가정전문간호사의 비판적 성찰을 통한 사례기반 학습: 가정용 인공호흡기 적용대상자 (Case-based Learning through Home Care Nursing Specialists' Critical Reflection: Application with Patients Using Home Mechanical Ventilator)

  • Purpose: This study is know the home care nursing specialists' copying method for life-threatening emergency situation occurred while changing the tracheotomy tube in the patient's home through case-based learning. Methods: Two cases were analyzed using structured critical reflection while changing the tracheal tube. In the first case, the patient presented with dyspnea, cyanosis, and low oxygen saturation when connected to home mechanical ventilator after replacement of the tracheotomy tube. In the second case, replacement of the tracheal tube was difficult due to a narrowed airway from proliferated granulation tissue. Results: From the case-based learning. using a critical reflection instrument, home care nursing specialists indicated that it was important to explain the possibility of danger to the client and that nurses must check the operation of the ambu-bag to prepare for an emergency when replacing a tracheotomy tube. Moreover, they stated a need for two nurses during client visits when there is the possibility of an emergency. Conclusion: Case-based learning through critical reflection provides actual practice-focused knowledge that is helpful to home care nurse specialists who face emergency situations in a restricted environment. Therefore, we hope that home care nursing specialists will use this method to strengthen their professional knowledge.

우리나라 보건의료법령에 명시된 간호에 관한 연구 (A Study on the Nursing Profession as Stipulated by Health & Medical Laws of Korea)

  • The purpose of the study is to find out how laws related to the nursing profession can be improved by analyzing the rules and regulations concerning nursing. Furthermore, to help settle legal matters in the process of doing nursing work. The data used for the study are the Health and Medical Act, the Maternal and Child Health Act, the School Health Act, the Special Act for Health and Medical Service in Rural Areas, the Industrial Health & Safety Act and the Notice on Nursing Professional Courses analyzed by age and content. The results of the study are as follows : First, basic nursing practice includes 'nursing care for recuperation and assistance in medical treatment and in special areas including the pre-vention of disease, maintenance of health, control of environment, and other therapeutic activities. It is suggested that the phrase 'assistance in medical treatment' should be eliminated as it limits the basic nursing practice to the assistance of the medical treatment. Second, Article 56 of the Health & Medical Act prescribes a special nurse but it does not prescribe a specific job. Accordingly, the new provison concerning the specific jobs of a special nurse should be added or a job guide should be inseated. Third, it is prescribed that those who have completed the training course after obtaining a license are qualified to be a midwife, a special nurse and a nurse practitioner working in special areas. However, school nurses, occupational health nurses and maternal and health workers are required to obtain a nurse license, but not to take an additional training course. Nurses working in special areas should be legally recognized as nurse specialists. The regulations to control various qualification standards consistently should be established. Fourth, the qualifications and types of nurses by area prescribed by Article 54 of the Health and Medical Act are not consistent with those of special nurses as recognized by affiliated organizations of the Korean Nurse Association and some hospitals. Accordingly, the qualifications and types of special nurses should be adjusted in consideration of special nurses. Fifth, as Article 16, Paragraph 2 of the Health and Medical Act does not prescribe the type and scope of first - aid treatment that nurses can provide, the first-aid treatment of nurses might be considered as an unlicensed practice. The specific regulations regarding these matters should be established. Sixth, the contents of the nursing record, which are prescribed by Article 21 of the Health and Medical Act as a duty, include 1) matters concerning body temperature, pulse, breath and blood pressure 2) matters concerning drug prescription 3) matters concerning input and output 4) matters concerning the treatment and nursing care (Article 17 of the Enforcement Regulations, Health and Medical Act). However, these matters are limited to basic nursing care and assistance in medical treatment. The new recording methods on nursing process are suggested to be adopted legally. Seventh, the prescription right entrusted to nurses which are prescribed by the School Health Act, the Special Act on Health and Medical Service for Rural Areas, and the Industrial Health and Safety Act are not consistent with the rights of nurses as prescribed by the Health and Medical Act. New regulations prescribing the partial right for medical treatment entrusted to nurses in consideration of the restraint of time and place in emergency situations should be established.

간호협회지를 통해 본 보수교육의 역사적 경향 1962년 ~ 1995년 (A Study of Trends in Continuing Education Published in the Korean Nurse)

  • This historical research was conducted to analyze and categorize the titles which were presented in the journal, The Korean Nurse, from August 1962 to October 1995. Titles which were published with the purpose of educating graduate nurses and to update 0 their nursing knowledge to improve professional practice were included. There were 348 articles published from the beginning of publication in August, 1962 to October, 1995. All of the journals were reviewed except nine missing journals which were not available in any library. According to the characteristics of the articles in the periodical, the articles were divided into three periods. In each of the three peroids there were five categories: Subject, Clinical Practice, Fundamentals of Nursing Science, Nursing Administration, Others. These categories were adopted from Kim's(1994) division system which was developed to analyze nurse's insurance education program. The special feature peroid was from August, 1962 to December, 1974. In this period the articles were presented in an unorganized manner in the area of special feature or main issue. The largest area was the subject category(44%). The second largest area wes the fundamental of nursing science category(31%). From May, 1975 to December, 1977, the articles with the educational purposes were published in a designated area called continuing education. This period was labelled as the continuing education period. Among the published articles in this period, 45% focused on the subject category and 45% on the fundamentals of nursing science category. In this period the articles were focusing on nurses 'work in specific areas such as industry, nurses' aid schools, and nursing administration, articles on physical assessment first started to appear. The written continuing education period was from January 1978 to October, 1995. All the educational articles published in this area were analyzed and categorized into five areas as for the other periods. 48% of the articles focused on the subject category. In the mid-eighties, the term nurse specialist first.appeared and ten years later in 1990, the titles were subdivided into more specific titles, such as, home nursing, industrial nursing, emergency nursing, 23% were in the fundamental nursing science category and they dealt with nursing process, nursing theories, theory development. For the content analysis, three articles, one from each period, dealing with cardiovscular diseases were selected for comparsion. First, the special feature period, the title of the article was on diet therapy for cardiovascular disease patients, but instead the content was about rest, hygiene, medication, observation. They recommended rings to prevent bed sores, which is now considered as a cause for bed sores. In the continuing education period, the content was more focused on rehabilitation rather than general nursing .care. It became more specific, systematic, and organized compared with the special feature period. In the written continuing education period, the focus was on rehabilitation, not broadly, but very specifically on exercise. The further research on the content analysis is recommended along with a comparison of the trends in the Journal of Nurses Academic Society.

응급구조과 학생의 구급차동승실습 만족도에 영향을 미치는 요인 (Influencing factors on satisfaction in ambulance ride practice of paramedic students)

  • Purpose: The purpose of this study was to provide an appropriate direction for effective field practice program by analyzing the influencing factors on satisfaction in ambulance ride practice of paramedic students in Korea. Methods: This study analyzed of 180 paramedic students from 20th to 30th september, 2019. The collected data were analyzed with MedCalc 18.1.0 program. Results: The results of the satisfaction of ambulance riding practice according to the degree of procedure practice are as follows. Satisfaction was high in all groups except the practice time. The lowest satisfaction rate was seen in the category of'There were many cases of applying theories learned at college'within the variable of practice environment(2.82±0.82). The factors affecting the satisfaction of ambulance riding practice were the nurse (β=-0.1472, p=.024) and emergency medical technician(β=-0.3318, p=.002) according to the ambulance worker qualification paramedic, and the practice score (β=0.0048, p<.001). Conclusions: For effective ambulance riding practice, it is necessary to be able to select the practice place where many cases can be experienced, the appropriate selection of the practice leader, and the degree of procedure practice.

중소병원 응급실 간호사의 감정노동, 직무스트레스가 환자안전관리활동에 미치는 영향 (The Impact of Emotional Labor and Job Stress on the Patient Safety Management Activities among Emergency Room Nurses in Small-Medium Sized General Hospitals)

  • 본 연구는 중소병원 응급실 간호사의 감정노동, 직무스트레스가 환자안전관리활동에 미치는 영향을 규명하고자 시도되었다. 중소병원 응급실에 근무하는 간호사 143명을 대상으로 자가보고 설문지를 이용하여 자료를 수집하였다. 환자안전관리활동 영향요인으로 감정노동, 직무스트레스, 인구사회학적특성, 근무특성 등을 조사하였다. 변수 특성에 따른 환자안전관리활동 차이는 평균비교분석, 관련성은 상관분석을 이용하였으며, 환자안전관리활동 영향요인은 단계적 회귀분석을 이용하였다. 단변수분석 결과 연령, 응급실 경력, 직위, 근무형태, 타 부서 이동의향이 환자안전관리활동에 유의한 차이가 있었다. 직무스트레스는 환자안전관리활동과 유의한 중간 정도의 양의 상관관계가 있었으며(r=.48), 감정노동은 환자안전관리활동과 유의하지만 약한 양의 상관관계가 있었다(r=.19). 회귀분석결과 환자안전관리활동에 유의한 영향을 미치는 변수는 직무스트레스, 응급실 근무 선택 여부였고, 이들 변수의 설명력은 27%로 나타났다. 직무스트레스가 높을수록, 응급실 근무가 타의 선택일 경우 환자안전관리활동이 감소하는 것으로 나타났다. 따라서 중소병원 응급실 간호사의 환자안전관리활동 향상을 위해 직무스트레스 관리와 개인의 요구에 따른 부서 배치가 필요하다.

간호사가 지각하는 간호조직문화와 조직몰입간의 관계 분석 (Analysis of Relationship between Nursing Organization Culture and Organizational Commitment)

  • Nursing organization culture is common patterns of believing, thinking and behaving resulted from a variety of experiences and interaction nurses in the same setting. This study was performed based on the assumption of existence of different subcultures within meta-culture, to identify the differences of subculture among 5 nursing units and to analyze the relationship between nursing organization culture and organizational commitment In this study, two kinds of instruments were used. One was the instrument of nursing organization culture developed by researcher through literature review and interview with nurses. The other one was Mowday's Organizational Commitment Questionnaire to measure organizational commitment of nurses. Both of them were structured with 5 scale. The answers were analyzed using SPSS program. The results were as follows : The meta-culture of the nursing organization culture was the people stable culture. There were significant differences in people development culture and people stable culture among 5 nursing units and all 5 units had different culture score. Especially, emergency room had strong development culture, and stable culture was dominant in operating room. Other units except emergency room and operating room had high scores in people stable culture than other culture types, but revealed different distribution. There were significant differences of the nursing organizational culture types -people development, task development, people stable, task stable-among 5 units. Four types of nursing organizational culture consisted of competing values in one organization. Nurses's organizational commitments were sign ificantly different among the units. The score per ceived by nurses who work in emergency room, operating room and leu was higher commitment score than that of medical/surgical nursing units Nurse's commitment to nursing organization was also related to total work period as nurse in any setting and work period in this hospital. Organizational commitment was significantly different among the nursing culture types, indicating that the scores of developmental culture were higher than stable culture. In conclusion, there were many different subcultures in nursing organization. In subculture, the organizational commitment was different. Therefore, the change of nursing organization culture or nursing unit culture needs to be considered to hire, give orientation, teach. and reallocate nurses efficiently. Research on nursing organization culture using both qualitative and quantitative method needs to be further considered. Furthermore, the strategy in nursing organization culture for nursing administrator to manage human resources efficiently and to change nursing unit effectively, needs to be developed.

한국형응급환자분류도구를 적용한 응급실에서 소아 환자의 중증도 분류 정확성 (Triage Accuracy of Pediatric Patients using the Korean Triage and Acuity Scale in Emergency Departments)

  • 본 연구는 한국형응급환자분류도구(Korean Triage and Acuity Scale: KTAS)를 사용한 소아 중증도 분류의 정확성을 파악하기 위한 후향적 조사연구이다. 연구자료는 2016년 10월부터 2017년 9월까지 1개 권역응급의료센터, 1개 지역응급의료센터에 방문한 소아환자의 자료 중 무작위로 추출한 250건의 간호초진기록지와 진료결과였다. 수집된 자료를 검정된 전문가가 분석하여 true-triage를 정하였다. 중증도 분류 정확도는 응급실간호사의 중증도 분류 결과와 전문가의 true-triage결과와의 일치도로 평가하였다. 전문가 의견에 따라 중증도 분류 오류의 원인이 분석되었고, KTAS 등급과 퇴원, 체류시간, 진료비와의 연관성이 비교되었다. 연구결과 전문가와 응급실 간호사의 중증도 분류 등급은 높은 일치도를 보였다(weighted kappa=.77). 중증도 분류 불일치의 원인 중 활력징후 결과를 KTAS 알고리즘 기준에 잘 못 적용한 경우가 가장 많았다(n=13). KTAS 1,2 등급과 같이 중증도가 높을수록 퇴원이 적었다(${\chi}=43.25$, p<.001). 연령을 보정했을 때 KTAS 등급에 따라 체류시간(F=12.39, p<.001)과 진료비(F=11.78, p<.001)는 차이가 있었다. 본 연구결과 KTAS는 국내 응급실에서 높은 정확도를 보였으므로, 새로 개발된 중증도 분류 도구가 국내 응급실에 잘 적용되고 있다고 할 수 있다.

COVID-19 팬데믹 상황에서 응급실 간호사의 소진에 영향을 미치는 융합적 요인 (Convergence factors Affecting Burnout of Emergency Room Nurses During the COVID-19 Pandemic)

  • 본 연구는 응급실 간호사를 대상으로 COVID-19 대유행 상황에서 COVID-19 스트레스, 대인관계(보호자-환자) 스트레스, 감정노동이 소진에 미치는 영향을 파악하기 위한 서술적 조사 연구이다. 자료수집은 H 의료원 산하기관 5개 상급종합병원과 종합병원 응급실 간호사를 대상으로 2021년 12월 9일부터 12월 23일까지 시행되었다. 대상자의 일반적 특성과 직무 관련 특성, COVID-19 스트레스, 대인관계(보호자-환자) 스트레스, 감정노동, 소진을 측정하기 위해 자기기입식 설문지로 진행하였다. 수집된 자료는 SPSS/WIN 25.0 통계 프로그램을 이용하여 빈도 분석, 기술통계분석, Independent sample t-test, One-way ANOVA, Scheffé test, Correlation analysis, 다중회귀분석을 실시하였다. 응급실 간호사의 COVID-19 스트레스의 평균은 3.64점, 대인관계(보호자-환자) 스트레스 4.35점, 감정노동 3.38점, 소진 3.44점이었다. 일반적 특성 및 직무 관련 특성에 따른 간호사의 소진은 성별, 결혼상태, 총 임상 경력, 근무 기관 형태에 따라 유의한 차이를 보였다. 소진은 COVID-19 스트레스, 대인관계 스트레스, 감정노동과 모두 유의한 정적 상관관계를 보였다. COVID-19 팬데믹 상황에서 응급실 간호사의 소진에 영향을 미치는 융합적 요인으로는 근무 기관 형태 중 권역 응급의료센터, 지역응급의료센터, 대인관계 스트레스, COVID-19 스트레스, 성별로 나타났고 설명력은 28.6%로 나타났다. 이와 같은 결과를 통해 신종감염병 대유행 시점에서 응급실 간호사의 소진을 예방하고 간호업무성과를 높이기 위한 중재프로그램 개발을 위한 기초자료를 제공하고자 한다.

초등학생의 응급처치 교육에 대한 인식 (Cognition about First Aid Education in Elementary School Students)

  • Purpose: The purpose of this study was to provide basic data to develop first aid education programs for elementary school students. Method: A self-report questionnaire was used to collect data from 720 5th and 6th grade elementary school students. Results: 1) The mean score for cognition of first aid education of the students was high with a score of 2.64 $({\pm}.30)$. 2) The mean scores for the 11 categories were, "Fire & Burns", 2.78 $({\pm}.40)$, "Poisoning", 2.77 $({\pm}.47)$, "Thermal injuries", 2.75 $({\pm}.51)$, "Rescue & moving", 2.73 $({\pm}.37)$, "Bites", 2.72 $({\pm}.44)$, "General first aid", 2.64 $({\pm}.37)$, "Wounds", 2.59 $({\pm}.39)$, "Removing foreign bodies", 2.58 $({\pm}.46)$, "Cardiopulmonary resuscitation", 2.57 $({\pm}.59)$, and "Musculoskeletal injuries" and "Others", 2.54 $({\pm}.51,\;{\pm}.53)$. 3) There was a significant difference in the cognition of first aid education according to student's gender (t=-3.012, p=.003), and judgement about the emergency situation (F=3.411, p=.034). Conclusion: The results indicate the necessity of developing effective first aid education programs for elementary school students.

보건과학분야의 마네킹 시뮬레이션 교육 (Mannequin Simulation in the Health Science)

  • 본 논문은 보건과학분야에서 활용되는 마네킹시뮬레이션 교육을 알아보고자 한다. 마네킹 시뮬레이션은 의과대학 교육뿐 아니라, 응급구조학과 교육, 구조구급 분야에 종사하는 소방공무원, 간호사, 치과대학, 마취과학등 의료의 여러 영역에서 다양하게 이용되고 있다. 본 연구는 시뮬레이션의 역사, 시뮬레이션의 진화과정, 시뮬레이션을 이용한 문제중심학습(problem-based learning, PBL)의 활용에 이르기까지 마네킹시뮬레이션 교육의 현황을 설명하고자 한다.
