IN VITRO COMPARISON OF VARIOUS DIAGNOSTIC METHODS OF OCCLUSAL CAR10US LESIONS (교합면 우식병소의 다양한 진단법에 관한 비교연구)

  • The aims of this study were to compare the accuracy, sensitivity and specificity of cnventional visual examination, radiography and a new laser fluorescence method, KaVo Diagnodent, for the detection of occlusal caries lesions. One hundred sound human premolars and molars which had no restorations or interproximal cavities were tested by three methods. Tooth lesions depth was assessed at histologic examination using Caries detector dye The following results were obtained. 1. Diagnodent show 7.8 in sound tooth, 25.4 in initial caries, 30.5 in enamel caries, and 53.8 in dentin caries with average score 2. Spearman and Pearson relation coefficient was high between tooth-specimen test with dye and Diagnodent(0.736, 0.619), visual examination(0.664, 0.666), and was low between tooth-specimen test with dye and radiographic examination(P<0.01, total) 3. Accuracy of occlusal caries was highest on Diagnodent(65%) and lowest on radiographic examination(35%) 4. In initial caries, the sensitivity and specificity of Diagnodent method was the highest. In enamel caries, the sensitivity of visual examination was the highest and specificity of Diagnodent method was the highest. In dentinal caries, the sensitivity and specificity of Diagnodent method was the highest and sensitivity of visual examination was the lowest.

THE EFFECTS OF NANO-SIZED HYDROXYAPATITE ON DEMINERALIZATION RESISTANCE AND BONDING STRENGTH IN LIGHT-CURED GLASS IONOMER DENTAL CEMENT (광중합형 글라스아이오노머 시멘트의 탈회 저항성과 결합 강도에 대한 나노미터 입자의 하이드록시아파타이트의 효과)

  • The aim of this study was to evaluate the effect of incorporated nano HA on the demineralization resistance and bonding strength of LC GIC in comparison with micro HA. Fuji II LC GIC was used as the control group and a base material for experimental groups. Two experimental groups were prepared. One was prepared by adding 15% micro HA to LC GIC by weight ratio (Exp. 1), and the other was prepared by adding 15% nano HA instead (Exp. 2). According to the results, the following conclusions could be obtained. 1. Observing under the CLSM, the control group showed thicker enamel demineralization layer than in the experimental groups, and the Exp. 2 group showed the thinnest demineralization layer. 2. In SEM analysis, there was greater enamel demineralization in the control group. The Exp. 2 group was more resistant to demineralization compared to the Exp. 1 group. 3. The bonding strength was found to be in the increasing order of control, Exp. 1, and Exp. 2 group (p < 0.05). 4. Observing the fractured surfaces under SEM after the bonding strength test was performed, there were bone-like apatite particles formed in HA-added experimental groups, and a greater number of bone-like apatite particles were formed in the Exp. 2 group compared to the Exp. 1 group.

INFLUENCE OF THE COLOR OF COMPOSITE RESINS APPLIED TO LINGUAL SURFACE ON THE LABIAL TOOTH COLOR (설측 복합레진 색상이 치아 순측 색상에 미치는 영향)

  • In this study we evaluated the influence of both the thickness of residual enamel and the color of the composite resins applied to lingual surface on the labial surface color. Background plates were made by randomly (A1, A2, A6D, B1, B2, B3, C1, C2, C6D) selected colors of Filtek Supreme (3M ESPE, St. Paul, U.S.A.) composite resin. Crown portion of 9 maxillary central incisors were cut off and embedded with acrylic resin except labial surface. Samples of average thickness of 2.2 mm were obtained after cutting it in a thickness of 2.5 mm from the labial surface and sandpaper polish. The shade of composite resin background was measured using Spectrophotometer ($Spectrolino^{(R)}$, GretagMacbeth, Regensdorf, Switzerland). And CIE $L^{\ast}a^{\ast}b^{\ast}$value of 2.2 mm thickness tooth samples were measured on the 9 composite resin backgrounds. And then, the cutting side of tooth samples was ground to the extent of 1.9 mm, 1.6 mm, 1.3 mm, 1.0 mm and placed on composite resin backgrounds and measured $L^{\ast}a^{\ast}b^{\ast}$values with the same method. In all samples, $L^{\ast}$value and $b^{\ast}$value seemed to have a tendency of decreasing as thickness of tooth sample becomes thinner regardless of background colors (p < 0.05). But, $a^{\ast}$value didn't show the significant differences depending on the thickness.

Effect of storage condition of resin cement on shear bond strength of the orthodontic bracket (레진시멘트의 보관 조건이 치열교정용 브라켓의 전단접착강도에 미치는 영향)

  • Purpose: For orthodontic bracket bonding, light curing resin cement is widely used because the process is convenient, and it can be polymerized at the desired time. This study compared the difference of bonding strength of orthodontic resin cement according to storage condition. Materials and Methods: After acid etching the bovine enamel surface with 37% phosphoric acid, 15 orthodontic brackets for mandible incisors were bonded with Ortho Connect and Orthomite LC according to following three conditions; 1) Immediate after 4℃ refrigeration for 3 months (IR), 2) One day room temperature after 4℃ refrigeration for 3 months (OR), 3) Room temperature for 3 months (RT). The shear bond strength was measured with a universal material tester and failure pattern of the specimen was observed. Two-way ANOVA and One-way ANOVA were used at the 95% significance level. Results: Ortho Connect that was applied immediately after refrigeration showed the maximum shear bond strength. Orthomite that was applied immediately after refrigeration showed the lowest shear bond strength, and the group stored at room temperature for three months showed the highest shear bond strength, and the difference between the two groups was significant. Conclusion: Ortho Connect can be used without worrying about bond strength even if it is used immediately after refrigeration, but Orthomite should be kept at room temperature sufficiently after refrigeration.

Full-mouth rehabilitation with CAD/CAM monolithic zirconia in dentinogenesis imperfecta: a case report (상아질 형성 부전증 환자에서 CAD/CAM 단일 구조 지르코니아를 이용한 완전구강회복 증례)

  • Dentinogenesis Imperfecta, with a high incidence rate of 1 : 6 - 8000, is inherited by autosomal dominant genetic transmission. This dental disorder causes discoloration of the teeth and the enamel and dentin show hypoplastic or hypocalcified defects which lead to frequent fractures and rapid attrition. Therefore, timely treatment is necessary for the preservation of the remaining teeth. In this particular case, a 19-year-old patient suffering from Type 1 dentinogenesis imperfecta showed signs of brownish hued teeth with multiple fractures, a loss of vertical dimension, excessive interdental space in the maxillary anterior teeth, and a lack of 5 posterior teeth. To improve the esthetic appearance of the anterior teeth, the vertical dimension was increased. Resin caps were used to alleviate the difficulty of taking an impression of multiple teeth at once. Monolithic zirconia materials used in this case showed high fracture strength and the ability to mask the discoloration of the teeth and therefore, functionally and esthetically favorable results were achieved.

A Study on thinner's Physicochemical property and its effect on genital organ of rat (시너(thinner)의 물리화학적 특성과 랫트의 생식기에 미치는 영향 연구)

  • The aim of this study is to confirm the physicochemical property and hazard of thinner (012), which is a diluent of enamel paint used for floor coating for waterproofing and oil painting for the outer wall. The literatures of physicochemical property and hazard of thinner were surveyed and its physicochemical property were evaluated. And then, the inhalation toxicity of thinner affecting the central nervous system and reproductive organs in rats were examined by subchronic (6 h./day. 5 days/ week for 13 weeks) inhalation test. 1) According to the 13-week subchronic inhalation test, there were no significant changes in clinical test and body weight. However, a significant evidence of toxicity was observed in the hematological test and organ weight such as heart, kidney, liver and brain (p<0.01) in the 200 ppm and 1,000 ppm exposure groups in a dose response manner. In the histopathology analysis, there were no significant evidence of toxicity. Therefore, thinner was not classified as an organ targeted toxic agent. In case of Harmfulness, it could be classified as a chronic toxic agent 3($500 ppm/4hr, rat). 2) The reproductive toxicity such as extension of the period of estrous cycle, reduction of serum estradiol concentration and increase of frequency of the abnormal sperm was observed in the 1,000 ppm exposed animals. 3) The result of the physicochemical property of the test material showed that the specific gravity was 0.793, boiling point $155.8^{\circ}C$, steam pressure 2.1 kPa, ignition point $34.5^{\circ}C$, and spontaneous ignition point $280^{\circ}C$. The endothermic and exothermic values were 371.4 J/g and 159.1 J/g. respectively. The explosion limit was 214 mg/l. These data showed that thinner could be classified as an explosion agent level 1.2 and ignitive liquid agent 3 ($23-60^{\circ}C$) according to the notification No. 2008-1 of the Labor Ministry, "Classifying Standard of Chemical Materials."

The effects of salivary contamination on tensile bond strength of resin modified glass ionomer cements in bonding brackets (브라켓 접착시 타액 오염이 레진 강화형 글래스 아이오노머 시멘트의 인장 접착 강도에 미치는 영향)

  • The purposes of this study were to evaluate clinical applicability of resin modified glass ionomer cements and to determine the effect of salivary contamination on the tensile bond strength. Fourty extracted human permanent premolars were prepared lot bonding and standard edgewise brackets were bonded with Ortho-One, Fuji Ortho LC, Vitremer and Advance. Fourty extracted human permanent premolars were contaminated with saliva, dried and bonded with same materials above. The tensile bond strength was tested by Instron testing device aster storage in normal saline at ,$37^{\circ}C$ for 24 hours from bonding. The results were as follows : 1. The tensile bond strength of Ortho-One group was $7.68\pm1.76$, Advance group was $7.87\pm2.80$, Fuji Ortho LC group was $4.99\pm2.53$, Vitremer group was $2.80\pm0.88$ MPa. The tensile bond strength in contaminated condition of Ortho-One group was $4.12\pm1.67$, Advance group was $5.37\pm0.68$, Fuji Ortho LC group was $4.41\pm1.61$, Vitremer group was $2.60\pm1.10$ Mpa. 2. Salivary contamination did not affect the tensile bond strength when compared with the uncontaminated enamel group in Fuji Ortho LC and Vitremer (p>0.05) and there was great significant difference in the tensile bond strength of Ortho-One and Advance. 3. Advance, Ortho-One and Fuji Ortho LC seemed to have clinically a proper bond strength.

THE EFFECT OF LIGHT CURED GLASS IONOMER CEMENT ON THE SHEAR BOND STRENGTH OF ORTHODONTIC BRACKETS (광중합형 글래스 아이오노머 시멘트 교정용 브라켓의 전단결합강도에 미치는 영향)

  • The purpose of this study was to evaluate clinical applicability of light cured glass ionomer cement as a othodontic adhesive. The metal brackets and plastic brackets were bonded with light cured glass ionomer cement(Fuji Ortho $LS^{(R)}$) after polishing with a slurry of pumice, surface conditioning with 10% polyacrylic acid and chemically cured resin(Mono-$Lok2^{(R)}$) after acid etching with 38% phosphoric acid on the extracted human bicuspids. The shear bond strength was tested with a universal testing machine(HGS-100A, Shimadzu Co., Japan) after storage in normal saline at $37^{\circ}C$ or 24 hours and 48 hours. The results were as follows: 1. The shear bond strength of light cured glass ionomer cement group polished with a slurry of pumice was significantly lower than that of chemically cured resin group(P<0.01). 2. The shear bond strength of light cured glass ionomer cement group conditioned with 10% polyacrylic acid was significantly lower than that of chemically cured resin group(P<0.01). 3. The shear bond strength of light cued glass ionorner cement group conditioned with 10% polyacrylic acid was slightly higher than that of light cured glass ionomer cement group polished with a slurry of pumice, but there was no significant difference(P>0.05). 4. There was no significant difference between metal bracket group and plastic bracket group irrelevant off enamel conditioning(P>005). In summary, although the shear bond strength of light cured glass lionomer cement was lower than that of chemically cured resin, it night be clinically applicable.

Effects of Sodium Fluoride Exposure on the Stages of Amelogenesis and Ameloblast Modulation in Rat Incisors (흰쥐 절치의 법랑질형성과 법랑모세포 변환주기에 불소가 미치는 영향)

  • Effects of sodium fluoride exposure on the amelogenesis during fetal formation were investigated using 11 days rat incisor of control group and two experimental groups. According to results of morphological analysis using an electron microscope, enamel organ in the rat incisor consisted of presecretory, secretory, and maturation zone, especially maturation zone had ruffle-ended ameloblasts (rAB) that additionally supply inorganic ions and smooth-ended ameloblasts (sAB) that remove water and organic compounds. Such a histological composition was same in fetal and adult rats. According to experimental results using calcein (green fluorescence) in order to reveal the modulation cycle of ameloblast, modulation cycle of experimental group decreased on an average one time than control group, as increase of density of sodium fluoride indicated that thickness of smooth-ended ameloblast decreased. Also ratio of thickness on sagittal total length of sAB increased than rAB in experimental groups than control group. In total length of teeth, an injected 100 ppm sodium fluoride group was similar control group but as injected 200 ppm group became short. In experimental group, thickness of sAB and rAB became narrow to the tip of cutting edge. According to concentration of sodium fluoride grows, the modulation cycle and total length of teeth were decreased, finally it prevented teeth growth.

Evaluation of Caries Status among Adolescents in Jeonju City with WHO Basic Methods, International Caries Detection and Assessment System II (ICDAS-II) (WHO basic methods와 International Caries Detection and Assessment System II (ICDAS-II)를 사용한 전주시 청소년의 우식상태 조사)

  • Since it is favorable to include the incipient caries lesion in the diagnostic criteria in adolescence, this study had surveyed dental caries status of adolescents at ages of 13 and 16 by using WHO basic methods and ICDAS-II codes. In this study, mean DMFT index was 3.71, and mean DT index was 1.94. For both indices, the age 16 group showed higher values than the age 13 group. By groups of teeth, DMFT index and DT index exhibited highest to the lowest values in molar, premolar, and anterior teeth, respectively. 77.46% of total numbers of teeth were classified as code 0 in ICDAS-II. Compared to anterior teeth, numbers of decayed teeth were increased in posterior teeth. All caries lesions in anterior teeth and premolars were limited to enamel. ICDAS-II code is an useful method to detect the incipient caries lesion, allowing preventive control on caries management.
