노인의 삶의 질 향상을 위한 주관적 구강건강에 영향을 미치는 요인 (Influencing factors for perceived oral health for the improvement of quality of life in Korean elderly)

  • Objectives: The purpose of the study was to investigate the influencing factors of the perceived oral health for improvement of quality of life in Korean elderly. Methods: The subjects were 1,289 elderly over 65 years old from the sixth National Health and Nutrition Examination Survey 2013. The dependent variable was subjective oral health status. The independent variable was sociodemographic characteristics. Results: The perceived oral health of the unemployed elderly and those having chewing problems were 1.65-fold(95% CI=1.12=2.44) and 3.45-fold(95% CI=2.37-5.02), respectively than employed and chewable elderly. The perceived oral health of the former was 2.49-fold worse(95% CI=1.73-3.60) than the latter. Conclusions: The influencing factors of perceived oral health status included occupation, perceived health status and chewing problems. To improve the oral health-related quality of life in the elderly, continuous education and hands-on programs should be provided for the elderly in the long term care.

치과종사자들의 노인에 대한 지식과 태도 연구 - 대전·충남지역을 중심으로 - (A study on knowledge and attitude toward the elderly in dental personnels - in Daejeon & Chungnam area -)

  • This study was surveyed the knowledge and attitude toward the elderly in dental personnels, who play an important role in oral health of the elderly in the aging society, In order to arrange the basic data that is conducive to development in the mouth care of the elderly, the research was conducted from January 7 to February 14, 2008, targeting 270 dental personnels in Daejeon & Chungnam area, who are in charge of oral duties in the current clinic. As a result of analyzing so that questionnaire can be prepared with the self-administered questionnaire, the following results were obtained 1. The knowledge level on the elderly in dental hygienist was 13.47 marks out of 25-mark perfection. There was no difference in knowledge depending on job category and volunteer-work activity experience, And, there was no difference even depending on physical & physiological sphere, psychological sphere, and family & social sphere. 2. The attitude level toward the elderly in dental hygienist was 91.63 marks out of 150-mark perfection. Dental hygienist showed positive attitude in personality characteristic, emotional characteristic, and self-management ability by sphere, and showed statistically significant difference (p=0.011). 3. In case of having volunteer-work activity experience, the attitude level toward the elderly was 92.57 marks out of 150-mark perfection, A case of having volunteer-work activity experience showed positive tendency in emotional characteristic, self-management ability, and judgement-ability characteristic by sphere, And, the attitude toward family relation was indicated to be negative tendency, thus there was statistical significance(p=0.022). 4. As for the correlation between knowledge and attitude toward the elderly, dental hygienist was indicated to have high interest in the volunteer-work experience, the elderly education experience, and the elderly problem(r=0. 444). The knowledge and attitude toward the elderly had slightly positive correlation(r=0.155). Dental hygienists are being required gradually as the primary staff for the elderly people's dental care in the aging society. A continuous education is needed so that dental hygienists can have positive sight in understanding, knowledge, and attitude. And, the development in a mouth care program for the elderly in line with it is considered to be necessary.

노인의 구강자각증상에 따른 구강건강행태 (Oral health behavior according to perceived oral symptoms in the elderly)

  • Objectives: The purpose of the study was to investigate the oral health behavior according to perceived oral symptoms in the elderly. Methods: The data were obtained from the 6th Korean National Health and Nutrition Examination Survey (2013). Data were analyzed by complex sample frequency t test, one-way ANOVA, ${\chi}^2$ test, and general analysis. The questionnaire consisted of general characteristics of the subjects and perceived oral symptoms. Results: Those who had higher education tended to brush teeth regularly. The use of oral hygiene product was also high in higher educated elderly and higher income. Those with good oral health perception showed regular toothbrushing habit. Those with good chewing and mastication used oral hygiene products. Male tended to have regular dental checkup. Those having spouse, higher income, younger age, regular dental checkup had good chewing ability. Poor oral health perception, toothache, and chewing difficulty were the main cause of dental visit (p<0.05). Conclusions: The elderly with poor perceived oral symptoms showed lower level of oral hygiene care. They tended to visit dental clinics only when they had oral symptoms.

The Effects of National Health Insurance Denture Coverage Policies for the Elderly on the Unmet Dental Needs of the Edentulous Elderly

  • As the elderly population increases, they are increasingly affected by oral health problems. Therefore, efforts are being made to improve the oral health of older people, alleviate mental discomfort, and reduce unmet dental needs. This study was conducted to confirm the relationship between the National Health Insurance Elderly Denture Coverage and the unmet dental need for the edentulous elderly, as part of the protection policy. We analyzed the 2011 and 2013 Community Health Survey data of the edentulous elderly, aged 75 years or older, before 2012. In order to more precisely confirm the effects of the denture donation policy on unmet dental care, basic life recipients who were subject to the free elderly prosthetic project were excluded from the analysis. The final analysis included 20,400 subjects. According to our investigation of the factors that affect the unmet dental needs of the elderly, the National Health Insurance Elderly Denture Coverage did not affect unmet dental needs. The statistically significant variables that affected the unmet dental needs of the elderly were education and income levels, which are representative socioeconomic status variables. The lower the level of education, the unhealthier the dental care experience, and income levels showed a similar tendency. The elderly who have a low socioeconomic status are more likely to experience unmet dental needs because they lack the knowledge and socioeconomic ability to pay for dental care. Therefore, the policy for health protection of the entire elderly population should be continuously expanded. In addition, the socioeconomically vulnerable groups may have health problems due to the restriction of medical use, which may lead to quality of life deterioration.

요양보호사의 노인 구강보건에 대한 지식과 인식도 연구 (A study on the knowledge and awareness of care workers on the geriatric oral health)

  • 본 연구는 노인요양시설에 근무 중인 요양보호사의 구강보건지식정도와 인식도를 조사하여 구강보건의료서비스의 질적 향상과 발전 방안 연구를 목적으로 실시하였다. 연구방법은 D광역시 소재 노인요양시설(10개소)에 근무하는 요양보호사 중 연구 참여에 동의한 163명을 대상으로 2015년 5월 1일부터 5월 31일까지 설문조사한 자료를 SPSS Statistics 18. 프로그램을 이용하여 분석 하였다. 그 결과 요양보호사들의 구강보건지식은 총점 26점 만점에 20.09점(77.2%)로, 가장 높은 지식은 '올바른 칫솔질 방법' 93.0%, 가장 낮은 지식은 '구강보조용품사용 지식' 58.1%로 조사되었다. 구강보건교육경험이 있는 사람이 구강보건교육 요구도가 높았으며 (P<0.001), 구강보건교육경험횟수가 많은 사람이 구강보건지식(P<0.001), 요구도(P<0.01), 인지도(P<0.05)가 높아 유의하였다. 요양보호사들의 구강보건인식정도에 영향을 주는 것은 구강보건지식점수(P<0.01), 구강보건교육 요구도(P<0.01)가 높을수록 구강보건인식정도는 높은 것으로 나타났다. 따라서 향후 요양병원 장기입원환자의 구강특성을 고려한 효율적, 지속적, 실질적인 요양보호사의 구강보건교육이 이루어 질 수 있도록 하여야 할 것이다. 또한 구강보건교육의 담당자로 치과위생사의 역할이 필요 할 것으로 전문 인력의 배치를 위한 국가의 제도적 지원도 함께 이루어져야 할 것이다.

노인의 본인인지 구강건강상태와 우울감 (Association of self-perceived oral health and depression in the elderly: the sixth Korea national health and nutrition examination survey(the 6th KNHANES))

  • Objectives: The aim of the study was to investigate the association of self-perceived oral health and depression in the Korean elderly. Methods: The subjects were 1,329 elderly in Korea from the sixth Korea National Health and Nutrition Examination Survey (the 6th KNHANES). The dependent variable was depression that continued more than two weeks. Model I for the impacts of depression on self-reported oral health, Model II for the impacts of depression on chewing problem, Model III for the impacts of depression on speaking problem was evaluated. PASW Statistics 18.0 version was used for frequency analysis, chi-square test and logistic regression analysis. Results: Female elderly were much more prone to depression than male. Female had lower monthly compensation, less medicaid, chewing problem, speaking problem, and less education. For these reasons, they tended to have more depression than male (p<0.001). Self-perceived oral health impacts on depression included poor self-reported oral health(p<0.01), poor chewing problem(p<0.01) and poor speaking problem(p<0.05). On the other hand, male did not show a statistically significant association. Conclusions: The study showed the self-perceived oral health related quality of life had a significant influence on depression in the elderly. The continuing lifelong oral health care can prevent depression in the later life because oral health care improvement can enhance the self-perceived oral health status.

취약계층 노인의 구강건강상태에 영향을 미치는 요인 (Factors Affecting the Oral Health Status in Vulnerable Elders)

  • Propose: This study was to identify the factors affecting the oral health status in vulnerable elders receiving home visiting health care service. Methods: A total of 444 elderly people over the age of 65 at 33 public health centers in Daegu and Gyungbuk areas were included in this survey. The data were collected by personal interview with nurses using oral health knowledge, behavior, and status questionnaires from the $10^{th}$ to $22^{nd}$ of July, 2012. The data were analyzed with t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, and stepwise multiple regression, using SPSS program. Results: Factors affecting the oral health status were chewing satisfaction with denture 14.5% (${\beta}=.391$, p<.001), denture care (wash with water) 3.2% (${\beta}=-.187$, p<.01), and types of health insurance (medical aid type1, 2) 1% (${\beta}=-.111$, p<.05). The factors explained 18.7% of variance in the oral health status of vulnerable elderly subjects. Conclusion: The results of this study indicate that in order to improve the oral health status of vulnerable elders, it is necessary to provide oral health education that includes the proper usage of denture to elders who receive home-visiting care.

일부 지역 노인의 구강 내 불편감에 영향을 미치는 요인 (Factors affecting oral discomfort in elderly in some areas)

  • Objectives: This study was conducted to analyze the factors affecting discomfort in the oral cavity for the elderly and to provide basic data for improving oral health in the elderly. Methods: It was conducted with ethical approval, and all subjects were explained about the research method and purpose before conducting the questionnaire. The final 178 were analyzed through a self-written questionnaire. SPSS 25.0 was used for statistical analysis of the collected data, and the significance level was 0.05. Results: Dental caries was 51.7%, periodontal disease was 69.1%, and lost tooth was 71.9%.The average score of discomfort in the oral cavity was 2.62±0.96. The factors affecting discomfort in the oral cavity were found to affect discomfort in the oral cavity in groups with education, periodontal disease, and missing teeth. Conclusions: Therefore, it is necessary to proceed with the oral health program according to the educational background of the elderly. Find out how to reduce oral discomfort caused by periodontal disease. It is believed that there is a need to expand health insurance for preventive care rather than treatment.

치과위생사의 노인장기요양보험제도에 대한 인식도 연구 (A Study on the Dental Hygienist's Awareness on the Elderly Long-Term Care Insurance System)

  • 본 연구는 일부 경남지역에 근무하고 있는 치과위생사 253명을 대상으로 2008년 7월 1일에 시행 된 노인장기요양보험제도와 관련한 인식정도를 파악하고 노인들의 구강건강 증진 및 유지에 대한 치과위생사의 역할을 제고하며 노인장기요양보험제도의 조기 정착을 위하는 목적으로 자가 기입식 설문법을 이용하여 자료를 조사하였으며 다음과 같은 결론을 얻었다. 1. 노인장기요양보험제도를 알고 있는 응답자는 41.9%였으며, 치과위생사가 장기요양요원으로서 활동할 수 있는 것에 대해 알고 있는 경우는 31.6%로 낮았다. 또한 향후 장기요양요원으로 참여하고 싶은 의사가 있는 경우는 43.1%이였으며, 방문간호센터에서 근무하고 싶은 의사가 있는 경우도 43.1%로 나타났다. 장기요양 요원에 대한 별도의 교육이 필요하다라는 의사를 보인 응답자는 76.7%이였으며, 장기요양요원으로서의 적정한 연령용 40~50대가 가장 적당하다라고 응답한 경우가 48.6% 가장 많았다. 2. 노인환자 구강관리 경험여부에서는 자주하는 경우는 51.8%로 조사되었으며 노인환자에 대한 친밀도에서는 편안하다라고 느끼는 경우는 31.6%로 나타났다. 자원봉사경험이 있는 경우는 33.6%이었으며 재학시절 노인과 관련 된 전문적인 교육을 받아 본 경험이 있는 경우가 34.8%, 재직 중에 노인에 대한 전문교육을 받아 본 경험이 없는 경우는 90.9%, 노인의 구강건강문제 관심도에서는 68.6%가 적은 것으로 나타났다. 3. 노인환자의 구강관리에 대한 경험여부와 치위생(학)과 재학 시 노인에 대한 전문적인 교육경험은 장기요양요원의 적정연령의사와 통계적으로도 유의한 차이가 나타냈다(p<0.01, p<0.05). 또한 조사대상자의 자원봉사경험여부와 재직 중 노인에 대한 전문적인 교육경험여부에 따라 노인장기요양보험제도에 대한 이해정도가 유의한 차이를 보였다(p<0.01). 4. 노인구강건강문제에 대한 관심도는 노인장기요양보험제도의 이해, 장기요양요원 활동정보에 대한 이해, 장기요양요원으로서의 근무의사여부, 방문간호센터 근무의사여부, 장기요양요원 별도교육 필요여부, 장기요양요원 적정연령의사에 대해 유의한 차이가 나타났다(p<0.05, p<0.01, p<0.001).

65세 이상 노인들의 순환기계 질환 의사 진단 여부에 따른 주관적 구강건강과 구강관리 행태 (Subjective Oral Health and Oral Care Behavior of Elderly People Over 65 According to the Diagnosis of Circulatory Diseases)

  • Purpose : As the number of patients with systemic diseases is increasing in the old, the relevance of oral health is gaining particular research interest. To provide fundamental resources for dental services, this study examined the relationship between doctors' diagnoses of circulatory diseases and patients' awareness of oral health and oral care behaviors. Methods : SPSS 26.0 was used to assess various variables, including doctor's diagnosis of circulatory diseases, gender, age, household income quintile, participation in economic activity, marital status, subjective level of oral health awareness, mastication discomfort, speaking and chewing discomfort, dental inspection, use of oral care goods, teeth brushing during the previous day, and untreated oral conditions. Results : The circulatory diseases suffered by the subjects were as follows: 56 % high blood pressure, 36 % dyslipidemia, 6 % stroke, and 8 % myocardial infarction or angina. A higher age meant a higher diagnosis rate of high blood pressure (p<.001) and stroke (p<.001). Those with dyslipidemia showed a higher rate of receiving oral inspection (p=.040), and an untreated oral condition was more frequently observed among those not diagnosed with the disease (p=.035). The subjects who were not diagnosed with stroke showed a higher rate of oral inspection (p<.001), while those who had a prior experience of stroke suffered a higher rate of mastication discomfort (p=.020). People who had high blood pressure showed a lower rate of using oral care goods (p<.001), and those diagnosed with stroke showed a lower rate of brushing teeth the previous day. Conclusion : This study found a correlation between the diagnosis of circulatory diseases and the awareness of oral health and oral care behavior. Consequently, oral health education should be included in mental health-related education, and customized training to teach teeth brushing and the use of oral care goods should be provided to patients with circulatory diseases during dentist visits.
