Effects of early clinical and basic laboratory exposure program on premedical students: a questionnaire survey

  • Background: Because premed students do not take courses related to medicine during their first 2 years, they cannot establish their identity as students at medical schools, making it difficult for them to set goals as future doctors. We conducted an early clinical and basic laboratory exposure program for premed students and studied the effects of the program and student satisfaction levels. Methods: We performed an early clinical and basic laboratory exposure program for premed students for 2 days and evaluated the effects of the program and student satisfaction with it. The program consisted of two types: type 1, where two to four students formed a group, which was assigned to a particular department to participate and make observations during ward rounds, outpatient clinics, examinations, procedures, and surgeries (in the case of basic laboratory work, the students partook in experimental observations); and type 2, where one student followed a medical school professor to observe the professor's day. After the program ended, an online survey was conducted to investigate the effects on students, their thoughts, and satisfaction levels. Results: In total, 114 students (91.2%) responded to the survey. Approximately 94% of them were satisfied with the program. They found that the program would be useful for deciding on future career paths, gaining knowledge about a department of interest, studying for a medical program after premedical studies, and befriending residents and professors in certain departments. Conclusion: Early clinical and basic laboratory exposure programs are recommended for premedical students.

Effect of early robot-assisted training using virtual reality program in patient with stroke (가상현실을 이용한 조기 로봇보조 보행 훈련이 뇌졸중 환자에 미치는 영향)

  • Purpose: The purpose of this study was to determine the effect of early Robot-assisted training on gait ability, function and ADL in patients with stroke. Methods: 26 patients with stroke were recruited for this study. The subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with 13 patients in each group. All subjects received a routine physical therapy. The robot-assisted training was for 30 min in the case of the EG subjects. The assessment tools of this study involved the gait ability, balance ability, function and ADL. The measurements were recorded before the intervention and after the intervention. Results: EG subjects and CG subjects, the variables measured after the intervention significantly differed from gait ability, balance ability, function and ADL without the FMA (p<0.05). The FMA was only effective experimental group after intervention. Also, there were significant differences in gait ability, balance ability, function and ADL without the FMA at post-test between the 2 groups (p<0.05). Conclusion: The findings indicate that early robot-assisted training exerts a positive effect on gait ability, balance ability, function and ADL in patients with stroke. This result indicates the possibility of application of the early Robot-assisted training to the management for stroke patients. Further studies are required to generalize the result for this study.

The Effects of Ward Exercise Program on the Improvement of Activity of Daily Living in Patients Who have Stroke (뇌졸중환자의 일상생활동작 증진을 위한 침상운동 프로그램의 효과)

  • This study was conducted with the subject of showing the importance of early rehabilitation and exercise therapy in patients who have stroke, of confirming the adavntage of ward exercise conducted by nurse, which had been conducted mainly by physical therapist only in physical thrapy room and of developing the exercise program as the independent rehabilitation nursing intervention. A total of 62 patients were selected as object from April, 5th, to May, 17th, 1995, who had been hospitalized in K medical center, and the half of them were assigned to Experimental group in ramdom assignment using a coin. It was ADL check list tool developed by Kang and Ward Exercise Program developed by the researcher that were used as a treatment. Ward Exercise Program was conducted by the reseacher and the physical therapist measured ADL score before and after Ward Exercise Program. The data were analyzed using ANOVA, pearson correlation, Chi-Square test and the effect of Ward Exercise Program was analyzed by t-tast. The result of this study is as follows. 1. The experimental group showed eminent improvement of ADL compared with the counter group with statistical significance. In eating(t=6.10, df=60, p=.000), personal hygiene performing(t=4.86, df=60, p=.000), wearing(t=5.86, df=60, p=.000), elimination(t=7.89, df=60, p=.000), mobility on the bed(t=13.36, df=60, p=.000), moving(t=9.11, df=60, p=.000), walking(t=7.45, df=60, p=.000) 2. There was no qualitative difference between experimental group and control group with the significance of $p{\le}.05$. 3. There was no relation between the general condition and the difference of ADL, while there was significant relation between the starting point of exercise and the difference of pre-exercise and post exercise ADL. As a result, it should be emphasized that the early rehabilitation and exercise therapy are important in patients who have stroke, and that it is necessary to extend the exercise therapy to the ward. Therefore, this Ward Exercise Program could be recommended as a independent clinical exercise nursing intervention in rehabilitation nursing of patients who have stroke.

Total Ankle Arthroplasty Management and Rehabilitation (족관절 인공관절 치환술 후 관리 및 재활)

  • Although total ankle arthroplasty (TAA) has increased considerably in the past ten years, reflecting improvements in implant design and survivorship, the clinical outcomes have been less satisfactory than total hip or total knee arthroplasties. Several issues under debate include postoperative management and rehabilitation in TAA. Especially, there is no consensus or evidence for the most appropriate postoperative management and rehabilitation for patients undergoing TAA. This study was therefore undertaken to suggest appropriate postoperative management and rehabilitation in TAA, after reviewing published articles and focusing on the following topics: prehabilitation, hospital stay, immobilization type and duration, weight-bearing management, pharmacological treatment, and adopted rehabilitation protocols. In previous studies, the postoperative management and rehabilitation proposed depended on the surgeon's preference, the patient's characteristics, and the associated surgical procedures performed after TAA. Nonetheless, our research indicates the best approach is to include a prehabilitation program, immobilization in the early postoperative stage (2~4 weeks), range of motion exercise with partial weight-bearing ambulation, followed by full weight-bearing ambulation after six weeks. Further studies are required to develop a standardized rehabilitation protocol and improve the overall quality of care after TAA.

Results of Conventional and Accelerated Rehabilitation Following ACL Reconstruction (전십자인대 재건술 후 고식적 재활과 적극적 재활의 결과)

  • The purpose of this study is to compare the results between conventional and accelerated rehabilitation program following ACL reconstruction using bone-patellar tendon-bone. Conventional rehabilitation focused on protecting the new ligament by blocking terminal extension and avoiding active quadriceps function in the terminal degrees of extension. But there is current trend toward early postoperative mobilization and intensive. so called 'accelerated', rehabilitation stressing hyperextension of the knee. The results of intraarticular ACL reconstruction with conventional and accelerated rehabilitation were prospectively compared for one year postoperatively in a series of 27 patients. Range of motion and thigh circumference were checked preoperatively, and weekly up to 8 weeks, 3 months. 6 months, and 1 year postoperatively. Stress radiologic test, KT-1000 arthrometer. Cybex II dynamometer were checked in preoperatively, and 3 months, 6 months, and 1 year postoperatively. There were no differences of objective stability and restoration of muscle power. But the accelerated group had a low incidence of extension loss. excellent range of motion, and less difference of thigh circumference. We concluded that accelerated rehabilitation program is recommendable due to superiority in terms of range of motion, especially less extension loss without increasing laxity of knee joint.

Characteristic Analysis of Lower Limbs Muscles in Young Normal Adults on a Tilting Bed Using an Unstable Platform (불안정판을 부착한 경사침대에서 하지운동 시 정상성인의 근력 특성 분석)

  • We investigated an early rehabilitation training system that increase the intensity of patient rehabilitation training to shorten the time it takes for patients to progress to a secondary rehabilitation training stage by allowing patients incapable of self-ambulation. It consisted of tilting bed, unstable platform using strong springs and training program for lower limb rehabilitation. We performed experimental study on the muscular activities of tibialis anterior(TA), soleus(SO), gastrocnemius(GA) in the lower extremities during training of straight line, circle, quardrangle pattern during tilting angle of $30^{\circ}$, $60^{\circ}$. The muscle activities were higher during tilting angle of $30^{\circ}$ than $60^{\circ}$. In straight line pattern, the muscle activities were higher by SO, GA and TA during medio-lateral direction, however, by TA, SO and GA during anterio-posterior direction. In circle and quardrangle pattern, the muscle activities were higher by TA, SO and GA during clockwise and counterclockwise direction. The results indicate that the early rehabilitation training system could be applied to improve the lower extremity muscular strength for elderly and patients, especially, stroke.

Application of 3-Dimensional MOIRE Topography to the School Screening Program for Adolescent Scoliosis (모아레 체형측정법이 청소년기 척추측만증의 조기집단검진 활용 가능성에 대한 평가)

  • Objectives : The purpose of this study is researching for possibility that Moire topography be applied in group school screening for scoliosis known school health problem, and find acceptable method of early detection and early treatment for scoliosis Methods : The authors practiced Moire topography for primary & middle school 1,895 students[male 976, female 919] in Jeonju, korea in 2001. After we distinguished students who had abnormal finding in Moire topography and then re-examined spinal X-ray analysis. The data was analysed and evaluated statistically Results : According to this research, the abnormal finding in Moire topography was 53.7% (1,018 students), and students needed X-ray re-examination were 11.2% (213 students). Students diagnosed scoliosis by X-ray re-examination were 1.8%. According to statistical analysis, interval between vertical base line of pelvis and vertical base line of neck, gap between left distance and right distance to the vertical base line of pelvis and difference of contour lines have strong correlations with deformity degree of the body surface examined by Moire. Conclusions : Following this research, throughout early detection for scoliosis by Moire topography could reduce exposure from scoliosis radiographs, and could detect trunk asymmetry that couldn't be found existing X-ray examination, so it made selecting students under observation who have bad posture possible.

The Relationship between Neurocognitive Functioning and Emotional Recognition in Chronic Schizophrenic Patients (만성 정신분열병 환자들의 인지 기능과 정서 인식 능력의 관련성)

  • Objective:The present study examined the association between basic neurocognitive functions and emotional recognition in chronic schizophrenia. Furthermore, to Investigate cognitive variable related to emotion recognition in Schizophrenia. Methods:Forty eight patients from the Yongin Psychiatric Rehabilitation Center were evaluated for neurocognitive function, and Emotional Recognition Test which has four subscales finding emotional clue, discriminating emotions, understanding emotional context and emotional capacity. Measures of neurocognitive functioning were selected based on hypothesized relationships to perception of emotion. These measures included:1) Letter Number Sequencing Test, a measure of working memory;2) Word Fluency and Block Design, a measure of executive function;3) Hopkins Verbal Learning Test-Korean version, a measure of verbal memory;4) Digit Span, a measure of immediate memory;5) Span of Apprehension Task, a measure of early visual processing, visual scanning;6) Continuous Performance Test, a measure of sustained attention functioning. Correlation analyses between specific neurocognitive measures and emotional recognition test were made. To examine the degree to which neurocognitive performance predicting emotional recognition, hierarchical regression analyses were also made. Results:Working memory, and verbal memory were closely related with emotional discrimination. Working memory, Span of Apprehension and Digit Span were closely related with contextual recognition. Among cognitive measures, Span of Apprehension, Working memory, Digit Span were most important variables in predicting emotional capacity. Conclusion:These results are relevant considering that emotional information processing depends, in part, on the abilities to scan the context and to use immediate working memory. These results indicated that mul- tifaceted cognitive training program added with Emotional Recognition Task(Cognitive Behavioral Rehabilitation Therapy added with Emotional Management Program) are promising.

Accelerated Rehabilitation After Arthroscopic Bankart Repair - A Prospective Randomized Clinical Study - (관절경적 Bankart 봉합술후 적극적 재활치료 - 전향적 임상연구 -)

  • Purpose: In this prospective, randomized study, we compared the results of early motion versus conventional immobilization after the arthroscopic Bankart repair. Materilal and Methods : We performed an arthroscopic Bankart repair using suture anchors in 62 patients with traumatic anterior shoulder instability and randomized them into two groups; Group 1 (n=28; mean age, 28 years) underwent three-week of immobilization and conventional rehabilitation program, while Group 2 (n=34; mean age, 29 years) underwent an accelerated rehabilitation program with staged range of motion and strengthening exercises starting from the immediate postoperative day. Selected patients were non-athletes with a classic Bankart lesion and a robust labrum. Analysis of outcome included pain scores (6-week and follow-up: 31(9 months), range of motion, return to activity, recurrence, patients’ satisfaction with each program, and shoulder scores (ASES, UCLA, and Rowe). Results : The recurrent rate was not different between the two groups (2 anterior apprehension from each group) (p=0.842). Patients with accelerated rehabilitation resumed functional range-of-motion faster and returned earlier to the functional level of activity (p<0.05). Accelerated rehabilitation decreased postoperative pain and more patients were satisfied with this program (p<0.05). No differences were found between the two groups at the follow-up with regards to the shoulder scores, return to activity, pain score, and the range-of-motion. Conclusions : Early mobilization after arthroscopic Bankart repair does not increase the recurrence rate in selected patients. Although the final outcomes are similar in both groups, the accelerated rehabilitation program promotes functional recovery and reduces postoperative pain, which enables patients an early institution of desired activities.

  • The purpose of this study is to suggest a professional consensus on the topics that should be dealt with importantly in the movement program for the recovery of cancer patients in Korea. As a result of calculating the importance of the finalized movement program component, First, the opinions about the environment for the promotion of movement were suggested as pleasant facilities, natural lighting, rest room space, facilities aspect of environment friendly space, education about diseases, participation programs, health feedback, information handouts. Second, the opinions on the purpose of the movement program were suggested to promote prevention and early screening of cancer, education, sharing effective treatment methods of cancer patients, cognitive aspect and psychological support for accurate information transmission, stress relief, and improvement of quality of life. Third, the opinions on the consideration of the movement program suggested stress relieving, cancer patient's needs, correct information delivery, quality of life, and participation of cancer patients. Fourth, the problems of the movement program were presented with different physical characteristics, physical inconvenience, the patient side of passive participation, space narrowness and lack, space aspect of program exclusive space etc.
