Miller-Fisher 증후군에서 나타난 지연형 안면마비 및 현훈 치험 1례 (A Case of Delayed Facial Palsy and Dizziness in Miller-Fisher Syndrome)

  • Objective : Miller-Fisher syndrome(MFS) is a type of acute inflammatory polyneuropathy, a disease characterized by abnormal muscle coordination, absence of tendon reflexes, and paralysis of the eye muscle. This study is to report a case of delayed facial palsy and dizziness in Miller-Fisher syndrome, treated with traditional Korean medicine. Methods : A 57-year-old male was diagnosed with Miller-Fisher syndrome accompanied by neurological symptoms such as right facial palsy and dizziness. He received traditional Korean medicine treatment such as acupuncture(including pharmacopuncture, needle embedding therapy, electroacupuncture) and herbal medicine for 17 days. Results : Each neurological symptoms were improved after traditional Korean medicine. In case of facial palsy, House Brackmann Grading System was reduced form grade 3 to grade 2, and Sunnybrook Scale was improved from 39 to 55. Also, other symptoms such as dizziness and headache was also improved. Conclusions : This case report suggests that traditional Korean medicine may have a role in treating neurological symptoms such as facial palsy and dizziness in patient diagnosed with Miller-Fisher syndrome.

  • Objective: In this care report, we address a case of a Parkinson's disease patient with nausea and vomiting. Methods: A patient was treated with Korean medicine therapies including herbal medication (Hyungbangjihwang-tang, Boikyangwie-tang), acupuncture, and electroacupuncture during 35 days of hospitalization. We evaluated the improvements of symptoms by checking the number of vomiting episodes and the presence of nausea. Results: During 35 days of Korean medicine treatment, there was improvement in patient's symptoms. The frequency of vomiting had decreased, and the nausea had decreased and not appeared for the last 7 days. Conclusions: This study suggested that Korean medicine therapies might be effective in relieving gastrointestinal symptoms of Parkinson's disease.

한방병원에 입원한 교통사고 환자의 후향적 연구: 상태-특성 불안 척도, 벡 우울 척도, 핵심감정척도-단축형과 통증 호전도의 상관관계 (A Retrospective Study of Traffic Accident Inpatients in a Korean Medicine Hospital: Correlation of STAI-I, STAI-II, BDI, and CSEI-s scores with Pain Improvement)

  • Objectives: The objective of this study was to investigate the correlation of the scores on the State-Trait Anxiety Inventory-I (STAI-I), State-Trait Anxiety Inventory-II (STAI-II), Beck's Depression Inventory (BDI), and Core Seven Emotions Inventory-short form (CSEI-s) scales with pain improvement. Methods: We retrospectively investigated the medical records of 66 traffic accident inpatients who satisfy the selection criteria. They had received Korean medical treatment including acupuncture, electroacupuncture, pharmacopuncture, herbal medicine, and Chuna during hospitalization. STAI-II, BDI, and CSEI-s scores on hospital day 1, and STAI-I and numerical rating scale(NRS) scores on hospital day 1, 4, 7, and 10 were used for analysis. Pain improvement was evaluated by difference in NRS scores between hospital day 1 and hospital day 4, 7, 10. Results: The STAI-I, BDI, and CSEI-s scores showed significant correlations with pain or pain improvement. Conclusions: This study may be used in the research on psychological state and pain management of traffic accident patients and for patient education. Large-scale, well-designed studies need to be conducted in future to strengthen the results in this regard.

뇌파를 평가도구로 사용한 침 중재 임상연구 동향 (A Review on Clinical Research of Acupuncture Using Electroencephalogram)

  • Objectives: The purpose of this study was to investigate the recent trends of clinical research on acupuncture using electroencephalogram (EEG) as the outcome measurement. Methods: Nine domestic and foreign databases were searched to collect related studies published up to November 3, 2021. The participants, intervention, outcomes, results of the included studies were extracted and analyzed. Results: A total of 18 studies were selected. Neurological diseases and mental disorders were included in most studies, and vascular dementia were most frequently investigated. Electroacupuncture and body acupuncture intervention were most frequently conducted in seven studies. The most commonly used outcomes using EEG was EEG abnormality. However, in most studies there was accurate description of the EEG measurement. Most studies showed significant difference in EEG outcomes after intervention. The quality of included studies was poor. Conclusions: EEG as diagnostic markers and outcome measurements is increasingly studied. Standardized EEG measurement and the consistent EEG finding for specific diseases are needed to perform the future rigorous studies on EEG as diagnostic and outcome tools.

뇌전증과 동반된 두통에 대한 한방치료 치험 1례 (Case Report on Headache Treated with Korean Medicine in a Patient with Epilepsy)

  • Objective: This study reports a clinical case of headache accompanied by epilepsy that showed improvement with Korean medicine treatments. Methods: A 67-year-old woman with headache was treated with Korean medicine therapies, including Cheongshimondam-tang, acupuncture, electroacupuncture, and cupping, for 14 days. The response before and after treatment was evaluated using the Numerical Rating Scale, Headache Impact Test-6 (HIT-6), and Quality of Life in Epilepsy-10 (QOLIE-10). Results: After the treatments, the headache was relieved from NRS 8 to NRS 1, and anxiety decreased from NRS 7 to NRS 2. The HIT-6 changed from 66 to 48, and the QOLIE-10 dropped from 44 to 30. Conclusion: This study suggests that combined Korean medicine treatments might be a therapeutic option for relieving headache with epilepsy.

소뇌천막에 발생한 경막하출혈 환자의 오령산 치험 1례 (A Case Study of a Patient with Tentorium cerebelli Subdural hematoma Using Oreong-san)

  • A case of a 73-year-old male patient who fell down, presented with unusual traumatic subdural hematoma localized on the lesion of tentorium cerebelli. He was treated with acupuncture, electroacupuncture and herbal medicine - 五苓散(Oreong-san). To evaluate the progress, we followed up computed tomography imaging and checked up changing of symptoms. After 17days of treatment, there were notable improvement in computed tomography imaging and symptoms. Oreong-san might be effective in treating tentorium cerebelli subdural hematoma.

염증성 장 질환에서 전침을 중심으로 한 미주신경자극에 대한 고찰 (Vagus Nerve Stimulation Through Electro-acupuncture for Inflammatory Bowel Disease)

  • Inflammatory bowel disease (IBD) is defined as a chronic inflammatory-mediated disease that causes ulceration and inflammation in the gastrointestinal tract. Among most patients, the course of chronic inflammation repeatedly shows intermittent exacerbations and alternating remissions. However, despite the various therapeutic options to relieve symptoms, such as corticosteroids, TNF-α inhibitors, and antibiotic drugs, there is no known cure for IBD. Nonetheless, previous research has revealed that the autonomic nervous system is involved in the pathophysiology of IBD. In this study, we reviewed clinical trials confirming the therapeutic effect of vagus nerve stimulation (VNS) on IBD in vivo. We searched in vivo and human studies on Pubmed using keywords combined with "vagus nerve stimulation", "VNS", and "inflammatory bowel disease". All studies included in this review reported that direct VNS is effective in relieving symptoms of IBD and has no severe adverse effects. The most frequently stimulated site was the unilateral cervical vagus nerve area, and parameters for stimulation were set as 5-20 Hz. Based on the results, we aim to summarize the evidence for the efficacy of VNS on IBD and suggest the possibility of auricular electroacupuncture treatment as a therapeutic option for IBD.

  • Objectives: The aim of this study is to describe the effects of treatment with traditional Korean medicine on a patient with voiding dysfunction caused by cerebral infarction. Methods: The patient was treated with herbal medicine, moxibustion, acupuncture, and electroacupuncture. The effects of these treatments were evaluated using the Korean version of the Modified Barthel Index and a post-void residual volume. Results: In this study, the Korean medicine Dangguishaoyao-san-hap-Jeoryeong-tang resulted in improvements in this patient with post-stroke urinary retention and successful removal of the indwelling catheter. Conclusion: The results suggest that traditional Korean medicine may be effective in ameliorating dysfunction caused by cerebral infarction.

급성 횡단성 척수염으로 의심된 길랑바레증후군 환자에 대한 한방 치료 증례보고 1례 (A Case Report of a Guillain-Barre Syndrome Patient Suspected of Having Acute Transverse Myelitis Treated with Korean Medicine)

  • A 67-year-old female patient diagnosed with acute transverse myelitis visited our hospital in February 2023. Consultation papers indicated that she was diagnosed with acute transverse myelitis and that she complained of quadriparesis and numbness in both hands and feet. However, her spine magnetic resonance imaging (MRI) did not clearly indicate that she suffered from acute transverse myelitis. She was then sent to a high-level hospital and diagnosed with Guillain-Barre Syndrome through electromyography, cerebrospinal fluid test, and spine MRI. On March 2023, she visited our hospital again, still complaining about weakness in both hands and inability to walk. She was treated with Korean medicine, including acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine such as Yanggyeoksanhwatang and Sipimijihwang-tanggami. After three months, she was discharged on cane walking. The manual muscle test (MMT) grade of her upper limbs increased from 3-/3-/3-/3- (shoulder/elbow/wrist/fingers) to 4/4/4/3+, and the MMT grade of her lower limbs increased from 2/2/2/2 (pelvis/knee/ankle/feet) to 4/4/4-/4-. Her numbness in both feet disappeared, and her numbness in both hands decreased from a numeric rating scale of 7 to 2. No adverse events were reported during treatment. This case implies the therapeutic potential of Korean medicine for Guillain-Barre Syndrome, especially for those who had delayed diagnosis and missed the opportunity to get plasma exchange.

Pubmed분석을 통한 경추통과 경추 척추증의 임상연구 최신동향 및 평가도구에 관한 고찰 (Review of Research Trends and Evaluation Tools for Clinical Studies of Neck Pain and Cervical Spondylosis : Using the Pubmed Database)

  • The purpose of this research is to contribute to clinical researches on neck pain and cervical spondylosis by reviewing the latest research trends and evaluation tools through the analyses of clinical studies on neck pain and cervical spondylosis over the last 5 years. 70 papers satisfying the selection conditions among the RCT papers that had been searched as "neck pain" or "cervical spondylosis" at Pubmed(https://www.ncbi.nlm.nih.gov/pubmed) from March 2011 to February 2016 were targeted. Papers were numbered in order of their publication dates and analyzed by classifying their contents into 1) pain classification, 2) treatment type, 3) treatment duration, 4) treatment time, 5) number of participants, 6) evaluation tools and methods of research, and 7) evaluation duration. 55 papers targeted chronic neck pain, 6 papers acute and subacute neck pain, and 2 papers subacute and chronic neck pain. In comparison by intervention, 43 papers corresponded to physical therapy, 3 papers to acupuncture, 1 to herbal fomentation, 5 to medication, and 18 papers corresponded to multilateral comparisons comparing the efficacy by various interventions. In research period, there were 50 papers based on treatment period, 16 papers based on the number of treatments, and 4 papers based on different periods depending on each group. In treatment duration, the cases from 1 month or more to less than 3 months were most, followed by the cases of less than 1 month, and the cases from 3 months or more to less than 6 months. In treatment frequency, the number of treatments of the treatment group was the same as that of the control group in 51 papers, and many treatments were conducted by the methods of acupuncture, manual therapy, and injection therapy in cases of once or twice of treatments, and physical therapy and electroacupuncture corresponded mainly to the cases from 3 times or more to less than 10 times of treatments, and retrospective observation and exercise programs corresponded mainly to the cases of more than 30 times of treatments. In the number of subjects of the researches, the cases from 50 or more to less than 100 were most, followed by the cases from 20 or more to less than 50. There were 7 evaluation tools cited 10 times or more: VAS, NRS, PPT, NDI, NPQ, CROM, and SF-36. In evaluation period, 37 papers evaluated only during the treatment period, and 33 papers conducted follow-up. In follow-up period, the cases of less than 3 months were most, followed by the cases from 6 months or more to less than 1 year, and the cases from 3 months or more to less than 6 months. When planning clinical researches on cervical pain in the future, appropriate intervention methods, frequency and duration of treatment, period of follow-up, appropriate number of subjects and selection of evaluation tools for objective validity will have to be considered. In addition, randomization, double-blind, etc. will have to be considered for researches with high basis level.
