Improvement of the pre-hospital emergency medical service system in China (중국의 병원 전 응급의료체계 개선방안)

  • Purpose : Emergency medical services in China are increase in demand by people and under the greater pressure than ever before. So it is, necessary to advance the pre-hospital system in order to promote the development of emergency medical services. Methods : This is based on China-related articles, books, journals, reports, statistical data and other literature. Results : First, pre-hospital emergency medical care with the introduction of specialist training program should be established. Second, to strengthen pre-hospital emergency services and to develop the EMS guidelines. Third, the "120" reporting systems unification and awareness activation. Fourth, the preparation of the EMS facilities equipment system. Fifth, the rapid transport system establishment to the selected medical institutions. Conclusion : It is necessary to strengthen the emergency medical personnel at the scene, rapid transport, rapid patient triage and to improve the survival rate of the patients.

A Study for General Emergency Medical Service Systems in Disaster (재난발생 시 일반응급의료체계에 관한 연구)

  • In Korea, There are many disasters, like the collapse of Sampung department store, the strike of severe typhoon 'Rusa' and the subway tragedy in Taegu, because of global warming, urbanization, high-density and high-rise of buildings. So, the government made 'The Framework Act' on the safe and management of disaster and 'The National Emergency Management Agency' was established. But emergency medical service systems in Korea is not growing so much. The purpose of this research is to give basic data for the development of emergency medical service systems in Disaster by comparing of disaster management systems and emergency medical systems among the nations of the world, analysing emergency medical systems in disaster in Korea and suggesting some improvement methods. The improvment methods are like this ; First, establishing the National Disaster Medical System in Korea, making the good triage by EMT, expansion of EMT's working area, developing protocols and framing of medical director increasing the working force of EMT, broad inner cavity of ambulance for treatment of patientent, supplement of professional equipments, active using of helicopters are needed in prehospital are. Second, equal establishment of emergency medical center and increase of working force of emergency medical team are needed in hospital area. Finally, enforcement of the dispatcher's qualification, smooth communication among EMSS systems and actualization of medical direction through screen are needed in the Telecommunication system.

  • This study focuses on making recommendations for the improvement of poor emergency medical services or EMS in Korea. Problems of the current EMS system is widely reviewed. As an alternative of the current system, a franchises system is introduced to EMS. A feasibility study of the new system is carried out as follows. 1) The current EMS system shows such problem as; - Inadequacy of emergency medical fee schedule, - Shortage of emergency medical resources, - Concentration of emergency medical facilities in metropolitan area, - Unestablishment of training program for emergency personnel, - Inappropriateness of patients' behvaviro pattern in emergency care system. 2) Recommendations for the improvement of EMS are as follows: - Well organized communication and transportation system needs to be established by utilizing all the availabl EMS is to be devised for the provision of appropriate emergency care. - A franchises system can be widly adopted so that private organizations are willing to establish a free standing emergency center. - Emergency care fee schedule of the franchises system needs to be appropriate for the system to provide high standard medical services.

A Study on the Analysis and Improvement methods of Emergency Medical Service Systems in Disaster Areas (재해현장에서의 응급의료체계에 대한 분석과 개선방안)

  • Various disasters have been continuously occurred in Korea from 1990s to now. However, there is no substantial improvement against damages as compared with the past due to various reasons such as lack of fundamental recognition, ineffective response systems and widespread insensitivity to safety. More worse, new types of disasters have been frequently generated due to rapid changes in social structures and industrial development, unusual changes in weather and changes of international situations. These disasters request comprehensive countermeasures. In particular, while material damages by disasters can be recovered, the losses of precious lives cannot be recuperated in any ways. Thus, it is critical to set effective disaster medical plans. The first way to minimize damages by disasters is the prevention and the next is to set the disaster medical plans focusing on preliminarily activating the emergency medical system to rapidly rescue and take appropriate emergency medical services for casualties in the early stage when any disaster occurs. Nevertheless, no sufficient researches or references do not exist up to now. Even worse, effective emergency medical systems that play critical roles in increasing survival rates of casualties in actual disaster areas is not deployed. For the United States, the consistent countermeasure system is established in FEMA through a close cooperative system with relevant organizations for serious accidents including terrorists' attacks or natural disasters. For the emergency medical services in disasters, the disaster medical plan is set to cope with any disasters in perfect order by special area as operating the National Disaster Medical SystemESF#8 Role by FEMA. Accordingly, we need to set the extensive and integrated disaster prevention system for rapid and flexible operation against various kinds of serious accidents. This study identified overall problems in disaster control plans in Korea and suggested how to improve the emergency medical service system in disaster areas. Furthermore, it aims to prepare the basic data to set the effective emergency medical service plans when substantial casualties break out and more reasonable and systematic disaster control plans to cope with the future occurrence of serious disasters.

Public perception of ambulances (일반인의 구급차 이미지에 대한 인식 유형)

  • Purpose: This study aimed to obtain, categorize, and analyze baseline data regarding the subjective perception of ambulances from the consumers' perspective, which can be utilized for improving emergency medical service systems. Methods: Fifty people aged 20 to 40 years old were selected as P samples. Data from 35 statements of Q-cards and Q-sample distribution plots were gathered using Q-sort and analyzed using the PC-QUANL program for major factor analysis. Results: We found four unique characteristics which accounted for 57.57% of the total variation: "Considerate of emergency situations," "Identifying emergency patients," "Trusting emergency vehicles," and "Sympathizing with emergency patients." Conclusion: Organizations that provide emergency medical services using ambulances should plan and render patient care according to the four types of public perception of ambulances.

Collateral damage of emergency medical services due to COVID-19 (COVID-19에 의한 EMS 동반손상)

  • Purpose: We aimed to provide effective emergency medical services (EMS) response strategies for coping with high acuity patients during the pandemic by analyzing the influence of the COVID-19 and social distancing on EMS. Methods: In this retrospective observational study, we analyzed the distribution of high acuity patients transported by Daejeon 1-1-9 EMS during the COVID-19 pandemic period, between February 1, 2020 and October 31, 2020 and the same period in 2019, as well as the level 3 social distancing enforced period, between July 27, 2020 and October 31, 2020. Results: The EMS dispatches decreased by 17% during the observed COVID-19 pandemic period compared to the same period in 2019. The number of cases with cardiac arrest and positive prehospital stroke scale rose by (p<.001). Patients with cardiac arrest, trauma, and positive prehospital stroke scale increased by during the level 3 social distancing period. Conclusion: Unlike the decreased EMS call volume and patient transports during the COVID-19 pandemic, cardiac arrest cases and the severity of high acuity patients tended to increase. We suggest that EMS systems should contrive a response strategy considering the collateral effect of major epidemics on the incidence rate of high acuity patients.

A Study on the Analysis and Improvement methods of Emergency Medical Service Systems for Large Scaled Fire (대형화재에 대비한 응급의료체계에 관한 분석과 개선방안)

  • In Korea, Crowds, as well as more complex and larger structures, have been caused more victims in the event of fire. In that situation, EMSS should have done triage the patients according to their severity in the field, treat with standard guidelines as like advanced burn life support and transfer to appropriate facility. But in many cases, they didn't practice like that. The purpose of this study is to give basic data for proper emergency medical services by analysing EMSS of large scaled fire in Korea and suggesting some improvement methods. The improvment methods are like this ; First, developing of protocols about burn patient is needed for EMT and dispatcher. Second, Legal approval for advanced treatment is needed for EMT. Third, Fire-helicoptors are needed in Daejeon and Jeju. And Emergency Medical Helicoptors are needed in EMSS. Forth, more advanced emergency medical centers of burn must be established. Sixth, more pocket-mask and burn sheet are needed for 119 rescuer. Finally, building owners must change inflammable materials as non-inflammable materials and educate fire-prevention and first-aid to employees.

Analysis of Factors Affecting Satisfaction of 119 Emergency Service Users in Korea

  • This study attempted to identify the service satisfaction with the 119 emergency service and the factors that affect it based on the responses of subjects who used the service. Data collection was conducted from February 11 to March 11, 2021, using convenience sampling. A total of 1842 subjects who used emergency medical services using 119 ambulance in 2020 participated. For the collected data, frequency, percentage, mean, and standard deviation were calculated, and t-test, analysis of variance, and multiple regression analysis were performed using SPSS 23.0. Based on these results, to further enhance the satisfaction of users of the 119 emergency service and the quality of emergency medical services, it is important to improve the quality of paramedics through education and systems. In addition, to increase user satisfaction, efficient and systematic communication education is required. The quality of emergency medical services will increase only when communication skills required for explanations and promoting understanding are improved.

  • Purpose: This study looked into the reality of the emergency medical technology department by analyzing the major indicators of university information disclosure systems and finding the competitiveness of the department of emergency medical technology by reviewing various evaluation indices. Methods: This study is a survey of 24 colleges with emergency medical technology and emergency medical technology departments across the country. Quantitative index data from 2017 to 2019 were collected and analyzed through the university information disclosure center web site. Results: Departments of emergency medical technology are generally higher than the target colleges in quantitative indicators, but the indices are somewhat insufficient in terms of "rate of faculty in full service" and "research performance of per one faculty in full service." Conclusion: Based on the results of this study, we recommend increasing the low indicators to enhance the competitiveness of the departments of emergency medical technology.

Identifying research trends in the emergency medical technician field using topic modeling (토픽모델링을 활용한 응급구조사 관련 연구동향)

  • Purpose: This study aimed to identify research topics in the emergency medical technician (EMT) field and examine research trends. Methods: In this study, 261 research papers published between January 2000 and May 2022 were collected, and EMT research topics and trends were analyzed using topic modeling techniques. This study used a text mining technique and was conducted using data collection flow, keyword preprocessing, and analysis. Keyword preprocessing and data analysis were done with the RStudio Version 4.0.0 program. Results: Keywords were derived through topic modeling analysis, and eight topics were ultimately identified: patient treatment, various roles, the performance of duties, cardiopulmonary resuscitation, triage systems, job stress, disaster management, and education programs. Conclusion: Based on the research results, it is believed that a study on the development and application of education programs that can successfully increase the emergency care capabilities of EMTs is needed.
