This study aims to provide a customized dynamic kiosk screen that considers user characteristics to cope with changes caused by increased use of kiosks. In order to optimize the screen composition according to the characteristics of the digital vulnerable group such as the visually impaired, the elderly, children, and wheelchair users, etc., users are classified into nine categories based on real-time analysis of user characteristics (wheelchair use, visual impairment, age, etc.). The kiosk screen is dynamically adjusted according to the characteristics of the user to provide efficient services. This study shows that the system communication and operation were performed in the embedded environment, and the used object detection, gait recognition, and speech recognition technologies showed accuracy of 74%, 98.9%, and 96%, respectively. The proposed technology was verified for its effectiveness by implementing a prototype, and through this, this study showed the possibility of reducing the digital gap and providing user-friendly "barrier-free kiosk" services.
Background: Recognizing the seriousness of the misuse and abuse of medical narcotics, the South Korean government introduced the world's first narcotic management system, the Narcotics Information Management System (NIMS). This study aimed to explore the recent one-year opioid prescribing patterns in South Korea using the NIMS database. Methods: This study analyzed opioid prescription records in South Korea for the year 2022, utilizing the dispensing/administration dataset provided by NIMS. Public data from the Korean Statistical Information Service were also utilized to explore prescription trends over the past four years. The examination covered 16 different opioid analgesics, assessed by the total number of units prescribed based on routes of administration, type of institutions, and patients' sex and age group. Additionally, the disposal rate for each ingredient was computed. Results: In total, 206,941 records of 87,792,968 opioid analgesic units were analyzed. Recently, the overall quantity of prescribed opioid analgesic units has remained relatively stable. The most prescribed ingredient was oral oxycodone, followed by tapentadol and sublingual fentanyl. Tertiary hospitals had the highest number of dispensed units (49.4%), followed by community pharmacies (40.2%). The highest number of prescribed units was attributed to male patients in their 60s. The disposal rates of the oral and transdermal formulations were less than 0.1%. Conclusions: Opioid prescription in South Korea features a high proportion of oral formulations, tertiary hospital administration, pharmacy dispensing, and elderly patients. Sustained education and surveillance of patients and healthcare providers is required.
Background: Sutureless aortic valves may enable shorter procedure times, which benefits patients with elevated surgical risk. We describe the outcomes of patients with aortic stenosis who underwent aortic valve replacement (AVR) using the sutureless Perceval aortic bioprosthesis. Methods: Data from a retrospective cohort were obtained from a clinical database. The study enrolled patients with symptomatic severe aortic stenosis who underwent surgical AVR with a sutureless bioprosthesis between August 2015 and December 2020. In total, 113 patients were included (mean age, 75.3±8.4 years; 57.5% women; median Society of Thoracic Surgeons score, 9.7%; mean follow-up period, 51.19±20.6 months). Of these patients, 41 were octogenarians (36.2%) and 3 were nonagenarians (2.6%). Transthoracic echocardiography was employed to assess changes in ejection fraction (EF), left ventricular mass index (LVMI), and mean pressure gradient (MPG). Results: The in-hospital mortality rate was 2.6%, and 13 patients developed new-onset atrial fibrillation. A permanent pacemaker was implanted in 3 patients (2.6%). The median intensive care unit stay was 1 day (interquartile range [IQR], 1-2 days), and the median hospital stay was 12 days (IQR, 9.5-15 days). The overall survival rate at 5 years was 95.9%. LVMI and MPG were reduced postoperatively, while EF increased over the follow-up period. No structural valve deterioration was observed, and no meaningful paravalvular leakage developed during follow-up. Conclusion: The use of a sutureless valve in the aortic position is safe and feasible, even for high-risk elderly patients requiring surgical AVR. LVMI and MPG decreased postoperatively, while EF increased over the follow-up period.
BACKGROUND/OBJECTIVES: There are concerns about the adverse health effects of behavioral changes linked to coronavirus disease 2019 (COVID-19). We conducted a survey to investigate changes in lifestyle habits, including exercise and eating, during the COVID-19 pandemic and their association with changes in weight. SUBJECTS/METHODS: We conducted a survey of 5,000 people in Hakui City, Japan, to assess their lifestyles and diets during the COVID-19 pandemic. A total of 3,992 complete responses were received. We also obtained pre- and post-pandemic health check-up data for 704 of the respondents. These health data were combined with the results of the questionnaire survey to identify factors associated with weight changes. Data were analyzed for 704 individuals. RESULTS: The mean body mass index was 22.5 ± 3.1 kg/m2 for respondents whose weight did not change and 25.1 ± 2.7 kg/m2 for respondents whose weight increased. The pre-pandemic mean values for abdominal circumference in females and for gamma-glutamyl transferase in males tended to be higher in those whose weight increased. Those with decreased weight tended to be older. Respondents who were already overweight were more likely to gain weight because of COVID-19-related changes in their lifestyle. In males, alcohol consumption was directly associated with weight, and in females, abdominal circumference was more important. CONCLUSION: The study found pre-pandemic overweight individuals likely gained more weight during it, with alcohol consumption being a significant factor for males. Weight loss was more crucial for those over 70, rather than weight gain.
Background and Objectives: Takotsubo syndrome (TTS) is a form of reversible cardiomyopathy often preceded by mental or physical stressors and predominantly affects elderly women. Several cardiac and inflammatory biomarkers are involved in the pathogenesis of the disease. We aimed to investigate the correlation of C-reactive protein (CRP) level with left ventricular ejection fraction (LVEF) and clinical outcomes in patients with TTS. Methods: The study included patients with discharge-diagnosis of Takotsubo through 2017-2022 from the cardiology department. Demographic, laboratory, echocardiographic, and clinical outcomes were retrospectively obtained. We investigated the relation between CRP and LVEF, length of stay (LOS), in-hospital complications, and recurrence. Results: A total of 86 patients (93% female, mean age 68.8±12.3 years) were included in the study. The median CRP level was 17.4 (interquartile range [IQR], 6.1-40.1) mg/L, and the mean LVEF was 41.5%, (IQR, 38-50%). Complications occurred in 24 (27.9%) of the patients, and the median LOS was 3 (IQR, 3-5) days. The level of CRP was associated with lower LVEF (r=-0.39, p<0.001), longer hospital stay (r=0.25, p=0.021), and recurrence. There was no correlation between CRP and in-hospital complications. In multivariate logistic regression, poor LVEF was associated with TTS recurrence (odds ratio, 1.22; 95% confidence interval, 1.08-1.37; p=0.001). Using linear regression, only CRP was correlated with longer LOS and lower LVEF (p<0.001). Conclusions: Among patients hospitalized with TTS, CRP level was associated with poor LVEF and prolonged hospital stay but not with in-hospital complications. Poor LVEF was also associated with TTS recurrence.

This study was carried out to find out the effect of dietary intake on cognitive function retardation in old age using dietary survey and cognitive function test. The subjects were 332 men of 50-94 years old and their activities of daily living were very similar. The cognitive function was tested by Mini Mental State Examination (MMSE)-K which was translated from MMSE, and the 24-hour recall method was used for dietary survey. Scoring of MMSE-K was a little different from MMSE, that is, in case of no education, one to four points were added to exclude the effect of education which has been considered as a confounder by many researchers. The number of subjects belonging to below 23 of MMSE-K score was increased by increasing age. Even though points were added in case of no education, the ratio of below 23 MMSE-K score group was diminished by increasing education. Therefore, education seems not to be a confounder but a independent variable on cognitive function. Income, past occupation, family type, self-evaluated health status did not play any effect on cognitive function significantly. On the other hand, the correlation between each nutrients and the score of cognitive function test showed that the more consumption of vitamin A and protein, the higher cognitive function score was obtained. In case of iron and Ca, even though it was not statistically significant, there was a tendency of increasing cognitive function score by increasing the intake of those nutrients. This study suggests that micronutrient intakes might be more related to cognitive function than macronutrients.
The purpose of this study was to analyze womens' health problems using Green & Kreuter's 1991 PRECEDE model and to develop a model for a womens' health care center located in the community. The subjects were recruited from Wonju City. 1. The results showed that 23% of the sample population felt there was a need for a womens' health care center. The mean number of health problems was 3.1. The prevalence rate, was 44.4%, and the rate for an artificial abortion, was 36.4%. Also 30.5% did not have a health examination in the past year. Women using the hospital for medical care accounted for 45% of the sample, while 40% used the drugstore. The average score on the HPLP was 2.41, and this was influenced by self-efficacy, family support, sexual role, and health locus of control. There are a few educational programs in the city provided by the Wonju Health Center and by community health nurse practitioners. 2. The nursing center, as defined in North America, is a nurse-anchored system of primary health care delivery or neighborhood health center. Centers offer various services ranging from primary care to the more traditional such as education, health promotion, wellness screening, and coordination of services by advanced practice nurses. For examples in Sweden MCH centers provide total services for childbearing women and their families, sexual counseling and education for adolescents, and screening by midwives for cervical cancer. 3. The developed model combines purpose, target population, organization, and services, and is related to health resources. The purpose is primary health care and promotion of the quality of life. The target population can be grouped according to the life cycle, (premarriaged age group, the childbearing/child rearing age group, and middle aged and elderly women) and focuses on self-help. The organization of the center includes an advisory committee to plan and evaluate, and a health services team that will be multidisciplinary to provide health care, counseling education, and research. The model development suggested that a variety of women's health care centers are needed to insure adequate management of women's health. Follow-up research using PROCEED is needed to analyze health outcomes. Also a health nursing specialist system is required to develop health promotion, and improve the quality of life of women.

Objective: This study aimed to report the relationship between walking activity and perceived physical activity barriers after classifying the age group(≤64, 65-74, ≥75). Methods: The subjects were conducted on 1500 residents of two areas, and the walking activity was defined as five or more times a week, 10 minutes or more at a time, and 30 minutes or more per day. Perceived physical activity barriers were lack of time, lack of social support, lack of energy, lack of willpower, and fear of falling. Results: In the group under 64 year, the lack of time (exp(B)=0.819, p=0.002) and the lack of willpower (exp(B)=0.656, p<0.001) were significantly associated. In the 65-74 year old group, the lack of willpower (exp(B)=0.714, p<0.001) and fear of falling (exp(B)=0.787, p=0.003) were a significant association. In the group aged 75 years and over, lack ofwill power (exp(B)=0.734, p<0.001) and fear of falling (exp(B)=0.807, p=0.003) were significantly associated. Conclusion: In order to solve the lack of willpower and lack of time, a walking activity should be performed together at a certain time in the village, and the elderly in the late ages should reduce the fear of falling by performing a fall prevention exercise in parallel.
The purpose of this study was to examine the effect of the education intervention program as a nutrition program and exercise for the improvement of metabolic syndrome in women who voluntarily participated in health program in public health centers over 30 years of age. The subjects of this study were 120 women who completed the 12 - week obesity clinic program among the 400 obesity clinic participants in the northeastern of Seoul. Exercise and nutrition programs were administered on a daily basis for 1 hour every week for 12 hours with exercise and nutrition for 1 week to assess the effect of the participants on the risk factors of metabolic syndrome in health program. The metabolic syndrome and the risk of the disease were examined by analyzing the metabolic syndrome risk factors, change magnitude and change rates before and after the intervention. Waist circumference(p <.001), systolic blood pressure (p <.001), diastolic blood pressure (p <.001) and body mass index (p <.001) decreased during the 12 - week health program. Body weight (p <.01), waist circumference (p <.05), BMI (p <.01) and fat mass (p <.05) decreased more in the age group below 50 years.

Purpose: The aims of this study are to analyze and to visualize distribution of patients visiting at a dental college hospital, using geographic information system (GIS). The visualized data can be utilized in patient care and treatment planning, ultimately leading to the assessment of risk evaluation and prevention of dental diseases. Materials and Methods: Patient information data was obtained from Dankook University Dental Hospital including the unit number, gender, date of birth, and address from 2007 to 2014. Patient distribution was visualized using GIS. Statistical analyses were performed using SAS 9.3 and ArcGIS 10.1. Five factors including proximity, accessibility, age, gender, and socioeconomic status were investigated as the explanatory variables of the patient distribution. Results: The visualized patient data showed a nationwide scale of the patient distribution. There was a little difference in characteristics for each department. As closer at Dankook University Dental Hospital, visitors increased. And it strongly showed that elderly patients in rural areas tend to visit more. Conclusion: The distribution of patients has been shown to be significantly affected by the proximity, accessibility, age, gender and socioeconomic status. The underlying reason remains to be further studied.
