For females, the group with subjective needs was 2.17 (95% CI: 1.57-2.98 ) times more likely to use dental services than the group without subjective needs. Conclusions: To conclude, the elderly with symptoms, used the dental services for pain relief and functional recovery. Fundamental efforts to improve oral health are needed, such as providing treatment, prevention and educational services, besides efforts to provide treatment-oriented dental services. Further research is needed for the middle and elderly individuals who are in the blind spot of the existing policy.

  • This is an effort to project health care resources in need for the elderly in the future when advanced transportation systems would greatly reduce geographical accessibility to health care services for the rural elderly. Two areas, Kimhae and Chuncheon, were selected for the study. Projection of health professionals and health care institutions for the elderly were made based on the analysis for the morbidity and illness behavior reported to two data sources, National Survey for the Elderly in 1992 by Korea Institute for Health and Social Affairs and beneficiary data by Korea Medical Insurance Cooperation for those living in the study areas in 1992. Projected number of health professionals and health care institutions were estimated for each area under study in years of 2010 and 2030, with those in a Japanese being used as a standard. Policy implications were discussed.

  • Home health care is one of the important components of health care services. Today, the need and demand for the home health care is increasing. To assess the effects of home visit health services by public health nurses in health center on the stat of health, use of medical services and quality of life by elderly people living at home, a randomized controlled trial was implemented for 65 years or more old people randomly allocated to intervention(93) and control(118) group. Intervention group received 2 visits a month over 6 months. Control group received no home visits. The data was collected in a one-year follow-up survey conducted at Kyongju before and after the intervention which composed of health risk assessment, risk factor education and health related behavior counseling. The prevalence rate of chronic illness was more decreased in intervention group than control group after intervention. The intervention group visited medical facilities less frequently than control group. And the home visit health services encouraged the elderly to practice regular exercise. After the intervention, the score of ADL(activities of daily living), LSI(life satisfaction index) and SSI(social support index) in intervention group were more increased than control group. And the increase of scores was more prominent in 70 years or more old people, female, non-smoker and non-chronic illed elderly rather than others. In conclusion, the regular home visit health services provided by public health nurses were beneficial for the elderly in terms of health promotion and quality of life.

  • Purpose: Ageing in place may improve the quality of life of frail elderly and decrease their costs of services. The purpose of this study was to examine the factors that influence the institutionalization of elderly using home care services in a Korean long-term care insurance system. Methods: This study used the data of '2009 Satisfaction survey of Korean long-term care system'. The survey proceeded to use a sampling data based on region, level of long-term care need, and insurance type among the beneficiaries between August and September 2009. The onset dates of institutionalization of 1,095 participants were ascertained from long-term care insurance claim data. This study calculated the hazard ratio through the Cox Proportional Hazard Model. Results: A total of 176 subjects who were institutionalized in nursing homes were included. There were higher risks in the group that included those who were 85 years and over, had dementia or fracture, used home-visit nursing service, and were not supported by direct family. Conclusion: The results of this study have policy implications to supplement the home care service system and postpone nursing home institutionalization of elderly.

  • In Korea, as the elderly population is growing, the quality of life of the rural elderly is becoming a major concern. By the way, social welfare services is less available and accessible to the rural elderly. And we have very limited information about community -oriented welfare services for the elderly. The lack of social welfare services in rural area resulted from mainly geographic isolation and economic deprivation. So, the present research aimed at; 1) to explore what the social welfare service is benefited from governmental or local assembly. 2) to explore what kind of community services is provided for the rural village and elderly. 3) to appear the political propose for the rural elderly. In South Korea, The Rural Development Administration currently operate 'villages which is supported special programs for the improvement of QOL of rural elderly' in 110 villages. It is a model governmental welfare service for rural area. For the purpose, the survey data is gathered from community level data per village (107 villages), individual survey data(881people) who live in the village and qualitative data. Two kinds of quantitative data is combined to form a data. The statistical methods used for data analysis are descriptive statistics, t-test and ANOVA. The major findings of this study were as follows : It was founded that the majority family type of the rural elderly is elderly-only households(75%). In case of poor elderly, they have very limited social insurance benefit and they can not get medical services with satisfaction. The result show that the welfare facilities per village and welfare service for the elderly is extremely low while the needs of welfare service is greate high. A distinctive characteristics in rural villages is that they receive a lot of services from private sectors, like as Women Farmers Union, Adult Union. They operate voluntary welfare services related to food supporting, education for the elderly, free haircut services and so on. In conclusion, the community care services from private sector has specialty in rural area. We conclude it is a distinguishing characteristic of rural community.

  • Government provides financial support to the 74 Home help service centers, 36 Day care service centers, and 18 Short stay service centers for the elderly. The number of service centers that receive financial support from the government is far less to meet the potential demand for the community care services. This paper applies cost-benefit analysis to evaluate the net social benefit of the services provided by the 3 types of the community care service centers sponsored by the government to justify the expansion of the government support. The benefit is calculated as community care services are provided privately in the market without financial support from the government. The potential market price is regarded as the benefit or value provided to the elderly. The price levels that potential users are willing to pay for these services are surveyed in the Census for the Elderly by the KIHASA, 1998. The market prices for the community services are generated by equating limited amount of service supply, as in number of users in one year in 3 types of community care service centers, and potential demand for the services. Market prices are multiplied to the number of users of 3 types of community centers to get the total benefit. Total operating cost of the community care service centers is regarded as cost. According to the cost-benefit analysis, Home-help service centers generated net social benefit of 137 billion Won, Day Care service centers generated 15 billion Won, and Short stay service centers generated 6 billion Won. Significant amount of net social benefit indicates that government should increase level of financial support to these service centers.

  • This study is about major symptoms of elderly and medical services for elderly in long-tenn care facilities. The subject of this study was 298 patients over 00 years old staying in two geriatric hospitals and two nursing homes. The symptoms and medical services were level of patient classification from RUG(Resource Utilization Group)-III which is applied for both Medicare and Medicaid for skilled nursing facilities reimbursement system in US and designed for measuring patient characteristics and medical staff time. This classification is explained by each patient resource(staff time) utilization level which is called CMI(Case-Mix Index). In this study, the symptoms and services were compared by facility type and they were categorized by level and compared by CMI. Major findings are as follows; 1. There were more elderly who have cognitive function problems in nursing homes than patients in geriatric hospitals. There were more patients with behavioral problems in geriatric hospitals than residents in nursing homes. These results were both statistically significant. 2. The patients in geriatric hospitals received significantly more nursing rehabilitation services, rehabilitation services and extensive services than residents in nursing homes. Other hands, special care services were provided significantly more to residents in nursing homes than elderly in geriatric hospitals. 3. ADL and depression variables had higher CMI when the symptoms were heavier condition. The CMI were not matched with levels of cognitive function problems and behavioral problems. 4. The CMI matched well significantly with levels of nursing rehabilitation services, special care services, and clinically complex services provided for the patient in geriatric hospitals and only nursing rehabilitation services in nursing homes. The CMI for rehabilitation services level and extensive services had regular trends. From the result of this study, the resource utilization level and services provided for elderly in each long-term care facilities were figured out.
