전자의무기록 기반의 심폐소생술금지 동의서의 전자서식 개발 (Development of an Electronic Document for DNR Informed Consent based on the Electronic Medical Record System)

  • Objectives : This study developed an iPad-based animation for an electronic informed consent to directly help patients prepare an informed consent. The goal was to raise patients' understanding about the contents contained in a DNR informed consent in the current medical situation in which DNR informed consents are mostly written by a guardian. Methods : The development of a DNR electronic informed consent was done in 3 stages: analysis, design and development. The analysis stage was done with a survey on the real status of preparing a DNR informed consent in a medical institution. The design stage was done with the contents in the DNR electronic informed consent through a primary and secondary Delphi survey. The development stage created a DNR electronic informed consent and evaluated it through a tertiary Delphi survey. Results : After evaluating the appropriateness of the composition of the contents, the understanding of the contents, the convenience of use, the reflection of an expert opinion, and the suitability of the application, all had scores higher than 4 points. Conclusions : The results of this study show that our proposed DNR electronic informed consent can help patients better understanding the contents of a DNR informed consent.

한방병원의 봉약침 시술 동의서의 사용 현황과 표준 시술 동의서 개발에 대한 제안 (Usage of informed consent form for Bee-venom pharmacopuncture Therapy at korean medicine hospitals and Proposal for development of a standard informed consent form.)

  • Objectives: We investigated the current status of the consent form for bee-venom pharmacopuncture therapy, which is using in Korean medicine hospitals. We suggest the development of a standard informed consent form. Method: Through the questionnaire survey, status of using informed consent form was identified at 24 Korean medicine hospitals. We analyze different types of informed consent form, which was developed by each hospitals. We investigated the types of informed consent forms for various medical procedures through electronic searches. A standard informed consent form for bee-venom pharmacopuncture therapy was developed based on the medical law and the standard informed consent form for medical procedures developed by Korea Fair Trade Mediation Agency. Result: In our survey, 65% of the hospitals do not use consent well, only 35% of the hospitals use informed consent form, and the most hospitals use self-developed informed consent form. As a result of analyzing the contents of informed consent form used in each hospitals, the explanation of diagnosis, treatment precautions, suggestions for other treatments, consequences of not performing the scheduled procedure, possibility of treatment change was insufficient. 48% of hospitals manage consent in recording on a chart, 39% in scanned documents, and 13% in digital electronic consent form. Conclusion: A standard informed consent form for Bee-venom pharmacopuncture therapy include diagnosis, effectivness, necessity, indications, method, skin reaction test, hypersensitivity questionnaire, treatment precautions, possible hypersensitivity reactions and countermeasures, suggestions for other treatments, consequences of not performing the scheduled procedure, possibility of treatment change and the name of doctor.

요양병원 입원환자의 애니메이션 모바일 전자동의서 적용: DNR 동의서를 중심으로 (Application of Animation Mobile Electronic Informed Consent in Inpatient of Long-term Care Hospital: Focused on DNR Informed Consent)

  • 본 연구는 요양병원 입원환자를 대상으로 애니메이션 모바일 DNR 전자동의서를 적용한 후, DNR 전자동의서의 내용이해도와 사용편리성을 평가하고자 시도되었다. 2014년12월1일부터 16일까지 요양병원 입원환자 100명을 대상으로 DNR 전자동의서를 적용하였으며, 자료분석은 SPSS 21.0K를 이용하여 빈도분석, 교차분석, t-test와 ANOVA를 실시하였다. 연구결과는 첫째, 향후 심정지가 발생했을 때, DNR 선택 33.0%, CPR 선택 54.0%로 나타났으며, 주진단에 따른 DNR과 CPR 선택에 통계적으로 유의한 차이가 있었다. 둘째, DNR 전자동의서의 내용이해도는 2.50점, 사용편리성은 2.37점이었다. 이상의 연구 결과를 근거로 애니메이션 전자동의서가 동의서 내용에 대한 이해를 높여서 환자 스스로 DNR 및 CPR 시행을 결정하고, DNR 동의서를 직접 작성하는데 도움을 줄 수 있을 것으로 사료된다.

의료기관에서 간호사와 환자의 전자동의서 사용 경험: 질적 연구 (Nurse and Patient's Experiences Regarding the Use of Electronic Informed Consent in Hospital: A Qualitative Study)

  • 본 연구의 목적은 의료기관에서 간호사와 환자의 전자동의서 작성 경험을 파악하고자 하는 것이다. 본 연구는 포커스 그룹 인터뷰를 적용한 질적 내용 분석 연구이며, 연구 참여자는 간호사 30명과 환자 27명이었다. 포커스 그룹 인터뷰와 심층 면담을 통해 자료를 수집하였고 내용분석 하였다. 본 연구결과 전자동의서 사용 경험과 관련된 주요 주제는 '전자 동의서 사용에 대한 유용성', '전자동의서 태블릿 PC 사용과 관련된 불편함', '전자동의서 기능향상에 대한 바람'이었다. 참여자들은 전자동의서는 간편하고 편리하며, 자원과 공간이 절약되고, 향상된 전자시스템의 기능이 있어 만족하나, 예기치 못한 기계의 오작동이 생기거가 충전 펜과 고정형 모니터를 사용할 때 또는 사용법이 익숙하지 않은 경우에는 불편하다고 하였다. 참여자들은 추후 점차 사용이 확대되고, 친필과 유사한 필체, 화면 확대 조정 및 시청각 보조 기능이 지원되기를 원했다. 전자동의서 사용자들의 불편함을 최소화하고 만족도를 최대화하기 위한 노력이 필요하다.

  • The purpose of this study is to find out the most successful way for the protection of medical information focusing on the electronic medical record(EMR). In this study, every aspect of the EMR is reviewed in terms of the hospital management. In particular, definitions, major functions, strengths and weaknesses of the EMR are considered. This study also examines the general development of the EMR as well as the current situation of applying the EMR. Important issues such as the protection of patient Medical information, informed consent, and the customer-oriented hospital information system are discussed and interpreted in light of the introduction of the EMR into the area of the hospital management. Finally, in this paper Protection of medical information by major Issues on Patient medical information.

이형협심증 환자의 혈관내피세포 기능저하에 영향을 미치는 심혈관질환 위험인자 (Cardiovascular Risk Factors Predicting Endothelial Dysfunction in Patients with Variant Angina)

  • Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of variant angina, and to examine the predicting factors on the vascular endothelial dysfunction of the patients with variant angina. Methods: A total of 134 patients diagnosed with variant angina were recruited from 2006 to 2008. The degrees of endothelial dysfunction were measured and recorded by the researcher using the values of flow-mediated vasodilation of their brachial arteries and Nitroglycerine-mediated dilation. Subjects' demographic data and risk factors were gathered after obtaining informed consent, and their electronic medical records were reviewed to collect laboratory data. Results: The mean age was $54.2{\pm}9.6$ years and 52% was male patients. More than 50% of the male patients were cigarette smokers and had hypercholesterolemia. 84% of the male patients and 70% of the female patients had more than one risk factor of cardiovascular disease. A stepwise multiple regression analysis showed that smoking and hypercholesterolemia predicted the decrease of flow-mediated vasodilation (Adjusted $R^2$ = .204, p < .001). Conclusion: Tailored educational interventions for smoking cessation and cholesterol management are needed to prevent recurrence of angina attack for patients with variant angina and to prevent cardiovascular disease for middle-aged workers.

산림 치유 관련 논문에 관한 체계적 고찰 (Systemic Review on Forest Healing Journals)

  • This review was conducted to investigate clinical trial trend, clinical effect, and study methods of forest healing. Literature search was performed using the search engines of eight electronic databases from inception to march 2010 without language limitation. The search terms were "forest healing", "forest experience", "forest therapy", "forest treatment", "clinical trial", and "clinical study". Inclusion criteria were randomized controlled trials (RCTs) among all kinds of study searched about forest healing. Selection literature and data extraction was performed by authors(SH Sun and SG Lee) independently. Total three studies were selected finally. The methodological quality in all study was low because of poor reporting. Study indications were metal health, depression, self-esteem, and stress. The scale of studies was very small, which was the level of pilot study. There were no description for obtaining informed consent and being reviewed by Institutional Review Board (IRB), and could be any problems for statistical method in any studies. It is difficult to accept the fact that each studies had the significant effect for study indications in the results of studies. It is concluded that the evidence that health healing have a significant effect is insufficient. Further systematic and methodological study and well-designed clinical trial will be needed.
