Screening Test for Heart Diseases in the First Grade Elementary School Children in Busan (부산 지역 초등학교 1학년 학생들의 심장질환의 집단검진에 관한 연구)

  • Purpose : We'd like to determine the incidence of congenital heart disease and arrhythmia in elementary school children in Busan, and to provide adequate prevention and treatment. Methods : A total of 23,802(male 12,909, female 10,893) 1st grade elementary school children living in Busan were studied. All children were 7-8 years old. We obtained their medical history by questionnaire and checked elecrocardiography(ECG). Subsequent screening tests including a 2nd ECG, chest X-ray, phonocardiogram and CBC for the students who had abnormal findings at the first screening test. The third screening test was done for students who had cardiac murmurs or abnormal ECG findings in the second screening test by echocardiogram, treadmill test and 24-hour Holter monitoring. Results : Among 23,802 children participants, 605(2.54%) had abnormal ECG findings at the first screening test. Q wave abnormality(0.58%) was observed most frequently, and complete right bundle branch block(RBBB)(0.26%), sinus tachycardia(0.24%), right axis deviation(0.22%) and ventricular premature contraction(VPC)(0.21%) followed in order. Four hundred and twenty four children participated in the second ECG screening test. Two hundred and two children(47.6%) had an abnormality such as sinus tachycardia(18.8%), VPC(17.8%), or complete RBBB(17.3%). After completing the third examination including echocardiogram, we couldn't find the students with ventricular tachycardia (VT) or SVT at the third arrhythmia screening test. Conclusion : A high incidence of arrhythmia was found in the 1st grade elementary school children in Busan despite their healthy appearances, although fatal heart diseases were not noted. Early diagnosis, adequate preventative measures and treatment will prevent and decrease the frequency of emergent situations like syncope and sudden death.

An ELISA-on-a-Chip Biosensor System for Early Screening of Listeria monocytogenes in Contaminated Food Products

  • An enzyme-linked immunosorbent assay (ELISA)-on-a-chip (EOC) biosensor combined with cell concentration technology based on immuno-magnetic separation (IMS) was investigated for use as a potential tool for early screening of Listeria monocytogenes (L. monocytogenes) in food products. The target analyte is a well-known pathogenic foodborne microorganism and outbreaks of the food poisoning typically occur due to contamination of normal food products. Thus, the aim of this study was to develop a rapid and reliable sensor that could be utilized on a daily basis to test food products for the presence of this pathogenic microorganism. The sensor was optimized to provide a high detection capability (e.g., 5.9 ${\times}\;10^3$ cells/mL) and, to eventually minimize cultivation time. The cell density was condensed using IMS prior to analysis. Since the concentration rate of IMS was greater than 100-fold, this combination resulted in a detection limit of 54 cells/mL. The EOC-IMS coupled analytical system was then applied to a real sample test of fish intestines. The system was able to detect L. monocytogenes at a concentration of 2.4 CFU/g after pre-enrichment for 6 h from the onset of cell cultivation. This may allow us to monitor the target analyte at a concentration less than 1 CFU/g within a 9 h-cultivation provided a doubling time of 40 min is typically maintained. Based on this estimation, the EOC-IMS system can screen and detect the presence of this microorganism in food products almost within working hours.

8 Years Report of Urine Organic Acid Analysis - Comparison before and after Introduction of Neonatal Screening Test using Tandem Mass Spectrometry - (소변 유기산 분석 8년의 정리 -탠덤매스(Tandem mass spectrometry)를 이용한 신생아 선별검사 도입 전후의 비교-)

  • Purpose: Disorders of organic acid metabolism have various clinical manifestations and it may be life-threatening. The prognoses of affected children are dependent on early diagnosis and treatment. We report this study to find out detection rate of referred samples, clinical manifestations and age distribution after introduction of neonatal screening test using tandem mass spectrometry in Hallym University Chuncheon Sacred Heart Hospital during 8 years and 9 months. Methods: The 2,794 patients referred from Jan. 2007 to Sep. 2015 were divided into four groups according to age. We conducted organic acid analysis of urine samples of patients and analyzed clinical manifestations and distributions of age at the diagnosis. For patients with ambiguous results, reanalysis of urine organic acid after diet restriction, protein loading and restriction, has been done. Results: A total of 626 patients with 20 disorders were diagnosed. Mitochondrial disorders (482 patients) were the most common diagnosis, followed by ketolytic defects (67), 3-hydroxyisobutyric aciduria (32), EPEMA syndrome (8), 3-methylcrotonyl glycinuria (7), glutaric aciduria type II (6) and type I (4), methylmalonic aciduria (3), isovaleric aciduria (3) and medium chain acyl-CoA dehydrogenase deficiency (3). Conclusion: As neonatal screening test using tandem mass spectrometry is increasingly common and medical environment is changed, detection rate of disorders of organic acid metabolism in this study has decreased compared to previous report. Because the deterioration can be prevented by early diagnosis and treatment, many pediatricians have to pay special attention to possibility of the disorders and make an effort for early diagnosis in clinical setting.

  • Hearing loss is the most common birth defect among infants. Yet most hearing-impaired children are not identified until 1 to 3 years of age - which is well beyond the critical period(6 month) for healthy speech and language development. However, if a hearing-impairment is identified and treated in its early stages, child's speech and language skills will be comparable to his or her normal- hearing peers. For these reasons, hearing screening at birth and routinely throughout childhood is extremely important. Auditory brain-stem response(ABR) is nowadays one of the most reliable diagnostic tools in the early detection of hard of hearing. In this study, we developed the system that detects auditory evoked potential using micro-processor. Furthermore, it is required to develop a portable system to measure in not only soundproof room but also nursery for infants.

Automatic Sputum Color Image Segmentation for Lung Cancer Diagnosis

  • Lung cancer is considered to be the leading cause of cancer death worldwide. A technique commonly used consists of analyzing sputum images for detecting lung cancer cells. However, the analysis of sputum is time consuming and requires highly trained personnel to avoid errors. The manual screening of sputum samples has to be improved by using image processing techniques. In this paper we present a Computer Aided Diagnosis (CAD) system for early detection and diagnosis of lung cancer based on the analysis of the sputum color image with the aim to attain a high accuracy rate and to reduce the time consumed to analyze such sputum samples. In order to form general diagnostic rules, we present a framework for segmentation and extraction of sputum cells in sputum images using respectively, a Bayesian classification method followed by region detection and feature extraction techniques to determine the shape of the nuclei inside the sputum cells. The final results will be used for a (CAD) system for early detection of lung cancer. We analyzed the performance of a Bayesian classification with respect to the color space representation and quantification. Our methods were validated via a series of experimentation conducted with a data set of 100 images. Our evaluation criteria were based on sensitivity, specificity and accuracy.

Chimeric RNAs as potential biomarkers for tumor diagnosis

  • Cancers claim millions of lives each year. Early detection that can enable a higher chance of cure is of paramount importance to cancer patients. However, diagnostic tools for many forms of tumors have been lacking. Over the last few years, studies of chimeric RNAs as biomarkers have emerged. Numerous reports using bioinformatics and screening methodologies have described more than 30,000 expressed sequence tags (EST) or cDNA sequences as putative chimeric RNAs. While cancer cells have been well known to contain fusion genes derived from chromosomal translocations, rearrangements or deletions, recent studies suggest that trans-splicing in cells may be another source of chimeric RNA production. Unlike cis-splicing, trans-splicing takes place between two pre-mRNA molecules, which are in most cases derived from two different genes, generating a chimeric non-co-linear RNA. It is possible that trans-splicing occurs in normal cells at high frequencies but the resulting chimeric RNAs exist only at low levels. However the levels of certain RNA chimeras may be elevated in cancers, leading to the formation of fusion genes. In light of the fact that chimeric RNAs have been shown to be overrepresented in various tumors, studies of the mechanisms that produce chimeric RNAs and identification of signature RNA chimeras as biomarkers present an opportunity for the development of diagnoses for early tumor detection.

Development of an Interaction Behaviors Checklist for Early Detection of Autistic Children (자폐아동의 조기 선별을 위한 상호작용행동체크리스트 개발)

  • Purpose: This study was conducted to develop a behavioral checklist to predict an autistic disorder and to identify the earliest detecting time. Method: One hundred and fifty eight children including normal, autistic, institutionalized normal, and retarded were assessed using critical interaction behavioral markers from literature review. Data was collected by semi-structured mother-child interaction by videotape recording and analyzed byfactor analysis, Cronbach a, Kappa, $x^2$, and Duncan. Result: Ten behavioral markers were sorted into 2 factors; joint-attention and synchronized behavior. Autistic children were impaired in pretend play, odeclarative pointing, proimperative pointing, gaze-monitoring, referential looking, showing, joint-attention, rhythmical vocal exchange, and synchronized laughing. The sychronized behavior was also a critical marker to predict the autistic disorder. However, it was difficult to differentiate autistic disorder from mental retardation. In addition, the appropriate detecting time was around 18 months after birth. Conclusion: This checklist should be behavior markers to predict autistic disorder and could be useful as educational material at children's clinics, parents class, and for caregivers in the health center. In addition, early detection should lead to treatment being started as soon after 18 months of age as possible.

Study on the Causes of Premature Cracking of Epoxy Coatings for Ship's Ballast Tanks

  • Premature cracking of the epoxy coatings applied on ship's ballast tanks(BT) can lead to damage of ship's hulls. To avoid this, it's important to have clear understanding of the underlying mechanism and primary factors of the coating crack. In this study, the efforts were made to clarify the integrated effects of main factors, i.e., initial coating shrinkage, thermally induced strain, steel-structural strain and the intrinsic coating flexibility at the initial and after aging, to the early cracking phenomena of epoxy coating in the ship's ballast tank. The coating crack is caused by combination of thermal stress, structural stress, and internal stresses which is closely related to chemical structures of the coatings. On the other hand, thermal stresses and dimensional stabilities would rarely play a major role in coating crack for ballast tank coatings with rather large flexibility. Crack resistance of the coatings at early stages can be estimated roughly by measuring internal stress, FT-IR and $T_g$ value of the coatings. A new screening test method was also proposed in this study, which can be possibly related to the long-term resistance of epoxy-based paints to cracking.

  • Fabry disease is an X-linked lysosomal disease caused by deficiency of ${\alpha}$-galactosidase, in which early diagnosis may be missed due to the wide variety of clinical symptoms presenting during disease progression. A 13 year-old boy visited our pain clinic complaining of pricking and burning pain in the toe tips of both feet. Continuous epidural infusion for pain management was performed because of oral analgesics ineffectiveness. The patient underwent ${\alpha}$-galactosidase A (GLA) enzyme analysis based on the clinical impression of Fabry disease from pain with a peripheral neuropathic component and history of anhidrosis. He was diagnosed with Fabry disease after confirming mutation of the GLA gene through a screening test of GLA activity. Enzyme replacement therapy was initiated and pain was tolerated with oral analgesics.

  • The risk assessment of thermal hazard to identify chemical or process hazard during early process developments have been considered. The early identification of thermal hazards associated with a process, such as rapid heats of reaction, exothermic decompositions, and the potential for thermal runaways before any large scale operations are undertaken. This paper presents to evaluate the safe operating parameters/envelope for exist plant operations. The assessment of thermal hazard with operating conditions such as amount of process materials, inhibitor, and catalyst on esterification process in manufacture of concrete mixture agents are described. The experiments were performed by a sort of calorimetry with the Multimax reactor system as a screening tool. The aim of the study was to evaluate the thermal risk of process material and mixture in terms of safety security to be practical applications in esterification process. It suggested that we should provide the thermal hazard of reaction materials to present safe operating conditions with cause of accident through this study.
