Delayed onset muscle soreness (DOMS) was the sensation of discomfort and stiffness in the muscle, often after taking part in unaccustomed physical activity. No universally accepted treatment exist. The aim of this study was to examine the influence of cryotherapy and intermittent compression on the delayed onset muscle soreness. Flexion elbow joint position and extension(Universal Goniometer). pain(Muscle Soreness Rating Scale) and mechanical pain threshold(Algometer) were measured before 30minutes DOMS was induced.The data were analyzed by measure of Mann-Whitney test and Kruskal-Wallis test. The result were as follow; 1. There were no significantly differences between groups or over time in relation to range of motion. 2. Muscle Soreness Rating was significantly high in cryotherapy and intermittent compression at 48, 72 hours after DOMS was induced(p<.05). 3. Mechanical pain threshold begin to increased at 24 hours and significantly in cryotherapy and intermittent compression groups at 48, 72 hours after DOMS was induced(p>.05). 4. A negative Correlation between muscle soreness rating scale and mechanical Pain threshold graphs at 24, 48, and 72 hours after exercise indicated in cryotherapy and intermittent compression groups.
During the period from March 1990 to November 1993, 22 children with diplaced supracondylar fracture of humerus were treated by closed reduction and percutaneous K-wire fixation at department of Orthopaedic surgery in Yeungnam University. All fracture were treated with closed reduction under the general anesthesia and percutaneous pinning. After the fracture was internally fixed, intraoperative anteroposterior reontgenograms of each distal humerus were compared. Only anatomical reduction ensured good result, because the main cause of late cubitus varus was medial tilting of distal fragment. We could follow up 10 patients more than one year. By Flynn's criteria, satisfactory result were obtained in 9 of the 10. Unsatisfactory one had a limitation in flexion of elbow joint, but had no problem in cosmetic feature. In short term follow up of 7 patients, 5 patients had a satisfactory result, too. There were no neurovascular complication and hospitalization was shortened to 11.4 days. Closed reduction and percutaneous pinning provided stabillity, vascular safety, simplified management, reduced hospital stay. This is safe and reliable technique for obtaining and maintaining an excellent reduction.
The objective of this paper was to evaluate the effectiveness of horizontal, vertical, asymmetric and coupling multipliers for manual material handling. Lifting tasks with 5 different horizontal distances ($30{\sim}70cm$) for 6 vertical distances(ankle, knee, waist, elbow, shoulder and head height) were experimented. The muscle activity and muscle exertion level during asymmetric load handling(without trunk flexion) was experimented. Lifting tasks with and without handle tote box for three postures(straight, bending, right angle posture) were experimented. The degrading tendency did not appeared almost in $60{\sim}70cm$ interval's horizontal distance. As a result of ANOVA, MVC paid attention to horizontal and vertical distance but cross effect was insignificant(p<0.01). The change of the MVC according to the horizontal, vertical distance appeared similar from of RWL. The results of normalized MVC measurement were decreased about 16%, 24%, 34% respectively as the asymmetry angle was $30^{\circ}$, $60^{\circ}$, $90^{\circ}$. RMS EMG values of right erector spinae muscles were decreased as the work posture went to $90^{\circ}$ and those of left erector spinae muscles were increased until the asymmetry angle was $40^{\circ}$ but decreased continually over $40^{\circ}$. 7 subjects, activities of left and right latissimus dorsi muscles were maintained constantly, while for remainer, those were irregular. MVC reduced maximum 23% by type of handle. MVC was highest in straight posture, but was lowest in right angle posture. As a result of ANOVA, MVC paid attention to posture, coupling(p<0.01). To all handle types, biceps brachii activity was increased in right angle posture, but reduced in straight posture. Based on the results of this study, it is suggested that the NIOSH guideline should not be directly applied to Korean without reasonable reexamination. In addition, we need to afterward study through an age classification.
We have made a survey of 40 patients in the university hospitals and oriental medical centers in Seoul from Sep. 1, 1997 to Mar. 1, 1998. We sampled 25 of them and the result shows that there were 12 MCA damaged patients(48%), 5 SAH(20%), 5 ACA(20%), 2 PCA (8%), 1 PCOA(4%). The number of MCA patients were the most. 1. As the cause of each disease, 4 of the 12 MCA damaged patients(33.35%) have infarction and cerebral hemorrhage, 2 of 5 SAH patients(40%) have cerebral hemorrhage and head injury, 3 ACA damaged patients have cerebral hemorrhage. 11 of 25 brain bloodvessel damaged patients(44%) were hemorrhage patients. 2. Rt. hemiparesis was the main symptom of 6 of 12 MCA damaged patients(50%) and 3 of 5 SAH patients(60%), and the main symptom of 3 of 5 ACA patients(60%) was Lt. hemiparesis. The main symptom of 13 of 25 brain bloodvessel damaged patients(52%) was Lt. hemiparesis 11 of them(44%) Rt. hemiparesis, and 1 of them(8.3%) Quadriplegia. 3. Language was the most well preserved function. 12 MCA damaged patients could understand language. 4. Retraction of shoulder girdle, among VIE flexor synergy, was the most frequent element because 9 of 12 MCA damaged patients had it. Among VIE flexor synergy, 5 SAH patient's most frequent synergy was Elbow flexion because all of them had it. All of 5 ACA damaged patients have shoulder girdle elevation, shoulder joint, hyperextension, abduction, and external rotation among VIE flexor synergy. 5. 7 of 12 MCA damaged patients(58.3%) were stereognosis handicapped patients, 3 of 5 SAH patients(60%) have handicap of position sense, light touch, and temperature, 3 of 5 ACA patients(60%) have position handicap. 13 of brain bloodvessel damaged patients(52%) have light touch handicap. 6. 8 of MCA damaged patients(66.7%) have facial palsy, 4 of SAH damaged patients(80%) have memory and action decline, and 3 of ACA damaged patients(60%) have action decline and facial palsy. The problem of Hemiplegia is very extensive from muscle weakness, atrophy, or deformation to psychical problems. Therefore physical therapists should have sufficient interest in psychological handicap as well as physical handicap as they deal with adult hemiplegia.
Purpose: This study examined the effects of the directions of neck rotation on the muscle activity of the upper trapezius and lower trapezius while rotating a shoulder externally. Methods: Twenty-five healthy males participated in this study. The subjects were asked to rotate their shoulder externally with 90° shoulder abduction and 90° elbow flexion in three different neck rotations (neutral, ipsilateral, and contralateral) in the prone position. The muscle activities of the upper and lower trapezius were measured using surface electromyography. One way repeated measures ANOVA was used to compare the muscle activity of the upper and lower trapezius depending on the different neck turning directions. Results: In the upper trapezius, turning the neck in the ipsilateral direction while turning a shoulder externally decreased the muscle activity significantly, but the muscle activity was increased significantly by turning the neck in the contralateral direction. On the other hand, in the lower trapezius, turning the neck in the ipsilateral direction increased the muscle activity significantly, but the muscle activity was decreased significantly by turning the neck in the contralateral direction decreased it significantly. Conclusion: When someone has an imbalance of shoulder function, turning the neck in the ipsilateral direction while turning the shoulder externally in the prone position is effective in decreasing the activity of the upper trapezius and increasing the activity of the lower trapezius. Therefore, these results could be used as basic evidence for researching patients with shoulder problems.

The purpose of this study was to analyze the shoulder joint torque and muscle pattern performing as Tennis serve by Isokinetic motions on Isomed 2000. The subject, who was 5 males Korean national high school tennis players. By analyzing the flexion/extension of the shoulder, the peak torque of the internal/external rotation motion(at 60,180 and 300 degree/sec) at peak torque degree, the weight, peak torque, and power. combined with the timing of the electrode of the attached trapezius and posterior deltoid at the three part. From the data analysis & discussion the following conclusions were drawn. When doing a shoulder extension, the peak torque can be widely seen at 60degree per second. However the degree may be different depending on angular velocity. When doing an internal rotation at 90degree abduction, peak torque per weight was seen at 60degree per second. The degree of peak torque was at 31.6-44.2 and peak power was faster when angular velocity was increased. The aspect of muscle pattern was seen more at the internal rotation in the 90degree abduction rather than the shoulder extension. However the angular velocity was not influenced by muscle mobilization(in order of anterior deltoid, posterior deltoid and trapezius. To properly apply the above conclusion, when tennis players serve it is better the elbow be impacted by the extension. when doing isokinetic motion it is better to increase angular velocity and improve muscle power. also the anterior deltoid amongst the shoulder muscle should be improved to develop serve speed.

Purpose: The purpose of this study is to figure out that the scapular stability exercise on unstable position can effect on the pain relief of the patient with lateral epicondylitis. Methods: The subject was 35 year old male, diagnosed as lateral epicondylitis. This patient was controled to do scapular stability exercise with crawling position on unstable surface, changing elbow movement from flexion to extension for 4 weeks, 5 times a week, 20 times in total. We used VAS to find the degree of pain and Cozen's test, Mill's test and Resisted middle finger test were determined for the measurement of epicondylitis changed. Results: The following is the result of this study. 1. Pain on lateral epicondylitis was relived from VAS 7 to VAS 0. 2. There were improvements that the results of tests for epicondylitis, Cozen's test, Mill's test and Resisted middle finger test, changed positive into negative signs. Conclusion: The result of this study indicates that the application scapular stability exercise on the unstable surface to the patient with lateral epicondylitis can relief the pain degree on the lateral epicondylitis and be used as one of lateral epicondylitis treatments.

Purpose : To identify whether isometric shoulder horizontal extension (ISHE) exercise could sufficiently activate the lumbar multifidus muscle, and to determine appropriate exercise intensity to increase muscle strength. Methods : Twenty healthy volunteers (10 males and 10 females) participated in this study. ISHE exercises on the dominant side were performed with $90^{\circ}$ shoulder abduction held in $90^{\circ}$ elbow flexion so that their arms were in the horizontal plane. Electromyographic (EMG) measurements of multifidus activation were performed in standing and supine positions, and were taken under four strength conditions: 75%, 50% and 25% of maximum shoulder horizontal extension strength, and maximum strength. Results : The EMG activations of both lumbar multifidus increased significantly with ISHE exercises of larger % strength (p<.05). In their multifidus EMG data measured in standing and supine positions, men differed significantly in their 75%, 50% and 25% strength for both sides (p<.05), and women differed significantly in their 75%, 50% and 25% strength on their 5th lumbar vertebrae's left side, but 75% only for their 5th lumbar vertebrae's right side (p<.05). The EMG data of 5th lumbar vertebrae's left and 5th lumbar vertebrae's right sides appeared to differ significantly at all strength levels for men while standing, but only at the 25% level for women in the supine position (p<.05). These findings indicate that ISHE exercises can be considered a beneficial method to enhance the multifidus strength. Conclusion : This study provides useful information for further study in this field.
This study was to perform the kinetic analysis of forward $1\frac{1}{2}$ somersault on the platform diving. Six men's diving players of the Korea national reserve athletes participated in this study. The variables were analyzed response time, velocity, center of mass (COM), angle, center of pressure (COP) and ground reaction force (GRF) of motion. For measure and analysis of this study, used to synchronized to 4 camcorder and 1 force plate, used to the Kwon3D XP (Ver. 4.0, Visol, Korea) and Kwon GRF (Ver. 2.0, Visol, Korea) for analyzed of variables. The results were as follows; Time factor were observed in maximum knee flexion depending on the extent of use at phase 1 of take-off to execute the somersault. This enabled the subject to secure the highest possible body position in space at the moment of jumping to execute the somersault and prepare for the entry into the water with more ease. Regarding the displacement of COM, all subjects showed rightward movement in the lateral displacement during technical execution. Changes in forward and downward movements were observed in the horizontal and vertical displacements, respectively. In terms of angular shift, the shoulder joint angle tended to decrease on average, and the elbow joints showed gradually increasing angles. This finding can be explained by the shift of the coordinate points of body segments around the rotational axis in order to execute the half-bending movement that can be implemented by pulling the lower limb segments toward the trunk using the upper limb segments. The hip joint angles gradually decreased; this accelerated the rotational movement by narrowing the distance to the trunk. Movement-specific shifts in the COP occurred in the front of and vertical directions. Regarding the changes in GRF, which is influenced by the strong compressive load exerted by the supporting feet, efficient aerial movements were executed through a vertical jump, with no energy lost to the lateral GRF.
Background: Scapular dyskinesis may cause not only rotator cuff (RC) tear but also weakness of the upper extremity, studies on scapular dyskinesis that may occur after RC repair is still lacking. Objects: To determine whether scapular dsykinesis was present in patients after arthroscopic RC repair and to investigate the influence of passive scapular stabilization on upper extremity strength. Methods: A total of 30 patients after RC repair participated in this study. To compare the scapula of the arthroscopic RC repair shoulder and the contralateral shoulder, the winged scapula (WS) was measured using a scapulometer and scapular dyskinesis was also classified by type. Fixed instruments for muscle strength measurements were used to measure upper extremity muscle strength differences depending on passive scapular stabilization position or natural scapular position. A chi-square test, an independent t-test and a 2-way mixed measures analysis of variance (ANOVA) was used as statistical analysis. In analyses, p < 0.05 was deemed to be statistically significant. Results: Postoperative shoulder had a significant association with scapular dyskinesis and the WS compared to the contralateral shoulder (F = 0.052, p < 0.01). Postoperative shoulder, muscle strength in the shoulder abduction (p < 0.01), elbow flexion (p < 0.01) and forearm supination (p < 0.05) were significantly greater in the scapular stabilization position than in the scapular natural position. Conclusion: Patients underwent arthroscopic RC repair had a significant association with scapular dyskinesis and muscle strength was improved by a passive scapular stabilization position, therefore scapular stabilization is important in rehabilitation program.
