The purpose of this study was to investigate the UVA radiation protection effects of summer hats currently on the market with the purpose of making it possible to choose a hat with suitable UVA protection. Twelve different summer hats from the market were selected for the experiment. The results are summarized as follows: It is more effective to wear a hat than not wear a hat to block UVA radiation. Summer hats with the greatest degree of protection, from highest to lowest, are cotton, straw2, and straw1. In the area of the forehead, which is rarely influenced by the irradiation angle, the cotton hat was the most effective in protecting from UVA radiation because the material density was greater than that of the straw hats. A hat with a 8.5 cm brim was more effective at blocking UVA radiation on the jaw than 6 cm, 4 cm, and 0 cm wide brims, but it still couldn't block the radiation completely. Irradiation amounts at 11:00 AM on the forehead, jaw! s and left and right cheeks were lower than amounts measured on the back of the neck. This revealed that irradiation amounts depend on the shape of the hat and time of day. A hat with a brim encircling the head was found to be more effective in blocking UVA radiation than a hat with only a front or side brim.
Background: We started postoperative pain management service using an intravenous patient-controlled analgesia (IV-PCA, PCA), which is known as convenient and effective analgesic method. In this report, we describe the efficacy and safety of PCA and the experience of developing an acute pain service to treat postoperative pain using a PCA. Methods: Practices of an acute pain service were started at a ward for general surgery after preparation of the standardized protocols for PCA. In each patient, PCA was connected following administration of initial loading doses of analgesics at recovery room after operation. All patients were checked by acute pain service team once or twice daily. The scope of acute pain service was gradually spread to other departments such as orthopedic, thoracic, obstetric and gynecologic departments by requests of patients or surgeons. We managed 1,590 patients during first 22 months. among them, nine hundred seventy two cases were prospectively evaluated for their analgesis efficacy and side effects of PCA. Results: The number of patients was increased day by day. the most common type of operation was gastrectomy (21.6%). Commonly used analgesics were nalbuphine (59%) and morphine (37%). The mean duration of PCA attachment was 3.3 days. The degree of analgesia on operation day was good in 44.8% and tolerable in 52.6% of patients. Only 3.9% of patients complained severe pain during their postoperative periods. One elderly patient experienced respiratory depression (0.06%) owing to accidental misuse of PCA by his relatives. Overall patient's satisfaction was over 93%. Conclusion: According to our experiences, we conclude that PCA is an effective, relatively safe and highly satisfactory method for postoperative pain management. Because of these advantages of PCA, the creation of our acute pain service using a PCA was successful and expanded rapidly.
Purpose: This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. Method: Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. Result: Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01). Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). Conclusion: Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.

We have got the following results from tests in the course of time retrogradation and taste change in bread with added flour of Angelica keiskei Koidz, which had been stored in room temperature (27±2oC), refrigerating(2±1oC) and freezing( 20±1oC), respectively. Bread with the added flour showed a little lower degree of retrogradation than control group, and every group in room temperature retrograded from the very first day. The degree of retrogradation of the group in refrigerating was far greater comparing with the group in room temperature. The longer bread was stored, the lower the degree of retrogradation was, and the higher the adding rate of the flour got, the later its retrogradation happened. Especially the flour of this vegetable's stem turned out to be the most effective in retarding its retrogradation. Group stored 30 days in freezing didn't show a wide difference in retrograde comparing with the group before storage. Preference for color and shape of the flour added bread stored in room temperatue, refrigerating and freezing did not change significantly from the group before storage. Although preference decreased for flavor, texture, mouth feel and overall quality with the lapse of time, flour added group improved in preference for these factors comparing with the control group. Above all, added flour of pretreated stem proved to be the most effective in the sensory tests. With respect to the storing method, the group stored in refrigerating showed greater preference for the texture and mouth feel over the groups in room temperature and freezing. Flavor preference of the group in freezing was the lowest of all.
Background : This study is to identify the inappropriate hospital services for elderly inpatients over 65 years in general hospital with acute care functioning. Consequently elderly inpatient care and the management of long-term care facilities are key issues for current government health policy. Method : The survey was conducted for two months for all inpatients over 65 in 7 general hospitals, 6 work sampling days randomly selected. In each survey day, the subjective judgement by medical staff on the degree of acute care needs and by nursing unit manager on hospital services of each inpatients was also conducted. Result : The total number of cases collected are 2,541 elderly inpatients, according to subjective judgements by medical staff on inpatient condition. However 46.8% of cases are turned out to be non-acute care group. The frequency of medical services provided to non-acute group are 2~3 vital sign checks per day 78.2%, IV injection 40.1%, antibiotics medication 20.2%. Conclusion : Lots of elderly patients' who are staving in acute hospitals, at present need to be transferred to long-term care facilities. However, there was been shortage of long-term care facilities. It is expected to identify the need of elderly inpatients and therefore, to provide cost-effective, appropriate and good quality health services to elderly inpatients depending on their needs.

Purpose: This study was to identify the effects of self-administered hand reflexology on level of fatigue in college student nurses during clinical practice. Method: A non-equivalent control group, quasi-experimental research with pre and post-test design was used. Forty-seven student nurses in clinical practice were assigned to either an experimental or control group. The experimental group performed self-hand reflexology for 10 minutes once a day during the 5 days of clinical practice. Results: 1. While there was a significant difference in degree of fatigue and fatigue scores in the experimental group over time, there was no significant difference between the two groups. 2. Vigor score for the experimental group was significantly higher than those of the control group during the 5 days except for day 5, but there was no significant difference between the two groups. 3, Levels of fatigue for the experimental group increased after 1 hour in the 1st post time, but there was no significant difference between the groups. Conclusion: Self-administered hand reflexology was not found to be an effective method for relieving fatigue for student nurses in clinical practice. Also physiological effects were not identified in this study, and so further research is needed.
K-Maryblyt has been developed for the effective control of secondary fire blight infections on blossoms and the elimination of primary inoculum sources from cankers and newly emerged shoots early in the season for both apple and pear trees. This model facilitates the precise determination of the blossom infection timing and identification of primary inoculum sources, akin to Maryblyt, predicting flower infections and the appearance of symptoms on various plant parts, including cankers, blossoms, and shoots. Nevertheless, K-Maryblyt has undergone significant improvements: Integration of Phenology Models for both apple and pear trees, Adoption of observed or predicted hourly temperatures for Epiphytic Infection Potential (EIP) calculation, incorporation of adjusted equations resulting in reduced mean error with 10.08 degree-hours (DH) for apple and 9.28 DH for pear, introduction of a relative humidity variable for pear EIP calculation, and adaptation of modified degree-day calculation methods for expected symptoms. Since the transition to a model-based control policy in 2022, the system has disseminated 158,440 messages related to blossom control and symptom prediction to farmers and professional managers in its inaugural year. Furthermore, the system has been refined to include control messages that account for the mechanism of action of pesticides distributed to farmers in specific counties, considering flower opening conditions and weather suitability for spraying. Operating as a pivotal module within the Fire Blight Forecasting Information System (FBcastS), K-Maryblyt plays a crucial role in providing essential fire blight information to farmers, professional managers, and policymakers.
Filgrastim is used as an indispensable adjuvant drug to reduce the degree and duration of chemotherapy-induced neutropenia. The purpose of this research is to study the use of filgrastim by reviewing retrospective medical records of breast cancer patients who have been treated by filgtastim in the National Cancer Center. 84 patients have received 323 cycles of chemotherapy, of which 134 cycles were treated by filgrastim $(41.5\%)$. Among those 134 cycles, 34 were for prophylaxis $(21.6\%)$, and 100 for treatment of neutropenia $(74.6\%)$. The frequence of filgrastim usage was more than $50\%$ in frequency with regimens containing docetaxel. For prophylaxis, the median of filgrastim initiation was measured on the day of chemotherapy (-3rd-13th). For the treatment, on the other hand, the median appeared on the 9th day (4th-2lst) after chemotherapy, which showed very wide distribution. Time to filgrastim initiation ranged between the 7th and the 9th day after chemotherapy in docetaxel+doxorubicin combination regimen and docetaxel single regimen, whereas it showed after the 10th day in doxorubicin+cyclophosphamide combination regimens. For the treatment, 48 out of 61 patients $(73.8\%)$ in 63 cycles have experienced fever, had to visit the emergency room, required hospitalization, caused infection, transfusion, dosage reduction and schedule changes in spite of using filgrastim with chemotherapy. For prophylaxis, 11 out of 19 patients $(17.9\%)$ in 11 cycles have experienced the same results. In conclusion, the guideline of time to the initiation and the last is required for cost-effective administration of filgrastim because of the difference occurring ANC nadir, the severity and duration of neutropenia by chemotherapy regimens.

The purpose of this study is to present the two treatment case of patient with dysfunctional uterine bleeding. Each patient has received the Korean Medicinal treatment including acupuncture, moxibustion, and herbal medication twice a week for a month or six months. This efficacy of treatment was evaluated with the degree of bleeding by the number of pad used per day. After finishing treatment, two patients had stopped bleeding, and their menstrual cycle returned to the normal range. Moreover, additional clinical manifestations including leukorrhea, hot flush, bloating, feeling tired and irritability which refers to the premenstrual syndrome were also alleviated. This case report showed that the acupuncture, moxibustion, and the herbal medicinal treatment can be effective for the treatment of the dysfunctional uterine bleeding.
Objective: To investigate short- and long-term treatment effects and side reactions of lobaplatin plus 5-Fu combined and concurrent radiotherapy in treating patients with inoperable middle-advanced stage esophageal cancer. Methods: Sixty patients with middle-advanced stage esophageal squamous cell cancer were retrospectively analyzed. All patients were administered lobaplatin (50 mg intravenously) for 2 h on day 1, and 5-Fu ($500mg/m^2$) injected intravenously from day 1 to 5 for 1 cycle, in an interval of 21 days for totally 4 cycles. At the same time, late-course accelerated hyperfractionated three-dimensional conformal radiotherapy was performed. Patients were firstly treated with conventional fractionated irradiation (1.8 Gy/d, 5 times/week, a total of 23 treatments, and DT41.4 Gy), and then treated with accelerated hyperfractionated irradiation (1.5 Gy, 2 times/d, a total of 27 Gy in 9 days, an entire course of 6-7 weeks, and DT 68.4Gy). Results: All patients completed treatment, including 10 complete response (CR), 41 partial response (PR), 7 stable disease (SD), and 2 progressive disease (PD). The total effective rate was 85.0% (51/60). Thirty-nine patients had an increased KPS score. One-, 2-, and 3-year survival rates were 85.3%, 57.5%, and 41.7%, respectively. The median survival time was 27 months. The adverse reactions included myelosuppression, which was mainly degree I and II. The occurrence rate of radiation esophagitis was 17.5%. No significant hepatic or renal toxicity was observed. Conclusion: Lobaplatin plus 5-Fu combined with concurrent radiotherapy is safe and effective in treating patients with middle-advanced stage esophageal cancer. However, this result warrants further evaluation by randomized clinical studies.
