Uniformity Evaluation of Elderly Hospital Outpatients' Waiting Space using Discrete Event Simulation (이산사건 시뮬레이션을 이용한 요양병원 외래부 대기공간 균일성 평가)

  • In recent years, the introduction of complex systems analysis based on various variables has become more active in order to identify and analyze complex problems of Modern Society. Prediction of patients' spatial perception and usability according to the spatial arrangement of the outpatient department is a very important factor for providing high quality hospital service. For objective analysis, the standard program procedure and analysis index for the diseases of the elderly were prepared and the uniformity of the atmospheric space was evaluated through heat map analysis and quantitative analysis. In this study, 73 cells were installed and simulated to analyze the uniformity of the four alternatives according to the change of the arrangement of the medical care space, receiving space, and consultation space using the complex system analysis method for the nursing hospitals. The resulting density was derived. The results are as follows. 1)The layout of the reception space has the greatest influence on the total spatial density of the waiting space. 2) The uniformity of the waiting space can be increased by separating the examination space and the examination space. 3)The closer the location of the receiving space is from the entrance, the greater the density of the waiting space. Finally, this study applied discrete event simulation to the evaluation of uniformity of atmosphere space, and proved that the actor - based model can be utilized for utilization and evaluation as spatial analysis methodology.

The Effects of Spaced Retrieval Training with Errorless Learning on Memory, IADL, Depression in Mild Cognitive Impairment: Single-Subject Design (오차배제훈련을 병행한 시간차 회상훈련이 경도인지장애 환자의 기억력에 미치는 효과와 수단적 일상생활(IADL) 및 우울에 미치는 영향: 단일대상연구)

  • Objective: The purpose of this study was to examine the effect of the Spaced Retrieval Training (SRT) with Errorless learning on the elderly with Mild Cognitive Impairment (MCI)'s memory, Instrumental Activities Daily Living, Depression symptom. Methods: A single subject experimental research with ABA design was conducted in this study on the 78-years-old person who was enrolled in day-care center. The total experimental sessions were 16 which composed of 3 sessions for baseline, 10 sessions for intervention and 3 sessions for second baseline. K-Auditory Verbal Learning Test (K-AVLT) was measured for the memory each session. For the measurement of cognitive function, IADL, depression Symptom, Korean version of Montreal Cognitive Assessment (MoCA-K), Philadelphia Geriatric Center Instrumental Activities Daily Living (PGC IADL), Geriatric Depression Scale Korean Version (GDS-K) was measured at pre-post test. Results: Memory at the phase B was improved than Phase A. At the phase B, the scores trend was ascending, but after the intervention at the phase A', the scores trend was descending. The scores of MoCA-K were improved, PGC IADL were maintained, GDS-K were decreased. Conclusion: This results support the evidence of the SRT with EL on the elderly with MCI in the clinical setting. In the future, the correlation researches about MCI's memory and other functional factors will be needed for effective occupational therapy service.

Association between job types of economically active population and sleep appropriateness among South Koreans (국내 경제활동 인구의 직업유형별 적정수면과의 연관성)

  • Background: As of 2016, average Koreans sleep 7 hours and 42 minutes, the lowest figure among Organization for Economic Cooperation and Development(OECD) countries, and the number of people with sleep disorders reached 561,000. Accordingly, the government has promoted the provision of 'Multiple Sleep Test' to strengthen the diagnosis service for patients with 'sleep disorder' in july 2018. As a result, healthcare costs for patients with sleep disorder is on the rise every year. In this study, we utilized 'Appropriate Sleep' criteria of United States's National Sleep Foundation(NSF) then investigated Korean's sleep pertinence using 「7th National Health and Nutrition Survey for 2016-2018」 by different occupational type, demographic characteristics, socio-economic characteristics, and health behaviors. Methods: We performed descriptive analysis to examine differences of sleep appropriateness by various sample characteristics. Multivariate logistic regression models were used to examine sleep appropriateness by occupational type and other variables. We also analyzed subgroup models to investigate. Results: As a result, a total of 1,948 (18.37%) study subjects experienced in-appropriate sleep. Results of the Multivariate logistic regression analysis revealed that blue color group had a higher odds ratio (OR) for experiencing in-appropriate sleep (OR=1.179). In addition, the odds ratio of experienced in-appropriate sleep among the elderly aged 70 and over was 2.698, and the odds ratio of the overstressed group was 1.299. Furthermore, sub-group analysis showed that blue color job of female(Or=1.334), high school or below(OR=1.404), divorce/death/separation(OR=2.039), 25%ile-50%lie income group(OR=1.411) more likely experienced in-appropriate sleep. Conclusion: Growing sleep disorder patients and related health care costs are expected. Government should apply detailed 'total periodic sleep disorder management policy' including pre-consultation, examination, diagnosis, treatment, post-consultation, self-management especially to vulnerable population that this study found.

  • This study examined the specific contents of social welfare policies of the Joseon Dynasty through the analysis of Mokminsimseo, Aemin Sixjo and Jinhwang Yukjo, and identified the connection between them and today's social welfare policies. As a result, In the Care of the People, it contained the basic contents of the Elderly Welfare Act and welfare services for the elderly, and in the Freedom, it included welfare policy services related to infants and children, and the value system of welfare for the disabled through government affairs. The policy direction was confirmed, and it was found that disaster relief calls for social integration in response to social crises and disasters. This also confirmed that major areas of social welfare policy such as family safety, income security, health policy, and social integration have been emphasized through social welfare policy indicators. In addition, through the analysis of Qinhwangyukjo, it was found that it contains not only the basic ideology and values of the social security system and social insurance system in terms of visa, scale, and power, but also the details of specific policy implementation. Today's social welfare policies and social service policies, which are equipped with social welfare facilities and are implementing systematic social welfare services for each target, were able to confirm their foundation through the Qinhwangyukjo. This confirms that the emphasis on social integration and income security is prominent in the Qinhwangyukjo through the social welfare policy indicators of the current policy system.

A Study on the Variables Affecting the Intention to Use Healing Agriculture (치유농업 이용의도에 영향을 미치는 변인 고찰)

  • The purpose of this study is to investigate the factors affecting the intention to use healing farming by setting aged readiness, rural healing supply condition, and rural healing service as independent variables. This study has been started based on the idea that it is necessary to provide healing service through healing agriculture to the rapidly increasing elderly people and urban people who need mental rest. Therefore, the purpose of this study is to find out the various factors influencing intention to use healing agriculture through empirical analysis. Second, we want to examine whether the variables influenced the intention to use more. Third, based on the results of these tests, we suggest the policy for activation of healing agriculture by examining the factors necessary for the promotion of physical and mental health of the elderly in the aging society and the revitalization of healing agriculture for the mental healing of the urban people. For this study, a questionnaire survey was conducted for men and women over 30 years old, and the final 356 copies were analyzed. The validity of the research hypothesis was verified by linear regression analysis. The results of the analysis are as follows. First, the physical preparation, emotional preparation, and economic preparation of aged care preparations were found to have a significant effect on intention to use. Second, natural landscape, accessibility, and stability of rural healing supply conditions were found to have a significant effect on intention to use. But economics and expertise were dismissed. Third, crop cultivation, animal medication and healing facilities of rural healing service were proved to have significant influence on intention to use. In order to increase the utilization of healing agriculture, it is necessary for the consumer to be well prepared for aging. In rural healing supply conditions, accessibility and safety should be provided for healing facilities in rural healing services.And to increase the intention to use it.

  • The study aims to develop a assessment tool to provide the in-home disabled in a local community with appropriate services in consideration of physical, emotional, social and environmental circumstances. After collection of assesment data of 200 in-home disabled through use of the tool, a desire-extracting algorithm was developed to provide a service to real needs through the use of decision tree analysis on data mining. The study was conducted for Five months from June 2006 through October 2006, and it is divided into development of an assessment tool and extraction of real needs through the use of the tool. The basic framework of the development of the tool was established through the examination of related literature, the subjective satisfaction of the assessment tool and items were developed through the use of a focus group and experts, and verification was implemented through the use of statistics to confirm the validity of the tool.   • The Comparison of Recognizing Personal Health Record Between Healthcare Students and Medical Students (보건대학생과 의과대학생 간의 개인건강기록(PHR) 인식 비교)

    • With the paradigm shifts towards consumer-centered health service, it is expected that more health care consumers will keep their health information and manage their own health in the future. Thus, this study was conducted to compare "Understanding", "Utilization" and "Management" of Personal Health Record(PHR) between medical users(healthcare students) and health care providers(medical students). We collected data from 208 health and medical students via using self-reported questionnaires form April to June, 2011. The collected data were analyzed using frequency, t-test, Chi-square on SPSS 19.0 version. There was no significant difference in "Understanding" of PHR between two groups. Looking at the order of the importance of PHR contents, two groups equally emphasized medical records, surgical history, and test results. There was significant difference in both time and effectiveness of PHR(p=0.02). Intergrated type of PHR was preferred by both groups. Recently, PHR reflects needs and demands of users more than ever. However there are many limitations to promote the utilization. In the future, it is necessary to implement targeted strategies for the elderly groups and specific types of disease.

    • The purpose of this study was to investigate the single-person household's demand for a wearable healthcare system that provides the ischemic heart disease and cardiopulmonary alarm to be developed. In this study, in-depth interviews were conducted with six unmarried individuals in their thirties who were in the workplace and the results were interpreted by Giorgi's phenomenological analysis method. The study results indicated that although the wearable healthcare system was implemented with high-technology functions that serve the intended purpose, it is necessary to comprehensively consider factors such as aesthetics, comfort, and ease of washing. In this study, we found that the variables influencing the interviewees' intent to accept healthcare services are complex and cannot be attributed to one factor alone; this study has also shown a variety of interpersonal correlations. In addition, although health-care research focusing on the elderly has been conducted in the past, it is suggested that younger generations such as those in their thirties can also be studied as health care service adopters.

    An Analysis of Infrastructure and Provision of Forest Welfare Service in Nursing Homes for the Elderly (노인요양시설의 산림복지서비스 인프라 및 제공 실태)

    • This is a cross-sectional study that suggests ways to activate forest welfare services (FWS) by investigating the infrastructure, service status, and perception on FWS in Korea. In August 2016, a structured email survey was conducted in nation widely. The respondents were mostly directors and general secretary (75.0%). The considerable number (16.3%) of nursing homes (NH) use some floors of the complex buildings that would be difficult to have FWS infrastructure and about 30% of those without forests near the facilities. The directors of NH recognize that FWS has positive effects on the elderly. However, FWS is not an requisite of the longterm care insurance benefit, and so costly and effort-intensive that FWS has not been activated so far. In order to activate FWS in NHs, it is necessary to develop and disseminate the guidelines on FWS that anyone can easily followed. In addition, when the National Health Insurance Corporation evaluates NHs, they should evaluate not only whether there is a wandering or walking space, but also whether it has forest healing factors such as forests. It is also necessary to create a barrier-free environment both inside and outside of NHs, increasing accessibility to the toilet in gardens, paving a passage for wheelchairs and lifts in forests near NHs. Through these efforts, it is expected that FWS will be activated to provide physical, mental rest and comfort, appropriate cognitive stimulation to the NH residents at the end of life.

    Factors Associated with Unmet Needs for Medical Care among Island Inhabitants in Korea (일개 섬 지역 주민의 미충족 의료와 관련 요인)

    • Objective:Korea has 3,170 islands with about 188,000 inhabitants. These inhabitants' needs for health services might go unmet because of geographic isolation, slimmer availability of health services, and higher proportion of the elderly compared with the mainland population. Unmet health service needs might result in serious health problems for these island residents. Therefore, the purpose of this study was to investigate their unmet health service needs and related factors from population of Nowha island. Methods: The survey was conducted from July 30 to August 1 by trained interviewers. Target population were residents in Nohwa island and the sampling method was incomplete quota sampling. General characteristics, socioeconomic status, utilization of health service, and unmet medical service needs were investigated. First univariate and then multivariate logistic analyses were done for the statistical analysis. Results: 324 residents were surveyed and the proportion of unmet health services needs was 26.5%. People living alone and of female sex had increased health services needs based on univariable analysis. From the multivariate analysis, those living alone increased the health services needs. Self-determined low socioeconomic status and medical aid increased the proportion of unmet medical needs, but these were not statistically significant. Conclusion:In Nowha island, those who are elder, and of lower socioeconomic status had much higher unmet medical needs. The results suggest the need for more social support by qualified health services to solve this unmet medical needs problem.
