denmark), OPD4 for helper T lymphocytes(l : 200, Dakopatt, Denmark) and alpha-1-antichymotrypsin for macrophages(l : 2000, Dakopatt, Denmark). The following results were obtained : 1. All the periapical lesions studied were infiltrated by T lymphocytes, plasma cells, B lymphocytes, and macrophages. T lymphocytes were more infiltrated than B lymphocytes, and B lymphocytes and macrophages were less infiltrated than T lymphocytes and plasma cells(P<0.05 : Oneway ANOVA test). 2. In untreated group and canal irrigated and enlarged group of all the periapical lesions, helper T lymphocytes were predominently infiltrated(P>0.05 : Oneway ANOVA test). 3. In canal filled groups of all lesions except periapical cyst, plasma cells were predominently infiltrated. But, in canal filled group of periapical cyst, helper T lymphocytes were the predominent cells(P>0.05 : Oneway ANOVA test). The above results shows that the immunologic responses play important role in pathogenesis of periapical lesions and the immunologic response involved undergoes certain changes after endodontic therapy.

한국 소아치과의사의 치수치료 술식에 대한 실태조사 (Status and Survey of Pulp Treatment by Korean Pediatric Dentists)

  • 유치 및 미성숙영구치의 치수치료는 소아 및 청소년 영역에서 빈번하게 행하지는 치료이며, 임상 양상과 치수 진단 뿐만 아니라 환아 및 보호자, 술자의 환경에 영향을 받는다. 이 연구의 목적은 실제 임상에서 행해지는 치수치료의 현황에 대한 조사와 더불어 치료방법을 선택하게 된 배경에 대해 조사하는 것이다. 이 연구는 소아치과학회 회원을 대상으로 총 153명의 설문조사 결과지를 바탕으로 시행되었으며, 2010년부터 2018년까지의 건강보험심사평가원의 통계자료를 함께 분석하였다. 설문조사 결과와 건강보험심사평가원의 통계자료 모두 유치의 1-visit pulpectomy가 가장 활발하게 시행되고 있는 술식으로 나타났으며, 유치열기 또는 혼합치열기 어린이에서 Ni-Ti file의 사용 빈도가 증가하였다. 미성숙영구치의 치료에서는 개인치과의원에서 치수재생치료의 시행 빈도가 대학병원보다 낮은 것으로 나타났다.
