THE EFFECT OF CPP-ACP PRE-TREATMENT ON BOND STRENGTH OF A SEALANT (CPP-ACP 적용이 치면열구전색제의 결합강도에 미치는 영향)

  • Recently the effect of CPP-ACP on early caries remineralization and prevention has hashbeen extensively researched. However, there has been a lack of research on micro-shear bond strength of a sealant applied on a CPP-ACP treated surface. Therefore, the purpose of this study was to evaluate the effect of CPP-ACP on the micro-shear bond strength of a sealant bonded with 3 different bonding systems. From the sound human 3rd molars, the 150 specimens were prepared and randomly assigned to six groups. Group I: Immersion in artificial saliva for 2 weeks + 35% phosphoric acid Group II: Immersion in artificial saliva for 2 weeks + 35% phosphoric acid + dentin bonding agent Group III: Immersion in artificial saliva for 2 weeks + self-etching adhesive Group IV: CPP-ACP application for 2 weeks + 35% phosphoric acid Group V: CPP-ACP application for 2 weeks + 35% phosphoric acid + dentin bonding agent Group VI: CPP-ACP application for 2 weeks + self-etching adhesive Sealant was applied and the micro-shear bond strength was measured. From the result of this study, it can be assumed that the CPP-ACP pretreatment can weaken the bond strength of a sealant if the enamel surface is conditioned with self-etch adhesive.

  • Multimodal cancer therapy including surgery, chemotherapy, and radiotherapy could not only improve the prognosis of malignancy but also reduce the dosage and toxicity of cancer drug for treatment of malignant tumor. The effects of radiotherapy are generally localized, additive, and accumulative, and depend on dosage, site and cell sensitivity. However, in growing individuals, the dental and skeletal sequelae to radiotherapy result in dental or facial abnormalities that are irreversible : arrested root development, disturbances in enamel formation, microdontia, anodontia, altered tooth eruption and mandibular or maxillary hypoplasia. Especially, the teeth which are developing is affected according to the stage. We report three cases of developmental disturbance of permanent teeth after radiotherapy. These children had received radiotherapy for malignant tumor at the age of 3 to 4 years, in which root hypoplasia, short tapered root and early apex closure were observed. For the management of radiation caries and radiotherapy-related teeth, periodic recall check and oral hygiene instruction are required.

DENS INVAGINATUS IN MANDIBULAR CENTRAL INCISORS (하악 중절치에 발생한 치내치)

  • Dens invaginatus is a rare malformation resulting from invagination of the enamel before calcification has occurred. It is mostly found in permanent maxillary lateral incisors and mandibular teeth are rarely affected by this anomaly. The malformation is estimated to affect between 0.04 % and 10 % of people and has been associated with other abnormalities such as taurodontism, microdontia, gemination and dentinogenesis imperfecta. Dens invaginatus is classified in three types with respect to the depth of invaginatus and has a broad spectrum of morphologic variations. Invagination frequently allows the entry of irritants and microorganism, which usually lead to caries, pulp infection and pulp necrosis. Root canal treatment on such invaginatus tooth may present severe problems because of its complex anatomy of the tooth. Therefore, the early diagnosis of such malformation is crucial and preventive approach is strongly recommended.

IDENTIFICATION OF THE AG I/II AND GTFD GENES FROM STREPTOCOCCUS MUTANS GS-5 (연쇄상구균 GS-5의 ag I/II와 gtfD 유전자 클로닝)

  • Streptococci are Gram-positive, facultative anaerobes and have no catalase activities. Among mutans streptococci containing ${\alpha}-type$ hemolytic activity, S. mutans is a causative agent for dental caries. As well as acid production yielding the demineralization of tooth enamel, adherence and colonization of S. mutans to the teeth are also important for its virulence. These early colonization are accomplished by the bacterial fibrillar protein, Antigen I/II (Ag I/II) and glucosyltransferase (GTF). Therefore, Ag I/II and GTF are reasonable targets for the development of vaccine against S. mutans GS-5. The ag I/II and gtfD genes from S. mutans GS-5 were cloned and sequenced. Sequence analyses showed the nucleotides sequence of cloned genes had high homology to the sequences previously reported. The sequence alignment of 280 nucleotides between the cloned Ag I/II and the available sequence of the corresponding S. mutans GS-5 showed the perfect match. Comparing with the sequence of gtfD from S. mutans UA159, the corresponding nucleotide sequence of S. mutans GS-5 showed some mismatches and the mismatches introduced changes in four residues out of 105 amino acids, yielding four missense mutations.

TREATMENT OF DENTAL CARIES BY ER:YAG LASER IN CHILDREN (소아 환자에서 Er:YAG Laser를 이용한 우식 병소의 처치)

  • The lasers have been used in dentistry for more than 30 years and the application of lasers for drilling dental hard tissue has been investigated since the early developement of lasers. Recently, the Er:YAG laser was invented for hard tissue ablation. The Er:YAG laser, having a wavelength of 2.94um, is highly absorbed in both water and hydroxiapatite, leading to a very effective material for hard tissue removal by bursting off the solid tissue component that is, enamel and dentin are removed by the Er :YAG laser by water vaporization and microexplosion, without any melting of inorganic tissues. Therefore, the Er:YAG laser produced round craters with well defined margins and the surrounding tissues had no cracks and no charring. When used for cavity preparation, pulpal damage should not occur if hear buildup is minimized by careful selection of exposure parameters and by use of a water spray. The present study demonstrated that the Er:YAG laser cut the tooth substance adequately for composite resin restoration, without having undesirable side effects such as harmful effects on the pulp, discoloration or cracking etc. Also, the child patients were well cooperative during laser treatment mainly because of little noise, lesser vibration and minimal pain compared to conventional means of cavity preparation.

IN VITRO STUDY OF CARIOGENIC POTENTIAL OF INFANT CONFECTIONARIES (유아용 과자의 우식유발능에 관한 생체 외 연구)

  • The purpose of this study was to assess the cariogenic potential of infant confectionaries. In vitro, as compaired with 10% sucrose solution and whole bovine milk. Buffering capability were determined by amount of 0.1N lactic acid consumed to titrate the 50ml specimen solutions to pH 4.0. The pH of the specimen solution inoculated by streptococcus mutans was measured by pH meter and the surface microhardness tester, before and after 48 hours incubation. The buffering capacity of infant confectionaries was higher than that of sucrose solution and lower than that of milk, and there were significant difference between infant confectionaries(p<0.05). The pH of infant confectionaries after 48 hours incubation was similar to 10% sucrose solution, and there were significant difference between infant confectionaries and milk(p<0.05). The microhardness change of primary tooth enamel of infant confectionaries group after 48 hours incubation was similar to that of 10% sucrose solution, and there were significant difference between infant confectionaries and milk(p<0.05). In conclusion, infant confectionaries seemed to have the ability to cause dental caries in primary teeth, and there were significant differences of cariogenic potential among infant confectionaries. Cooperative efforts of dentistry and manufacturers to reduce the cariogenic potential of infant confectionaries would be necessary to prevent the early childhood caries in children.
