  • 전자의무기록(EMR)[1]을 도입한 이후 현재 의료장비의 EMR 연동상황이 극히 미비한 상태로 대두되고 있다. 종합병원 의료장비의 약 30%정도는 검사 결과물이 출력이 안되고 보여지기만 하는 구조로 되어 있어 직접 수기로 EMR 에 연동할 수 있는 소프트웨어에 입력하여 EMR 에 연동하고 있으며, 약 60%는 의료장비에서 프린터 혹은 시리얼통신으로 데이터를 출력할 수 있는 구조로 되어있다. 하지만 의료장비의 노후화 및 검사결과물의 인터페이스 특성이 다양하기 때문에 쉽게 연동하지 못하고 있으며, 대부분 종이 출력 결과물을 스캔을 통해 이미지를 저장하거나 받아서 EMR 에 연동하고 있다. 그 외 나머지 10%는 의료장비 자체의 저장장치 또는 네트워크를 통해 검사결과 데이터를 EMR 에 전송하는 구조로[2] 되어있다. 본 논문에서는 의료장비의 프린터를 통한 결과물을 EMR 에 연동하기 위한 인터페이스 솔루션을 구현하고, 검사결과의 출력 데이터를 이미지 복원 및 관리하는 방법을 통해 많은 수의 의료장비의 출력 데이터를 EMR 연동을 하여 인력 및 기타 사무용품의 소모를 줄이며, 검사결과를 실시간 진단할 수 있는 방법을 제시한다.

A Study on the Development of a Selection System for Preservation Formats of Image-Type Electronic Records (이미지 유형 전자기록물의 보존포맷 선정체계 구축방안 연구)

  • Electronic records, characterized by their inherent volatility and instability, necessitate sustainable preservation measures to ensure their long-term accessibility. The National Archives of Korea has instituted a selection system for preservation formats tailored predominantly for document-type electronic records. However, this system falls short in accommodating other record types such as audiovisual records. This study endeavors to broaden the applicability of the existing system, with a concentrated focus on image-type electronic records, and to formulate foundational guidelines for their long-term preservation. In South Korea, image-type electronic records rank as the second most prevalent category following document-type. The image-type electronic records are the most basic form of audio-visual records, and research on this lays the foundation for future discussions on other audio-visual records. Consequently, this research has led to the development of a selection system for preservation formats specifically for image-type electronic records. This system is designed to facilitate the prompt and efficient evaluation of preservation format suitability, even in the context of emerging image formats. The efficacy of this system was validated through its application to extant image formats, resulting in the selection of TIFF, JFIF, and PNG as the optimal preservation formats. The outcomes of this study offer valuable insights and practical reference points for future preservation format evaluations within the field of electronic record management.

A Study of Standard Model for Electronic Document Archiving (전자문서 아카이빙 표준모델 연구)

  • Requirements concerning production of electronic documents and storage are stipulated in the act on document management of public institutions revised in 2003. However, provisions or standards for long term preservation of electronic documents are insufficient and in need of strengthening. This study aims to provide standard factors for long term preservation of electronic documents and thus lay foundation for long term preservation related matters for the establishment of management strategy, ISO 15489 management factor is analyzed as a necessary framework for long term preservation of electronic record at a production stage. Preservation description information is derived from ISO 14721 which is suggesting document management systems to archival institutions. Through this case study, standard registry factors reflecting ISO 15489 and 14721's are suggested in an attempt to improve the act and system environment for long term preservation and archiving.

  • HL7 released V3 CDA(Clinical Document Architecture) and V2.x message standards for medical information exchange. Currently, these standards are successfully adopted by a number of nations across the globe. However, substantial amount of time is required to develop and implement these standards. Moreover, developers need a lot of time to understand these standards. To solve these issues from 2011, the HL7 standard framework started to discuss Fast Healthcare Interoperability Resources(FHIR) as next generation standard of healthcare information exchange. People's interests toward personal health record and smartphone penetration rate are growing and increasing rapidly. Therefore, our research team believes it is necessary to develop a PHR profiling system which could be accessed by using a smartphone and we developed the system. Through a FHIR Profile editor tool developed in Furore, we found that improvements could be made in generating and changing the profile. In order to build the PHR Profiling system, an Open-API on FHIR is used for exchanging information between electronic medical record system and PHR Profiling system. In the PHR Profiling system, the transactions of information between two systems are provided by RESTful service. In this study, we verify the efficiency of development of the PHR Profiling system through FHIR.

  • This study is a case study for in-house developed Record Maintenance and Management (RMMS) which is a digital data storage/retrieval system in a GIS-enabled solution. RMMS is designed for reorganizing submitted architectural drawings associated with scanned drawings files & geodatabase, managing metadata/table and geometric features in ArcSDE/Enterprise geodatabase format. The birth of the RMMS was borne out of a necessity for a modernized approach in digital file basis to provide improved customer services for various architectural drawings and associated with geo-spatial and its attribute information. Through a case study for building permit & review practice implemented on an internet-based Electronic Architectural administration Information System (EAIS) at the local governments in Korea, this study is able to derive differences between GIS-enabled RMMS system implemented in the Richmond City and internet-based EAIS system implemented in the local government in Korea. In an effort to be the most effective, it presents meaningful ways of maximizing efficiency in record maintenance & management system derived from the case studies that looked into in ways of a method in RMMS' operation, implementation on GIS platform, mutual interface among various programs with various digital files and finally technical supports and system development /upgrade.

Use Case Development for Next Generation Electronic Nursing Record Systems Utilizing Clinical Workflow Analysis and a Delphi Survey (차세대 전자간호기록 시스템 유스케이스 개발: 업무흐름 분석과 전문가 델파이 기법 적용)

  • Purpose: To identify user requirements for electronic nursing record (ENR) systems so as to ensure system usability. Methods: A mixed methods approach were applied in three steps : (i) task and workflow analysis with literature review of nursing documentation, (ii) literature reviews of system usability, and (iii) Use Case idenfication and consensus-based validation. We analyzed the nursing activity logs collected from a time-motion investigation of six hospitals. The Use Cases were validated by eight clinical experts from different hospitals and two experts from academia in a sequential Delphi survey. Consensus was achieved for the significance score and agreement among the panel. Results: Eight task groups and patterns of task flow were observed, which were translated into nine Use Cases. The specification of Use Cases was derived from principles, guidelines, and recommendations on nursing documentation and electronic health record systems, which was organized into three requirements of each Use Case: functionality, information, and design characteristics. Each Use Case achieved an agreement of 50~70%, and significance scores of 4 or 5 on a 5-point Likert scale. Conclusion: The nine Use Case identified were considered to be important and adequate in terms of both clinical and informatics contexts.

  • The evolution of information technology and proliferation of hospital management and managerial applications of computing has led to change in the characteristics, uses and evaluations of software for the hospital management. With the growing proliferation of microcomputer use and the value-added for management strategies, more and more software has been massively developed, produced and distributed for the hospital industry. The user is faced with an increasingly difficult choice in the evaluation and selection of software. For many reasons, users frequently must rely on expert evaluations of the technical functions and quality of software. The objectives of this study are to provide selection criteria for an Electronic Medical Record (EMR) and to develop an evaluation framework for the Hospital Information Systems. The major findings of our study are as follows (1) the identification of EMR evaluation characteristics (2) the design and development of EMR selection model and (3) the evaluation of the importance for EMR characteristics using Analytic Hierarchy Process (AHP). We identify 6 characteristics and 22 sub-characteristics of the EMR, calculate their weights, and decide the best configuration. Especially, the AHP methodology can be applied to gather knowledge from multiple experts. Because AHP can 1) facilitate the participation of multiple experts 2) increase group productivity and therefore result in both quantitatively and qualitatively superior outcomes than that of a single individual's work 3) provide a mechanism for reconciling conflict from multiple expert 4) validate the acquired knowledge, providing consistency of facts, and 5) enhance the accuracy reliability of the acquired knowledge increase through of the reliability provided by consensus across multiple experts. Although some further research is required, the proposed model can be regarded as a basis for the selection of EMR.

Analysis of Standard Nursing Statements Recorded in an Electronic Nursing Record System and User Satisfaction (전자간호기록에 사용된 표준간호진술문 활용실태와 시스템 사용자 만족도)

  • Purpose: The aims of this study were to analyze the frequency of standard nursing statements used in the Electronic Nursing Record (ENR) and to evaluate the degree of satisfaction by users of the ENR system. Methods: We retrospectively reviewed the ENR of 1914 patients who were admitted to our center between 1 May 2011 and 31 May 2011. Additionally, we collected questionnaires from 100 doctors and 300 nurses to evaluate the satisfaction of the users. Results: The frequency of use for the following standard nursing statements was investigated: standard nursing assessment statements (43.6%), standard nursing diagnosis statements (61.8%), standard nursing plan statements (46.7%), standard nursing intervention statements (56.9%), and standard nursing evaluation statements (41.7%). The mean satisfaction score was 3.03 out of 5 in the nurse's group, and 3.11 in the doctor's group. The nurses said the advantages of the ENR system were as follows: easy to access, informative, and standardized terms. However 75.7% of the nurse answered that they cannot express actual nursing situations exactly with the currently limited standard nursing statements. Conclusion: Development of various standard nursing statements is needed to meet the demands of the users. As a result, the use of the ENR system would become easier and more efficient for its users.

  • This study aims to propose the adoption of a business management system, as well as suggest the functions and development directions for public organizations, which are required to establish the record management and information disclosure system under Government 3.0, rapidly respond to the needs for strengthening the responsibilities for explanation, and improve work efficiency. Recently, some of the public organizations that introduced the record management and information disclosure system adopted the Electronic Document System, which focuses on the function of electronic approval, and developed a records classification scheme for the system. This study aims to review the case of A organization, which recently developed an in-house records management system and established information strategy planning to adopt a customized business management system after establishing a business reference model throughout the organization, and suggests the directions of the electronic record production system for public organizations.

Development of a Flexible Critical Pathway with Electronic Medical Record for Gastrectomy Patients in a University Hospital (위 절제술 환자의 진료계획표 개발 및 전자 의무 기록화)

  • Objectives : This study was conducted to evaluate the effect of fixed critical pathway with emr (electronic medical record) on the length of hospital stay, the cost and quality of care provided to gastrectomy patients in a university hospital and to develop flexible critical pathway with emr which can be used excluded or drop-out patients. Methods : Thirty-eight patients with gastrectomy were included as case group and Thirty-four patients included as control group. The comparison between control and case with using fixed critical pathway were done. To develop and to evaluate usefulness of flexible critical pathway with flexible data base, simulation was done for flexible critical pathway with drop-out patients. Result : The major results of this study were as follows: There were no significant differences in patient clinical conditions and no sign of deterioration of quality from critical pathway. The length of hospital stay was 11 days in control group, 8 days in path group(P<0.01). The total costs during the hospital stay were reduced in path group. However the cost per day was significantly increased from reduction of hospital stay(554,352 won in control, 645,669 won in path group). One hundred percentage of drop out patients(60) in the simulation of flexible critical pathway was successful. Conclusion : Computerized critical pathway reduced the length of hospital stay, total hospital costs and resource utilization without harming quality of patient care. The flexible critical pathway program can be used as one of the powerful management tools for reducing the practice variations and increasing the efficiency of care process and decreasing the workload of doctors and nurses in Korean hospital settings.
