The study of Health Care Utilization and Direct Medical Cost in the Diabetes Mellitus Client (당뇨병 질환자의 의료이용 및 직접의료비 연구)

  • This study was aimed to make data how much spent money of medical utilization and direct medical cost. In order to research we were using Korea Health panel 2012 Statistics which data contained Diabetes mellitus client 812 people in age 19. The method of this study was emergency cost, admission medical cost, out patient department cost(client own due, National Health insurance service due, not insurance fee). The result of this study, Diabete Mellitus client were using 198 times during 1 year per 100, total medical direct cost were 859,942 won, 447,359 won, 363,255,508. And admission times were 5.6 times per year, total direct cost was 772,240 won, 4,061,982 won, and 3,298,329,384 won, and out patient clinic using number was 10 times, medical cost total direct cost containing total direct cost was 11,978 won, 26,020 won, and 21,129,240 won. From this research we conclusion that the occurrence of diabetes mellitus can be increased medical cost and direct medical cost and it can be huge burden to client including their family and quality of life in the future. We suggest that in order to prevention and management of diabetes mellitus healthy diet, activity, blood sugar, and blood management should be encouragement.

Influence of Consumers' Purchasing Selection Criteria for Golf-wear upon Clothing Pursuit Benefit and Purchase Satisfaction (골프웨어 소비자의 구매선택기준이 의복추구혜택과 구매만족도에 미치는 영향)

  • The purpose of this study is to analyze a research on the influence of consumers' purchasing selection criteria for golf-wear upon clothing pursuit benefit and purchase satisfaction. It selected people who are using golf games and golf courses in each city and county where are located in Gangwon-do Province, and surveyed targeting totally 206 people by using convenience sampling. As for data processing, the collected materials in this study were carried out frequency & percentage, reliability analysis, correlation & multiple regression analysis by using SPSS 13.0 program. The results are as follows. First, as a result of analyzing socio-demographic characteristics, the similar distribution was shown by gender with 51.0% in men and 49.0% in women. By age, the age group from their 40s to 50s was indicated to be the largest. The martial status was indicated to be a litter higher in the married with 57.3% compared to the unmarried with 42.7%. By healthy condition, what tends to be healthy can be known to be the largest with 79.4%. Second, positive correlation was indicated all in correlation of consumers' purchasing selection criteria for golf-wear with the clothing pursuit benefit and satisfaction. Third, as a result of analyzing on relationship of the golf-wear purchasing selection criteria with the clothing pursuit benefit, the purchasing selection criteria were indicated to have influence upon the ideal pursuit, the fashion & brand, comfort pursuit, and youthand-individuality pursuit, which are the clothing pursuit benefits. Fourth, the analytical result on the purchasing selection criteria for golf-wear with purchase satisfaction was indicated to have significant influence upon consumers' golf-wear purchase satisfaction.

Potential Effects of Nature-friendly Hill Climb Sportsmen's Participatory Motivation and Commitment Experience upon Their Sense of Self-achievement and Living Satisfaction (자연친화적 힐클라임 스포츠 참가자의 참가동기가 몰입경험, 자아성취감, 생활만족에 미치는 영향)

  • The purpose of this study was to examine potential effects of nature- friendly hill climb sportsmen's participatory motivation and commitment experience on their sense of self-achievement and living satisfaction. To meet the goal, this study surveyed total 206 participants (male and female) in hill climb tournaments held at Daegwallyeong and Misiryeong, Gangwon province. For data processing, this study used SPSS 14.0 and amos 7.0 to conduct frequency analysis, reliability analysis, correlation analysis, descriptive statistic analysis and structural equation model analysis respectively. As a result, this study could come to the following conclusions: First, according to descriptive statistic analysis, it was found that our respondents who participated in nature-friendly hill climb sports scored relatively high points in their participatory motivation, commitment experience, sense of self-achievement and living satisfaction. Secondly, this study adopted Pearson's moment correlation analysis in order to determine potential associations among our respondents' participatory motivation, commitment experience, sense of self-achievement and living satisfaction. As a result, it was found that their participatory motivation was positively correlated with their living satisfaction, while their commitment experience was positively correlated with their sense of self-achievement. Thirdly, it was found that there were certain causalities among our respondents' participatory motivation, living satisfaction, commitment experience and sense of self-achievement.

Treatment of Combat-related Gunshot and Explosive Injuries to the Extremities (전투 상황에서 발생한 사지 총상 및 폭발창의 치료)

  • Purpose: We should prepare proper medical service for disaster control as South Korea is not free from terrorism and war, as we experienced through the two naval battles of the Yeonpyeong, one in 1999 and the other in 2002, the sinking of Cheonan in 2010, and the attack against the border island of Yeonpyeong in 2010. Moreover, North Korea's increasingly bellicose rhetoric and mounting military threats against the world demand instant action to address the issue. The aim of this article is to describe our experience with three patients with combat-related gunshot and explosive injuries to their extremities and to establish useful methods for the management of patients with combat-related injuries. Methods: Three personnel who had been injured by gunshot or explosion during either the second naval battle of the Yeonpyeong in 2002 or the attack against the border island of Yeonpyeong in 2010 were included in our retrospective analysis. There were one case of gunshot injury and two cases of explosive injuries to the extremities, and the injured regions were the left hand, the right foot, and the right humerus. In one case, the patient had accompanying abdominal injuries, and his vital signs were unstable. He recovered after early initial management and appropriate emergency surgery. Results: All patients underwent emergent surgical debridement and temporary fixation surgery in the same military hospital immediately after their evacuations from the combat area. After that, continuous administration of antibiotics and wound care were performed, and definite reconstructions were carried out in a delayed manner. In the two cases in which flap operations for soft tissue coverage were required, one operation was performed 5 weeks after the injury, and the other operation was performed 7 weeks after the injury. Definite procedures for osteosynthesis were performed at 3 months in all cases. Complete union and adequate functional recovery were achieved in all cases. Conclusion: The patient should be stabilized and any life-threatening injuries must first be evaluated and treated with damage control surgery. Staged treatment and strict adherence to traditional principles for open fractures are recommended for combat-related gunshot and explosive injuries to the extremities.

An Ergonomic Analysis for Heavy Manual Material Handling Jobs by Fire Fighters (소방대원의 중량물작업에 대한 인간공학적 분석)

  • Modern fire fighting jobs have been expanded to include areas of rescue, emergency medical service as well as conventional fire suppression, so that load for fire fighting jobs has been increased. Specifically, musculoskeletal disorders (MSDs) such as low back injury have been considered as one of major industrial hazards in heavy manual material handling during fire fighting jobs. This study tried to evaluate risk levels and to prepare background for reducing risk levels associated with heavy manual material handling during fire fighting jobs. This study applied two major tools in evaluating heavy manual material handling jobs which were NLE (NIOSH Lifting Equation) and 3DSSPP (3D Static Strength Prediction Program). A risk index in terms of heavy manual material handling during fire fighting jobs was identified. This index consisted of seven risk levels ranged from nine points (the first level) to three points (the seventh level). There was no job associated with the first level (the highest risk level) of index. There was only one job (life saving job) belonging to the second level (the second highest risk level) of index. The third level had jobs such as usage of destruction equipment and lifting patient. A total of basic eighteen jobs was categorized into six different levels (2nd-7th levels) of index. The outcome of the study could provide a good basis for conducting job intervention, preparing good equipment and developing good education program in order to prevent and reduce MSDs including low back injury of fire fighting jobs.

Impact of Changes in Medical Aid Status on Health Care Utilization

  • Background: South Korea operates a Medical Aid (MA) program targeting selected low-income individuals to ensure medical service delivery to the disadvantaged while enhancing self-sufficiency of work-capable beneficiaries. However, as reasons behind welfare exits are diverse and do not always infer poverty relief or the provision of appropriate levels of health care services, this study aimed to investigate the association between changes in MA status and health care utilization. Methods: This study used the 2006 to 2015 National Health Insurance claims data. The impact of changes in annual MA status on health care utilization (yearly number of outpatient visits, inpatient visits, length of stay, and emergency department [ED] visits) was investigated using the generalized estimating equation model. Results: In 117,943 adult subjects aged 20 to 64, compared to the 'MA to MA' group, the 'MA to MA exit' group showed general decreases in utilization (outpatient visits: β=-3.93, p<0.0001; hospital admissions: relative risk [RR], 0.87; 95% confidence interval [CI], 0.83-0.91; length of stay: β=-3.64, p<0.0001; ED visits: RR, 0.83; 95% CI, 0.77-0.90). Similar patterns were found in the 'MA exit to MA exit' group (outpatient visits: β=-5.72, p<0.0001; admissions: RR, 0.91; 95% CI, 0.87-0.94; length of stay: β=-5.87; p<0.0001; ED visits: RR, 0.81; 95% CI, 0.75-0.88). Likewise, in 74,747 older adult subjects aged 65 or above, the 'MA to MA exit' group showed reduced levels of utilization (outpatient visits: β=-1.51; p=0.0020), as well as the 'MA exit to MA exit' group (admissions: RR, 0.92; 95% CI, 0.89-0.95; length of stay: β, -5.45; p<0.0001; ED visits: RR, 0.90; 95% CI, 0.83-0.97). Conclusion: MA exit was associated with general decreases in health care utilization. Utilization patterns of individuals with experiences of receiving MA benefits should be monitored to promote the ideal use of health care services while preventing potential financial barriers present in accessing medical care.

Special Issue for the 30th Anniversary of the Korean Academy of Health Policy and Management (한국보건행정학회 30주년 기념 특별호)

  • The Korean Academy of Health Policy and Management (KAHPM) has shown remarkable achievements in the field of health policy and management in Korea for the last 30 years. The KAHPM consists of experts in various fields of health policy and management, and has been the leading academic discussion forum for health policy agendas of interest to the public. Health Policy and Management (HPM), the official journal of the KAHPM, published the first issue of volume 1 in October, 1991 and is publishing the second issue of volume 28 as of 2018. Currently, it is one of Korea' main journals in the field of health policy and management. HPM has published a special issue in commemoration of the 30th anniversary of the KAHPM. The HPM invited authors, including former presidents of the KAHPM and current board members, to write about main issues in health policy and management. Although the HPM tried to set up an invited author on all subjects in the health policy and management field, 19 papers are published, that completed the peer review process by August, 2018. The authors of the special issue of the 30th anniversary of the KAHPM include six former presidents, a senior professor, and 12 board members. The subjects of this issue are reform of the healthcare delivery system, health insurance and medical policy, reform of health system governance, the role of National Health Insurance Service (NHIS), the Korea Institute for Health and Social Affairs (KIHASA) and the National Evidence-based healthcare Collaborating Agency (NECA), ethical aspects of health policy change, regional disparities of healthcare, healthcare accreditation, new healthcare technology evaluation system, globalization of the healthcare industry, the epidemiological investigator system, the quarantine system, safety and disaster, and official development assistance. There are some remaining topics to deal with for the KAHPM: aged society, anti-smoking, non-infectious disease, suicide, healthcare resources, emergency medical care, out-of-pocket money, medical fee payment system, medical aid system, long-term care insurance, industrial accident compensation insurance, community-centered health welfare system, and central government and local government of health. The HPM will continue to publish review articles on the main topics in health policy and management. This is because the KAHPM, which has been the leading academic society of Korea's health policy and management for the last 30 years, feels responsible for continuing its mission for the next 30 years.

Effect of Cooperative Learning Applying Jigsaw Model in Simulation-Based Infection Control Education on Perception of Infection Control, Intrinsic Motive and Learning Satisfaction (시뮬레이션기반 감염관리교육에서 직소(Jigsaw)모형을 응용한 협동학습이 감염관리 인식도, 내적동기, 학습만족도에 미치는 효과)

  • This study has the purpose to evaluate the effects of cooperative learning applying Jigsaw model in simulation based infection control education by comparing perception of infection control, intrinsic motive and learning satisfaction, 54 first-year health related students. It is composed of 27 students of the experimental group and 27 students of the control group. In order to evaluate the homogeneity between the two groups, it surveyed in advance perception of infection control, intrinsic motive and learning satisfaction. There was no significant difference between the two groups, The education program composed cooperative learning applying Jigsaw Model, simulation practice and debriefing to the experimental group was performed twice for 2 weeks and lecture and skill training, simulation practice and debriefing were performed to the control group. After two-week education, perception of infection control, intrinsic motive and learning satisfaction of the participants the research were surveyed. As a result of this study, the Jigsow Model indicated the perception of infection control and learning satisfaction experimental group was significantly high in statistic. Based on this research result, I suggests that utilization of cooperative learning applying the Jigsaw Model as a strategy to improve the scholatic achievement and learning satisfaction of students in the variable simulation based education.

Digit ratio(2D:4D) and gender role orientation in health and medical students (보건의료 대학생의 손가락 길이비(2D:4D)와 성역할 정체감)

  • The ratio of index finger length to ring finger length(2D:4D) is an index of prenatal testosterone and/or estrogen exposure. The aim of this study is to investigate digit ratio and the type of gender-role orientation, to identify the correlation between 2D:4D and gender-role orientation in health and medical students. Participants were 120 male and 146 female university students majoring in health or medicine. After filling in KSRI(Korean Sex Role Inventory), 2nd and 4th finger lengths were measured on both hands using vernier callipers with photocopy. This study showed 2D:4D in the males was 0.95, and that in the females was 0.96 (p<0.01). 38.3% male students revealed androgyny, on the other hand, 35.6% female students revealed undifferentiated type from KSRI. And also, there was a significant difference in masculinity score between male and female but not in feminity score. There was no significant difference in 2D:4D according gender role orientation. The KSRI masculinity score was negatively related to 2D:4D of left hand in male (p<0.05). The results of this study suggest that a more masculine gender role orientation in males is associated with low 2D:4D revealing higher testosterone and/or lower estrogen level in utero. However, more research in 2D:4D ratio to Korean is needed for support of this result.

A Characteristics of the Vulnerable Area for Emergency Medical Service in Daejeon by Analysis of Geographic Information System (지리정보시스템(GIS)으로 분석한 대전광역시 응급의료서비스 취약지 특성)

  • 본 연구는 대전광역시의 응급의료서비스 취약지를 도출하고 취약지역의 보건학적 특성 및 응급의료서비스 취약여부와의 관련성을 분석하여 응급의료 관련 정책의 의사결정에 유용한 기초자료로 제공하기 위한 연구이다. 응급의료서비스 취약지 도출은 Arc GIS의 공간분석 방법 중 가중분석(Cost Weighted distance) 방법으로 응급의료센터로부터의 접근성 분석을 하였으며, 응급의료서비스 취약지의 보건학적 특성 및 응급의료서비스 취약여부와의 관련성은 SPSS 17.0을 이용하여 비모수 t-검정 및 다중회귀분석을 시행하였다. 본 연구의 주요 결과는 다음과 같다. 연구지역의 응급의료기관 분포는 동구와 유성구, 대덕구는 지정된 응급의료센터가 없으나 서구와 중구는 응급의료센터가 2개소 이상 위치하고 있어 응급의료기관 분포가 편중되어 있으며, GIS를 활용하여 응급의료센터와의 접근성 분석을 수행한 결과, 대전광역시 자치구별 전체 면적 대비 응급의료서비스 취약지의 비율이 높은 자치구는 동구가 41.2%로 가장 높았다. GIS를 활용하여 행정동별 응급의료서비스 취약지를 분석한 결과, 대덕구 신탄진동, 동구 대청동과 산내동, 유성구 구즉동과 노은2동, 서구 기성동, 중구 산성동으로 나타났으며, 응급의료서비스 취약지 중 기성동, 대청동이 노인 인구밀도가 높게 나타났다. 응급의료서비스 취약여부에 따른 보건학적 특성별 차이를 분석한 결과, 국민기초생활수급권자, 장애인등록자, 농업인구 비율의 평균은 취약지가 비취약지에 비해 높았으며 통계적으로 유의한 차이를 보였다(p<0.01). 응급의료서비스 취약여부를 종속변수로 하고 지역별 보건학적 특성을 독립변수로 하여 로지스틱 회귀분석을 시행한 결과, 농업인구 비율과 국민기초생활수급권자 비율이 높았으며 이는 통계적으로 유의하여 응급의료서비스 취약여부를 설명할 수 있는 변수인 것으로 나타났다(p<0.01, p<0.05). 이상의 결과를 종합하면 대전광역시 5개 자치구의 행정동 중 응급의료서비스 접근 불평등지역이 도출되었고 이러한 지역은 보건학적 특성 중 농업인구 비율과 국민기초생활수급권자의 비율이 높았으며 이는 통계적으로 유의하여 응급의료서비스 취약여부와 관련성이 있는 것으로 나타났다. 향후 효율적인 응급의료 자원 분배를 위해서는 GIS를 활용한 의사결정이 필요하며, 응급의료서비스 이용의 형평성을 증진시키기 위해서 응급의료서비스의 사각지대에 놓여있는 지역의 보건학적 특성을 고려한 정책이 시행되어야 할 것으로 사료된다.
