한의학적 침구치료원리에 근거한 새로운 자극방식을 통해 치료효과를 증대시킬 수 있는 구강안면 동통 진단/치료기를 개발하였다. 이 새로운 DDTS-1 치료기는 기존방식과는 달리 캐패시터를 이용하여 동통발생 원인인 인체의 과잉 이온전하를 흡수하여 치료하는 방식이다. 과도한 이온전하를 흡수하여 치료하기 위한 최적 파라미터 추출은 실험을 통해, 캐패시터의 충방전을 제어하기 위한 제어신호의 최적의 주파수는 6hz, 캐패시턴스는 $0.1\~0.33{\mu}F$으로 결정하였다. 임상실험을 통해 시스템의 검증을 위해 치성동통환자, 3차 신경통환자 등을 대상으로 정상부위와 이상부위의 전위를 측정하였다. 그 결과, 기존 자극기는 치료전과 후의 전위차가 매우 작았으며 DDTS-1은 생체전위차가 동일하거나 비슷해지는 뚜렷한 차이를 보였다. 따라서, DDTS-1의 자극방식이 기존의 전기적 자극방식보다 치료효과 측면에서 더 효과적이며 이는 캐패시터를 이용한 인체 이온전하 흡수방식의 타당성을 검증하였다. 즉, DDTS-1 치료기의 자극방식에 따른 동통조절기전의 이론적 타당성을 검증하고 성능 및 신뢰성을 검증하였다.

More than 80% of cancer patients experience cancer pain. Among them, more than 50% experience moderate to severe pain. To control cancer pain, a variety of methods have been used, including medications and nerve blocks. In some patients, however, it is impossible to perform nerve blocks due to caner metastasis into the epidural space, while in other patients, opioid dose escalation is impossible due to opioid side effects; thus, cancer pain management is difficult. Scrambler therapy is a novel approach for pain control that uses EKG-like pads, which are applied above and below the site of pain. Scrambler therapy synthesizes 16 different types of nerve action potentials that provide "non-pain" information via cutaneous nerves. The advantages of this treatment are that it is non-invasive and safe and has no significant side effects. In this case series, we report the treatment results of using scrambler therapy in three cancer patients with intractable pain.
Objectives : The aim of this study is evaluation for significance of skin meridian-acupoints energy measurement system, and is verification of principle to measure the bio-ion charge on acupoints. Methods : To check a correlation between the MEP(Meridian Energy Potential) and bio-electric charge condition, ten acupoints(left/right BL meridian : BL13, BL15, BL18, BL20, BL23) were chosen. Based on basic theory concerning the upward trend of the bio-electric charge by cupping therapy, we measured MEP change before and after 600mmHg(80kPa) Negative pressure stimulation using cupping. Results : We could identify the correlation between MEP and the condition of bio-electric charge in acupoints. Also, we checked the balance or imbalance of left/right the MEP and unusual cases. Conclusions : We confirmed significance of proposed principle and meridian energy measurement system.
바쁜 일상 속에서 신체 활동량의 감소와 식생활의 서구화 등으로 인한 복부 비만의 발생률이 높아지고 있다. 따라서 체형 관리에 대한 욕구를 충족시키기 위해 시간 대비 고 효율의 관리 프로그램이 필요하다. 본 연구에서는 전기근육자극요법 (electric muscle stimulation therapy)을 활용하여 다이어트젤(슬리밍 화장품)과의 복합사용에 의하여 허리둘레 및 체지방량 (또는 복부의 지방)를 감소 효과를 촉진시켰다. 연구 대상자는 20대 성인 남녀로 다이어트젤을 허리 부위에 단순 도포를 한 후, 그 위에 EMS를 착용하여 기상 후와 취침 전에 1일 2회 20분간 마사지를 실시하였다. 실험 기간은 2주 동안 실시되었다. 지방분해 촉진 효과는 비 조영 CT를 활용하여 피하지방의 감소에 대한 관찰이 되었으며, 이와 함께 허리둘레와 체지방량 측정으로 확인되었다. 결론적으로 개발된 EMS와 다이어트젤의 동시 사용 프로그램은 약 3 cm (p<.001)의 허리둘레 감소와 약 1 kg (p<.01)의 체지방량 감소를 나타내어 허리라인 관리에 탁월한 효과가 있음을 입증하였다. 이는 EMS의 활용이 근육 자극에 의한 마사지 효과를 나타내며 다이어트젤의 흡수를 도와 지방 분해를 촉진시키는 역할을 하는 것으로 사료된다.
한의학에서 경락은 생체에너지의 전달 통로이며, 진단치료의 기본대상이다. 그러나 서양의학에서는 경락을 신경-면역계통의 활동통로로 간주하고 있다. 침구치료 효과를 단순한 자극에 의한 인체의 반응으로 간주하고 있다. 본 논문에서는 시술자의 생체에너지가 전달될 때와 전달되지 않을 때, 즉 절연자침과 비절연자침을 시행하였을 경우, 경락과 비경락에서의 전위를 측정하였다. 그 결과 비절연자침 시행시에는 각 시술에 대한 반응이 뚜렷하게 나타나고 있으나 절연자침 시행시에는 첫 번째 자침 시술에만 반응하였다. 이는 침구치료의 효과가 단순한 침자극에 의한 효과라기 보다는 시술자의 생체에너지 전달과 더 밀접한 관계가 있음을 의미한다. 따라서, 침 자극에 의한 생체에너지 전달에 의한 인체의 반응을 전위의 변화로 분석하여 침구치료효과를 규명하였다.
본 연구는 저작근의 과활성으로 유발된 근피로와 근육통증에 경피성 신경자극이 어떤 영향을 미치는지 평가하기 위해 측두하악장애의 병력과 현증이 없는 평균연령 26.1세인 29명의 정상 성인 (남자 18명, 여자 11명)을 대상으로 교근의 주관적 통증강도, 압력통각역치, 근전도 power spectrum의 변화양상을 측정하여 분석하였다. 모든 피험자는 적어도 3일 이상의 간격으로 2 회의 실험에 참여하여 하루는 실험군으로서 경피성 신경자극(TENS)을 적용하였고, 다른 하루는 대조군으로서 위경피성 신경자극(sham-TENS)을 적용하였다. 실험군과 대조군에서 각각 교근의 지속적인 등척성 수축 전과 후의 주관적 통증강도, 압력통각역치, 인내시간까지의 근수축 동안 근전도 power spectrum을 측정 분석하여 다음과 같은 결론을 얻었다. 1. 지속적인 등척성 수축으로 유발된 근피로 후의 압력통각역치와 중간주파수는 근피로 유발 전보다 유의하게 감소하였다. 2. 근피로 유발 전과 후의 압력통각역치는 TENS를 적용한 실험군에서 통계적으로 유의하게 증가하였다. 3. 근피로로 유발된 압력통각역치의 변화량은 TENS를 적용한 실험군에서 유의하게 감소하였다. 4. 근피로 후의 주관적 통증강도는 TENS를 적용한 실험군에서 유의하게 감소하였다. 5. 인내시간은 TENS를 적용한 실험군에서 증가하였고, sham-TENS를 적용한 대조군에서는 감소하였으나, 모두 통계적 유의성은 나타나지 않았다. 6. 인내시간 동안 중간주파수 변화량은 TENS를 적용한 실험군에서 더욱 적게 감소하였고 중간주파수 변화의 기울기는 더욱 크게 증가하였으나 모두 통계적 유의성은 나타나지 않았다.
Facial nerve paralysis(or Bell's palsy) which commonly occurs unilaterally, gives rise to paralysis of facial expression muscle. This condition is classified into symptomatic facial nerve paralysis due to intracranial tumor, post operative trauma, etc. and idiopathic facial nerve paralysis. To explain the etiology of idiopathic facial nerve paralysis, many hypothesis including ischemic theory, viral infection, exposure to cold, immune theory etc. were suggested, but there is no agreement at this point. The method to evaluate the facial nerve paralysis, when it occurs, consists of three stage scale method, image thechnics like CT and MRI, laboratory test to examine the antibody titers of viral infection, neurophysiologic test to evaluate the degree and prognosis of paralysis. Treatment includes medication, stellate ganglion block(SGB), surgery, physical therapy and other home care therapy. In medication, systemic steroids, vitamins, vasodilating-drug and ATP drugs were used. SGB was also used repeatedly to attempt the improvement of circulation and to stimulate the recovery of nerve function. Physical therapy including electric acupuncture stimulation therapy(EAST) and hot pack was used to prevent the muscle atrophy. When No response was showed to this conservative therapies, surgery was considered. After treating two patients complaining of Bell's palsy with medication(systemic steroids) and EAST, favorable result was obtained. so author report the case of facial nerve paralysis.
The purpose of this study was to investigate the effect of the body temperature peripheral circulation with vibration shoes in healthy 10 adults. The magnetic vibration device with non-electric power was mounted in the midsole of the vibration shoes. The experiment was divided into two groups: vibration shoes and no vibration shoes. Subjects were randomly selected and measured body surface temperature by digital infrared thermal imaging (DITI) and non-invasive capillaries change by nailfold microscope (NFM). After walking in a treadmill for 15 minutes at 4.0 km/h speed wearing normal shoes or vibration shoes, DITI and NFM were measured. The walking with vibration shoes showed the body surface temperature shift from the proximal to the distal. In addition, the diameter of the nailfold capillary in the vibration shoes group was thicker and clearer due to the increased blood flow than that of the no vibration shoes group. The vibration shoes are easy to apply to anyone who can walk because it can give vibration stimulation by walking without additional time, cost, and effort in daily life. Further studies are needed to explain the physiological effects of vibration frequency and intensity on the long-term perspective of target subjects resulting from vascular dysfunction.
The purpose of this study is to evaluate the effectiveness of traditional Korean medicine treatment of cervical vertigo. Three patients were diagnosed as cervical vertigo with correlating symptoms of imbalance and dizziness with neck pain. The diagnosis of cervical vertigo is also dependent on excluding other vestibular disorders on the basis of history, examination, and vestibular function tests. They were treated by acupuncture, transcutaneous electrical nerve stimulation therapy and low-intensity pulsed ultrasound at acupoints and sympathetic ganglion chain of their cervical and upper thoracic region. The evaluation of clinical outcome was done by numeric rating scale (NRS), dizziness handicap inventory (DHI) and neck disability index (NDI), EuroQol-five dimensions questionnaire (EQ-5D) index. After the treatment, the value of their NRS, NDI, DHI was significantly decreased and their EQ-5D index was significantly increased. The traditional Korean medicine treatment at cervical and upper thoracic region could be an effective way to treat cervical vertigo.
Purpose: Nowadays spinal cord stimulator is frequently used for the patients diagnosed as complex regional pain syndrome. The lead is placed above the spinal cord and connected to the stimulation generator, which is mostly placed in the subcutaneous layer of the abdomen. When the complication occurs in the generator inserted site, such as infection or generator exposure, replacement of the new generator to another site or pocket of the abdomen would be the classical choice. The objective of our study is to present our experience of the effective replacement of the existing stimulation generator from subcutaneous layer to another layer in same site after the wound infection at inexpensive cost and avoidance of new scar formation. Methods: A 50-year-old man who was diagnosed as complex regional pain syndrome after traffic accident received spinal cord stimulator, Synergy$^{(R)}$ (Medtronic, Minneapolis, USA) insertion 1 month ago by anesthetist. The patient was referred to our department for wound infection management. The patient was presented with erythema, swelling, thick discharge and wound disruption in the left upper quadrant of the abdomen. After surgical debridement of the capsule, the existing generator replacement beneath the anterior layer of rectus sheath was performed after sterilization by alcohol. Results: Patient's postoperative course was uneventful without any complication and had no evidence of infection for 3 months follow-up period. Conclusion: Replacement of existing spinal cord stimulation generator after sterilization between the anterior layer of rectus sheath and rectus abdominis muscle in the abdomen will be an alternative treatment in wound infection of stimulator generator.
