Newly Developed Urinary Retention and Motor Weakness of Lower Extremities in a Postherpetic Neuralgia Patient

  • During the early stage of postherpetic neuralgia, an epidural block on the affected segment is helpful in controlling pain and preventing progression to a chronic state. The main neurologic complication following an epidural block is cord compression symptom due to an epidural hematoma. When neurologic complications arise from an epidural block for the treatment of postherpetic neuralgia, it is important to determine whether the complications are due to the procedure or due to the herpes zoster itself. We report a case of a patient who was diagnosed with herpes zoster myelitis during treatment for postherpetic neuralgia. The patient complained of motor weakness in the lower extremities after receiving a thoracic epidural block six times. Although initially, we believed that the complications were due to the epidural block, it was ultimately determined to be from the herpes zoster myelitis.

  • The joint dysfunction is the major cause of pain in trunk and extremities according to J. McM. Mennell who mentioned that there were no pathological changes that was found in the painful joint. The referred pain due to joint dysfunction has to be differentiated from other painful conditions, particularly of the neuromusculoskeletal system. Most of the pain in the lower extremities is due to the sacroiliac joint dysfunction according to Setsuo Hakata who had developed arthrokinematic approach(AKA) for sacroiliac joint in 1989. AKA for sacroiliac dysfunction is made up of distraction & gliding technics that are no violent to recover the disturbed intra-articular movement and result in immediate relief of pain.

산후풍(産後風)을 동반한 산후 복부 지방 저류 환자의 치험1례 (A Case Study on an Abdominally Obese Patient with Postpartum Disease)

  • We experienced an abdominal obese patient with postpartum disease. From the 15th of July 2006 to the 16th of September 2006 we applied herbal medications (Gungguijohyeoleumgamibang and Taeeumjouitanggamibang), acupuncture, auricular acupuncture, electrolipolysis, low calorie diet, aerobic exercise, behavioral therapy to her. We examined the changes in body weight, BMI, PBF and WHR and the changes in cold hypersensitivity of lower extremities and pain of wrists and ankles. Her weight decreased from 49.6kg to 44.5kg, BMI from $20.9kg/m^2$ to $18.8kg/m^2$, PBF from 27.7% to 23.9%, WHR from 0.84 to 0.78. Cold hypersensitivity of lower extremities and pain of wrists and ankles, symptoms typical of postpartum disease disappeared after treatment. Despite a low calorie diet, lactational performance was not impaired.

진행성 색소성 자반병 한방 치험 1례 (A Case of Progressive Pigmented Purpuric Dermatosis)

  • Progressive pigmented purpuric dermatosis(Schamberg's disease, purpura simplex) is an uncommon eruption characterized by petechiae and patches of brownish pigmentation, particularly on the lower extremities. Lesions remain for months or years and present only a cosmetic problem. there is no hematologic disease, venous insufficiency, or associated internal disease. The most characteristic feature is orange brown, pinhead-sized "cayenne pepper" spots. It is hard to find similar disease in Oriental Medicine, however it could be though related with 瘀血. We observed and treated a 25 old female with progressive pigmented purpuric dermatosis on her lower extremities, without pain and itching sign. About 1 year after our treatment, herb-medication. acupuncture treatment, negative therapy and applied aroma oil in order to remove the 瘀血(a kind of congestion) & inner heat and promote the circulation of her blood, the area of pigmented purpuric dermatosis was decreased remarkably and the colour was lighter. She is been treated continuously now and satisfied with the efficacy of treatment.

Intramedullary Spinal Cord Lipoma Extending from the Cervicomedullary Junction to the Upper Thoracic Cord

  • A case of intramedullary spinal cord lipoma is presented. A one month-old male infant presented with irritability and weakness on his upper extremities. A magnetic resonance[MR] image of the cervical spine demonstrated a well defined, high signal intensity lesion on both T1 and T2-weighted MR images and suppression on the fat saturation sequence. The tumor mass extended from the foramen magnum to T2 vertebra level. Ventral displacement of the spinal cord with kinking of the cervico-medullary junction was evident on the T2-weighted sagittal image. Partial resection of the tumor mass through laminoplastic laminotomy from C1 to T2 resulted in improved motor weakness on his upper extremities.

  • Syncope is defined as a transient loss of consciousness and postural tone, characterized by rapid onset, short duration, and spontaneous recovery. Stellate ganglion block (SGB) is a nerve block method that is used for treatment of neuropathic pain in the head, neck and upper extremities, especially trigeminal neuralgia, postherpetic neuralgia and complex regional pain syndrome. SGB can modulate and stabilize the sympathetic nervous system, which prevents it from overexcitation and improves symptoms of syncope. The authors report a patient who was treated for pain and edema of both upper extremities with SGB, then showed improvement in recurrent syncope followed by chest pain and overall quality of life.

성대의 자유연에 발생한 후두 지방종 1례 (A Case of Laryngeal Lipoma Arising at the Free Margin of the Vocal Cord)

  • Lipomas are benign tumors and most commonly occurs in trunk, upper extremities and lower extremities. About 13-15% of lipomas are located in the head and neck area. However, lipomas of larynx are very rare and only about 100 cases have been reported. Laryngeal lipomas occur mainly in epiglottis, aryepiglottic fold and false vocal cords, which have adipose tissue. Author experienced an unusual presentation of laryngeal lipoma. Tumor seemed to be located in the supraglottis in the preoperative laryngoscopy, but it was found to be located at the free margin of the true vocal cord. To date, only one case has been reported in the world literature. We report this case with a review of the literature.

태권도 경기력 및 운동수행능력과 PNF운동 (PNF Exercise, the Athletic Performances and the Ability of Exercise Capacity in Taekwondo)

  • Purpose : The purpose of this study was to investigate PNF exercise, the athletic performances and the ability of exercise capacity in Taekwondo. Methods : This is a literature study with books and articles, seminar note and books for PNF international course. Results : The PNF exercise was efficient at improving flexibility and muscle strengthening in Taekwondo athletes. Kinematic characters of Yupchagi and Dollyuchagi in Taekwondo were analogous to Lower extremity Flexion-Abduction-Internal Rotation patterns in PNF, and the kinematic character of Apchagi was to analogous to Lower extremity Flexion-Adduction-External Rotation pattern in PNF. Movements of Upper extremities and Lower extremities during kicking were similar to PNF patterns. The PNF is efficient at improving athletic performances and the ability of exercise capacity in Taekwondo. Conclusion : The result of this study showed that PNF patterns and kicking that is one of the basic movement in Taekwondo have many similarities. With that PNF patterns was the training methods to improve flexibility and muscle strengthening, PNF patterns are considered to improve athletic performances and the ability of exercise capacity in Taekwondo.

오수혈 자침 깊이와 맥기의 천심 (Needling Depth of Five-Phase Acupoints and Depth of Meridian Qi)

  • Objectives : The purpose of this study was to investigate the needling depth of five-phase acupoints and discuss the association with the depth of meridian qi. Methods : DongUiBoGam was used to determine the depth of five-phase acupoints. The depth of needling at 60 five-phase acupoints was compared between well, spring, stream, river, and sea acupoints. Results : The proximal part of the extremities had deeper needling depth than the distal part of the extremities. The targeted deqi sensation can be related to the needling depth. Conclusions : The depth of the meridian qi is related to the distinct patterns of needling depth of five-phase acupoints.

Varicose Veins and the Diagnosis of Chronic Venous Disease in the Lower Extremities

  • Varicose veins usually present in the superficial veins of the lower extremities and are one of the main clinical presentations of chronic venous disease (CVD). Patients' symptoms may vary according to the pathophysiology, location, and severity of CVD. The prevalence of CVD in Korea has been increasing gradually. However, due to its broad clinical spectrum and the subjective nature of its diagnosis using ultrasound, discrepancies in diagnostic and treatment quality may exist among treating physicians. There have been recent efforts to improve the quality of the diagnosis and treatment of varicose veins in Korea by standardizing the diagnostic criteria and the indications for treatment. This study is a comprehensive review of the clinical manifestations and diagnostic criteria of CVD based on the most recent international and domestic guidelines and reports.
