Guillain-Barre syndrome (GBS) is a group of autoimmune syndromes consisting of demyelinating and acute axonal degenerating forms of the disease. Typically, Gullain-Barre syndrome can be diagnosed from the patient's symptoms and physical examination such as the rapid onset of weakness, paralysis and loss of reflexes. In most patients, resolution is complete or near complete. Treatment consists of supportive care, ventilatory management (in about one third of patients), and specific therapy with intravenous immunoglobulin or plasmapheresis. This clinical report is about suspected acute severe Guillain-Barre syndrome patient, 61-year-old man had quadriplegia, facial palsy, dysphasia, respiratory failure. After 5 weeks of East-West integrative medicine therapy - Conventional Conservative therapy(plasmaphresis and intravenous immunoglobulin) and Korean traditional medicine(Sasang medicine and acupuncture treatment) - most symptoms improved.
Objectives : The purpose of this study was to present the clinical practice guidelines for breast cancer through the analysis of existing clinical practice guidelines in Western and Chinese medicine. Methods : The literature search was performed regarding conventional and integrative medical treatment of breast cancer using Google Scholar, KIOM OASIS, PUBMED and Jisan library of Daejeon university. Results : Breast cancer is the fifth most common cancer in Korea. Recent studies revealed that combining conventional and integrative medical treatment can reduce the sequela of surgical operation and improve survival rate. However there isn’t still a systemic clinical guideline for breast cancer in korean medicine. This study will be a preliminary study to establish clinical practice guidelines of Korean Medicine for breast cancer. Conclusions : Further objective and systemic clinical studies related to Korean Medicine are needed to develope more advanced clinical practice guidelines of breast cancer.
Objectives: The purpose of this study is to present clinical practice guidelines for nasopharyngeal cancer (NPC).Methods: Data related to western and oriental medical treatment of NPC were collected using various search engines such as Google Scholar, KIOM OASIS, PUBMED, and the library of Woosuk University. Results: In recent studies, applying combined oriental and western medicine has been shown to improve survival, quality of life, and immune function and to decrease side effects with respect to NPC. However, there still is no objective and systematic clinical guideline for NPC, so we have proposed one. This study will be meaningful in establishing clinical practice guidelines of Korean medicine for NPC.Conclusion: Further studies related to Korean medicine are needed to develop more advanced clinical practice NPC guidelines.
Objectives The purpose of this survey was to investigate the using experiences and attitudes of general public toward the East-West collaborative medical practices (EWCMP). Methods We selected objects in general public and structured questionnaires were administrated to both 295 offline participants and 795 online participants. The data was collected from 15 to 28 February 2016 and analyzed statistically by frequency, ${\chi}^2$ cross-over analysis with SPSS software. Results The total number of participants was 1,090 and 66.1% of them has heard but only 19.5% has ever used EWCMP. For participants who has used EWCMP, the decision to use it was made by themselves (53.2%), 44.0% of them was satisfied with EWCMP, 89.4% of them showed revisiting intention to EWCMP hospital, and 72.2% of them showed the intention to introduce EWCMP to others. And 52.3% of them were optimistic about the prospects of EWCMP, especially in the field of disease treatment (49.9%) and rehabilitation (20.6%). On the reasons for lack of development of EWCMP, 31.9% of them listed prejudices existing on both sides firstly (31.9%). Preferred type of EWCMP was to use both western and Korean medical treatment at the same weight (53.9%). To promote EWCMP, it is required to confirm the concept of collaborative medical practice to the medical professions and government authorities (29.5%). Conclusions Considering low proportion of utilization and positive attitude toward the EWCMP among general populations, it is need to introduce or advertise EWCMP to them.
Purpose : To propose various types of clinical research which is feasible for botanical new drug (IND) development processes, and suggest essential steps to development of study protocol for IND. Methods : Literature-based discussions and one research group's experience is given regarding domestic act, regulation, and system. Results : In order to get an approval of IND for botanical drug in Korea there are several types of clinical research to conduct. In quality control steps for standardized medicinal herbs, case reports or case series can be conducted, and for good manufacturing practice(GMP) steps, we can conduct case reports, case series, and retrospective cohort studies. In addition, as long as we gathered good laboratory practice(GLP) data we can conduct up to quasi-experimental studies and clinical trials including investigator initiated trials. In order to conduct these studies development of study protocol is essential. First, we obtain historical evidence including target disease and indication, efficacy, safety, and endpoints by reviewing medical classics. Second, we obtain clinically and statistically important data by conducting non-clinical studies, observation studies, and quasi-experimental studies. Third, we generate research hypotheses and purposes and explore methodologies, endpoints, clinical practice guidelines, cost-effectiveness, and commercial potential. Finally, we develop study protocol with aid of biostatistician or expert in contract research organization. Discussions and conclusions : This study have obvious limitations in that most thoughts, suggestions, and proposes are from one research group's experience. Therefore, we hope to see various types of research in this topic and process from other research group as well.
Objectives: Toad venom, called Chan-Su, is a traditional Oriental medicine secreted from the auricular and the skin glands of the Bufo bufo gargarizanz Cantor or B. melanosticus Schneider and has been widely used in China, Korea and other parts of Asia for the treatment of pain, heart conditions, and cancer. We examined the concentrations of the main chemical constituents within a commercially available toad venom product and compared the levels for different extraction methods. Methods: Toad venom was extracted using either cold or hot water, ethanol (EtOH), methanol (MeOH), or ethyl acetate (EtOAc), was fractionated using precipitation or reflux, and was then analyzed using thin layer chromatography (TLC), high-performance liquid chromatography (HTLC), and liquid chroma-tography - mass spectrometry (LC-MS). Individual components were identified by comparisons of the retention times, the ultraviolet spectra, and mass spectras and differences in chemical constituents for different solvents and extraction methods are presented. Results: Components with authentic standards, including serotonin and bufodienolides (cinobufagen, bufalin, cinobufalin, and resibufogenin), were detected. The water extract of toad venom contained the greatest amount of serotonin ($75.7{\pm}0.1$ mg/g), but very small amounts of bufodienolides ($3.8{\pm}0.0$ mg/g). In contrast, the use of MeOH or EtOH extraction solutions resulted in 5-26 times higher concentrations of bufodienolides, with only trace amounts of serotonin. The relative and the absolute concentrations of the component also varied based on the extraction method; i.e., EtOH extracts yielded the greatest total amounts of bufodienolides, and EtOAc precipitation had the lowest amounts of bufodienolides. Conclusions: Toad venom consists of serotonin and several bufodienolides, and the choice of solvent to extract chemical the constituents is important as a way to enrich the purported active components for treating different conditions.
The aim of this study was to examine the effects of aqua-acupuncture a mixture of bos taurus domesticus and selenarctos thiberanus, and bos taurus domesticus, selenarctos thiberanus and Moschus moschiferus on an animal model of arthritic pain. Under halothane anesthesia, arthritic pain was induced by the injection of $2\%$ carrageenan into the left knee joint cavity of male Sprague-Dawley rats. The responses of afferents to a movement cycle were recorded before and after aqua-acupuncture. The aqua-acupuncture at acupoints reduced neural responses to noxious movement stimulation. Aqua-acupuncture at Zusanli inhibited neural responses of articular afferents to noxious stimulation more than at Hegu. These results indicate that aqua-acupuncture of a mixture of bos taurus domesticus and selenarctos thiberanus, and bos taurus domesticus, selenarctos thiberanus and Moschus moschiferusmay provide a potent strategy in relieving arthritic pain.
Chunghyul-dan (CHD) is a combinatorial drug known to exert anti-inflammatory effects in endothelial cells. In this study, we employed global transcriptional profiling using cDNA microarrays to identify molecular mechanisms responsible for the anti-inflammatory activity of CHD in endothelial cells. An analysis of the microarray data revealed that transcript levels of monocyte chemotactic protein-1 (MCP-1), vascular cell-adhesion molecule-1 (VCAM-1) and activated leukocyte cell-adhesion molecule were dramatically altered in CHD-treated endothelial cells. These changes in gene expression were confirmed by RT-PCR, Western blotting and ELISA. Chronic CHD treatment also appeared to decrease MCP-1 secretion, probably as a result of decreased MCP-1 expression. In addition, we determined that chronic CHD treatment inhibited lipopolysaccharide-stimulated adhesion of THP-1 leukocytes to endothelial cells. The inhibitory effect of CHD on LPS-stimulated adhesion resulted from downregulation of VCAM-1 expression. Transmigration of THP-1 leukocytes through endothelial cells was also inhibited by chronic CHD treatment. In conclusion, CHD controls a variety of inflammatory activities by regulating MCP-1 and VCAM-1 gene expression.
Purposes: This study is to examine the causes of limitations of modern western science and philosophy through Ayurvedic thought, and to seek the path of holism for the development of national healing projects. Methods: Therefore, it was inferred that the background of modern Western civilization and the reality of the national healing project would share the flow. In order to examine this, the difference between the East and West's 'recognition of the body and the mind' and the 'perspective on the origin of existence' was examined, and the characteristics of the Ayurvedic five-element novel were examined. Results: Through that process, the "mind and body dualism" in the West and the absence of the "empty element( 空 Śunya)" in Western four-element novels were pointed out as the cause of the limitations. In the sense that the 'empty element' is an invisible world and an immaterial world, it was concluded that the absence of justice was a limitation of Western civilization and a problem of the reality of the national healing project. Conclusions: I hope that this study will lead to continuous research on Ayurvedic ideology and research in the field of natural healing, thus contributing to the development of national healing projects and leading to health, well-being, and self-healing of the people.
