Multiple casualty disaster scene response management: a survey of 119 paramedics (119구급대원의 다수사상자 발생 재난 현장의 대응 역량에 관한 연구)

  • Purpose: The purpose of this study is to understand currently active Korean paramedics' disaster response abilities, including immediate response, severity classification, patient treatment, and patient transfer, in a disaster situation with multiple casualties. Methods: A structured questionnaire consisting of a total of 25 questions was used, including 5 questions on the subject's general characteristics and 20 questions on disaster-related emergency response abilities. Results: Among the disaster response abilities of the participants, the patient transport ability scores were high and the cooperative support ability scores were low. In terms of general characteristics, there was a significant difference in age, and it was high in the 40s, and there was a significant positive correlation between each competency. Conclusion: These results suggest that there is an urgent need to develop a systematic and specialized educational system with components inside and outside fire departments related to multiple casualty disasters to improve overall abilities.

  • Purpose: To evaluate the emergency medical protocols and need assessment through the case report Method: The study investigated the emergency call system through the 1339 emergency patients information center in 1997. Results: The emergency calls tend to follow general trends of the type of cases : abdominal pain (13.5%), unconsciousness(12.0%), traffic accidents (7.4%), bleeding (6.6%), respiration difficulty (5.9%). This was all collected through emergency medical protocols injury and poisoning (22.3%), symptom (51.6%), others (26.1%). Conclusion: The results suggest that a specific training program for emergency medical dispatchers should be established, and emergency medical dispatcher managers must review the cases on the basis of emergency medical protocols every year. Emergency medical services, furthermore, will continue to raise the standard for emergency medical dispatching.

A study on the cardiopulmonary resuscitation by the emergency medical dispatcher (구급상황관리사에 의한 심폐소생술 안내 실태 연구)

  • Purpose: The purpose of the study is to check up the status of 119 emergency control centers usage. Therefore, the status of use of 119 emergency control centers and the incidence of pre-hospital cardiac arrest patients were investigated. Methods: The emergency activity daily reports and first aid diaries of 119 emergency control centers from January to December 2018 were reviewed. For more accurate status analysis, Among the first aid guidance received in the emergency rescue standard system, the cardiopulmonary resuscitation guide log was reviewed. Results: In 2018, the total usage of the 119 emergency control centers was 1,358,356 calls, hospital guidance werethe most commom (n=629,676, 46.4%), followed by first aid (n=428,027, 31.5%), disease consultation (n=170,238, 12.5%), medical oversight (n=111,188, 8.2%), and interhospital transfer (n=5,052, 0.4%). Regarding the user number per 1,000 persons, Jeju was the greatest at 48.0, whereas Changwon was the lowest at 13.0. A total number of dispatcher-assisted cardiopulmonary resuscitation was 12.181. The time from report to chest compression were 156.2±80.8 seconds for those with previous cardiopulmonary resuscitation training and 168.0±79.3 seconds for those without such training (p<.05). Conclusion: The ratio of first aid instructions, including dispatcher-assisted cardiopulmonary resuscitation, among total usage of the 119 emergency control centers increased. Therefore, additional efforts are required to improve the quality and expertise of information provided through the 119 emergency control centers.

Change in the severity of patients visiting emergency medical centers through the 119 ambulance during the COVID-19 pandemic (COVID-19 기간 119구급차를 통해 일개 응급의료센터에 내원한 환자 중증도의 변화)

  • Purpose: Despite the fact that emergency medical centers (ED) are used by emergency patients, more than 50% of non-emergency patients have been reported to be admitted in EDs, of which more than 10% of them used the 119 ambulance. Therefore, this study investigated whether there was a change in the severity of patients visiting EDs during the first year of the COVID-19 pandemic. Methods: This was a retrospective study of patients who visited the ED through the 119 ambulance in 2019 (13,735) and 2020 (11,015). Patient data were analyzed using the medical information system. Results: There was an increase in non-emergency patients at the emergency departments (KTAS levels 4-5) in 2020 with a rate of 58.0% (6,393), as compared to the 2019 data with a rate of 44.9% (6,169). However, the 2020 median length of stay in the emergency department was 188 minutes (IQR: 0-5,909minutes) (p=.000), which was decreased as compared to the 2019 median length of stay of 231 minutes (IQR: 5-6,211minutes) (p=.003). Conclusion: Providing emergency ambulance guidelines for selecting proper hospitals and educating patients to refrain from using the ED for non-emergency patients should be taken into consideration to prevent overcrowding and construct a more effective emergency medical system (EMS).

A Survey on actual state or patients visited one emergency medical center (일개 응급센타에 내원한 일부 내원자의 실태조사)

  • We have reviewed 2.876 patients who visited the emergency room from May, 1996 to september, 1996 in order to look for a more appropriate management of the emergency medical system. The result were summarized as followings; 1. Considering sexual distribution, male patients were 59.6% more than female patients 40.4% and according to age categories, first decade of life was most frequent. 2. Distribution according to ca use is most common for the disease 79.8%, and next injury and traffic accident is 11.6%, 6.8%, respectively. 3. According to the department distribution, pediatrics accounted for 37.4%, internal medicine 21.1%, and emergency medicine 10.0%, and these three departments was 66.3% of total number of patients. 4. 67.0% of patients visited emergency room were discharged. Arrived time in emergency room, patients of 41.9% was visited from 17 to 1 hours. 5. Among 196 patients of traffic accident, 160 were male(81.5%) and 36 were female (18.5%). Distribution of the age is most common in 3rd and 4th decade for 63.2%, and region of injury were face 33.3%, head 22.2%, and chest 22.2% and follow-up measures after visiting emergency room is most common transfer hospital 40.8%.

Study of Personal Emergency Response System among Community Residing Elderly (119무선페이징의 이용실태에 관한 연구 - 일 지역을 중심으로 -)

  • Health and disease related characteristics of 226 selected by systematic sampling from 452 personal emergency response system(PERS) beneficiaries and actual conditions of using PERS by them are as follows. Over 86% of PERS beneficiaries have not good health conditions and 70.7% of them have chronic diseases. On social supports family was highest as 52.2% and cases having social workers' assistance were 15.2%, but 53 as 23.7% had not visit or call from anyone and showed very low social supports. 86.2% was given PERS within 3years and 79.1% had it by the recommendation from related agencies and 4.0% was by their demand. On wearing it, 78.3% didn't bring it with them and 92% of them answered they were not sick and then it was found that they didn't use it because they had not special emergency. On satisfaction with paging system's operation, 81.3% answered they were satisfied with it, 48.8% used it for 'acute and emergency diseases' and 29.3% called ambulance for 'appointed medical treatment'. Time required for ambulance to arrive at the field was within 10 min. in 87.8% and after 10 min. in 12.2% and emergency service for beneficiaries by fire service was very good. On satisfaction with use of PERS, 85.4% were satisfied with it, 81.9% who requested repair or replacement of radio paging got it back after one or two days of their request and they answered they were satisfied with A/S. 45.5% answered they powered off it because 'they didn't use it' and 12.1% had 'economical reason of phone charge'.

Comparison of Continuing Education Program for Emergency Medical Technician in Korea and Abroad (국내·외 응급구조사의 보수교육프로그램 비교조사)

  • Purpose: This study aims to investigate the operation of continuing education system and continuing education program for emergency medical technician in Korea, Japan and United States and develop reasonable operating scheme of continuing education and curriculum in order to provide the base data for the improvement of continuing education for the improved practice capability and its maintenance, Method: The overall review of continuing education for domestic 1st class emergency medical technician was performed and also the content of continuing education for Emergency Medical Technician - Paramedic in the United States and Paramedics of Fire Department with the license for the paramedic in Japan, have been analyzed through literature, books, articles, agencies' data, laws, and internet date. Result: Hours for domestic continuing education was 4-8 hours and it was only 3-11% compared to 72 hours in the United States and 128 hours in the Japan. And with respect to the types and methods of programs, there were differences both in quantity and quality. Conclusion: As an education, providing and supplementing the changed content and technical information for the improvement of the capability and qualification of emergency medical technician, selection of education hours and various continuing education should be continuously and regularly provided and conducted. The introduction of various continuing education system and programs for this is considered to be required.

  • Nurses are medical personnels under the Medical Service Act and perform medical practice such as medical assistance at medical institutions. The nurse, a medical personnel, provides emergency medical service to emergency patients in the pre-hospital emergency medical system as a 119 rescuer based on the Act on 119 Rescue and Emergency Medical Services. The scope of practice of nurses is comprehensively defined in the Medical Service Act and specified through precedents. In contrast, The scope of work of emergency medical technician is listed in detail. It is understood that nurses in the pre-hospital emergency medical service system have a wider scope of practice than emergency medical technician. In particular, the scope of practice of nurses as emergency medical personnel in the pre-hospital emergency medical system should be interpreted differently within the medical institution, considering the urgency of the patient, being transferred to the emergency medical institution, and the specificity of medical direction through tele-communication.

Under the current provisions in law concerning emergency care, and enforceable rules on enforcement issues and identify practical issues for consideration and improvements to enumerate the reasons for the demotion, the following conclusions can be sure until they improve. In addition, the National Assembly recently passed a bill AED use, and enforceable rules to enforce party that can teach AED necessarily a grade, including 1st EMT's to protect people's lives and to actively deal with the emergency situation so that it can be. Resolving these issues in the country where the emergency medical system can create a foundation for the advancement of emergency patients can get a good length of life, opens.

Factors Affecting Stress of 119 Emergency Staffs (119구급대의 스트레스에 영향을 미치는 요인)

  • The results of analyzing the questionnaires with 119 emergency staffs working in Gwangju and Jeonnam region are as follows. 1. The whole stress score was mean 2.94 and the degree of stress from personal relationship was highest as 3.22. 2. There were significant differences in possession of qualification related to public health and medical treatment (F=3.090, p=.030) in service factors, total career of emergency staffs (F=2.979, p=.023) in working environment factors, age (F=2.835, p=.042) and sex (F=2.375, p=.019) in social position factors, possession of medical qualification (F=3.995, p=.010) and household account burden (F=2.777, p=.045) in personal relationship factors. 3. There were correlations (p=.475) between working environment factors and social position (p<0.01) and correlations (p=.508) between working environment factors and personal relationship factors (p<0.01). There was correlation (p=.415) between social position and personal relationship factors(p<0.01). More concrete research that examines whether current fire station operation system is desirable or not by comparing the degree of stress of the whole 119 emergency staffs in Korea is needed and it is considered that real allowance level should be secured to reduce their stresses and facility and guarantee system to satisfy their demands for welfare are required.
