노인시설의 물리적 환경에 대한 거주노인 의견 조사방법의 분석 : 1990년 이후 해외 학술논문자료를 중심으로 (An Analysis on the Research Method of Elderly Residents' Opinion towards the Physical Environments of the Facilities for the Elderly : Focusing on Foreign Academic Journal Articles since 1990)

  • The purpose of this study was to provide information for research about residents' opinion toward the physical environments of elderly facilities, through the analysis and investigation on the research methodology of foreign academic journal articles from 1990 to 2014. The study results were as follows: Firstly, purposive sampling was a large majority of both facilities and elderly residents. In quantitative studies, many researchers have conducted simple random, cluster, or stratified sampling. Diverse facilities in area, size, location, and etc. should be considered for participation. The qualifications for residents' participation should be considered as well, so that they all could have autonomy for study participation. Secondly, questionnaire and semi-structured guide were likely to be used in independent and resident care facilities. On the other hand in assisted living and long-term care facilities, open questions and visual material were used as well. A compatible scale should be developed so that elderly having variable functional level could participate independently in the study. Thirdly, in data collection process, compliance with research ethics and well trained interviewer's skill were important for residents' active responses and minimization of response errors. Enough research period of time and mixed study in data collection will decrease the response error.

소형 아파트 주거환경의 유니버설 디자인 적용성 평가연구 - 전용면적 18평형 아파트를 중심으로 - (A Case Study on the Evaluation of Small-Sized Apartment Houses in aspects of Universal Design - Focused on the 18 pyung Apartment Houses -)

  • This study was intended to suggest that new small-sized apartment houses should be designed and provided on the base of the concept of universal design. This is based on the diversity of each resident, that is, resident of not as a family, but as an individual. On the background of this, this study evaluated the small-sized apartment house in aspects of universal design. The walk-through and the residents' evaluation were carried out by using 60 items related to the principles of universal design. The 18 pyung apartment complex and housing unit in Jangyou new town area was evaluated. Also, 100 residents living in that apartment housing unit were asked to evaluate these 60 items by using 5 point Likert scale. The walk-through with measuring and taking pictures was carried out by researchers. In general, the apartment house was not met the fundamental elements of universal design. Thus, it was not appropriate for the disabled or the elderly to live in. Specially, the door width of bathroom and kitchen balcony was too narrow to pass through. Also, the floor of entrance and bathroom was not flat, so that the wheelchair user cannot enter by alone. The residents evaluated negatively many items related to design, such as shape, color, and material. Therefore, in order to provide the living environments for diverse residents such as the elderly, or the disabled to live together, the new small-sized apartment houses should be designed on the base of the principles of universal design.

노인의 스트레스, 면역세포 변화, 신체적 건강상태 및 우울 (Stress, immune cells, physical health status and depression of elderly)

  • This study was to identify the levels of perceived stress, immunity cells, physical health and depression, and their relationships among those variables in the elderly who institutionalized comparing home residents. The result of this study can be used as basic data when applying nursing interventions to increase quality of life in the elderly. The questionnaires to estimate stress, health status and depression were collected through direct interview from July to August in 1999 and immunity cells were measured by venous blood specimen collected from 9 to 10 A.M. during the same period. The collected data were analysed using SAS program. The results were as follows. The score of perceived stress of all subjects was 38.49 and perceived stress score of institutionalized elderly(42.62) was significantly higher than that in home resident elderly(34.52). All immune cells tested in this study such as total T cell, helper T cell, suppressor T cell, T4/T8 ration, total B cell, and NK cell, were all under the standard criteria of cells distributions. Most elderly who institutionalized and reside home replied that their health status was not good. However their physical health activity was mostly good even though institutionalized elderly had more disability than home residents. The highest rate was 67.3% as disability due to arthritis. The score of depression in all subjects was 8.2 that indicated having depressive symptom. There was no difference in the depression level between institutionalized elderly and home resided elderly. There was a significant correlationship between physical health and depression, however, the rest of varibles did not show any significant relationships. In summary, the immune cells in the elderly who replied perceiving low level stress, was under normal range. Their health status was perceived as 'not good' but physical health activity was perceived as 'good'. The relationships of stress, immunity, physical health and depression were partially significant but not had evidence as enough as theoretically the suggested relationship. We suggest that further studies using large sample size and more diverse variables should be performed.

Behavioral Symptoms in Nursing Home Residents with Dementia: Developing a Nursing Practice Model

  • Purpose: Behavioral symptoms in dementia (BSD) are one of the most disturbing behaviors to caregivers and a major reason for nursing home placement. Behavioral symptoms are often treated with psychotropic drugs (PD), however, the effect of such drugs for the frail elderly dementia patient is not certain because of their critical adverse effects. Theoretical model applicable to nursing practice for BSD in nursing homes, which is essential in guiding and evaluating such interventions, is absent. This article presents the process of developing a theoretical model of BSD in nursing homes. Method: Using Walker and Avants' theory synthesis method, three behavior models and two system models were incorporated into the proposed model to provide the theoretical and analytical explanation of the relationships between PD usage, its determinants, and BSD. Results: Resident variables and nursing home variables related to the two focal concepts (i.e., PD usage and BSD) were identified. Resident variables include demographical characteristics such as age and gender, and dementia-compromised functions such as cognitive and functional impairment. Nursing home variables include facility characteristics such as ownership type and size, and physical and psychosocial environment. Conclusion: The proposed model suggests that fulfillment of resident unmet needs through improvement of physical and psychosocial environment may produce better health outcomes of nursing home residents with BSD. Assessment and intervening environmental triggers of such behaviors are also suggested to be prior to the PD usage.

주거공간에서의 고령자 행위별 자립도에 따른 세부행위 분석 (Analysis of Sub-Behaviors according to Elderly Resident's Level of Independence for Performing the Behaviors at Home)

  • This study specifies daily behaviors that elderly residents perform in their living space and verifies the behaviors that they have difficulties in performing. The study categorized elderly people according to the level of difficulties they have when performing household activities (independence of behavior performance) and verified characteristics of the sub-behaviors and the level of difficulties of performing them in each type. For this purpose, the study conducted observation investigation on the behavior of elderly people by directly visiting houses of 52 elderly people over the age of 65. The characteristics of sub-behavior were also examined through photograph shooting and in-depth interview. In this study, behaviors of elderly people inside living space were categorized into the following nine behaviors. The study investigated the difficulties of performing these sub-behaviors according to the elderly people's level of independence by each behavior. Analyzing the difficulties of sub-behaviors according to the independence level, elderly people felt more structural problems and inconvenience in using facilities when they have lower independence level. Moving from independence to dependence, their performance of behaviors gradually became difficult, making them inevitable to use tools. At last, they came to need caregiver's help. For continuous sustenance of elderly people's independent living inside house, policy measures are required that can address the difficulties of sub-behaviors that are observed in a series of process of one's becoming dependent from independent in this study.

주거지 재생지역의 기존주택 기능전환에 대한 노인의 반응연구 (Elderly response to alteration of existing house function in housing improvement area)

  • As an aging population has increased vastly and nuclear families have been a dominating family type in modern Korean society, the numbers of the elderly who suffer Empty Nest Syndrome and LID (Loss Isolation Depression) syndrome have been accelerated. These syndromes involve psychological instability, melancholy, and lethargy. To make the elderly get out of this phenomenon and live actively, the scheme of converting their existing house into shared housing was set as a hypothetical solution in this study. This study sets out to find out responses of the elderly to alter their own house's function in housing improvement area. A Small Workshop Panel method was used and the elderly aged 55 years upwards took part and they own a detached house in a housing improvement area that is relatively large for the elderly or the elderly couple to live alone. Through the workshop, problems that the elderly have been experiencing within their houses were looked into and responses of the elderly on altering their house to shared housing by introducing developed schematic plans. Although, the first response was negative, positive responses from the house owners were carried out when a visual support i.e. developed shared housing plan was provided. The positive response was based on the fact that subsidies from the government on house renovation and tenants matching programs are supported as shared housing could provide home owners economical support with rents and help them to feel less lonely. This is a way of relieving the elderly from being neglected and supporting them to live and age actively in their later lives. Furthermore, by providing a visual media to the elder residents in housing improvement area, their greater understandings on the development and addressing their opinions were possible. Therefore, more tools that promote elder residents' participation are needed for future housing improvement projects and an aging society.

노인의 의미 있는 활동에 관한 융합 연구 (A Convergence Study on the Meaningful Activity Experience of the Elderly)

  • 이 연구에서는 노년기 의미 있는 활동의 요인을 분석할 목적으로 시행되었으며, 최종연구대상은 65세 이상 110명을 대상으로 자기기입식 설문을 수행하였다. 연구방법은 SPSS 통계프로그램을 이용하여 빈도분석 및 student t-test와 one-way ANOVA를 실시하였으며, 위계적 회귀분석을 통해 노인의 의미 있는 활동에 미치는 요인을 파악하였다. 연구결과 '나는 가족과 함께 하는 일(활동)이 보람 있다고 생각 한다'가 3.95±0.64점으로 가장 높게 나타났다. 위계적 회귀분석 결과 노인의 의미 있는 활동에 미치는 요인은 1.2.3단계 각각 거주지(서울)(β=-.308, p=.002), (β=-.330, p=<.001), (β=-.281, p=<.001)로 나타났고. 2단계에서 연령(β=-.215, p=.026) 나타나 노인의 의미 있는 활동에 미치는 요인은 거주지(서울)와 연령으로 나타났다. 결과적으로 앞으로 노년기 의미 있는 활동을 위해서는 지역을 고려하여 연소노인(65-74세) 대상으로 한 가족과 함께 하는 일(활동)이면서 대화 및 의사소통에 우선한 프로그램의 개발이 필요하다. 추후 노년기 의미 있는 활동을 위한 다양한 프로그램의 체계적인 적용이 필요할 것이다.

퍼소나 기반 시나리오 방법론을 적용한 주택에서의 노인 행위 시나리오 개발 (Developing Scenario for Elderly Residents' Behaviors at Home Using Persona-based Scenario Method)

  • An analysis of elderly behavior at home needs to be meticulously understood in order to build a spatial environment where the elderly can live safe and independent lives. To this end, the following study analyzed elderly behavior that occurs in the home, constructed personas, and developed persona-based scenarios of elderly behavior at home. Persona was composed of behavioral characteristics, spatial characteristics, and personal characteristics (gender, age, disease etc) that have a direct effect on living activities in the home, so it was intended to express a more real persona. The study developed six personas based on the behavioral and mobility characteristics of the elderly, and scenarios for each persona developed bsed on the following eight behaviors: toilet use, bating/Shower, washing and grooming, having meal, sleeping, dressing/undressing, laundering clothes, and getting out of home). Through this, the study determined the characteristics and difficulty level of various sub-behaviors and actions that occur in elderly behaviors at home. It also determined characteristics related to the use of physical elements such as equipment and furniture that are a part of those sub-behaviors. The behavior scenarios developed in this study can be utilized to understand the difficulties involved in elderly behavior, and to present a spatial plan and remodeling direction that alleviates such difficulties. This study is also significant that it investigates the applicability of persona-based scenario method on residential architecture.

요양시설 입소 노인 가족의 가족참여 요구도에 관한 연구 (Expected Family Involvement of Family of Elderly Residents in Nursing Homes)

  • Purpose: The purpose of this study was to investigate the expected family involvement of family members of elderly residents in nursing homes. Methods: This study utilized a descriptive design with a survey research method. Data were collected using the Expect Family Involvement Questionnaire Scale, which was completed by 112 family caregivers of older adults in nursing homes in Seoul and Gyeong-gi Province. The collected data were analyzed using descriptive statistics. Results: Responses to the questionnaire indicated the highest value was recorded for the item, 'use of a room for family privacy' with $2.41{\pm}0.43$ points, followed by the item, 'staff calling family for advice with resident problems' with $2.38{\pm}0.38$ points and 'education about nursing home programs' with $2.32{\pm}0.42$ points. Conclusion: In order to promote family involvement in long-term care facilities, various approaches, interventions, and government supports with the respective of the family are required. The nursing home placement process should include understanding discuss on the importance of the role of family, and should help promote positive relationships between the elderly residents and their families. Staffs and administrators of nursing home are to consider the family as a partner rather than visitor.

Linking Clinical Events in Elderly to In-home Monitoring Sensor Data: A Brief Review and a Pilot Study on Predicting Pulse Pressure

  • Technology has had a tremendous impact on our daily lives. Recently, technology and its impact on aging has become an expanding field of inquiry. A major reason for this interest is that the use of technology can help older people who experience deteriorating health to live independently. In this paper we give a brief review of the in-home monitoring technologies for the elderly. In the pilot study, we analyze the possibility of employing the data generated by a continuous, unobtrusive nursing home monitoring system for predicting elevated(abnormal)pulse pressure(PP) in elderly(PP=systolic blood pressure-diastolic blood pressure). Our sensor data capture external information(behavioral) about the resident that is subsequently reflected in the predicted PP. By continuously predicting the possibility of elevated pulse pressure we may alert the nursing staff when some predefined threshold is exceeded. This approach may provide additional blood pressure monitoring for the elderly persons susceptible to blood pressure variations during the time between two nursing visits. We conducted a retrospective pilot study on two residents of the TigerPlace aging in place facility with age over 70, that had blood pressure measured between 100 and 300 times during a period of two years. The pilot study suggested that abnormal pulse pressure can be reasonably well estimated (an area under ROC curve of about 0.75) using apartment bed and motion sensors.
