자궁경부암: 개정된 2018 FIGO 병기와 자기공명영상을 중심으로 (Uterine Cervical Cancer: Emphasis on Revised FIGO Staging 2018 and MRI)

  • 자궁경부암(uterine cervical cancer)은 발생 빈도가 높은 부인암 중 하나로서 병변의 조기발견과 정확한 병기설정 및 치료가 예후에 큰 영향을 미치며 영상 검사는 병기설정 및 치료계획과 추적 관찰에 있어 중요한 역할을 하고 있다. 특히 자기공명영상은 높은 연부 조직의 대조도를 통해 병변의 크기와 침범 정도를 파악하는 데 유용하다. 2018년 국제 산부인과 학회(International Federation of Gynecology and Obstetrics; 이하 FIGO)에서 병기설정이 개정되었으며, 원발종양의 크기를 세분화하고 림프절 전이를 포함하게 되었다. 본 종설에서는 이전 연구 및 최근 개정된 FIGO 병기를 바탕으로 한 자궁경부암의 병기설정과 여러 가지 치료 후 영상에 대해 자기공명영상 소견을 중심으로 기술하고자 한다.

자궁경부암 예방을 위한 자기효능증진 프로그램이 간호사의 자기효능감과 예방행위 수행에 미치는 영향 (Effects of the Self-efficacy Promoting Program for Prevention of Uterine Cervical Cancer on Self-efficacy and Performance of Preventive Health Behaviors in Nurses)

  • Purpose: This study was conducted to identify the effects of the self efficacy promoting program on self efficacy and performance of preventive health behavior of uterine cervical cancer for community nurses. Method: A one group quasi-experimental research with pre and post test design was used. The subjects of the study were consisted of twenty two community nurses in a city in Korea. The whole program was carried out from July to September, 2003. The effect of the program was analyzed at the first week, the fifth week, and the ninth week since the experiment input began. Data were analyzed with repeated ANOVA to determine the effects of program. Results: 1) After the self efficacy promoting program, self efficacy scores were significantly increased compared to those before the program(F=12.029. p=0.005). 2) After the self efficacy promoting program, preventive health behavior scores were significantly increased than those before the program(F=10.431. p=0.006). Conclusions: This 9-week program showed much affirmative effect on the prevention of uterine cervical cancer for community nurses. Thus this program can be recommended to the management of the prevention and early detection of uterine cancer.

Clinical and histopathologic analysis of gynecological cancer: a single institute experience over 7 years

  • Background: Approximately 100,000 women are diagnosed with cancer each year in Korea. According to a survey by the Korean central cancer registry in 2016, uterine cervical cancer, uterine corpus cancer, and ovarian cancer were the 5th, 7th, and 8th most prevalent cancers respectively among Korean women. The present study aims to review the clinico-pathologic characteristics of patients who were treated for major gynecological malignancies at Yeungnam University Medical Center. Methods: Patients with invasive gynecological cancers from January 2012 to February 2019 were retrospectively identified. We analyzed the clinical features, demographic profiles, pathologic data, treatment modality used, adjuvant treatment used, complications, recurrence, and survival outcomes. Results: A total of 287 patients (cervical cancer 115; corporal cancer 86; and ovarian, tubal, or primary peritoneal cancer 90) were included. Most cervical (82.7%) and corporal cancers (89.5%) were diagnosed in the early stages (stage I or II), while more than half (58.9%) the cases of ovarian, tubal or peritoneal cancers were diagnosed in the advanced stages (stage III or IV). Surgical complications were observed in 12.2% of cervical cancers, 16.3% of uterine corpus cancers, and 11.1% of ovarian, tubal, and peritoneal cancers, respectively. The 5-year overall survival rate was 94.1%, 91.0%, and 77.1% for cervical, corporal, and ovarian, tubal, or peritoneal cancers, respectively. Conclusion: Surgical treatment was satisfactory in terms of the incidence of complications, and survival outcomes were generally good. Clinicians should be aware of the clinical and histopathological characteristics of patients with gynecological cancers to be able to provide optimal strategies and counseling.

Clinical outcomes of adjuvant radiation therapy and prognostic factors in early stage uterine cervical cancer

  • Purpose: To evaluate the outcomes of adjuvant radiotherapy (RT) and to analyze prognostic factors of survival in the International Federation of Gynecology and Obstetrics (FIGO) IB-IIA uterine cervical cancer. Materials and Methods: We retrospectively reviewed the medical records of 148 patients with FIGO IB-IIA uterine cervical cancer who underwent surgery followed by adjuvant RT at the Yonsei Cancer Center between June 1997 and December 2011. Adjuvant radiotherapy was delivered to the whole pelvis or an extended field with or without brachytherapy. Among all patients, 57 (38.5%) received adjuvant chemotherapy either concurrently or sequentially. To analyze prognostic factors, we assessed clinicopathologic variables and metabolic parameters measured on preoperative 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT). To evaluate the predictive performance of metabolic parameters, receiver operating characteristic curve analysis was used. Overall survival (OS) and disease-free survival (DFS) were analyzed by the Kaplan-Meier method. Results: The median follow-up period was 63.2 months (range, 2.7 to 206.8 months). Locoregional recurrence alone occurred in 6 patients, while distant metastasis was present in 16 patients, including 2 patients with simultaneous regional failure. The 5-year and 10-year OSs were 87.0% and 85.4%, respectively. The 5-year and 10-year DFSs were 83.8% and 82.5%, respectively. In multivariate analysis, pathologic type and tumor size were shown to be significant prognostic factors associated with both DFS and OS. In subset analysis of 40 patients who underwent preoperative PET/CT, total lesion glycolysis was shown to be the most significant prognostic factor among the clinicopathologic variables and metabolic parameters for DFS. Conclusion: Our results demonstrated that adjuvant RT following hysterectomy effectively improves local control. From the subset analysis of preoperative PET/CT, we can consider that metabolic parameters may hold prognostic significance in early uterine cervical cancer patients. More effective systemic treatments might be needed to reduce distant metastasis in these patients.

High Dose Rate Cobalt-60 After Loading Intracavitary Therapy of the Uterine Cervical Carcinoma in Srinagarind Hospital, Analysis of Residual Disease

  • Objectives: To evaluate residual disease in uterine cervical cancer patients treated with teletherapy using combined high dose rate Cobalt-60 brachytherapy. Materials and Methods: A retrospective study of uterine cervical cancer patients, FIGO stages IB-IVB (International Federation of Gynecologists and Obstetricians recommendations), treated by radiotherapy alone between April 1986 and December 1988 was conducted and the outcomes analysed. The patients were treated using teletherapy 50 Gy/25 fractions, five fractions per week to the whole pelvis together with HDR Cobalt -60 afterloading brachytherapy of 850 cGy/fraction, weekly to point A for 2 fractions. Results: The study covered 141 patients with uterine cervical cancer. The mean age was 50.0 years with a range of 30-78 years. The mean tumor size was 4.1 cm in diameter (range 1-8 cm). Mean follow - up time was 2.94 years (range 1 month-6.92 years). The overall incidence of residual locoregional disease was 3.5%. Residual disease, according to stage IIB, IIIB and IVA was present in 2.78%, 3.37% and 50.0%. It was noted that there was no evidence of residual disease in stage IB and IIA cases. Conclusion: Combined teletherapy along with high dose rate Cobalt -60 brachytherapy of 850 cGy/fraction, weekly to point A for 2 fractions resulted in overall 3.5% residual disease and a 96.5% complete response. The proposed recommendation for improving outcome is initiation of measurements for early detection of disease.

자궁 경부암의 수술후 방사선 치료에서 선량 분포에 관한 연구 (A Study of Dose Distribution in Postoperative Radiotherapy in Uterine Cervical Cancer)

  • 1986년 4월부터 1991년 3월까지 영남대학교 의과대학 부속병원에서 자궁 경부암으로 확진되어 수술후 방사선치료를 시행한 22명의 환자에서 방사선 선량 분포에 따른 치료성적은 다음과 같다. 1. 22명중 3명(13.6%)에서 골반이나 복부에 재발이 생겼으며 임상적 병기가 진행될수록, 외부조사선량이 적을수록 재발이 많았다. 2. 골반내 임파선 전이와 임파혈관 침범인 경우에 재발이 되었다. 3. 강내조사 선량이나 수술방법은 재발과 무관하였다. 4. 중대한 합병중은 임상적 병기가 앞설수록 외부조사 선량이나, 총조사 선량이 증가할수록 많이 발생하였다. 이상의 결과를 보면 본원 치료방사선과의 자궁 경부암의 수술후 방사선 치료원칙은 적절하며 재발방지와 합병증의 예방을 위한 세심한 주의와 항암제를 위시한 전신요법의 추가를 고려하여야 하겠다.

Terahertz Pulse Imaging of Micro-metastatic Lymph Nodes in Early-stage Cervical Cancer Patients

  • Lymph node metastasis is an important prognostic factor in cervical cancer patients. We report THz imaging for detecting micro-metastatic foci in the lymph nodes of early-stage uterine cervical cancer patients. Five paraffin-embedded metastatic lymph nodes from two cervical cancer patients were imaged using a THz time-domain spectroscopy system in the reflection mode. The size and shape of the tumor regions were compared with those from histopathologic examinations. The metastatic portions of lymph nodes as small as 3 mm were well delineated by THz imaging. The reflected peak amplitudes were lower in metastatic portions than in the normal portions of lymph nodes, and the difference in their peak-to-peak amplitudes was ~5%.

한국인 여성에서 자궁경부암의 위험요인에 관한 환자-대조군 연구 (A Case-Control Study on Risk Factors of Uterine Cervix Cancer in Korea)

  • A hospital-based case-control study was carried out to investigate the risk factors of uterine cervical cancer in Korea. Information on a wide-range of life-styles, which might be related with uterine cervix cancer, has been routinely collected through a dual application of the self-administered questionnaire and the direct interview by a well-trained nurse at the Department of Gynecology, Seoul National University Hosiptal since 1992. The number of cervical cancer cases, histologically confirmed at the hospital, were 284. Included were 939 women as controls, who were free of past history of any malignancies. Adjusted odds ratio and 95% confidence limits were based on the unconditional logistic regression model. The multivariate logistic model was constructed under the consideration of biologic characteristics on the natural history of the malignancy. In the multivariate results, the uterine cervical cancer risk was higher in women of shorter height$(P_{trend}<0.05)$, less educated spouse$(P_{trend}<0.001)$(0.001), multiple marriages(adjusted OR=2,70, 95% C.I. $1.64\sim4.47$), ever had a family history (adjusted OR=2.14., 95% C.I. $1.18\sim3.89$), multiparity$(P_{trend}<0.001)$, and early age at first $(P_{trend}<0.001)$. These results strongly suggest that the uterine cervix cancer might be related to the reproductive factors, and probably with sexual behaviour of both women and men in Korea.

Current Status of Knowledge, Attitude and Practice (KAP) and Screening for Cervical Cancer in Countries at Different Levels of Development

  • Cancer of the uterine cervix is a worldwide menace taking innumerable womens' lives. The literature is vast and a large number of studies have been conducted in this field. Analyses have shown significant differences exist in terms of screening and HPV testing facilities among high income and low to middle income countries. In addition, acute lack of awareness and knowledge among the concerned population is particularly noted in rural areas of the low income countries. A detailed review of Indian case studies revealed that early age of marriage and childbirth, multiparity, poor personal hygiene and low socio-economic status among others are the principal risk factors for this disease. This review concludes that a two pronged strategy involving strong government and NGO action is necessary to minimize the occurrence of cervical cancer especially in low and medium income countries.

Lack of any Impact of Histopathology Type on Prognosis in Patients with Early-Stage Adenocarcinoma and Squamous Cell Carcinoma of the Uterine Cervix

  • Background: The aim of this study was to evaluate the prognosis of patients with stage IA-IIB cervical carcinoma and to investigate a possible correlation of histology with prognosis. Materials and Methods: Two hundred fifty one patients with adenocarcinoma and squamous cell carcinoma (SCC) histology for FIGO (International Federation of Gynecology and Obstetrics) stage IA-IIB uterine cervical carcinomas at the Radiation Oncology Clinic of GH Okmeydan Training and Research Hospital between January 1996 and December 2006 were selected, analyzed retrospectively and evaluated in terms of general characteristics and survival. Disease-free survival (DFS) and overall survival (OS) was calculated using the Kaplan-Meier method and differences were compared with the log-rank test. Multivariate analysis using a Cox-proportional hazards model was used to adjust for prognostic factors and to estimate hazard ratio (HR) with 95% confidence interval (CI). Results: There was no differences between the two tumour types in age, stage, pelvic nodal metastasis, parametrial invasion, surgical margin status, DSI, LVSI, maximal tumor diameter, grade, and treatment modalities. 5-year OS and DFS were 73% and 77%, versus 64% and 69%, for SCC and adenocarcinoma, respectively (p> 0.05). Multivariate analysis revealed independent prognostic factors including pelvic nodal metastasis and resection margin status for OS (p=0.008, p=0.002, respectively). Conclusions: Prognosis of FIGO stage IA-IIB cervical cancer patients was found to be the same for those with adenocarcinoma and SCC.
