We have experienced 2 cases of the hunshot wound sof the chest involving cardiac injuries at department of the thoracic surgery, Capital Armed Forces General Hospital during I year from April I 1979 to Jan. 1980. In one case of two patients , he was a 22 years old man who was transported to this emergency room 4 hour 10 minutes after having gunshot wound of the left chest by helicopter. Physical examination showed small inlet in left 3rd ICS and left parasternal border, large outlet in left 8th ICS and left scapular line, no breath sound on left side and distant heart sound. chest roentgenography demonstrated marked pleural effusion in left side and mediastinum shifted to right. As soon as chest X-ray was taken, the bleeding through penetrating wound became profuse and cardiac arrest ensued. Closed chest cardiac massage was started and vigorous transfusion continued, but no effective cardiac activity could not be obtained. The patient was pronounced dead due to exsanguinating hemorrhage from wuwpected cardiac wounds. In this critically injured patient with evidence of intrathoracic hemorrhage and suspected cardiac penetration, only emergency thoracic exploration and immediate surgical control of bleeding points might offer the maximum possibility of survival. The other case was a 23 years old man who was transferred to the emergency room 4 hours 50 minutes after having kmultiple communicated fractures of sternum and linear fracture of right mandible by a missile. Examination revealed about 30% skin loss of the anterior chest wall, weak pulse of 96 beats/min., distant heart sound and decreased breath sounds bilaterally. finding on the chest X-ray films showed multiple sternal fractures, marked pericardial effusion indicating hemopericardium. So, the patient was moved immediately to the operation room where, after endotracheal tube inserted, a median sternotomy was performced. A hemorrhagic congestion of the right upper lobe and marked bulging pericardium were disclosed. The pericardium was opened anterior to right phrenic nerve and exsanguinating hemorrhage ensued from the 0.5cm lacerated wound in the auricle of right atrium. The rupture site of right atrium was occluded with non-crushing vascular clamps and then was over sewn with interrupted sutures. It was thought to be highly possible that he was alive long enough to have cardiorrhaphy because of cardiac tamponade, which prevented exsanguinating hemorrhage. He was taken closed reduction for linear fracture of right mandible 2 weeks after repair of ruptured right auricle in dental clinic. This patient's post-operative course was not eventful.
Background: The diffuse infiltrative lung disease requires surgical lung biopsy for its final diagnosis. We evaluated the effect of surgical lung biopsy for final diagnosis of duffuse interstitial lung disease and compared video assisted thoracoscopic lung biopsy(TLB) with open lung biopsy(OLB). Material and Method: We evaluated the patients who underwent surgical lung biopsy from March 2000 from December 2005, retrospectively, We divide to two groups(OLB and TLB group) and compared them. Result: There were 36 patients and cough was the most common pre- operative symptom. Surgery time, anesthetic time, hospital stay, duration of chest tube indwelling, specimen volume and the rate of post-operative complication were not significantly different between two groups. Histologic diagnosis was confirmed in all cases. There was one post-operative death who had suffered from respiratory failure since pre-operative period. Conclusion: Surgical lung biopsy is effective method in final diagnosis for diffuse infiltrative lung disease. Video assisted thoracoscopic lung biopsy is lesser invasive method than open lung biopsy and provide similar results, so it is basic diagnostic method of surgical lung biopsy.
High-dose aprotinin(Hammersmith regimen) has been widely used for years to control postoperative bleeding and reduce blood consumption in cardiac surgery but had known to cause some side-effects and had disadvantage in cost-effectiveness. The prospective controlled study of 33 patients undergoing cardiopulmonary bypass was performed to evaluate the efficacy for reducing postoperative bleeding and unfavorable effects of low-dose aprotinin. The level of hemoglobin and platelet in the blood and the amount of postoperative bleeding were assessed preoperatively, and postoperatively for the study of hemostatic function. The level of BUN and serum creatinine in the blood, levels of urine creatinine, total protein, albumin, alpha-1-microglobulin and creatinine clearance were assessed before and after the operation for the study of renal function. The aprotinin group had a significant reduction in chest tube drainage; 243$\pm$ 123 ml versus 406$\pm$303 ml(P=0.037) during 6 hours immediate-postoperatively, 494$\pm$358 ml versus 869$\pm$570 ml(P=0.045) during 24 hours postoperatively. The ratio of alpha-1-microglobulin/creatinine and microalbumin/creatinine in the urine were slightly increased in the aprotinin group postoperatively in comparison with the control group but there were no statistically significant difference(55$\pm$23 versus 24$\pm$10 in the alpha-1-microglobulin/creatinine, 56$\pm$19 versus 38$\pm$25 in the microalbumin/creatinine at post- operative 3rd day). There were no significant difference between two groups in other parameters of renal function, too. This study showed that low-dose aprotinin is an effective means of reducing postoperative bleeding without inducing significant renal dysfunction.

Chitosan is known to be one of the natural chelating agents. It is derived from chitin which is a cellulose-like biopolymer distributed widely in nature, especially in shellfish, insects, fungi, and yeast. There are two forms of chitosans, water soluble and insoluble, The purpose of the present study is to investigate whether water soluble chitosan can be applied to reduce the bioabailability of radios-trontium in foods. We compared the effect of water soluble and insoluble chitosans on the absorption of ingested radiostrontium ($^{85}Sr$). Three percent water soluble and insoluble chitosan solutions were given orally, and immediately after $^{85}SrCl_2$ ($0.2{\mu}Ci$) was administered to rats using a orogastric tube. In one group water soluble chitosan solution was given for additional 4 days. And in control group no chitosan was given. Each group consisted of 6 rats. The whole-body retention of $^{85}Sr$, determined by in vivo counting method, was lower in water soluble chitosan group than that of water insoluble chitosan group and that of control. Urinary excretion of $^{85}Sr$ in chitosan-treated rats was higher than that of control. And 5 day ingested group of water soluble chitosan showed least whole body retention of $^{85}Sr$. In conclusion water soluble chitosan was more effective in reducing bioavailability of ingested radiostrontium in the gastrointestinal tract than insoluble chitosan.

Radiostrontium is one of fallouts. It can be absorbed through intestine and causing radiation injury to bones. The purpose of this study is to evaluate the inhibitory effect of 10% chitosan (water soluble and insoluble) and 10% alginate (water soluble and insoluble) on radiostrontium adsorption. Water soluble and insoluble chitosans and alginates were given to 10 NIH male mice in each group for 7 days. At the 7th day, 74 MBq of $^{85}Sr$ were given through orogastric tube. Chitosans and alginates were given for additional 7 days. During the 7 days, radioactivities of feces were counted daily. Finally animals were sacrificed, and radioactivities of bones were counted. Fecal excretion was significantly higher in chitosan and alginate group as compared to control from the 1st day (p < 0.01). Water soluble chitosan group showed highest fecal excretion. Bony retention was significantly lower in the treated group than the control (p < 0.01). There was no difference among treated groups. In conclusion, both water soluble and insoluble chitosans and alginates were effective agents on lowering orally ingested radiostrontium ($^{85}Sr$).
Recently, video-assisted thoracoscopic surgery for mediastinal lesions has been considered a new effective therapeutic method. From March, 1992 to April, 1997, 33 cases of video assisted thoracoscopic surgery for mediastinal lesions were performed. Gender distribution was 16 males and 17 females. Average age was 42 years old(ranged from 14 to 69). The locations of lesions were anterior mediastinum in 14 cases, middle mediastinum in 5 cases, posterior mediastinum in 11 cases, and superior mediastinum in 3 cases. These included 9 neurilemmomas, 5 benign cystic teratoma, 4 pericardial cysts, 2 ganglioneuroma, 2 thymus, 2 thymic cyst, 1 thymoma, 2 esophageal leiomyomas, 1 dermoid cyst, 1 lipoma, 1 malignant lymphoma, 1 bronchogenic cyst, 1 pericardial effusion, and 1 Boerhaave's disease with empyema. Working window was needed in 6 cases. We converted to open thoracotomy in 6 cases. Reasons of convertion to open thoracotomy were large sized mass(1), severe adhesion(3), and difficult location to approach(2). The average operation time was 116min($\pm$56 min). The average chest tube drainage time was 4.7days. The average hospital stay was 8.7 days. Operative complications were atelectasis(2), empyema with mediastinitis(1), recurrent laryngeal nerve palsy(1), and plenic nerve palsy(1). In conclusion, VATS for mediastinal lesions were performed with shorter operation time and hospital stay, and lesser complications and pain than those of conventional thoracotomy.
Purpose : The Purpose of this research was to investigate the effects of antithromb otic activities of Honghwadangguisan(HDS) Methods : Measured the effect which was given to blood flow rate through the regular volume of glass tube after the blood was diluted five times with ACD soulution. Antithrombotic effect was calculated as a percentage of the experimental animal figure protected from the paralysis of hind legs or death of the mouse that is caused from the administration of platelet aggregation regent. Each of the groups consisted in 8 mice, was divided into Normal, Control, and HBS. All of these 3 group were supplied a saline solution and after an hour the control group brought the dextran extravasated blood. Also the HDS group was dosed to the experimental mice with Oral Zonde one day before the experiment. After that, the mice were abstained from food. And then we gave a measured amount of it before an hour. Finally, it gave rise to dextran extravasated blood as well as the Control group. Results : The results were obtained as follows. In vitro, HDS inhibited platelet aggregation induced by ADP and epinephrine significantly as compared with the control group. HDS showed fibrinolytic activity insignificantly as compared with the control group. HDS reduced blood flow rate in significantly as compared with the control group. In vivo HDS inhibited pulmonary embolism induced by collagen and epinephrine (inhibitive rate 50%). HDS increased number of platelet, fibrinogen amount and shortened prothrombin time, activated partial thromboplastin time significantly but reduced blood flow rate insignificantly as compared with the control group in thrombus model induced by dextran. Conclusion : HDS is effective antithrombotic activity from experimental result.
Objectives : It has been reported that CG was effective in decreasing injury to neural tissues. To investigate neural responses in the injured spinal cord, an extract of CG was examined to determine its effect on neural responses in the injured spinal cords of rats. Methods : After CG treatment was applied to the spinal cord of rats given a contusion injury, the re-growth responses of injured neural tissues and corticospinal tract axons was observed by measuring the number of GAP-43, Cdc2, and phospho-Erk1/2 proteins, CST axons, GFAP-stained astrocytes, and Glial scarring in the injured spinal cord. Results : Levels of GAP-43, Cdc2, and phospho-Erk1/2 proteins were found to have increased in the injured spinal cord region. The number of GFAP-stained astrocytes also increased within and around the injury cavity. Glial scarring, which was identified by CSPG immunofluorescence staining, was reduced by CG treatment. Anterograde tracing by Dil dye showed that the elongation of the CST axons in the dorso-medial white matter area was almost completely prevented at the injury site. Collateral sprouting was observed in the spinal cord rostrally close to the injury site, and CG treatment further increased axonal arborization in the corresponding region. In vivo migration of CST axons and astrocytes using an implanted polymer tube system showed more of an increase in enhanced migration of axons and astrocytes in CG-treated group compared to the injury control group. Conclusions : These results suggest that CG activated neural responses - including astrocyte migration - and promotes axonal regenerative activity in the injured spinal cord area.

Recently, Due to the increased industry using radiation inspection equipment in the semiconductor, this demand of technology research is increasing. Although semiconductor inspection equipment is using low energy X-ray from 40 keV to 120 keV, Studies of radiation damage about the low energy X-ray are lacking circumstance in our country. Therefore, It is study that BJT (bipolar junction transistor) of one type of semiconductor elements are received radiation damage by low energy X-ray. BJT were used to the NXP semiconductor company's BC817-25 (NPN type), and Used the X-ray generator for the irradiation. Radiation damage of BJT was evaluated that confirm to analyse change of collector-emitter voltage of before and after X-ray irradiation when current gain fixed to 10. X-ray generator of tube voltage was setting 40 kVp, 60 kVp, 80 kVp, 100 kVp, 120 kVp and irradiation time was setting 180s, 360s, 540s into 180s intervals. As the result, We confirmed radiation damage in BJT by low energy X-ray under 120 keV energy, and Especially the biggest radiation damage was appeared at the 80 kVp. It is expected that ELDRS (enhanced low dose rate sensitivity) phenomenon occurs on the basis of 80 kVp. This studies expect to contribute effective dose administration of semiconductor inspection equipment using low energy X-ray, Also Research and Development of X-ray filter.

Polyphenol oxidase in apple (Golden Delicious) was extracted, partially purified and its properties were found as follows; Polyphenol oxidase showed optimum pH for activity at 6.5 and optimum temperature at $30^{\circ}C$ and high affinity to o-diphenol compounds. Cysteine, ascorbic acid and sodium metabisulfite appeared to be most effective inhibitors. EDTA showed a slight inhibition. During the enzyme was kept in test tube at $4^{\circ}C\;and\;20^{\circ}C$ for a week, polyphenol oxidase activity decreased sharply during the first four days at $20^{\circ}C$, then decreased slowly as the storage was prolonged. At $4^{\circ}C$, the polyphenol oxidase activity appeared to be relatively stable during the first two days before activity began to decline sharply. Polyacrylamide disc gel electrophoresis indicated four bands with polyphenol oxidase activity. Three bands and one band of the active bands were observed after heating for 1hr at $60^{\circ}C\;and\;70^{\circ}C$ respectively. The enzyme activity was observed 40% after treatment at $60^{\circ}C$ and 5% after treatment at $70^{\circ}C$. Therefore, no difference in the thermal stability was observed between active bands and the enzyme activity.
