Purpose: Near drowning refers to immediate survival after asphyxia due to submersion or immersion in water, which is a crucial public safety problem worldwide. Acute lung injury or acute respiratory distress syndrome (ARDS) is a common complication of near drowning. The purpose of this study was to investigate the feasibility and effectiveness of noninvasive nasal positive pressure ventilation (NINPPV). Methods: This retrospective study was conducted at a tertiary emergency department. NINPPV was administered for moderate ARDS caused by submersion or immersion in patients who were older than 18 years, from January 2015 to December 2018. We collected the demographic (age, sex, length of hospital stay, and outcome), laboratory (arterial blood gas, lactate, oxygen saturation, partial pressure of oxygen divided by the fraction of inspired oxygen, complete blood count, blood urea nitrogen, and creatinine), and clinical data (acute lung injury index and ventilator failure) of the patients. A statistical analysis was performed using Statistical Package for the Social Sciences version 20.0 for Windows. Results: NINPPV treatment was provided to 57 patients for near drowning, 45 of whom (78.9%) were successfully treated without complications; in 12 (21.1%), treatment was changed to invasive mechanical ventilation within 48 hours due to ARDS or acute kidney injury. NINPPV treatment was successful in 31 (75.6%) out of 41 sea-water near drowning patients. They were more difficult to treat with NINPPV compared with the fresh-water near drowning patients (p<0.05). Conclusions: NINPPV would be useful and feasible as the initial treatment of moderate ARDS caused by near drowning.
The power quality of Single Phase Grid-Connected Inverters (GCIs) has received much attention with the increasing number of Distributed Generation (DG) systems. However, the performance of single phase GCIs get degraded due to several factors such as the grid voltage harmonics, the dead time effect, and the turn ON/OFF of the switches, which causes the harmonics at the output of GCIs. Therefore, it is not easy to satisfy the harmonic standards such as IEEE 519 and P1547 without the help of harmonic compensator. To meet the harmonic standards a certain kind of harmonic controller needs to be added to the current control loop to effectively mitigate the low order harmonics. In this paper, the harmonic compensation is performed using a novel robust harmonic compensation method based on Digital Lock-in Amplifier (DLA). In the proposed technique, DLAs are used to extract the amplitude and phase information of the harmonics from the output current and compensate it by using a simple PI controller in the feedforward manner. In order to show the superior performance of the proposed harmonic compensation technique, it is compared with those of conventional harmonic compensation methods in terms of the effectiveness of harmonic elimination, complexity, and implementation. The validity of the proposed harmonic compensation techniques for the single phase GCIs is verified through the experimental results with a 5kW single phase GCI. Purpose: This study compares and examines the effects of proprioceptive neuromuscular facilitation (PNF) on patients with chronic low back pain through systematic literature review and meta-analysis. Methods: Domestic literature was searched with combinations of keywords including "proprioceptive neuromuscular facilitation," "PNF," "back pain," and "low back pain" using the Research Information Sharing Service (RISS), Korean Studies Information Service System (KISS), and Korean Medical Database (KMbase). Six studies (n=148) were finally included in the analysis through a selection and exclusion process. The quality of the studies was evaluated using the PEDro scale. Results: According to the meta-analysis results, the low back pains of the PNF group and the contrast group showed a standardized mean difference (SMD) of 2.21 (95% CI: -3.35, -1.07, p=0.01, $I^2=83%$) after intervention. Thus, the PNF group showed a statistically significant decrease in low back pain compared with the control group. In addition, the SMDs of the Oswestry Disability Index (ODI), lung function, and the Roland and Morris Disability Questionnaire (RMDQ) were -1.34 (95% CI: -1.88, -0.79, p<0.01, $I^2=35%$), 1.14 (95% CI: 0.49, 1.79, p=0.01, $I^2=0%$), and -1.59 (95% CI: -2.56, -0.62, p=0.01, $I^2=46%$), respectively. Thus, the PNF group showed statistically significant differences from the control group. Conclusion: At present, there is some limit to obtaining definite results about effect sizes because there are relatively few randomized controlled experiments that analyze the effects of PNF exercise in patients with chronic low back pain. Therefore, continuous efforts should be made to conduct randomized clinical trials and long-term efficacy studies in the future.
Control algorithms are the most important aspects in successful control of structures against earthquakes. In recent years, intelligent control methods rather than classical control methods have been more considered by researchers, due to some specific capabilities such as handling nonlinear and complex systems, adaptability, and robustness to errors and uncertainties. However, due to lack of learning ability of fuzzy controller, it is used in combination with a genetic algorithm, which in turn suffers from some problems like premature convergence around an incorrect target. Therefore in this research, the introduction and design of the Fuzzy Cooperative Coevolution (Fuzzy CoCo) controller and Adaptive Neural-Fuzzy Inference System (ANFIS) have been innovatively presented for semi-active seismic control. In this research, in order to improve the seismic behavior of structures, a semi-active control of building using Magneto Rheological (MR) damper is proposed to determine input voltage of Magneto Rheological (MR) dampers using ANFIS and Fuzzy CoCo. Genetic Algorithm (GA) is used to optimize the performance of controllers. In this paper, the design of controllers is based on the reduction of the Park-Ang damage index. In order to assess the effectiveness of the designed control system, its function is numerically studied on a 9-story benchmark building, and is compared to those of a Wavelet Neural Network (WNN), fuzzy logic controller optimized by genetic algorithm (GAFLC), Linear Quadratic Gaussian (LQG) and Clipped Optimal Control (COC) systems in terms of seismic performance. The results showed desirable performance of the ANFIS and Fuzzy CoCo controllers in considerably reducing the structure responses under different earthquakes; for instance ANFIS and Fuzzy CoCo controllers showed respectively 38 and 46% reductions in peak inter-story drift ($J_1$) compared to the LQG controller; 30 and 39% reductions in $J_1$ compared to the COC controller and 3 and 16% reductions in $J_1$ compared to the GAFLC controller. When compared to other controllers, one can conclude that Fuzzy CoCo controller performs better.

Objectives: To appraise the effectiveness of platelet-rich fibrin (PRF) in the management of established dry socket in terms of pain, inflammation, and wound healing. Materials and Methods: Two hundred patients with established alveolar osteitis were studied to determine the efficacy of PRF and zinc oxide eugenol (ZOE) for pain control, inflammation reduction, and wound healing. Patients were randomly allocated to Group A (PRF) or Group B (ZOE). Patients were examined on the 1st, 3rd, 7th, and 14th postoperative day and evaluated for pain using visual analogue scale scores, inflammation with a gingival index score, and wound healing through a determination of the number of bony walls exposed. Results: Group A showed better results in terms of pain remission, control of inflammation, and wound healing compared to Group B. Results between groups were statistically significant (P<0.05). Conclusion: PRF is a better alternative than ZOE for the effective management of alveolar osteitis.
Objective: This study examined the effectiveness of Pyung-Hyung acupuncture and herbal medicine for a hemiplegic patient diagnosed with intracerebral hemorrhage. Methods: The patient was treated with Pyung-Hyung acupuncture and herbal medicine for one month. Intracerebral hemorrhage symptoms were evaluated using the Korean version of the Modified Barthel Index (K-MBI), the Manual Muscle Test (MMT), and brain CT images. Results: Following Pyung-Hyung acupuncture and herbal medicine, K-MBI, MMT, and brain CT image results improved. Conclusion: This case showed that Pyung-Hyung acupuncture and herbal medicine effectively treated intracerebral hemorrhage with hemiplegia.
The purpose of this study was to evaluate the effectiveness and safety of acupotomy for the treatment of patients with knee osteoarthritis. There were 9 databases searched to retrieve randomized controlled trials until August 3, 2019 regarding acupotomy versus conventional Western medicine, conventional Western medicine treatment with and without acupotomy, and Korean medicine treatment with and without acupotomy, and meta-analysis was performed. Of 303 potentially relevant studies retrieved, 43 were systematically reviewed. All studies were conducted in China. Effective rate, visual analogue scale, and Western Ontario and McMaster Universities Osteoarthritis index were used as the evaluation scales. The Ashi point was selected most frequently. In all studies, the intervention group was more effective than the control group. Meta-analysis revealed that acupotomy showed statistically significant beneficial results. Although acupotomy had a beneficial effect on knee osteoarthritis, the risk of bias of the included studies was not low. The majority of the results from the evaluation scales used were highly heterogeneous (> 50%) which reduced confidence in the estimation of effect, or had a small sample size. Further clinical research and development is required in the future.
Ulcerative colitis (UC) is one of inflammatory bowel disease (IBD), characterized by chronic inflammatory response and dysregulation of immune function. The severity of US has been influenced by environmental factors and food habit. The immune modulatory, anti-inflammatory and steroidal medicine have been used for the treatment of UC. However, long-term administration of those medicine is accompanied with side-effect. So, it is necessary to develop the non side-effect medicine using natural product. Prebiotics influences intestinal condition and food consumption. The heredity, immunity and environmental condition are related with occurrence of UC. In recent study, UC patients had lower level of prebiotics such as Lactobacillus and Bifidobacterium compared with healthy people. Also, previous study announced that imbalance of enteric flora aggravates the severity of UC. The effectiveness of probiotics might affect colon ability and viable bacteria also could promote the proliferation of beneficial intestinal bacteria. Prebiotics, such as herbal medicine, could lead to balance of intestinal bacteria or increase beneficial bacteria. So, proper choice of herbal medicine could control the intestinal condition. This study aimed to investigate the effect of mixture of probiotics and prebiotics (synbiotics) on dextran sulfate sodium (DSS)-induced UC in vivo. The synbiotics consist of Lactobacillus buchneri, Polymnia sonchifolia and Glycine max Merr. in this study. To evaluate the effect of synbiotics, 3% DSS was administered in BALB/c mice and synbiotics was daily administered for experimental days. The administration of synbiotics regulated colon length shortening, body weight change and disease activity index effectively. Also, extract of synbiotics upregulated survival ability of Lactobacillus buchneri in gut condition. These results suggest that mixture of probiotics and prebiotics, called as synbiotics, could influence intestinal condition also regulate the colon disease. Synbiotics might be a therapeutic agent for treatment of UC.
광고효과를 둘러싼 논의는 광고 연구의 가장 큰 갈래라고 해도 지나치지 않는다. 하지만 정부 공공광고의 효과를 측정하기 위한 모델과 관련한 본격적인 논의는 아직 부족한 상황이다. 본 연구는 이러한 점에 주목하여 정부광고를 평가할 수 있는 지수 모델을 개발하는데 초점을 맞추고 있다. 이를 위해 전국 7대 도시 800명을 대상으로 설문조사를 실시하였으며, 정부 공공광고의 창의성 평가 요인(독창성, 명료성, 적합성)과 정책 내용성 평가 요인(흥미성, 회피성, 상관성)을 토대로 수용자들의 인지, 태도, 행동 반응에 영향을 미치는 인과관계 모형을 설정하였다. 분석 결과, 적합성과 회피성을 제외한 독창성, 명료성, 흥미성, 상관성은 정책 이해, 수용자 태도, 정책 구전의향 등 정부광고 효과를 창출하는 유의미한 변인으로 작용하고 있었다. 또한 정책 이해도와 수용자 태도는 정책 구전의향에 영향을 미치는 주요한 매개 변인으로 나타났다. 연구결과를 바탕으로 향후 효과적인 정부 공공광고를 제작하기 위한 방향에 대해 논의하였다.
본 연구에서는 데이터 시각화 교육에 초점을 맞춘 스크래치를 초등학교 예비교사 26명에게 적용하여 창의성 향상에 미치는 효과를 분석해 보았다. 현직 초등교사 60명을 대상으로 실시한 요구분석 결과를 바탕으로, 데이터 시각화의 절차에 따른 스크래치를 활용한 프로그램을 개발하였다. 개발한 교육 프로그램을 초등학교 예비교사 26명에게 총 4개월 동안 42시간의 수업을 진행하였다. 창의성 증진을 살펴보기 위해 창의성 검사를 실시하여 사전·사후 비교 검사를 통해 적용한 프로그램의 효과를 분석하였다. 분석 결과 초등학교 예비 교사를 대상으로 한 데이터 시각화 교육 중심 스크래치를 활용한 소프트웨어 교육은 초등학교 예비교사들의 창의성 하위요소 중 유창성, 정교성, 창의성 평균, 창의성 지수에 유의미한 효과가 있는 것으로 확인되었다.
