The Clinical Study of Muscle Energy Techniques in Elector Spinae Muscle through Meridian Electromyography on Subjects (일반인에서의 근에너지 기법 시술 전과 후의 척추기립근 경근전도 변화)

  • Objectives : To evaluate the clinical utility of Muscle Energy Techniques(MET) in Elector Spinae Muscle on subjects. Methods : We compared electrical activity between a before MET and a after MET in Elector Spinae Muscle on subjects in same group(n=26) in dynamic flexion-reextension state during five seconds. We analyzed amplitudes and areas of electrical activity and Asymmetry Index(AI) and Median Edge Frequency(MEF). Results : 1. After MET in Elector Spinae Muscle on subjects were lower electrical activity than before MET in Elector Spinae Muscle on subjects but it is not a pointless observation(p<0.05). 2. AI of the after MET in Elector Spinae Muscle on subjects significantly decreased compared with before MET in Elector Spinae Muscle on subjects(p<0.05). 3. MEF of the after MET in Elector Spinae Muscle on subjects decreased compared with before MET in Elector Spinae Muscle on subjects but it is not a pointless observation(p<0.05). Conclusions : According to above results, there is clinical effect MET on subjects.

AI of the after MET in elector spinae muscle on low back pain patients significantly decreased compared with before MET in elector spinae muscle on low back pain patients(p<0.05). Conclusions : According to above results, there is clinical effect MET on low back pain patients.

The Impacts of Muscle Energy Technique (MET) and Transcutaneous Electrical Nerve Stimulation (TENS) Performed on Elector Spinae Muscle of Acute Low Back Pain Patients on Pain and Meridian Electromyography (경근추나요법과 경피전기자극치료가 급성 요통 환자의 통증 및 척추기립근 경근전도에 미치는 영향)

  • Objectives To evaluate the clinical usefulness between muscle energy techniques (MET) and transcutaneous electrical nerve stimulator (TENS), we performed both on elector spinae muscle of acute low back pain patients. Methods After performing MET and TENS, we compared both in terms of electrical activity. We performed MET or TENS on elector spinae muscle of acute low back pain patients in each group (n=15,15). After performing MET or TENS, we analyzed root mean square (RMS), median edge frequency (MEF) and asymmetry index (AI). Results 1. After performing MET on elector spinae muscle of acute low back pain patients, RMS was significantly decreased compared with before (p<0.005). 2. After performing TENS on elector spinae muscle of acute low back pain patients, RMS was significantly decreased compared with before (p<0.005). 3. After performing MET on elector spinae muscle of acute low back pain patients, asymmetric index was significantly decreased compared with before (p<0.05). Conclusions According to above results, performing MET on elector spinae muscle of acute low back pain patients has effect in terms of RMS and asymmetric index. And performing TENS on elector spinae muscle of acute low back pain patients also has similar effect in terms of RMS but has not in terms of asymmetric index.

The Effect of Low Back Pain on the EMG of Professional Golfer's Drive Swing (요통에 따른 프로 골퍼의 드라이버 스윙에 관한 근전도 분석)

  • The purpose of this study is to compare and analyze the muscle activations between the professional golfers without low back pain symptom and the professional golfers with low back pain symptom, and so identify the stress related to golf swings, and provide the basic data to minimize the low back pain and the injury risk. Using surface electrode electromyography, we evaluated muscle activity in 6 male professional golfers during the golf drive swing. Surface electrodes were used to record the level of muscle activity in the Abdominal Oblique, Elector Spinae, Rectus Abdominis, Gluteus Maximus muscles during the golfer's swing. These signals were compared with %RVC(Reference voluntary contraction) which was normalized by IEMG(Integrated EMG). The golf swing was divided into five phases: take away, forward swing, acceleration, early follow through, late follow through. we observed patterns of trunk muscle activity throughout five phases of the golf swing. The results can be summarized as follows: RES(Right Elector Spinae) had statistically significant difference in take away phase, LGM(Left Gluteus Maximus), LRA(Left Rectus Abdominis), LOA(Left Oblique Abdominal) had statistically significant difference in forward swing phase, RES(Right Elector Spinae), RGM(Right Gluteus Maximus), ROA(Right Oblique Abdominal) had statistically significant difference in acceleration phase, RES(Right Elector Spinae), RGM(Right Gluteus Maximus) had statistically significant difference in early follow-through phase, LES(Left Elector Spinae), RGM(Right Gluteus Maximus) had statistically significant difference in late follow through phase.

The effects of the angles of the knee and heel-off on the muscle activity during a bridge exercise

  • PURPOSE: The purpose of this study is to investigate the effects of the angles of the knee and heel-off on the muscle activity during a bridge exercise. METHODS: 15 healthy adult men and women with the balance ability and joint working range required for performing a bridge exercise participated in this study, in which $120^{\circ}$, $90^{\circ}$ and $60^{\circ}$ angles of the knee-flexion and heel-off were applied during the bridge exercise. RESULTS: Our data showed that there were significant differences in muscle activities of elector spinae and rectus abdominis when $120^{\circ}$ and $60^{\circ}$ angles of the knee were applied, of internal oblique when $120^{\circ}$ and $60^{\circ}$ were applied, and external oblique when $90^{\circ}$ and $60^{\circ}$ were applied. When heel-off was applied, there were significant differences in muscle activities of elector spinae and rectus abdominis when $120^{\circ}$ and $60^{\circ}$ were applied, of internal oblique when $120^{\circ}$ and $60^{\circ}$ were applied, and external oblique when $90^{\circ}$ and $60^{\circ}$ were applied. CONCLUSION: In this study on an application of heel-off to the bridge exercise, we showed that the effect of the angles of the knee on the muscle activities of elector spinae, rectus abdominis, internal oblique and external oblique were all similar to the regular bridge exercise, but overall muscle activities were increased with heel-off when compared with the regular bridge exercise.

Analysis of Trunk Muscle Fatigue as the Frequency of Lifting/Lowering and Recovery Time Change (들기/내리기 빈도와 회복시간 변화에 따른 몸통 근육의 피로도 분석)

  • The purpose of this study was to investigate the relationship between the cumulative fatigue of trunk muscles and the period of recovery time during repetitive lifting and lowering task with two different frequencies(4 times/min and 6 times/ min). Eight healthy males with no prior history of LBD(low back disorders) volunteered for this study. Subjects had 2, 3, 4, and 5 minutes recovery time respectively while they were preforming the lifting and lowering task at 15% level of MVC. EMG signals from six trunk muscles were collected and the fatigue level was analyzed quantitatively. In results, the fatigue levels of LES(left elector spinae), RLD(right lattissimus dorsi), LLD(left lattissimus dorsi), RRA(right rectus abdominis) and LRA(left rectus abdominis) were recovered when 3 minutes recovery time was given at 4 times/min frequency. However, the fatgue level of RES(right elector spinae) was recovered when 4 minutes recovery was given. On the other hand, when 6 times/min frequency was used, the RLD, LLD, RRA and LRA were recovered at 5 minutes of recovery time. But for RES and LES, it took longer than 5 minutes to be recovered. This results can be applied to design the adequate length of recovery time to control the cumulative fatigue of trunk muscles in industry with repetitive lifting and lowering task.

Difference in Muscle Activities According to Stability on Support Surface During Plank Exercise

  • PURPOSE: The present study aimed to measure muscle activities in the pectoralis major, the erector spinae, and the quadriceps femoris according to support surface states of arms and legs during plank exercise. METHODS: The subjects of this study were 21 healthy males in their 20s and their muscle activities at three states were measured as follows: The first state was where the support surface of arms and legs was stable. The second state was where only arms were unstable, and the third state was where only legs were unstable. Electromyography (EMG) was used to measure muscle activities. Pectoralis major, quadriceps femoris, and elector spinae were measured for muscle activities. RESULTS: The muscle activities in the pectoralis major were statistically high when arms were unstable. The muscle activities in the quadriceps femoris were statistically high when legs were unstable. The muscle activities in the erector spinae were higher when arms and legs were unstable compared to that at the stable support surface. No significant difference was revealed statistically when arms and legs were unstable. CONCLUSION: If the instability of arms and legs is employed during plank exercise, exercise on the upper and lower bodies or the erector spinae is expected to be more effective.

Comparative Analysis of Muscles Activation relate to Core Stability during 5 Therapeutic Exercise in Children with Spastic Diplegia (경직성 양하지 마비아의 5가지 치료적 운동시 핵심 안정성과 관련된 근활동 비교 분석)

  • PURPOSE: This study aimed to investigate muscles activation relate to core stability during 5 therapeutic exercise in children with spastic diplegia. METHODS: The subjects of this study were 8 children with typical development and 12 children with spastic diplegia who have been treated in Bobath children's hospital, all of whom agreed to participate in the study. All subjects were measured to see their muscles activation of rectus abdominalis, external oblique abdominalis, elector spinae, gluteus maximus, rectus femoris, and semitendinosus with surface EMG. RESULTS: The results of this study were as follows : 1) There were statistically significant difference in the root mean square(RMS) of all muscles according to 5 therapeutic exercise in children with spastic diplegia. 2) There were statistically significant difference in the RMS of all muscles according to 5 therapeutic exercise in children with typical development. 3) Significant differences of the RMS between diplegic children and normal children were found in elector spinae and rectus femoris only curl-up exercise. CONCLUSION: As the above results, we suggest that 5 therapeutic exercises could be used for a core stability or core strengthening program. Depending on the individual needs of children with cerebral palsy, some of exercises may be more beneficial than others for achieving strength.

Pelvic Compression Belt Convergence Impact on the Thickness of Multifidus and Erector Spinae Muscles (골반압박벨트가 뭇갈래근과 척추세움근의 근두께에 융합적으로 미치는 영향)

  • The purpose of this study was to determine the effectiveness of the pelvic compression belt on the thickness of the erector spinae and multifidus during hip extension on quadruped position. Thirty male university students volunteered to participate in this study. The pelvic compression belt was positioned below the anterior superior iliac spines with the stabilizing pressure using elastic compression bands. Subjects were instructed to perform hip extension in quadruped position with and without applying the pelvic compression belt. The thickness of the erector spinae and multifidus was measured ultrasound during prone position, quadruped position without applying pelvic compression belt and quadruped position applying pelvic compression belt. Data were analyzed using repeated ANOVA. Muscle thickness of multifidus was significantly higher applying the pelvic compression belt than without applying the pelvic compression belt (p<.05). Muscle thickness of elector spinae was significantly higher applying the pelvic compression belt than without applying the pelvic compression belt (p<.05). Therefore, the research can contribute to the prescription and application of quadruped position exercises in clinical practices.

Effect of Sprinter Pattern Bridging Exercise using Theraband on Activation of Lower Extremity and Abdominal Muscle

  • Objective: The purpose of this study was to investigate the effect of sprinter pattern bridging exercise using theraband on activation of lower extremity and abdominal muscle and to find out postures that can effectively improve abdominal and lower extremity muscle strength and increase abdominal stability. Methods: This study was designed as a cross-sectional study. The following research was done with applicants attending S university in Seoul to compare the difference in muscle activity between one-leg-Support bridging exercise and sprinter-pattern bridging exercise using theraband. For 48 study participants, we first measured their MVC. Then, we applied one-leg-support bridging exercise and sprinter-pattern bridging exercise at random order. These data were expressed as the percentage of maximal voluntary contraction (%MVC).Electromyography analysis was performed by measuring the external obliques, internal obliques, biceps femoris, and gluteus maximus. Results: There was a statistically significant increment of muscle activity in external and internal oblique muscle(p<0.001)by sprinter-pattern bridging exercise using theraband. On the lower body, statistically significant increment of muscle activity in biceps femoris and gluteus maximus was found(p<0.05). On the other hand, on erector spinae, there was statistically significant decrease in muscle activity(p<0.05). Conclusions: Efficient treatment is expected when sprinter-pattern bridging exercise using theraband is applied clinically.For patients with chronic knee and ankle pain who have difficulty bearing weight, including low back pain and internal rotation of the femur, starting with a low weight bearing, we think it will be helpful in planning systematic training aimed at progressively strengthening the lower extremities.
