Dental caries is the most common oral disease. There are many factors contributing to its development, but complete understanding and prevention are not fully known. However, it is possible to remineralize the early enamel curious lesion by fluoride containing remineralization solution. Recently the pH-cycling model has been used to examine the effect of fluoride solution on remineralization of artificial caries in vitro as it can closely simulate the conditions encountered in vivo within a carefully controlled environment. The aim of this study was to evaluate the remineralizing effects of supersaturated buffer solutions under pH-cycling model. The specimen with 3mm-diameter was made using mature bovine incisors which has no caries and has sound enamel surface. Early curious lesions were produced by suspending each specimens into demineralization solution at pH 5.0 for 33 hours and the specimen whose surface hardness value ranged from 25 to 45 VHN were used. The pH cycling treatment regimen consisted of 5 min soaks of three treatment solutions four times per days for 15 days and the continuous cycling of demineralization and remineralization were carried out for 15 days. Following the pH-cycling treatment regimen, the specimens' surface microhardness were measured by the Vickers hardness test (VHN) and analyzed by ANOVA and Duncan's multiple-range test. 1. The surface microhardness value of supersaturated solution, Senstime, and Gagline groups were increased after pH cycling, and that of supersaturated solution was significantly Increased compared to saline group(P<0.05). 2. The surface remineralization effect of fluoride containing solutions was accelerated by saliva under pH-cycling mode 3. The pH cycling model was considered appropriate to mimic the intra-oral pH changes when evaluating demineralization and remineralization in vitro. Under the results of above study, salivary remineralization effect can be improved by fluoride containing remineralization solution. The pH-cycling model was considered appropriate to mimic the intra-oral pH changes when evaluating demineralization and remineralization in vitro.

The aim of this study was to apply the quantitative light-induced fluorescence(QLF) and use of fluorescein-enhanced QLF method for quantitative assessment of early enamel demineralization in vitro, comparing effectiveness of light sources : argon laser, halogen lamp, light emitting diode (LED) and plasma arc lamp. Sixty extracted teeth were used, prepared by gentle pumicing and coating in an acid-resistant nail-varnish, except for an exposure to a demineralizing solution. Teeth were removed at regular intervals (24, 48 and 72hrs), air-dried and QLF image were taken. Mineral loss, as measured by difference of optical density, was recorded. For dyeenhanced QLF a 0.075% sodium fluorescein dye was applied after QLF examination and mineral loss was recorded. The following result were obtained : 1. Comparing with mean difference of optical density, plasma arc lamp was higher than other light sources in all group (p<0.05). 2. Comparing each group with mean difference of optical density, there was significant different using plasma are lamp and halogen lamp. 3. For use of dye-enhanced QLF, comparing with mean difference of optical density, plasma arc lamp were higher than other light sources in all group(p<0.05). 4. With this model dye-enhanced QLF compared with QLF in optical density, dye-enhanced QLF was higher than QLF in optical density. From the results presented in this paper, it was concluded that plasma light source was more effective compared with other light source for the detection and the quantification of early enamel caries.
Purpose: The aim of this study was to analyse and review deep learning convolutional neural networks for detecting and diagnosing early-stage dental caries on periapical radiographs. Materials and Methods: In order to conduct this review, the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA) guidelines were followed. Studies published from 2015 to 2021 under the keywords(deep convolutional neural network) AND (caries), (deep learning caries) AND (convolutional neural network) AND (caries) were systematically reviewed. Results: When dental caries is improperly diagnosed, the lesion may eventually invade the enamel, dentin, and pulp tissue, leading to loss of tooth function. Rapid and precise detection and diagnosis are vital for implementing appropriate prevention and treatment of dental caries. Radiography and intraoral images are considered to play a vital role in detecting dental caries; nevertheless, studies have shown that 20% of suspicious areas are mistakenly diagnosed as dental caries using this technique; hence, diagnosis via radiography alone without an objective assessment is inaccurate. Identifying caries with a deep convolutional neural network-based detector enables the operator to distinguish changes in the location and morphological features of dental caries lesions. Deep learning algorithms have broader and more profound layers and are continually being developed, remarkably enhancing their precision in detecting and segmenting objects. Conclusion: Clinical applications of deep learning convolutional neural networks in the dental field have shown significant accuracy in detecting and diagnosing dental caries, and these models hold promise in supporting dental practitioners to improve patient outcomes.

This study aimed to evaluate surface morphology and resin tag penetration of resin infiltration into primary anterior teeth after enamel deproteinization with sodium hypochlorite (NaOCl) prior to phosphoric acid ($H_3PO_4$) etching. Ninety primary anterior teeth with non-cavitated caries lesion were devided five groups according to enamel pretreatment as follows, group I-15% hydrochloric acid (HCl) 2min. ; group II-5.25% NaOCl 1min., 35% $H_3PO_4$ 1min. ; group III-5.25% NaOCl 2min., 35% $H_3PO_4$ 1min. ; group IV-5.25% NaOCl 1min., 35% $H_3PO_4$ 2min. ; group V-5.25% NaOCl 2min., 35% $H_3PO_4$ 2min. Fifteen teeth were examined etched surface structure using field emission-scanning electron microscope. Seventy five teeth were infiltrated with resin, maximum penetration depth and percentage penetration were analysed using dual fluorescence confocal microscopy. As the application time of NaOCl increased, ratio of enamel type I, II were increased. Percentage penetration (PP) was higher in group V than group II, III (p < 0.05). PP of group IV, V did not show any differences. Non-cavitated caries of primary anterior teeth can be treated with resin infiltration. Enamel deproteinization with NaOCl prior to 35% $H_3PO_4$ etching could be an alternative of 15% HCl etching in resin infiltration.

A new method for early caries diagnosis was proposed and tested through a home-made optical examination system that used quantitative light fluorescence (QLF) and digital imaging fiber optic transillumination (FOTI) (DIFOTI), with light sources across a wide spectral range, from 350 nm to 1,000 nm. The front-illuminated infrared light scattering image (FIR) showed similar diagnostic abilities to that of DIFOTI. The FIR method was invented based on the observation that caries lesions lose the high transmittance and low scattering properties of benign enamel tissue. There are various methods for the early diagnosis of caries, such as visual examination, exploration, X-ray radiography, QLF, FOTI, and infrared fluorescence (diagnodent). Among them, methods based on optical properties are regarded as having the most potential. A comparative study was performed between the FOTI, QLF, diagnodent, optical coherence tomography, and FIR scattering image methods, using 20 extracted teeth samples with early caries. A scale of lesion measurement based on optical image contrast was proposed. The statistical analysis showed a significant correlation between the DIFOTI and FIR methods (r = 0.35, p < 0.05). However, the QLF and diagnodent methods showed little association with FIR images, as they have different detection principles as compared with FIR. Tomographic images obtained by OCT, using 1,330 nm super luminescent LED as a gold standard of tooth structure, verified that the FOTI and FIR results correctly represented the lack of homogeneity in dental tissue. The newly proposed FIR method attained similar diagnostic results to those of FOTI, but with an easier approach.
Background: In our study, a silver diamine fluoride (SDF) compound for the treatment of dental caries was synthesized to characterize its remineralization activity upon direct application to deciduous teeth. This study aimed to use microscopic computed tomography (microCT) and nanoindentation to evaluate whether SDF composite application could effectively arrest dental caries in five exfoliated primary molars. Methods: Carious teeth were extracted and visually examined using quantitative photofluorescence devices (Qraycam and QraypenTM). After performing microCT, the SDF composite was applied to the teeth according to the manufacturer's instructions. The researchers exchanged and precipitated the irritant saliva once daily for 1 week. The teeth were sectioned longitudinally through the centers of the mesial and distal surfaces, embedded, polished, and measured using nanoindentation. Thereafter, microCT was repeated. Statistical analyses were performed using GraphPad Prism software. Results: Following SDF composite application, a remineralized layer was observed on microCT images, and the hardness increased when measured using nanoindentation. We found that demineralized enamel presented with an increased number of irregular crystals in the deep carious lesion group compared with those in the shallow carious lesion group, resulting in a rougher surface. Conclusion: The SDF composite may be used for remineralization of early caries and cessation of advanced caries in primary molars.

This study was performed to evaluate clinical use of laser fluorescence (LF) to identify early childhood caries lesions suitable for applying resin infiltration. 20 exfoliated primary molars with proximal caries were selected and cut buccolingually cross the central pit for regarding the mesial and distal surfaces respectively. 27 specimens corresponding to ICDAS code 1 and 2 were selected and the LF values were measured. When infiltrant resin was applied, double staining for microscopy detection has done simultaneously. Tooth samples were sliced with 0.7 mm thick. The maximum lesion depth, maximum penetration depth, and average penetration rate were measured from the confocal scanning laser microscope image. Pearson correlation analysis was performed. The intraclass correlation coefficient of LF values shows excellent agreement. LF values had positive correlation with penetration rate, but not lesion depth and penetration depth. Significant correlation between LF readings and penetration rate was verified in deep enamel caries and dentin caries except shallow enamel caries. Infiltrant resin could penetrate with a higher rate and LF values could be increased in more active caries lesions. In assessing radiologically similar caries lesion, laser fluorescence might be useful for identifying caries activity.

Current treatment concept of dental caries has been changed, because it has been proved that it is a preventable disease. The philosophy has been changed from purely restorative treatment to preventive caries control. Therefore the methods or criteria of oral examination has been changed. The clinician have to detect not only cavitation, but also the lesion of non-cavitation stage. International Caries Detection and Assessment System II (ICDAS II) was developed recently, which is a new criteria of classification of dental caries. This system was based on the current concept of prevention, early detection and patient-centered management of caries. Therefore this philosophy is in accord with the perspective of pediatric dentistry. The purpose of this article is to introduce this system for oral examination of children.

Moebius syndrome is a rare, congenital neurological disease involving facial paralysis and limitation of eye movements. It results from maldevelopment of the sixth and seventh cranial nerves. Dental features of this syndrome include micrognathia, microstomia, tongue deformity, cleft palate, hypoplasia of the teeth, and congenital missing teeth. A 7-year-old female with Moebius syndrome was referred from a local dental clinic for caries treatment. She presented with facial paralysis and microstomia. Oral findings included multiple caries with enamel hypoplasia, congenital missing teeth, and tongue deformity. Dental treatments including restorative and preventive procedures were performed. Oral findings and management aspects of Moebius syndrome for this case are discussed. Early evaluation and multidisciplinary care are needed for children with Moebius syndrome.

Hidden caries is a subtype of the occlusal pit and fissure caries type and is defined as a dentinal caries lesion near the occlusal surface of the tooth seen on a radiograph. In visual examination, the occlusal enamel is seen intact or is minimally perforated. Covert caries, Occlult caries or Fluoride syndrome are used as synonym. The percentage of occlusal dentin lesions that are clinically undetected ranges from 1.4-50%. Little is known about the mechanisms involved in the development of hidden caries. But it is thought that extensive use of topical fluoride or the special bacteriological profile has been a major factor. This case report is about detection and treatment of hidden caries of two children who visited the department of pediatric dentistry, Yonsei University Dental Hospital. The color of caries dentin found in hidden caries lesion is lighter than cavity forming caries, which makes it more difficult to detect caries by visual examination. Therefore diagnosis of hidden caries is often accomplished after clinical sign is recognized by patients. The use of advanced caries detection aids such as Diagnodent.. with periodic radiographic examination is seemed to be helpful for early detection of hidden caries.
