Comparison between Subjective Scoring and Computer-Based Asymmetry Assessment in Facial Nerve Palsy

  • Background and Objectives: The aim of the present study was to assess the feasibility of a PC-based facial asymmetry assessment program (PC-FAAP) and to compare the results of PC-FAAP with subjective regional scoring by raters in acute unilateral peripheral facial nerve paralysis (FNP). Subjects and Methods: Participants were divided into 3 groups with 8 participants per group: group I, normal; group II, mild to moderate FNP; and group III, severe FNP. Using the PC-FAAP, the mouth asymmetry ratio (MAR), eyebrow asymmetry ratio (EAR), and complete eye closure asymmetry ratio (CAR) were calculated by comparing the movement of tracking points on both sides. The FNP grading scale (FGS) integrated each score, and the scores were weighted with a ratio of 5:3:2 (MAR:CAR:EAR). Subjective regional scoring was measured on a 0-100 scale score by three otologists. PC-FAAP and subjective scoring were compared in each group regarding the consistency of the results. Results: The mean scores of the MAR, EAR, CAR, and FGS of each group were significantly different. PC-FAAP showed significant differences between the three groups in terms of MAR, EAC, CAR, and FGS. PC-FAAP showed more consistent results than subjective assessment (p<0.001). The PC-FAAP was significantly more consistent in group I and group III (p<0.001 and p=0.002, respectively). FGS in group III was the only parameter that showed a more consistent result in PC-FAAP than the subjective scoring (p=0.008). Conclusions: An FNP grading system using a PC-based program may provide more consistent results, especially for severe forms.

A Case Report of Acute Primary Pontine Hemorrhage with One-and-a-Half Syndrome, Vertigo, and Paresthesia Treated with Eastern-Western Integrative Medicine (급성 원발성 교뇌 출혈 환자의 하나반증후군, 현기증, 이상감각에 대한 한양방 협진치료 : 증례보고)

  • Background: The purpose of this study was to report the improvement of a patient with one-and-a-half syndrome, vertigo, and paresthesia caused by acute primary pontine hemorrhage (PPH) after a combination treatment of traditional Korean and Western medicine. Case report: A 51-year-old female with one-and-a-half syndrome, vertigo, and paresthesia after PPH was treated with Korean medicine, including herbal medication, acupuncture, and moxibustion, and Western medicine, including medication and rehabilitation therapy during hospitalization. Her progress was evaluated by checking for changes in symptoms with the extraocular muscle (EOM) function test, numeral rating scale (NRS), and follow-up brain computed tomography scans and magnetic resonance imaging (MRI). After 41 days of treatment, the EOM movement was improved, leaving limited abduction of the left eye. The NRS scores for vertigo and paresthesia decreased from 10 to 5 and from 10 to 3, respectively. Improvement was noted in hematoma in MRI, but a new ischemic lesion was also discovered. Conclusion: This case reports the clinical course of one-and-a-half syndrome and suggests that a combined therapy of traditional Korean and Western medicine can be useful for PPH patients with one-and-a-half syndrome, vertigo, and paresthesia. However, studies of larger populations are required.

  • Objectives: The aim of this study is to modernize Nutrient Qi(營氣) and Defense Qi(衛氣) from the point of view of Visceral Body and Somatic Body. Methods: Investigate the areas of body and function of Nutrient Qi(營氣) and Defense Qi(衛氣) recorded in the 『HuangdiNeijing(黃帝內經)』 and 『Dongeuibogam(東醫寶鑑)』. And investigate Visceral Body and Somatic Body that anatomical structure is divided according to function based on the book 『Life and Rhythm』. Results: Nutrient Qi(營氣) works to nourish the body such as viscera and bowels in the meridian. Defense Qi(衛氣) works to protect the body such as skin, limb joint, eye outside the meridian. The human body is divided into Visceral Body called a vegetable organ and Somatic Body called an animal organ. Visceral body is the organ that manages "nutrition-reproductive" and is divided into the intestine(腸管), blood vessels(血管), and the nephridium(腎管). Somatic Body is an organ that manages 'sensory-motion' and is divided into a three-layer structure: the outer layer covering the body surface, the nerve layer connecting the outer layer and the muscle layer, and the muscle layer controling contraction and movement. Conclusions: It is estimated that Nutrient Qi(營氣) is similar to Visceral Body, and Defense Qi(衛氣) is similar to Somatic Body.

  • The size of the virtual reality market is growing every year, but cybersickness that occurs in virtual reality has not been resolved yet. In this paper, results were derived through experiments on the relationship between cybersickness and gaze guidance occurring in virtual reality contents. Using eye tracking technique, the relationship of gaze movement with cybersickness was identified. The experiment was divided into two groups to find out whether visual induction affects cyber sickness. In addition, the results were analyzed by dividing the two groups to check whether cyber sickness showed different results according to gender. We also analyzed using the SSQ questionnaire to measure cybersickness. We tried to understand the relationship between gaze guidance and cybersickness through two methods. As a result of the experiment, it was found that the induction of a clear gaze caused the concentration of the gaze, and it was effective in cybersickness through the rotation of the camera. In order to alleviate cyber sickness, it has been confirmed that concentrating one's eyes through gaze-guided production is effective for cyber sickness. It is hoped that this result will be used as a way to alleviate cyber sickness for producers who want to use virtual reality to produce content.

Effect of 4 Weeks of Pilates Exercise in Women using Light LED EMG Feedback System on Posture and Balance; Randomized Controlled Trial

  • Objective: This study aimed to assess the impact of using the LED light electromyographyfeedback system (EMG-light) during Pilates exercises in women to maintain consistent muscle contraction in the abdominal external oblique muscles and reduce muscle contraction in the upper trapezius muscles. This study compared Pilates training using the EMG-light with Pilates training only in healthy women for 4 weeks. Design: This study was conducted as a cross-sectional study. Methods: A total of 17 healthy women were divided into an experimental group (n=9) and a control group (n=8). Both groups performed Pilates exercises as assigned, twice per week for four weeks, with each session lasting fifty minutes. The experimental group were used the EMG-light feedback system during pilates exercise while the control group did not use EMG-light. We used cervical vertebral angle (CVA), shoulder tilt angle to evaluate neck posture and standing balance with closed eye for 30s before and after exercise. Results: The control group exhibited a significant change in CVA (p<0.05). Both groups showed significant changes in shoulder tilt, center of pressure (COP) path-length, and COP velocity during eyes-closed conditions (p<0.05). However, there were significant differences between the experimental and control groups in terms of CVA, shoulder tilt, COP path-length, and COP velocity. Conclusions: This study demonstrated that Pilates exercises had positive effects on shoulder posture and balance. The use of EMG-light provided real-time visual feedback on muscle contraction during Pilates exercise. However, the experimental group did not show significant improvements compared to the control group, which performed Pilates exercises without feedback.

Cold Feet and Sleep Quality : An Exploratory Study Using Polysomnography and Pittsburgh Sleep Quality Index (족냉과 수면의 질 : 수면다원검사와 피츠버그 수면의 질 지수를 이용한 탐색적 연구)

  • Objectives : This study aimed to investigate the relationship between cold feet and sleep quality using polysomnography (PSG) and Pittsburgh Sleep Quality Index (PSQI). Methods : We divided 11 adults (6 females, 5 males) with Insomnia Severity Index score below 21 into cold feet (CF) and a non-cold feet (NCF) group based on the median feet temperature (Taichong, LR3). PSG and PSQI were administered to assess sleep characteristics and subjective sleep quality. Results : CF group exhibited significantly lower time in bed, sleep period time, and total sleep time compared to NCF group. While there were no significant group differences in sleep latency, wakefulness after sleep onset, or total arousal index, NCF group had significantly lower minimum oxygen saturation and apnea-hypopnea index in REM (rapid eye movement) sleep compared to CF group. Although the PSQI score and the proportion of poor sleepers were both higher in the CF group (7.40 and 80%) compared to the NCF group (5.50 and 50%), these differences did not reach statistical significance. Conclusions : This study showed that foot temperature affects sleep characteristics and suggests the need to utilize PSG in sleep research in Korean medicine.

A Case Report of Miller-Fisher Syndrome with Ophthalmoplegia and Facial Palsy (양안의 완전 외안근마비와 편측 안면마비를 동반한 밀러-피셔 증후군 환자 치험 1례)

  • Objective : The purpose of this study is to report that the effect of Korean medical treatments on Miller-Fisher Syndrome with ophtalmoplegia and facial palsy. Methods : We treated a 69-year-old female diagnosed with Miller-Fisher syndrome with ophthalmoplegia, right facial palsy and other symptoms. She received Korean medical treatments such as herbal medicine(Gamiboik-tang), cupping therapy and acupuncture(including pharmacopuncture). The severity of ophthalmoplegia was evaluated by length of the eyeball movement and Scott and Kraft score. The severity of facial palsy was evaluated by Yanagihara score and the severity of other symptoms such as diplopia, dizziness, gait disturbance and neck&shoulder pain was evaluated by VAS. Results : Each neurological symptoms were improved after Korean medical treatments. In case of ophthalmop legia, Scott and Kraft score increase from -4 to 0. There were no restrictions on eye movements. In case of facial palsy, Yanagihara score increased from 10 to 40. Also, other symptoms such as diplopia, dizziness, gait disturbance and neck&shoulder pain was improved. Conclusions : This case report suggests that Korean medical treatments can be effective for Miller-Fisher Syndrome patient with Ophthalmoplegia and Facial Palsy.

Automatic Detection of Stage 1 Sleep (자동 분석을 이용한 1단계 수면탐지)

  • Stage 1 sleep provides important information regarding interpretation of nocturnal polysomnography, particularly sleep onset. It is a short transition period from wakeful consciousness to sleep. Lack of prominent sleep events characterizing stage 1 sleep is a major obstacle in automatic sleep stage scoring. In this study, we attempted to utilize simultaneous EEC and EOG processing and analyses to detect stage 1 sleep automatically. Relative powers of the alpha waves and the theta waves were calculated from spectral estimation. Either the relative power of alpha waves less than 50% or the relative power of theta waves more than 23% was regarded as stage 1 sleep. SEM (slow eye movement) was defined as the duration of both eye movement ranging from 1.5 to 4 seconds and regarded also as stage 1 sleep. If one of these three criteria was met, the epoch was regarded as stage 1 sleep. Results f ere compared to the manual rating results done by two polysomnography experts. Total of 169 epochs was analyzed. Agreement rate for stage 1 sleep between automatic detection and manual scoring was 79.3% and Cohen's Kappa was 0.586 (p<0.01). A significant portion (32%) of automatically detected stage 1 sleep included SEM. Generally, digitally-scored sleep s1aging shows the accuracy up to 70%. Considering potential difficulties in stage 1 sleep scoring, the accuracy of 79.3% in this study seems to be robust enough. Simultaneous analysis of EOG provides differential value to the present study from previous oneswhich mainly depended on EEG analysis. The issue of close relationship between SEM and stage 1 sleep raised by Kinnariet at. remains to be a valid one in this study.

A Study on the Spatial Control Effect of Panjang in Donggwoldo (동궐도(東闕圖) 판장(板墻)의 공간통제 효과에 관한 연구)

  • This study compared and analyzed the spatial division function and role of partitions by comparing the entire space and the spatial changes before and after the installation of partitions in <Donggwoldo>, which was manufactured in the late Joseon Dynasty. As a research method, a set standard was prepared to decompose the space of <Donggwoldo> into a unit space, and the standard was set according to the role and height of the space by classifying it into a main space, sub space, and transition space. Two convex maps were prepared according to before and after the installation of the Panjang, and the values of connectivity, control, and integration, which are spatial syntax variables, were calculated and analyzed. The results of the study are as follows. First, the partition in <Donggwoldo(東闕圖)> did not affect the overall spatial arrangement and control or connection of Donggwol, but the movement and access of space is limited to specific areas. Second, the partition was a facility intensively distributed in Naejeon(內殿) and Donggung(東宮) to be used actively in the way of space utilization. It shows that the unit space increased rapidly due to the installation of the partition. Since the partition was installed in the spaces that were open and under high control in the case of Naejeon(內殿), it helped to secure private spaces as closed ones under low control. On the other hand, for Donggung(東宮), the spaces were compartmented and divided with the partition to guide the movement path through narrow gates of the partition and increase the depth of the space. This helped to create spaces that are free and can be hidden as it increased the number of spaces coming through. Third, In addition to the functions of "eye blocking, space division, and movement path control" revealed in prior research, the partition has created a "space that is easy to control" within a specific area. The installation of the partition reduced the scale through the separation of spaces, but it occurred the expansion of the movement path and space. Also, the partition functioned to strengthen hiding and closure or increase openness as well through space division. This study is significant in that it revealed the value of the spatial control function of panjang through the analysis of spatial control and depth by analyzing the function of the partition with a mathematical model in addition to the analysis and study of the function and role of panjang. In addition, it is valuable in that it has prepared a framework for analysis tools that can be applied to traditional residential complexes similar to palaces by applying space syntax to <Donggungdo> to create convex spaces according to unit space division and connection types of palace architecture and landscape elements.
