Is It possible To Apply Placebo Auricular Acupuncture to Korean? - The Effects of Placebo Auricular Acupuncture through Single-blind Method and Randomized Controlled Trial - (한국인에게 플라세보 이침의 적용이 가능한가? - 단일검맹과 무작위배정법을 통한 플라세보 이침의 효과 -)

  • Objective : This study was designed to find out whether placebo auricular acupuncture is able to be constituted as an appropriate control group for needle insertion to Korean. Methods : We performed a randomized controlled, single-blind study(n=48) comparing subjective evaluations of needles insertion and avoidance of needles insertion. Both ears were needled concurrently, one real acupuncture(needle insertion) and the other placebo acupuncture(avoidance of needle insertion with blunt tip). Subjects then completed a questionnaire rating the intensity of five sensations in each ear, and also attempted to identity which ear received placebo and which ear received real acupuncture. Results : As for the effects of real and placebo acupuncture on each item(Warmth, Fullness, Pain, Activity, Radiating), real acupuncture was significantly warmer, fuller, more painful than placebo acupuncture. In particular, the real acupuncture only in experienced acupuncture recipients was significantly warmer, fuller, more painful than placebo acupuncture. About the ability to differentiate real and placebo acupuncture, 54.2 percent correctly identified which ear received real acupuncture and which ear received placebo acupuncture. However, the ability to differentiate real and placebo acupuncture for naive and experienced acupuncture recipients was not significantly different. Conclusions : Most of the subjects(Korean) identified which ear received placebo and which ear received real acupuncture. We found that this placebo auricular acupuncture is not able to be applied to Korean, in particular for experienced acupuncture recipients. Further study is needed for new placebo auricular acupuncture or racial difference between American.

  • An osteoma of the external auditory canal(EAC) is an uncommon benign tumor. If a canal obstruction occurs, ear fullness or conductive hearing loss can arise. But, the association of an osteoma with a cholesteatoma is extremely rare. Therefore, we report a case of a 35-year-old female patient with osteoma of temporal bone that was complicated with EAC cholesteatoma.

  • Meniere's disease is a disorder of the membranous labyrinth that is characterized by vertigo, hearing loss, tinnitus and ear fullness. The etiology of Meniere's disease is yet to be described and the management is also very difficult. Fifty-one years old female patient had paroxysm of severe tinnitus and dizziness after medical therapy and surgical intervention during several years. Patient received stellate ganglion block (SGB) and her symptoms were completely relieved. Therefore, SGB could be considered another therapeutic modality for Meniere's disease.

  • Objectives The purpose of this study is to report the effect of oriental demonstration treatment for episodic vertigo of Meniere's disease. Meniere's Disease characterized by recurrent prostrating episodic vertigo, nausea, vomiting, sensory hearing loss, tinnitus, and a feeling of fullness in the ear associated with generalized dilation of the membranous labyrinth (endolymphatic hydrops). The cause of Meniere's disease is unknown, and the pathophysiology is poorly understood. Methods We experienced one case of Meniere's disease treated with Asian medical treatment, acupuncture and herbal medication. 64-year-old female patient was diagnosed with Meniere's disease by western medical doctor. Methods: Saam acupuncture Sojangjungkyuk applied to the patient. Results After treatment her episodic vertigo, nausea, vomiting and endolymphatic hydrops were decreased but the level of tinnitus it was not improved as much as other symptoms. Conclusions Saam Acupuncture of Asian medical treatment could be safe and effective.

  • Meniere's disease is characterized by episodic vertigo, fluctuating hearing loss, tinnitus and ear fullness. The main pathological finding is endolymphatic hydrops, but the etiology of disease and effective treatment methods are still disputed. Recently, Cervical spine disorders(CSD) and Temporomandibular disorders(TMD) have been attracting attention as one of the causes of Meniere's disease. A 65-year-old female Meniere patient with musculoskeletal problems in the cervical and mandible area was treated by Korean medical therapies including pharmacoacupuncture treatment. After 5 weeks of treatment, there was no meaningful change in hearing level evaluated with pure tone audiometry, but the subjective symptoms of Meniere improved significantly. Numerical rating scale (NRS) decreased from 10 to 0 for hearing loss, 10 to 3 for tinnitus and 8 to 3 for ear fullness. Also NRS of cervicalgia was reduced from 5 to 0 after treatment. The result suggests that the Korean medical therapy including pharmacoapuncture targeting CSD and TMD could be safe and effective method for patients with Meniere's disease.

  • Meniere's disease is an idiopathic syndrome of endolymphatic hydrops characterized by episodic vertigo, tinnitus, fluctuating hearing loss and ear fullness. The etiology and pathophysiology of the disease is still disputed. As yet, no treatment has conclusively modified the clinical course of the condition and thereby prevented the associated progressive hearing loss. We observed two cases of Meniere's disease treated with oriental herbal medication by the diagnosis of Deficiency-Excess. One patient had taken BangHyunOnDam-tang, and the other had taken ChungGanESa-tang. After treatment, vertigo attacks were controlled in both. Tinnitus and hearing loss were improved in one patient and unchanged in the other. Therefore, we believe that oriental herbal treatment may be a therapeutic modality that is effective in controlling Meniere's disease.

Viability of Somatic Cell Nuclear Transfer Embryos following Embryo Transfer in Korean Native Striped Cattle (Bos namadicus Falconer, Chikso)

  • This study was conducted to examine the viability of Korean native striped cattle (Bos namadicus Falconer, Chikso) clone embryos after embryo transfer. Chikso somatic cell nuclear transfer (SCNT) embryos were produced by fusion of ear skin cells derived from a female Chikso with enucleated oocytes matured in vitro for 18-24 hr. After in vitro culture of SCNT embryos for 7 to 8 days, fresh or vitrified blastocysts derived from SCNT were transferred into a uterine horn of recipient cows. Fifteen of total 43 recipients were pregnant at Day 50 and 4 recipients were maintained to term. Three IVF-derived calves and 1 clone Chikso calf were born. Pregnancy rate was higher when fresh embryos were transferred to recipients compared to vitrified embryos, but development to term was not different between both groups. The clone Chikso calf died at 5 days after birth due to the fullness of amniotic fluid in rumen and the infection of umbilical cord. The result of the present study shows that clone Chikso calf can produced from the embryo transfer of SCNT embryos, however, solution of abortion problem is necessary to improve the cloning efficiency.

Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital (동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.

A Case of Adult Otitis Media with Effusion Accompanied by Atopic Dermatitis (아토피피부염을 동반한 성인 삼출성 중이염 치험례)

  • Objectives : This study is to report a case of otitis media with effusion accompanied by atopic dermatitis improved by Korean medicine monotherapy. Methods : A 31-year-old woman patient visited our clinic for aural fullness, hearing loss, and tinnitus of the left ear on August 14th, 2021. She also had abdominal pain, diarrhea, and general weakness because of side effects of antibiotics. Besides, she had nasal congestion, rhinorrhea, and dry skin. Through otoscopy, her tympanum was fully filled with yellow effusion. She only took Korean medicine therapy including acupuncture and herbal medicine weekly. The treatment continued for about 4 months(total 14 visits) until December 4th, 2021. Otoscopy, numerical rating scale(NRS), scoring atopic dermatitis(SCORAD) index were used to assess the symptoms. Results : On August 30, 2021, her intestinal symptoms improved. The left tympanum was recovered to normal condition as the treatment continued. The effusion disappeared on October 16th, 2021. On November 6th, 2021, peritympanic blood congestion also almost alleviated. Subjective symptoms including aural fullness, hearing loss, and tinnitus disappeared on October 16, 2021. Nasal symptoms and dry skin also improved. The SCORAD index dropped from 15 to 4. Conclusions : These results suggests that otitis media with effusion can be sufficiently managed by Korean medicine therapy. Further, active application of Korean medicine therapy to common otolaryngological diseases is necessary.

The Effect of Korean Medicine Treatment on Suspected Meniere's Disease with Headache in a Patient Hospitalized at a Korean Medicine Hospital: A Case Report (한방병원에 입원한 두통을 동반한 메니에르 병 의증 환자에 대한 한방 치료 1례)

  • Objectives: This study reports on the efficacy of using Korean medical treatment with suspected Meniere's Disease. Methods: We treated a patient with suspected Meniere's disease using Korean medical treatments (herbal medicines, acupuncture, herbal acupuncture therapy). To evaluate the results, we used the visual analogue scale (VAS) to measure dizziness, tinnitus, ear fullness, and headache. The degree of tinnitus was also evaluated using the Korean Tinnitus Handicap Inventory (K-THI). The generic health status was evaluated using the European Quality of Life-5 Dimensions (EQ-5D) scale. Results: Improvements in the VAS and EQ-5D scale scores were observed after the treatments. The change in the K-THI scores showed that the patient's tinnitus handicap was relieved. Conclusions: The results of this case study suggest that Korean medicine therapies may be effective in the treatment of suspected Meniere's Disease.
