Korea faces new social needs of community-based service for senior citizen as aging is getting fasten. Targeting elderly people who are beneficiaries of long-term care insurance and caring service, this paper aims to explore decent and proper services for senior citizens in terms of rehabilitation, health improvement and quality of life. Especially this research approaches from the perspective of service providers, including service facilities and care-service givers. Using FGI methods with care-providers and in-depth interviews with community social workers as well as nurses, convergence approaches are explored. As a result, welfare field and nursing practices are clearly divided in the community, so that the convergence education and team work tool would be developed in the near future.

This study was performed to determine the levels of MMSE-K among the elderly people affiliated with long-term care insurance services in rural areas, and to reveal its association with related variables. The interviews were performed, during the period from March 1st, to May 31th, 2009, to 410 elderlies in rural areas. As a results, The prevalence of cognitive functions among all subjects were 80.5% of severe cognitive impairment, 11.7% of mild cognitive impairment, and 7.8% of normal. The prevalence of cognitive functions were lower according to the higher grade of long-term care insurance. And the prevalence of cognitive functions were lower in the group of lower educational level, unemployed, live alone, lower monthly income, bear for living expenses by sons and daughters and medical aid, lower visual acuity, with urinary incontinence, with amnesia, without regular exercise and lower frequency of going out than their respective counterparts. The hierarchial multiple regression analysis was used to reveal the explanatory powers of factors influencing on the cognitive functions level. Such factors as age, bear for living expenses, visual acuity, urinary incontinence, amnesia, regular exercise and grade of long-term care services were significantly influenced with cognitive functions. especially the factors of health status were significantly influenced with cognitive functions.
The purpose of this study was 1) to review communal housing in the UK, 2) to consider the policy implications for elderly communal housing in Korea. The research methods used were 1) literature review about communal housing and related policy in the UK 2) field survey in the UK 3) interpretative suggestion for the proper policy implication to develope communal housing for the elderly in Korea. Sheltered housing in the UK had been developed as communal housing for the elderly with special needs since the 1970s. The type of sheltered housing were category 1 and category 2. Very sheltered housing with more facilities and meal services was added in 1980s. Sheltered housing was evaluated as the most humanistic solution for older people in the UK in 1980s. Because of the policy of moving institutional care to community care, sheltered housing became less in demand because of more options for older people including being able to stay in their own home. So new completion of sheltered housing by registered social landlords reduced saliently. Sheltered housing already totalled over half million units in which 5% of all elderly over 65 still lived and a small quantity of private sector for sale schemes emerged in the 1990s. The reason why the residents moved to sheltered housing was for sociable, secure, and manageable living arrangements. In general the residents were satisfied with these characteristics but dissatisfied with the service charge and quality of meals, especially in category 2.5 schemes. The degree of utilisation of communal spaces and facilities depended on the wardens ability and enthusiasm. Evaluation of sheltered housing indicated several problems such as wardens duty as a \"good neighbour\" ; difficult-to-let problems with poor location or individual units of bedsittiing type with shared bathroom ; and the under use of communal spaces and facilities. Some ideas to solve these problems were suggested by researchers through expanding wardens duty as a professional, opening the scheme to the public, improving interior standards, and accepting non-elderly applicants who need support. Some researchers insisted continuing development of sheltered housing, but higher standards must be considered for the minority who want to live in communal living arrangement. Recently, enhanced sheltered housing with greater involvement of relatives and with tied up policy in registration and funding suggested as an alternative for residential care. In conclusion, the rights of choice for older people should be policy support for special needs housing. Elderly communal housing, especially a model similar to sheltered housing category 2 with at least 1 meal a day might be recommended for a Korean Model. For special needs housing development either for rent or for sale, participation of the public sector and long term and low interest financial support for the private sector must be developed in Korea. Providing a system for scheme managers to train and retrain must be encouraged. The professional ability of the scheme manager to plan and to deliver services might be the most important factor for the success of elderly communal housing projects in Korea. In addition the expansion of a public health care service, the development of leisure programs in Senior Citizens Centre, home helper both for the elderly in communal housing and the elderly in mainstream housing of the community as well. Providing of elderly communal housing through the modified general Construction Act rather than the present Elderly Welfare Act might be more helpful to encourage the access of general people in Korea. in Korea.

According to Elderly Long-term Care Insurance, MCI people can be provided cognitive program at dementia center and daycare center. However, Elderly Alone Household cannot be the proper beneficiaries in the insurance. Despite of increasing ederly alone household, consideration of daily life ability as well as easy-use cognitive program are not enough in the community. Therefore, this paper explores personal need and usability of ICT and VR cognitive program for elderly people in the community. Research results shows that elderly people are keenly interested in new technology. Especially human service and emotional bonding is very important contacting new device or technology.
Purpose: The purpose of this study was to examine effectiveness and usefulness of Information Communication Technology (ICT) in communication between physician and visiting nurses who provide visiting nursing services under long-term care insurance. Methods: Structured questionnaires were used to measure usefulness and satisfaction of the system, both accessibility and convenience to visiting nurses (31 people) and users (182 people). Results: From the user perspective, accessibility and convenience in terms of service users were both satisfactory as shown by users' percentage. No statistically significant difference was found for satisfaction between before and after using the system. The usefulness of the system for visiting nurses was satisfactory for most the nurses. Also most nurses answered that the system is needed and is very useful. Most of the participants (both visiting nurse and service users) were satisfied with use of the ICT system. However, there was no statistically significant difference in satisfaction between the pre and post service because the service provision period was too short (three months). Conclusion: The consensus from both users and service providers is that an ICT based visiting nursing system needs to be introduced but a more user - friendly environment for system development will be needed.

This study was performed to determine the levels of quality of life(QOL) according to the grade of long-term care service in the elderly people who were judged from long-term care insurance, and to reveal its association with related variables. The interviews were made to 958 elderlies in urban and rural areas from March 1 to May 31, 2009. The mean scores of QOL among all subjects were $55.4{\pm}15.62$(Grade I; $49.7{\pm}14.17$, Grade II; $56.8{\pm}14.62$, Grade III; $59.4{\pm}16.36$), and they were lower according to the higher grade of long-term care insurance. The multiple regression analysis was used to reveal the explanatory powers of factors influencing on the level QOL. Such factors as educational level, monthly income, subjective health status, depression and MMSE-K were shown to affect their QOL in Grade I, Grade II, and Grade III with explanatory powers of 45~62%.
A high-quality workforce is essential to delivering the high-quality care for the older people who are beneficiaries of long term care insurance. Providing adequate education and ongoing training opportunities could be one answer that may lead to the ultimate goal of improving the quality of long-term care services. Thus this study aimed to examine a workforce crisis and difficulties that care workers face in current education system, especially in contents and administration aspects and draw out suitable solutions or improvement measures. The focus group interview approach was chosen to reflect opinions of long-term care workers, directors of the education system in long-term care facilities and managers. The findings from the focus group interviews were analyzed by theme and classified into several categories by common subjects:'Training contents', 'Training environment', 'Administrator', 'Compulsory training', 'Care givers supply', 'Working condition'. Also, four content areas were presented as follows:'Job training', 'Dementia care training', 'Administration system', 'Workforce'. The caregivers demand very practical training programmes and trainers with a lot of field experience. Improving the working condition and treatments for them leeds to expand the opportunity of the on-going training.
Objectives: The purpose of this study is to investigate the perception of LTCP trainees' on their professional. To accomplish the purpose, the perceived occupational nature and perceived clients' nature among the participants in the LTCP education are examined with the predicting variables. Methods: The participants of this study were composed of 320 people who have completed the long-term care provider certificate program. The participants were from 5 different long-term care provider licensing institution in the region of Gangdong-Gu, Seoul. Total of 320 self-administered questionnaires were distributed from June 2, 2008 to October 2, 2008 and 297 questionnaires were collected. Among them 283 surveys were analyzed. Results: In this model, active family support and recognition of task characteristics for the LTCP were the significantly predicting with beta of .187 and .507. The effect from active family support was remained significantly as model progressed. The magnitude of F-value and R-square value were significant. Conclusion: The results from this study will be helpful for the scholars who have investigated on the LTCP education program development as well as policy makers who have tried to supply dependable LTCP to the needy elderly and their families. In turn, the results from this study may empower constituents to make informed decisions about, and easily access, existing health and long-term care options.
The purpose of this research is to analyze the overall problems at the moment of October 2008, and then to find the improvements of home-help services of the Long-Term Care Insurance(LTCI), which has been revealed many problems since it was released in July 2008. The research uses the literature survey which analyzes 2nd-hand materials studied by other people already, and survey research was executed from active social workers in the area of LTCI. Based on the policy analysis framework of Gilbert and Specht, all the data are analyzed in the scopes of client·benefit(service)·finance·transferring system. This research has found the problems in each scope of home-help services of the LTCI. Firstly, the client system has some problems in mismatching between registered and service clients, estimating client number, and judging service levels. Secondly, the service system reveals deficiency in professionality of social workers, service quality lowering by loose qualification criteria on workers, non-reasonable limitation of service time available, and the same fare system applied to visiting-help service in spite of different levels. Thirdly, in financing system, clients need to pay additional money to get extra services such as meal, hair cutting, bathing etc., due to government financial support stopped, some organizations have to reduce services and replace full-time workers to part-time ones, which makes the service quality worse. Lastly, in the transferring system, the management system for service quality is not well prepared. There are too much competion because of allowing too many home-help service organizations and care worker academies. The suggestions that this research has found to improve the policy are as follows. ① It is desirable to make the registered clients the service ones as many as possible in the long term perspective. ② The LTCI organization requires more workers and higher professionality. ③ Many elderly people who are not eligible now require connection system to be more served. ④ Management system and service manual for care worker are to be developed. ⑤ Laws related to the service contents and process should be modified, the proportion of client charge needs to adjust. ⑥ Home-help service organization licensed by the LTCI needs to be financially supported publicly. ⑦ Monitoring system to home-help service organization needs to be strengthened. ⑧ Evaluation tools to home-help service organization and workers is required. ⑨ Specification to open the home-help service organization needs to be more strict.
As the population is getting older, medical expenses amount of the whole is keep increasing. So, the pressure of the finances, Health Insurance, Medical Care Assistance Act and etc, is getting higher. The share of healthcare-expense is increasing due to elderly illness. And it became a social problem; we analysed present state of senior healthcare in South Korea-looked into current laws and policies, and found problems. We tried to suggest improvements that drew from the current state of foreign country senior healthcare of those problems. For the result, we found the problem in relevant-law system of senior healthcare guarantee. In this study, we proposed the ways to qualitatively upgrade of medical standard that considered on elderly' features: the strengthened guarantee for healthcare, financial secure for long-term convalescence benefit, linking and functional reinforcement for elderly welfare and long-term convalescence insurance, the solution for overlapped laws about convalescence in long-term convalescence insurance and elderly welfare, a betterment of grading, and a home service consolidation. We need to secure right amount of emergency medical service budget, and effective management system for the improved level of senior severely emergency medical service. Furthermore, we suggested that South Korea needs to legislate [The Law for Senior Medical Secure] to respond to rapidly increasing senior healthcare fee.
