Application of GIS Technology for Developing Emergency Management System on High-Speed Lines

  • Due to almost all tracks of new Kyoungbu high-speed lines(HSL) consisting of bridges and tunnels, HSR operational safety management is very important factor for passenger safety and preventing catastrophic event. Especially, it is important to approach the scene of the accident and assist in saving lives as soon as possible under emergency conditions. Therefore it is essential to prepare access methods and available resources in advance in order to support emergency correspodances promptly and effectively. This paper persents an application of geographical information system(GIS) for developing emergency management system as well as batabase system on tracks and facilities, electric/communication equipment and safety equipment required to support emergencies. The on-site information on the proximity of HSL is expressed on the numerical map, which can be helpful to call external supports such as medical service, local authorities, police offices, fire brigades, and etc. for the emergency situations.

  • In this paper, we propose the architecture of the policy-based emergency bio data transmission for the smart healthcare service. the medical staff can quickly and accurately monitor the emergency bio data of the remote patient through the proposed architecture. The proposed system consists of three parts: IEEE 11073-based agents and managers performing the aggregation function and transmission function of the bio data; the emergency management server performing the converting function between IEEE 11073 and HL7 and auto-diagnosis function of the policy-based; HL7 medical system based on HL7. Finally, by implementing the proposed system, we shows that the aggregation of the bio data and management of the emergency bio data in the smart healthcare service are possible.

  • As the elderly population is becoming an aging society, the elderly are experiencing many problems. Social security costs for the elderly are increasing and the un-linked social phenomenon is emerging. Thus, the social infrastructure and welfare system established in the past economic growth period are in danger of not functioning properly. People socially isolated or with chronic diseases among the elderly are exposed to various accidents. Thus, an active healthcare management service is imperative. Additionally, in the event of a dangerous situation, the system must have ways to notify guardians (family or medical personnel) regarding appropriate action. Thus, in this paper, we propose the smartphone-based healthcare and emergency response service platform. The proposed service platform aggregates movement of relevant data in real-time using a smartphone. Based on aggregated data, it will always recognize the user's movements and current state using the human motion recognition mechanism. Thus, the proposed service platform provides real-time status monitoring, activity reports, a health calendar, location-based hospital information, emergency situation detection, and cloud messaging server-based efficient notification to several subscribers such as family, guardians, and medical personnel. Through this service, users or guardians can augment the level of care for the elderly through the reports. Also, if an emergency situation is detected, the system immediately informs guardians so as to minimize the risk through immediate response.

Comparison of bimanual laryngoscopy, backward-rightward pressure, and cricoid pressure in difficult airway management: A manikin study (어려운 기도를 가진 마네킹에게 양손후두경법, 우측-후방압박법과 반지연골압박법 효율성 비교)

  • Purpose: The purpose of this study was to establish novice user guidelines for efficient external laryngeal manipulation for intubation in difficult airway management. Methods: This study included 59 pre-qualified junior and senior emergency medical service students. The participants were instructed at random to intubate a manikin equipped with a cervical collar, thus simulating a difficult airway, using three types of external laryngeal manipulation: bimanual laryngoscopy, backward-rightward pressure, and cricoid pressure. The resultant intubation time and glottic view grade scores were estimated. Results: Intubation time was longest using the bimanual manipulation method, followed by cricoid pressure and backward-rightward pressure. A low Cormack-Lehane glottic view score was obtained regardless of the assisted compression method used. Conclusion: Backward-rightward pressure may be the most efficient method of external laryngeal manipulation on the basis of the intubation time and improvement in glottic view.

  • Today, the basic problem of functional safe management of Korean Disaster Control system was separately administrated 33 acts that are relating to safe management in 13 ministries. Because of the facts that the fire service is not provided practically, the control system and risk management for safe administration are not operated, the information can not be shared with each other, and the various laws have the lacks of linkage, the National Safe System was appeared unsteady. The roles and functions of fire service have started with restriction to operate structurally and institutionally, which operational structure of fire service is becoming weak. As a result, the federal and local fire organizations have not reached yet to the institutionalization and the local fire service agencies have bias with the task regarding the fire service because of the relation between organizational structure and the local fire agency. With the enforcement of the federal and local fire system, professionality and autonomy for making policy, and dealing with changes of fire service positively, the national fire service on the policy performance can be established. Promotion of research and development and education training to strengthen innovation in technology and competition in fire industry will contribute to the firmly establishment of control system to prevent from fire, flood, terror and national disaster. This article proposed that (a) the established law and administration, agency are required efforts to effectively operate fire service system; (b) the national fire service agency, national college of fire, national institute of science fire, and national fire service hospital should be early established to make firmly policy to operate effectively and practically. These kinds of innovational acts are known the best ways of operating solid policy of national fire service system.

  • Emergency room process is very important in the whole hospital processes because it is first diagnosis for patient. Above all, triage is important activity which quickly diagnose the status of emergency patient and sets the priority for treatment. This paper analyzes the treatment process pattern by triage type. The results show that the treatment process after triage such as residence time, diagnosis and checkup type, and joint treatment are dependent on triage types. We can use these analysis results for improving the current triage system and developing the new triage system considering a domestic emergency medical service environment.

The Effect of Job Stress and Musculoskeletal Symptoms on Depression among Emergency Medical Technicians (응급구조사의 업무스트레스와 근골격계 증상이 우울에 미치는 영향)

  • Objectives : The aim of this study was to examine how emergency medical technicians(EMT) are affected by job stress, musculoskeletal symptoms(MSSs) and depression. Methods : Data were collected from EMTs at emergency medical institutions and fire stations (n=257). With a questionnaire, the association of job stress and MSSs and depression were examined in this study. Results : When examining job stress by place of work, the level of stress in field situations was significantly higher in EMTs at fire stations than in EMTs at emergency medical institutions. The MSSs of EMTs at emergency medical institutions were significantly higher compared to EMTs at fire stations, For depression, the score was 14.09 out of 60, and MSSs were significantly more common in women. Additionally, the level of job stress for 20-24 and over 35 year olds was significantly higher than that in the other age groups. Job stress had a positive correlation with MSSs and depression of EMTs, and MSSs were associated with depression. Conclusions : To improve the work environment of EMTs, attention should be paid to job stress, MSS, and depression and the presence of positive organizational support should be provided which can prevent negative effects.

EMS fund, is it functioning or fumbling? (응급의료기금, 무엇이 문제인가?)

  • Purpose. Based on the comparison and analysis with those of United States, the aim of this research is to find the problems in current management, operation and future directions of emergency medical service (EMS) fund in S. Korea and to provide basic resources and appropriate measures to make a right decision in policies for EMS fund. Methods. Data from Ministry of Health and Welfare and other various sources during 1995 to 2012 were collected and analyzed. Results. From our analyses, several problems are identified in EMS fund operation. In brief, problems discerned are as follows. First, whereas the purpose and direction of EMS fund operations in United States are highlighted and focused on pre-hospital EMS system and associated infrastructures which need to be constructed, those of S. Korea are emphasized mainly on the in-hospital EMS system so far. Second, on the contrary to the fact that the EMS funds in United States are tuned to pre-hospital EMS system to provide prompt and efficient emergency care at the emergency scene of pre-hospital stage and to achieve the development, design, planning and demonstration projects for pre-hospital EMS systems, up to date, our investment of EMS funds demonstrated an excess biased inclination toward the construction of in-hospital EMS system, which is far from the realization of constructive and vital pre-hospital infrastructures. Third, while emergency medical technician is important and principal body in the management of emergency medical funds in United States, so far, no EMS funds in S. Korea existed for EMTs including the job condition, improved treatment and working environment for them. Conclusion. In conclusion, we strongly suggest that the problems pointed out must be revised and corrected. Current usage of EMS fund needs to be redirected predominatly to pre-hospital EMS system. Otherwise, unless current management and investment of emergency medical funds in S. Korea are applied and used for the vital necessities and demands of EMTs, public EMS units and private EMS units as well as related units in pre-hospital EMS system as in the cases of United States, in our consideration, they must be suspended or abolished.

Forecast Driven Simulation Model for Service Quality Improvement of the Emergency Department in the Moses H. Cone Memorial Hospital

  • Patient satisfaction with the Emergency Department(ED) in a hospital is related to the length of stay, and especially to the amount of waiting time for medical treatments. ED overcrowding decreases quality and efficiency, therefore affecting hospitals' profitability. This paper presents a forecasting and simulation model for resource management of the ED at Moses H. Cone Memorial Hospital. A linear regression forecasting model is proposed to predict the number of ED patient arrivals, and then a simulation model is provided to estimate the length of stay of ED patients, system throughput, and the utilization of resources such as triage nurses, patient beds, registered nurses, and medical doctors. The near future load level of each resource is presented using the proposed models.
