아동의 학교부적응에 영향을 미치는 관련 변인의 구조분석 (Linear Structural Relationships in Children's School Maladjustment and Related Variables)

  • Causal relationships between school maladjustment and related variables were examined in 371 5th grade elementary school students. School maladjustment was ascertained by revised the School Adjustment Test(Lim, 1993; Song, 1999). After analysis of differences in internal locus of control, academic stress, and academic achievement between the school maladjusted and adjusted groups, the study built a structural equation model of school maladjustment-related variables goodness of fit was tested with LISREL 8 for Windows. Significant differences were found in internal locus of control, academic stress, and academic achievement by level of school maladjustment. Internal locus of control, academic stress, and academic achievement both directly and/or indirectly influenced school maladjustment. These variables explained 64% of the variances in school maladjustment.

  • The purpose of this study was to investigate whether there are any differences among kindergarten teachers', primary school teachers', and children's mothers's perceptions-belief, knowledge, practice-about the educational articulation between kindergarten and primary school. The results of this study were as follows; First, there were differences among kindergarten teachers, primary school teachers, and mothers about the belief of articulation content. Second, in the knowledge of educational articulation, kindergarten teachers's understandings of the counterpart's curriculum were higher than that of primary school teachers's. Third, in the practice of educational articulation, kindergarten teachers and mothers were focused language and mathematics learning for educational articulation. But primary school teachers were focused school adjustment.

Variation in clinical usefulness of biomarkers of acute kidney injury in young children undergoing cardiac surgery

  • Background: Acute kidney injury (AKI) is one of the most significant postoperative complications of pediatric cardiac surgery. Because serum creatinine has limitations as a diagnostic marker of AKI, new biomarkers including neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), and interleukin-18 (IL-18) are being evaluated to overcome these limitations and detect AKI at an early stage after cardiac surgery. Purpose: This study aimed to investigate the clinical usefulness of these biomarkers in young children. Methods: Thirty patients with congenital heart diseases who underwent cardiac surgery using cardiopulmonary bypass (CPB) were selected, and their urine and blood samples were collected at baseline and 6, 24, and 48 hours after surgery. Serum creatinine and blood urea nitrogen levels as well as NGAL, KIM-1, and IL-18 levels in urine samples were measured, and clinical parameters were evaluated. Results: Of the 30 patients, 12 developed AKI within 48 hours after cardiac surgery. In the AKI group, 8 of 12 (66.6%) met AKI criteria after 24 hours, and urine KIM-1/creatinine (Cr) level (with adjustment of urine creatinine) peaked at 24 hours with significant difference from baseline level. Additionally, urine KIM-1/Cr level in the AKI group was significantly higher than in the non-AKI group at 6 hours. However, urine NGAL/Cr and IL-18/Cr levels showed no specific trend with time for 48 hours after cardiac surgery. Conclusion: It is suggested that urine KIM-1/Cr concentration could be considered a good biomarker for early AKI prediction after open cardiac surgery using CPB in young children with congenital heart diseases.

Growth observation and orthodontic treatment of a hemifacial microsomia patient treated with distraction osteogenesis

  • Hemifacial microsomia (HFM) patients may experience emotional withdrawal during their growth period due to their abnormal facial appearance. Distraction osteogenesis at an early age to improve their appearance can encourage these patients. Some abnormalities of the affected side can be overcome by distraction osteogenesis at an early age. However, differences in the growth rate between the affected and unaffected sides during the rest of the growth period are inevitable due to the characteristics of HFM. Therefore, re-evaluation should be performed after completion of growth in order to achieve stable occlusion through either orthognathic surgery or camouflage orthodontic treatment. An eight-year-old patient visited the clinic exhibiting features of HFM with slight mandibular involvement. He received phase I treatment with distraction osteogenesis and a functional appliance. Distraction osteogenesis was performed at the right ramus, which resulted in an open bite at the right posterior dentition. After distraction osteogenesis, a functional appliance and partial fixed appliance were used to achieve extrusion of the affected posterior dentition and settlement of the occlusion adjustment on the unaffected posterior dentition. The patient visited the clinic regularly for follow-up assessments, and at the age of 20 years, he showed facial asymmetry of the mandible, which had deviated to the right side. He received orthodontic treatment to improve the occlusion of his posterior dentition after the growth period. Without orthognathic surgery, stable occlusion and a satisfactory facial appearance were obtained through camouflage orthodontic treatment.

Inter-Facility Transport on Extracorporeal Life Support: Clinical Outcomes and Comparative Analysis with In-house Patients

  • Background: Extracorporeal life support (ECLS) is widely used in refractory heart or lung failure, and the demand for inter-facility transportation on ECLS is expanding. However, little is known about post-transportation outcomes, the clinical safety of such transportation, or the characteristics of the transported patients. Methods: This was a retrospective review of a 3-year, single-institution experience with inter-facility ECLS transport, as well as a comparative analysis of clinical outcomes with those of in-house patients. We also analyzed the risk factors for hospital mortality in the entire ECLS population using univariate and multivariate analyses to investigate the effects of transport. Results: All 44 patients were safely transported without adverse events. The average travel distance was 178.7 km, with an average travel time of 74.0 minutes. Early survival of the transported group seemed to be better than that of the in-house group, but the difference was not statistically significant (70.5% vs. 56.6%, p=0.096). The incidence of complications was similar between the 2 groups, except for critical limb ischemia, which was significantly more common in the transported group than in the in-house group (25.0% vs. 8.1%, p=0.017). After adjusting for confounders, being part of the transported group was not a predictor of early death (adjusted odds ratio, 0.689; p=0.397). Conclusion: Transportation of patients on ECLS is relatively safe, and the clinical outcomes of transported patients are comparable to those of in-house ECLS patients. Although matched studies are required, our study demonstrates that transporting patients on ECLS did not increase their risk of hospital mortality after adjustment for other factors.

학교폭력 피해자의 정신병 실태 조사 (A SURVEY OF THE PSYCHOSIS AMONG SCHOOL VIOLENCE VICTIMS)

  • 본 연구는 학교폭력의 피해자를 대상으로 정신병리에 대한 특성을 알아내고, 조기 정신병과의 관계를 규명함을 기본 목적으로 시행되었다. 더 나아가서는 조기정신병의 발견과 아울러, 조기 치료를 시행함으로써 심각한 정신병의 예방을 도모하고자 하였다. 서울시 전 구역 중 16개 중학교를 선정하여 중학교 1, 2학년 남 ${\cdot}$ 녀 학생에게 간이정신진단검사, 피어스-해리스 자아상 검사의 인기도와 학교 적응 소척도, 저자들이 개발한 따돌림 척도를 배포하여 따돌림을 당하는 정도의 점수가 평균보다 통계적으로 유의하게 차이가 나는 대상을 선정하고, 이들 중에서 스스로 인기가 없다고 여기면서 정신병적 증상이 유의하게 높게 보고된 대상을 선정하여'정신병의 전구기에 있는 군'으로 판정하여, 이들의 대상의 수를 계산하여 위험도를 산출하고, 정신병 전구기에 있다고 판단되는 대상의 임상적 특성을 상관 분석 및 회귀 분석을 통해 정신병적 증상과 관련된 임상적 척도들을 파악하였다. 결과는 다음과 같다. 1) 따돌림 척도에서 통계적으로 유의하게 높은 점수를 받은 군은 총 157명(5.3%)이었고, 그 중에서 47명(29.9%)이 간이정신진단검사의 정신병적 증상 수치가 유의하게 높았다. 따돌림 척도에서 높지 않은 점수를 보인 2,135명 중 정신병적 증상이 높은 군은 50명이어서, 따돌림 척도에서 높게 나온 군의 정신병적 증상이 높게 나올 위험도(odds ratio)는 17.8이었다. 이는 통계적으로 유의한 수치였다(${\chi}^2$=274.8, df=1, p<0.05). 즉 따돌림을 당한다고 호소한 집단은 정신병적 증상이 유의하게 높았고, 이를 통해 왕따 피해자는 정신병 발병 위험이 통계적으로 유의하게 높다고 할 수 있었다. 2) 자신은 남들에게 인기도 없고, 따돌림을 당한다고 여기며 정신병적 증상을 가진 군(47명, 남자 31명, 여자 16명)은 단순히 정신병적 증상이 높은 군에 비해 불안 척도, 우울증 척도, 공격성, 대인 민감성, 학교 및 사회 적응 척도, 강박증상 척도, 공포불안 척도, 정신병적 증상, 신체적 증상 호소 등에서는 차이를 보이지 않았으나 자신이 따돌림을 받는다는 호소와 편집증은 더 높게 측정되었다. 그리고, 학교폭력 피해자 중에서 정신병적 증상이 유의하게 높게 측정되었고, 스스로 인기가 없다고 생각하는 대상들에서는, 불안 증상, 우울증, 공격성, 대인 예민성, 강박증상, 편집증, 신체 증상 호소 등이 정신병적 증상을 잘 설명해주었다 ($r^2$=0.93). 즉, 따돌림을 당한다고 호소한 집단은 정신병적 증상이 유의하게 높았고, 이를 통해 왕따 피해자는 정신병 발병 위험이 통계적으로 유의하게 높다고 할 수 있었다. 이를 통해서 따돌림과 같은 학교폭력 피해자가 불안증상, 우울증, 공격성, 대인 예민성, 강박증상, 편집증, 신체 증상 호소와 같은 문제를 보였을 경우에는 정신병적 증상 유무와 학교 적응, 집중력 장애, 사고개념 형성 장애 등을 확인하여 정신병의 진단을 고려해야 한다고 할 수 있다.

아동의 우울보고에 따른 부모 아동행동평가의 차이 (Difference between Children's Self-Reports on Depression and Parents' Assessment of Children's Behaviors)

  • Objectives : Childhood is a critical period involving various developmental tasks that need to be accomplished. Childhood depression has overall negative implications for certain areas of development, including cognition, emotion, social skills, academic achievement, and ability to cope with stress. Yet, because depression can be "masked" by accompanying behavioral problems, early detection and diagnosis of childhood depression is somewhat challenging. In this study, using the Korean version of the Child Behavior Checklist (K-CBCL), we evaluated the association between children's self reports on depression and parents' assessment of children's behaviors. Methods : Subjects were recruited from the S city, a cohort comprising a non-random convenience sample of 226, 10-year-old ethnic Koreans in their fourth year of elementary school and their parents. All participants underwent several tests, including Children's Depression Inventory (CDI) and K-CBCL. Results : A total of 226 children, including 166 boys (73.5%) and 60 girls (26.5%), participated in the study. The average CDI for the participants was 14.57 (SD=7.54). Two items on the K-CBCL, total scale of adjustment scale and social withdrawal problems, showed a close association with the CDI. Conclusion : Although much remains to be elucidated, after controlling for covariates, the results of this study suggested that behavioral problems observed in children may be closely associated with depression.

The Association of Anxiety Severity With Health Risk Behaviors in a Large Representative Sample of Korean Adolescents

  • Objectives: Anxiety disorders are the most common psychiatric disorders in adolescents and seem to occur the earliest among all forms of psychopathology. The aim of this study was to investigate the association of anxiety severity with health risk behaviors and mental health in adolescents. Methods: Data from the 2020 Korean Youth Risk Behavior Web-Based Survey were analyzed. A total of 54948 adolescents responded to the 7-item Generalized Anxiety Disorder Scale (GAD-7) for the assessment of their anxiety severity as well as to the mental health and health risk behavior survey. Logistic regression analysis, t tests, and variance analysis of a complex sample general linear model were used to examine the association of anxiety severity with health behaviors and mental health. Results: After statistical adjustment for sociodemographic characteristics, the subjects in the severe anxiety group were significantly more likely to be current smokers (odds ratio [OR]: 2.08, 95% confidence interval [CI]: 1.72-2.50), current drinkers (OR: 1.91, 95% CI: 1.67-2.19), experience habitual substance use (OR: 10.89, 95% CI: 8.22-14.42), have sexual intercourse (OR: 2.10, 95% CI: 1.76-2.51), and have unprotected intercourse (OR: 2.21, 95% CI: 1.67-2.92) than those in the normal group. Anxiety severity negatively correlated with sleep satisfaction and happiness, but positively correlated with stress perception, loneliness, depressive symptoms, and suicidality. Conclusion: Adolescent anxiety is associated with health risk behaviors and poor mental health. Thus, early screening and intervention for anxiety in adolescents could contribute to the management and coping of youth health risk behaviors in the community.

초기 청소년의 자아존중감과 스트레스 대처방식 (A Study of Self-esteem and Stress Coping Skills in Early Adolescence)

  • Purpose: This study was conducted to investigate the relationship between self-esteem and coping skills among middle school students. Method: The participants were 674 students in three middle schools in GyeongGi-Do. The data were collected from June 10 to 20, 2005. The instruments used were the Rosenberg Self-esteem Scale and the Ways of Coping Checklist. The data were analyzed using the SPSS-Win 12.0 program. Result: Self-esteem showed a significant positive correlation with positive coping skills (r= .162. p= .000). As for the positive coping skills, there was a significant difference by grade (F= 4.69, p= .010), relationship with parent (F=4.036, p= .02), and relationship with siblings (F=3.902, p= .02). As for the negative coping skills, there was a significant difference by grade (F=3.05, p= .04), condition of living with parents (F=3.559, p= .02), economic status (F=3.736, p= .02), relationship with sibling (F=3.408, p= .03), and troubling matters (F=5.422, p= .005). Conclusion: Positive coping skills may be effective for adjustment because they positively related to self-esteem. Self-esteem and coping skills should be considered when an adolescent's mental health enhancement program is designed.

Strategic focus for substantial rewards

  • Due principally to the desire to seek lower production costs, the bulk of the world's textile and clothing manufacture migrated to low-cost zones, mainly outside Europe, over the course of the late-twentieth century. In the early-twenty-first century, fast fashion became a dominant force worldwide, with 'Western' retail buyers hunting cheaper deals from clothing manufacturers (mainly in Asia), and with occasional disasters not changing matters beyond the duration of a fashion season. Progressively, seams became narrower, cheaper raw materials were used and durability was no longer an aim. Why bother to do otherwise? This was what the 'Western' consumer wanted: fashion to be worn only a few times and then discarded, despite the fact that vast amounts of human, technological and financial resources were wasted in such a quest. By the end of the second decade of the twenty-first century, the production of textile and clothing products continued to contribute substantially to global warming. This paper reviews briefly the current conditions of manufacture, and argues that the research agenda should be focused on addressing the implications of a progressively changed focus, not on fast-fashion products, but instead on the production of products with greater durability. Meanwhile 'Western' consumers need to turn away from fast fashion and realise that waste is bad for their economy and their society. It is argued further, that after a period of re-adjustment, substantial financial rewards await the national textile and clothing industries that undergo such a turn around.
