Safety and effectiveness of early cardiac rehabilitation in a stroke patient with heart failure and atrial fibrillation: a case report

  • Stroke patients have reduced aerobic capacity. Therefore, intensive structured exercise programs are needed. We report the case of a patient with stroke and cardiac disease who underwent early inpatient cardiac rehabilitation (CR). A 38-year-old male patient with atrial fibrillation, heart failure, and cerebral infarction underwent a symptom-limited exercise tolerance test (ETT) without any problems on day 45 after admission. He completed a 2-week inpatient program and an 8-week home-based CR program. Follow-up ETT showed increased exercise capacity. The present case might be the first to report a safely performed CR program in a patient with stroke and cardiac comorbidity in Korea. Systematic guidance is needed for post-stroke patients to receive safe and effective CR for the secondary prevention of stroke and cardiovascular risk.

Time-frequency domain characteristics of intact and cracked red sandstone based on acoustic emission waveforms

  • This study conducts uniaxial compression tests on intact and single crack-contained rocks to investigate the time-frequency domain characteristics of acoustic emission (AE) signals monitored during the deformation failure process. A processing approach, short-time Fourier transform (STFT), is performed to obtain the evolution characteristics of time-frequency domain of AE signals. The AE signal modes at different deformation stages of rocks are different. Five modes of AE signal are observed during the cracking process of rocks. The evolution characteristics of time-frequency domain of AE signals processed by STFT can be utilized to evaluate the damage process of rocks. The difference of time-frequency domain characteristics between intact and cracked rocks is comparatively analyzed. The distribution characteristics of frequency changing from a single band-shaped cluster to multiple band-shaped clusters can be regarded as an early warning information of damage and failure of rocks. Meanwhile, the attenuation of frequency enables the exploration of rock failure trends.

Early Phase Contingency Trajectory Design for the Failure of the First Lunar Orbit Insertion Maneuver: Direct Recovery Options

  • To ensure the successful launch of the Korea pathfinder lunar orbiter (KPLO) mission, the Korea Aerospace Research Institute (KARI) is now performing extensive trajectory design and analysis studies. From the trajectory design perspective, it is crucial to prepare contingency trajectory options for the failure of the first lunar brake or the failure of the first lunar orbit insertion (LOI) maneuver. As part of the early phase trajectory design and analysis activities, the required time of flight (TOF) and associated delta-V magnitudes for each recovery maneuver (RM) to recover the KPLO mission trajectory are analyzed. There are two typical trajectory recovery options, direct recovery and low energy recovery. The current work is focused on the direct recovery option. Results indicate that a quicker execution of the first RM after the failure of the first LOI plays a significant role in saving the magnitudes of the RMs. Under the conditions of the extremely tight delta-V budget that is currently allocated for the KPLO mission, it is found that the recovery of the KPLO without altering the originally planned mission orbit (a 100 km circular orbit) cannot be achieved via direct recovery options. However, feasible recovery options are suggested within the boundaries of the currently planned delta-V budget. By changing the shape and orientation of the recovered final mission orbit, it is expected that the KPLO mission may partially pursue its scientific mission after successful recovery, though it will be limited.

  • Galvanized Steel pipes have been widely used in industries and apartments, Unexpected early leakage has been found in an apartment. Tunneling corrosion or penetration was found in the water supply pipes. The chemical compositions of the pipes and properties of coating layer were evaluated. The pipes met the specification of KS D 3507. The cause of early failure was analyzed through the examination of macrostructures and microstructures, It was found that the pipes were failed by grooving corrosion, which resulted from galvanic corrosion of weld bead and matrix.

  • Purpose: The purpose of this paper is to suggest an environmental test plan for reducing early failure. Methods: We study the preferred test design and tests that are performed substantially in the company. We investigate the environmental profile on the actual supply chain and identify the effectiveness for this test design. Results: Proposed sequential test designed to implement the supply chain on the environment profile showed more effective results compared to existing tests. Conclusion: Suggested sequential test in this paper will be an effective guidance for the given environment test.

  • Videophone service was developed and marketed by AT&T in the late 1960s and early 1970s. The market did not respond to the service offering, and the service was withdrawn. However, Videophone returned in the late 1980s and early 1990s. Due to the technological advance, establishment of ITU standards and fall of videophone price, many analysts are very optimistic about videophone market. But on the other hand, some analysts anticipate that recent videophone will only replicate the past with the same results: namely, market failure. First, this article reviews the history of videophone service, the recent development trend and reasons for its market failure. Second, this article reviews prospects for videophone service. Finally authors examine some of the technological and service features that might shift the value of videophone service.

  • There are many methodologies fur doing analysis of system's reliability in early design stage. Among the methods, PRISM is, as compared to MIL-HDBK-217, a newly developed technology but not easy to use. Because PRISM provides models that predict a part failure rate and field database, called EPRD and NPRD that can be combined with prediction models. This paper presents some capabilities of the prediction models in PRISM and usability of EPRD and NPRD database in system level reliability prediction.

Early Valve Replacement in Patient with Native Valve Endocarditis - Report of Seven Cases - (활동기 자가판 심내막염의 판막치환술: 7례 경험)

  • From October 1988 to November 1989, seven patients underwent valve replacement during the active phase of native valve endocarditis. There were 4 males and 3 females whose mean age was 41 years[range, 16 to 68 years]. Preoperative two-dimensional and Doppler echocardiography showed vegetations and severe valvular regurgitation in all patients. Blood cultures were positive in 4, and negative in 3 patients Organisms were alpha-hemolytic Streptococcus in 2, Staphylococcus epidermidis in 1, Erysipelothrix rhusiopathiae in 1 patient Valve tissue cultures were negative in all patients. Intravenous antibiotic therapy had been done for 3 to 18 days in 5 patients pre-operatively and was not done in 2 patients, Indications for operation were heart failure in h, and systemic emboli in 1 patient. The aortic valve was involved in 3, mitral in 1, and both aortic and mitral in 3 patients, One operative death[14.4%] occurred in patient with cardiogenic shock before operation. Late death occurred in one on 14 months after operation. The remaining 5 patients were followed up over a two year period in good condition. In conclusion, native valve endocarditis with severe heart failure must be considered for early operation.

Early Surgical Intervention of Active Infective Endocarditis (심내막염 환자의 수술적 치료)

  • During one year period from Sep. 1986 to Sep. 1987, we have experienced 6 cases of infective endocarditis requiring surgical interventions. All 6 patients had class IV or V cardiac disability at the time of surgery. The indication for surgery was rapidly progressive congestive heart failure in all cases. Four patients underwent aortic valve replacement including one double valve replacement. Two other patients required other surgical procedures, removal of large left atrial vegetation mass in one patient and excision of destroyed pulmonary valve and aortic vegetation in the other patient. Two patients died; one of mitral annulus rupture after release of aortic clamp and the other of mediastinal bleeding 3 months after replacement of aortic valve. Three out of 4 survivors are in NYHA Class I and the remaining patient is in Class II. We emphasize that early operative intervention is life-saving in patients with persistent or progressive congestive heart failure, irrespective of the activity of the infective process or the duration of antibiotic therapy.

Improvement of an Early Failure Rate By Using Neural Control Chart

  • Even though the impact of manufacturing quality to reliability is not considered much as well as that of design area, a major cause of an early failure of the product is known as manufacturing problem. This research applies two different types of neural network algorithms, the Back propagation (BP) algorithm and Learning Vector Quantization (LVQ) algorithm, to identify and classify the nonrandom variation pattern on the control chart based on knowledge-based diagnosis of dimensional variation. The performance and efficiency of both algorithms are evaluated to choose the better pattern recognition system for auto body assembly process. To analyze hundred percent of the data obtained by Optical Coordinate Measurement Machine (OCMM), this research considers an application in which individual observations rather than subsample means are used. A case study for analysis of OCMM data in underbody assembly process is presented to demonstrate the proposed knowledge-based pattern recognition system.
