In remote USV (Unmanned Surface Vehicle) maritime operation, the remote operation and control technic and autonomous control technic is required and the emergency mode algorithm is needed certainly for sailing and accomplishing various surveillance, reconnaissance, and underwater search missions of USV. In this paper, we review the countermeasures in emergency situation of the existing USV system (Barracuda) and propose the emergency mode algorithm considering the operation and control, and autonomous control level for the stable USV operation in case of emergency. We analyzed the autonomous control level in view of the mission complexity and environmental difficulty, and human interface, and verified the performance of the autonomous control level when we apply four emergency mode algorithms. It is expected that more stable and reliable operation and cotrol are possible if the proposed algorithm is applied to the environments requiring the various multi-mission USV sailing and mission achievement.
Purpose: The purpose of the study was to contribute to the prevention of musculoskeletal disorders (MSDs) in 119 emergency medical technicians (EMTs) by evaluating ergonomic risk factors of patient lifting work-postures. Methods: Four procedures were evaluated: using long back-board (LBB) on the sitting and standing main stretcher, using variable stretcher on the sitting and standing main stretcher. Wok-postures were assessed during training. Results: In using LBB on the sitting main stretcher, the OWAS-score was Mode:3 (Mean:2.30, Maximum:3), the REBA-score was Mode:9 (Mean:7.61, Maximum:11), requiring improvement soon. In using LBB on the standing main stretcher, the OWAS-score was Mode:3 (Mean:2.33, Maximum:3), requiring as soon as possible corrective action, the REBA-score was Mode:6 (Mean:5.44, Maximum:11), requiring improvement. In using variable stretcher on the sitting main stretcher, the OWAS-score was Mode:1 (Mean:1.85, Maximum:3), not requiring corrective action, the REBA-score was Mode:6 (Mean:6.78, Maximum:11), requiring improvement. In using variable stretcher on the standing main stretcher, the OWAS-score was Mode:3 (Mean:2.84, Maximum:3), requiring as soon as possible corrective action, the REBA-score was Mode:11 (Mean:9.38, Maximum:11), requiring immediate improvement. Conclusion: All four-procedures showed improvement in work-posture. Thereby, required attention and management in training, occupational health professionals should participate in change of lifting-method, and programs aimed at preventing MSDs should be developed and implemented in fire-academy and fire-station.
Purpose: This study aims to examine spatial characteristics of emergency department Chronologically through case studies; and to consider the social implication of these spatial changes. Methods: In this study, a total of eight emergency departments, one for each period, were selected in order to analyze the spatial characteristics. The spatial maps of Space Syntax were employed for analyzing case studies. Results: The spatial configuration of emergency department has been changed from five or more emergency department specialties to four or less specialties such as psychiatric, pediatrics, emergency, and trauma. In the case of care initiation area, the concentrated arrangement mode was switched from the distributed arrangement mode. The spatial maps indicate that it can be seen that the tree structure changes to an annular structure emphasizing the connection between the spaces in the emergency department. This shows that the space efficiency, safe, visual control and flexibility in the planning of the emergency department are important factors affecting the spatial structure of the emergency department. Implications: In the future, it is expected that Korean emergency department will be more focused on efficiency, safe, visual control, and flexibility in the planning as in the case studies of the United States.
Purpose: The purpose of this study was to determine whether, in patients with acute coronary syndrome (ACS), according to the mode of arrival affect the emergency medical process. Methods: The participants of this study were 118 adult patients (46 patients admitted by emergency medical services (EMS), 48 walk-in patients, and 24 transferred from other hospitals) admitted to the emergency departments at one regional-level medical center who underwent coronary angiography between January 1, 2016 and December 31, 2017. To compare treatment courses, the median values of the following variables were compared among groups: symptom to door time; door to triage time; and door to ECG time. All data were analyzed using SPSS program. Results: Based on the initial assessment at triage, there was a significantly greater proportion of Korean Triage and Acuity Stage (KTAS) Level 1 or 2 among patients admitted by EMS than among walk-in patients. All three analyzed variables were lower in patients admitted by EMS than in the other two groups. Conclusion: Our results show that ACS patients who accessed EMS reached the emergency center faster after symptom onset, received initial triage assessment at earlier stages, and underwent sooner important examinations (i.e., the 12-lead ECG).
This study proposes a technology to ensure the seismic stability of a 1,000 kW diesel engine-type emergency generator by applying a seismic isolated bed system. The technology allows the static analysis by making the first natural frequency of the installed entire emergency generator larger than the earthquake cutoff frequency of 33 Hz. First a three dimensional model for the generator was made with simplification for mode analysis. A new bed system with springs, shock absorbers, stoppers was then devised. Next, The mode analysis for the finite element model equipped by the bed system was performed. the 1st natural frequency above 33 Hz, the seismic safety cutoff frequency, was calculated to be 152.92 Hz. Finally, based on the seismic stability theory, the von-Mises equivalent stresses derived by structural analysis under the Upset and Faulted conditions were 0.01603 Mpa, and 32.06 Mpa, respectively. so seismic stability was confirmed.
The purposes of this study which was conducted by studying the literature on Emergency Medical Dispatch System are to provide some developmental policies of quality management, pre-arrival instructions, priority dispatch protocols, training program for the dispatchers(Emergency Medical Dispatchers or EMDs) in Korea and to promote understanding emergency medical dispatching. The conclusions from this summarized as follows; (1) It is confirmed that there has been little study on the Emergency Medical Dispatch System in Korea, because for the first time, the real Emergency Medical Services were introduced into Korea in 1994, and the importance of the Emergency Medical Dispatch System has not been realized. (2) Only some squads are using a set of dispatch protocols, others aren't. (3) In spite of trying to introduce a new set of dispatch protocols, it isn't the priority dispatch system using a complete set of dispatch protocols which has key questions, pre-arrival instructions, mode & configuration based on patient assessment. (4) The EMS is unable to promote the service capacity by using quality management, because there is no medical control on the emergency medical dispatching and the EMDs. (5) There are no medical directors in the communications center who are in charge of the medical control to detect problems derived from the EMS and to solve them. (6) There are no systematic training program for the EMDs who are taking charge of dispatching. (7) Having a deep relation to the elements of the EMS, the emergency medical dispatching is subject to restriction of those elements.
This study proposes to ensure the seismic stability of an existing switchboard for emergency diesel generator by applying mode analysis, static analysis and dynamic analysis. First, a three dimensional model for the swithboard was made with simplification for mode analysis. Next, The mode analysis for the finite element model of the existing switchboard was performed. The 1st natural frequency below 33 Hz, the seismic safety cutoff frequency, was calculated to be 21.943 Hz. Finally, based on the seismic stability theory, the von-Mises equivalent stresses derived by structural analysis and response spectrum analysis under the normal and faulted conditions were 74.179 MPa and 49.769 MPa, respectively. These are less than specified allowable stresses. So seismic stability was confirmed.
Purpose: The purpose of this study was to identify the effects of multi-mode simulation learning on critical thinking disposition, on the problem solving process and on clinical competence of nursing students. Methods: A non-equivalent control group with pre-posttest was designed. The participants in this study were 65 students who were enrolled in an emergency and critical nursing course at N university. The treatment group consisted of 33 juniors in 2010 and the control group 32 juniors in 2011. Collected data were analyzed using chi-square, independent t-test, and ANCOVA with the SPSS/WIN 18.0 for Window Program. Results: There were significant increases in problem solving process and clinical competence in the treatment group who participated in the multi-mode simulation learning compared to the control group who did not (t=-2.39, p=.020; F=12.76, p=.001). However, there were no significant differences in critical thinking disposition between the treatment and control group (t=0.40, p=.692). Conclusion: Multi-mode simulation is an effective teaching and learning method to enhance the problem solving process and clinical competence of nursing students. Further exploration is needed to develop and utilize multi-mode simulation for diverse scenarios, depending on emergency nursing educational goals and environments and to develop a universal method to measure outcomes.
Objectives : The purpose of this study was to examine the health service utilization of elderly patients who visited an emergency medical center in the Gyeongbuk region and to provide basic information for the effective management of emergency medical centers. Methods : This study analyzed the characteristics of the hospital visits and the actual situation for the use of emergency medical care of 10,264 elderly patients that visited an emergency medical center in the Gyeongbuk region from January, 2014 to December, 2014. Frequency analysis and chi-square test were done in this study. Results : This study showed that there is a difference in the characteristics of health service utilization which included hospital visits, duration of hospital visits and mode of arrival to the emergency medical center according to age, gender and other characteristics. Conclusions : Providing efficient emergency services is necessary as well as establishing an emergency medical center management plan that takes into consideration the difference in health service utilization of elderly patients.
Purpose: This study aims to evaluate the risk of work-related musculoskeletal disorders (WMSDs) in 119 EMT during patient-carrying tasks using a long backboard (LBB) and a variable stretcher. Methods: Manikins were used as patients, and 45 new 119 EMT from K Fire Academy were filmed performing patient-carrying tasks on stairs. The tasks were analyzed using Ovako working posture analysis system (OWAS) and Rapid entire body assessment (REBA). Results: In using LBB, the OWAS score was Mode 3(Mean 2.37, Maximum 3), requiring as soon as possible corrective action, the REBA score was Mode 11(Mean 9.16, Maximum 11), requiring immediate improvement. In using variable stretcher, the OWAS score was Mode 1(Mean 2.33, Maximum 3), non-necessity for corrective action, the REBA score was Mode 9(Mean 8.0, Maximum 11), requiring as soon as possible. Conclusion: In conclusion, improvement was needed in one task (carrying a patient using a LBB) in the OWAS and in two tasks (carrying a patient using a LBB, carrying a patient using a variable stretcher) in the REBA. Thereby, required attention and management of WMSDs during training. In addition, it is essential to carry out objective and quantitative assessments through ergonomic analysis by occupational health professionals when designing future training programs to prevent WMSDs.
