내분비요법 중인 폐경 전 유방암 환자의 디스트레스 영향 요인 (Factors Influencing Distress in Premenopausal Patients with Breast Cancer Undergoing Endocrine Therapy)

  • Purpose: The purpose of this study was to investigate relationships among menopausal symptoms, functional status, and distress and to identify factors influencing distress in premenopausal breast cancer patients who had been on endocrine therapy. Methods: A descriptive correlational study was conducted. Data were collected using questionnaires from 140 patients with breast cancer undergoing endocrine therapy at a general hospital. Data were analyzed using descriptive statistics, t-test, ANOVA, Tukey HSD test, Pearson's correlation analysis and hierarchical regression analysis. Results: The mean scores for menopausal symptom, functional status, and distress were $19.65{\pm}7.86$, $2.67{\pm}0.33$ and $3.69{\pm}2.19$, respectively. The menopausal symptoms and distress were positively correlated (r=.76, p<.001). The menopausal symptoms and functional status (r=-.43, p<.001) and functional status and distress (r=-.31, p<.001) were negatively correlated. The most influential factor for distress was menopausal symptoms (${\beta}=.79$, p<.001). Conclusion: Based on the findings of this study, developing nursing intervention programs focusing on decreasing menopausal symptoms and distress are recommended.

심리측정 패러다임을 이용한 대구 지역 주부들의 식품 위해요소에 대한 위험 지각 수준 및 우려도에 영향을 미치는 요인 분석 (Analysis of Levels of Risk Perception using Psychometric Paradigm and Factors Affecting Concerns about Food Risk Elements of Housewives in Daegu)

  • This study examined the levels of risk perception of food risk elements by adopting a psychometric paradigm and analyzed factors affecting concerns about food risk elements to obtain basic materials for food safety policy. The data were collected from 296 housewives in Daegu, Korea, by a self-administered questionnaire. Frequency distributions, Pearson's correlation coefficient analysis, factor analysis, Cronbach's ${alpha}$, and multiple regression analyses were conducted by SPSS 21.0. The mean level of concern for food safety was 3.75/5.00 points, and the mean percentage of correct answers about heavy metal contamination was highest among food risk elements. The respondents perceived radioactive contaminated foods, GM foods, and endocrine disruptors as a new, delayed, scientifically unknown, involuntary, serious, and uncontrollable risk in risk perception. According to the result of factor analysis for risk perception, two factors such as non-controllability and dread were categorized. In the risk perception map, radioactive contaminated foods and GM foods were considered as an uncontrollable and dreaded risk, heavy metal contamination, endocrine disruptors, and pesticide residues as a controllable and dreaded risk, and foodborne illness and food additives as a controllable and less dreaded risk. On the other hand, the levels of concerns about food risk elements were higher in order of radioactive contaminated foods, GM foods, and endocrine disruptors. The results of multiple regression analysis showed that age, concern for food safety, percentage of correct answers about food risk elements, non-controllability, and dread influenced the concerns about food risk elements. These results imply that food safety policies should consider differences in consumer's risk perception of food risk elements.

  • Growth hormone (GH) treatment has been increasingly widely used for children with GH deficiencies as the survival rate of pediatric patients with malignancies has increased. Both GH and insulin-like growth factor-I have mitogenic and antiapoptotic activity, prompting concern that GH treatment may be associated with tumor development. In this review, the authors examined the relationship between GH treatment and cancer risk in terms of de novo malignancy, recurrence, and secondary neoplasm. Although the results from numerous studies were not entirely consistent, this review of various clinical and epidemiological studies demonstrated that there is no clear evidence of a causal relationship between GH treatment and tumor development. Nonetheless, a small number of studies reported that childhood cancer survivors who receive GH treatment have a small increased risk of developing de novo cancer and secondary malignant neoplasm. Therefore, regular follow-ups and careful examination for development of cancer should be required in children who receive GH treatment. Continued surveillance for an extended period is essential for monitoring long-term safety.

Intensity-modulated Radiotherapy Combined with Endocrine Therapy for Intermediate and Advanced Prostate Cancer: Long-term Outcome of Chinese Patients

  • Aim: The aim of this study was to evaluate acute adverse events and efficacy of three-dimensional intensitymodulated radiotherapy (IMRT) combined with endocrine therapy for intermediate and advanced prostate cancer. Methods: Sixty-seven patients were treated with three-dimensional IMRT combined with maximum androgen blockade. The correlation between radiation-induced rectal injury and clinical factors was further analyzed. Results: After treatment, 21 patients had complete remission (CR), 37 had partial remission (PR), and nine had stable disease (SD), with an overall response rate of 86.5%. The follow-up period ranged from 12.5 to 99.6 months. Thirty-nine patients had a follow-up time of ${\geq}$ five years. In this group, three-year and five-year overall survival rates were 89% and 89.5%, respectively; three-year and five-year progression-free survival rates were 72% and 63%. In univariate analyses, gross tumor volume was found to be prognostic for survival ($X^2$ = 5.70, P = 0.037). Rates of leucopenia and anemia were 91.1% and 89.5%, respectively. Two patients developed acute liver injury, and a majority of patients developed acute radiation proctitis and cystitis, mainly grade 1/2. Tumor volume before treatment was the only prognostic factor influencing the severity of acute radiation proctitis (P < 0.05). Conclusions: IMRT combined with endocrine therapy demonstrated promising efficacy and was well tolerated in patients with intermediate and advanced prostate cancer.

  • The relation of endocrine in the western medicine and zangfu-organ functions in Korean medicine, related to growth disturbance, was studied and derived the following results. 1. The hormone most related to growth disturbance is, directly secreted from the anterior pituitary or is stimulated and secreted in the target grands, growth hormone, thyroid, adrenocortical hormone, gonadial hormone and is insulin secreted from $\beta$ cell of langerhans' slands of pancreas. 2. the pituitary has the most close relation with the kidney in the five zang-organ. Because the kidney is innate origin(先天之本) and promotes qi and blood(生化氣血), stores the essence of life(藏精), dominates the bones(主骨) and promotes the marrow(生骨髓). Especially it is connected with brain(通於腦). 3. In the children growth, the endocrine action in the pituitary has the most close relation with the kidney, As in the reports of the brain and spinal cord, bone, store essence of life, sexual maturation and decline(kidney-qi, sexual functions of both sexes(天癸)) etc, and cause of cretinism, dwarf in the main subject. 4. Somatomedin is the most important factor of the growth factors, IGF in another word. The unification of IGF and secretion is controlled firstly according to growth hormone, however is very closely related to the nutrition status in the non-hormonic causes. Also, it is affected very much by the insulin. 5. Insulin is one of the important hormone related to the growth and is secreted from the pancreas. Pancreas belongs to the functional system of spleen in oriental medicine, thus the growth disturbance, occurred due to error in insulin secretion and nutrition(in another words, the lack of postnatal essential substance from food-stuff(水穀精氣)), is closely related to the spleen. 6. From the results driven above, the hormone action of endocrine and problem in secretion, related to the growth disturbance, must be focused on the differentiation of symptoms and signs of the kidney and the spleen in oriental medicine.

  • The somatotropic axis plays a key role in proliferation and differentiation of avian organs during both pre- and posthatching periods. This review discusses the complexity of regulation of the endocrine system for chicken development and growth by growth hormone (GH), insulin-like growth factor (IGF), and IGF binding protein (IGFBP). In addition, the thyrotropic axis, including thyrotropin-releasing hormone (TRH) and thyroid hormones ($T_4$ and $T_3$), is also involved in the GH-secreting pattern. In mammals, IGFI and -II are always sequestered in a 150 kDa non-covalent ternary complex. This complex consists of one molecule each of IGF-I or IGF-II, IGFBP-3 or IGFBP-5 and an acid labile subunit (ALS). Chick ALS is identified in different strains for the first time, and further investigation of the expression of ALS on developmental stage and ALS effect on IGF bioavailability may be addressed in the future.

  • Bisphenol A(BPA) is a monomer widely used in the manufacturing polycarbonate plastic or epoxy resin and Mancozeb (MCZ), a polymeric complex of zinc and manganese salts of ethylene bisthiocarbamate, is widely used in agriculture as fungicide, insecticide and herbcide. These chemicals have been recently known as endocrine disruptors. To investigate the effects of BPA and MCZ on LPS-induced cytokine production (TNF-${\alpha}$) in vivo, female ICR mice were administered to various concentration of these materials (BPA; 100, 500, 1000 mg/kg/day, and MCZ; 250, 500, 1000, 1500 mg/kg/day) for 30 days and serum cytokine levels were measured at 1h post LPS injection (day 32) in BPA- or MCZ-administered mice.   • Seborrheic dermatitis is acute or chronic inflammatory disease frequently caused by heredtary factor, endocrine abnormality, alcohol, etc. and characterized by red spots with clear boundary itching. It is similar to Myunyoupung in Oriental Medicine. I prescribed the Seungmahwangryun-tang(升麻黃連湯) and Zowiseunggi-tang(調胃承氣湯) to a patient with Seborrheic dermatitis. After taking above medicines. the patient was cured.

Toxicogenomic Analysis and Identification of Estrogen Responsive Genes of Di (n-ethylhexyl) Phthalate in MCF-7 Cells

  • Di (n-ethylhexyl) phthalate (DEHP) is thought to mimic estrogens in their action, and are called endocrine disrupting chemicals. DEHP is used in numerous consumer products, especially those made of flexible polyvinyl chloride and have been reported to be weakly estrogenic. In this study, DEHP were tested for estrogenic properties in vitro models and with microarray analysis. First, the E-screen assay was used to measure the proliferation of DEHP in MCF-7 cells, a human breast cancer cell line. DEHP induced an increase in MCF-7 cell proliferation at concentration of $10^{-4}M$. Second, we carried out a microarray analysis of MCF-7 cells treated with DEHP using human c-DNA microarray including 401 endocrine system related genes. Of the genes analyzed, 60 genes were identified showing significant changes in gene expression resulting from DEHP. Especially, 4 genes were repressed and 4 genes were induced by DEHP compared to $17{\beta}-estradiol$. Among these genes, trefoil factor 3 (intestinal), breast cancer 1, early onset and CYP1B1 are involved in estrogen metabolism and regulation. Therefore it suggests that these genes may be associated with estrogenic effect of the DEHP on transcriptional level. The rationale is that, as gene expression is a sensitive endpoint, alterations of these genes may act as useful biomarkers to define more precisely the nature and level of exposure to kinds of phthalates.

Effects of Postnatal Exposure to Octylphenol on the Transcriptions of Steroidogenic Enzymes in Mouse Testis

  • The effects of postnatal exposure to octylphenol(OP) on the expressions of the steroidogenic enzymes and testosterone production were evaluated. Postnatal male mice (15-day-old) were injected with 2 or 20mg $kg^{-l}$ body weight (BW) of OP for 5 days and sacrificed on postnatal day 21. Testosterone concentration was measured by radioimmunoassay and the expressions of the testicular genes were determined by RT-PCR analyses. Significant reductions in the mean body and testis weight were observed in the OP treated animals. No marked alteration in the histological structure of the testis were observed, however, slight reduction in the seminiferous tubule diameter and the number of Leydig cells and several pyknotic cells could be identified in the 20 mg $kg^{-l}$ BW of the OP treated animals. Serum testosterone concentration was dramatically reduced and the mRNA expressions of the steroidogenic acute regulatory protein (StAR), cholesterol side-chain cleavage enzyme (P450scc) and $17\beta$-hydroxylase/Cl7-20 lyase $(P450_{17\alpha})$ were decreased. No significant changes of the gene expressions of the steroidogenic factor-l (SF-I) and estrogen and androgen receptor after the OP treatment showed that the decreased expressions of the steroidogenic enzymes in the present study did not correlate with these genes. Altogether, the present study demonstrates that postnatal treatment of OP inhibits steroidogenesis by decreasing the transcriptional expressions of the StAR and steroidogenic enzymes. The alteration in steroidogenesis may adversely affect the normal development of the testis and sper- matogenesis.
