확률론적 모의실험 기법을 이용한 일부 지하철 근무자들의 이산화질소 개인노출 시나리오 분석 (Scenario Analysis of Personal Nitrogen Dioxide Exposure with Monte Carlo Simulation on Subway Station Workers in Seoul)

  • The personal exposures of nitrogen dioxide(NO$_2$), microenvironmental levels and daily time activity patterns on Seoul subway station workers were measured from February 10 to March 12, 1999. Personal NO$_2$exposure for 24 hours were 29.40$\pm$9.75 ppb. NO$_2$level of occupational environment were 27.87$\pm$7.15 ppb in office, 33.60$\pm$8.64 ppb in platform and 50.13$\pm$13.04 ppb in outdoor. Personal exposure time of subway station workers was constituted as survey results with $7.94\pm$3.00 hours in office, $2.82\pm$1.63 hours in platform and 1 hours in outdoor. With above results, personal $NO_2$exposure distributions on subway station workers in Seoul were estimated with Monte Carlo simulation which uses statistical probabilistic theory on various exposure scenario testing. Some of distributions which did not have any formal patterns were assumed as custom distribution type. Estimated personal occupational $NO_2$exposure using time weighted average (TWA) model was 31.$29\pm$5.57 ppb, which were under Annual Ambient Standard (50ppb) of Korea. Though arithmetic means of measured personal $NO_2$exposure was lower than that of occupational $NO_2$exposure estimated by TWA model, considering probability distribution type simulated, probability distribution of measured personal $NO_2$exposures for 24 hours was over ambient standard with 3.23%, which was higher than those of occupational exposure(0.02%). Further research is needed for reducing these 24 hour $NO_2$personal excess exposures besides occupational exposure on subway station workers in Seoul.

Biomonitoring of Metal Exposure During Additive Manufacturing (3D Printing)

  • Background: Additive manufacturing (AM) is a rapidly expanding new technology involving challenges to occupational health. Here, metal exposure in an AM facility with large-scale metallic component production was investigated during two consecutive years with preventive actions in between. Methods: Gravimetric analyzes measured airborne particle concentrations, and filters were analyzed for metal content. In addition, concentrations of airborne particles <300 nm were investigated. Particles from recycled powder were characterized. Biomonitoring of urine and dermal contamination among AM operators, office personnel, and welders was performed. Results: Total and inhalable dust levels were almost all below occupational exposure limits, but inductively coupled plasma mass spectrometry showed that AM operators had a significant increase in cobalt exposure compared with welders. Airborne particle concentrations (<300 nm) showed transient peaks in the AM facility but were lower than those of the welding facility. Particle characterization of recycled powder showed fragmentation and condensates enriched in volatile metals. Biomonitoring showed a nonsignificant increase in the level of metals in urine in AM operators. Dermal cobalt and a trend for increasing urine metals during Workweek Year 1, but not in Year 2, indicated reduced exposure after preventive actions. Conclusion: Gravimetric analyses showed low total and inhalable dust exposure in AM operators. However, transient emission of smaller particles constitutes exposure risks. Preventive actions implemented by the company reduced the workers' metal exposure despite unchanged emissions of particles, indicating a need for careful design and regulation of the AM environments. It also emphasizes the need for relevant exposure markers and biomonitoring of health risks.

식이와 함께 섭취시킨 미량의 납 수준과 납 섭취기간이 납의 체내 축적 및 분포에 미치는 영향 (Effect of Very Low Levels of Lead in Diet and Exposure Time on the Accumulation and Distribution of Lead in Rat Organs)

  • Effects of very low level of lead in diet and exposure time on the accumulation and distribution in organs and tissues was investigated with growing rats. 21 days old Sprague-Dawley rats were exposed to lead for 7, 14 and 21 days by feeding of 0.03, 0.42, 0.92 and 1.46mg/kg Pb as Pb-acetate containing diet, respectively. Lead concentrations in blood, liver, kidney and bone exhibit a linear relationship with lead levels in diet. After 7 days of exposure, the greatest dose dependent accumulation of lead was found in kidney and followed in bone. However, after 14 and 21 days, the dose dependent accumlation of lead in bone was about two fold greater than that in kidney. The accumulation of lead in liver and blood was relatively low. As continuous exposure to lead, the concentrations of lead in liver, kidney, blood and intestinal tracts were rather not increased with exposure time. However, bone lead concentration was increased with exposure time by feeding of 0.92 and 1.46mg/kg Pb in diet, but not 0.42mg/kg. The lead concentration in gastrointestinal tracts tends also to increasing with lead levels in diet after 7 and 14 days of exposure. However, by 21 days of exposure the lead concentration revealed relatively constant value regardless of the dietary lead levels. It is concluded that the binding capacity of the lead in blood, liver, kidney and bone seems to be increased with increasing lead levels in diet. The lead concentration in these organs, with the exception of the lead in bone, seems, however, to be standing under steady state regulation by continued exposure with the same dietary lead level. Therefore, by chronic exposure condition with environmental relevant lead level bone might be a principle targe organ for lead and blood lead repesents better the current lead exposure than the lead body burden.

Moderating Effects of Media Exposure on Associations between Socioeconomic Position and Cancer Worry

  • Reducing fear of cancer is significant in developing cancer screening interventions, but the levels of fear may vary depending on the degrees of media exposure as well as individuals' socioeconomic positions (SEP). However, few studies have examined how the SEP influences the fear of cancer under the moderating process of general and specific forms of media exposure. We investigated the moderating effect of media exposure on the relationship between SEP and the level of fear of cancer by assuming that cancer knowledge is a covariate between those two. In particular, this study examined how exposure to both general and specific media changes the series of processes from SEP to fear of cancer. We conducted path analyses with three types of media - television, radio and the Internet- using data from a health communication survey of 613 adults in Massachusetts in the United States. We found that SEP influences cancer knowledge directly and fear of cancer indirectly, as moderated by the level of media exposure. Health-specific exposure, however, had a more consistent effect than general media exposure in lowering the fear of cancer by increasing knowledge about cancer. A higher level of health-specific exposure and greater amount of cancer knowledge lessened the fear of cancer. In addition, the more people were exposed to health information on television and the Internet, the lower the level of fear of cancer as a result. These findings indicate a relationship between SEP and fear of cancer, as moderated by the level and type of media exposure. Furthermore, the findings suggest that for early detection or cancer prevention strategies, health communication approaches through mass media need to be considered.

작업환경측정 결과를 활용한 벤젠 노출 매트릭스에 대한 연구 (Benzene Exposure Matrices Using Employees's Exposure Assessment Data)

  • Objectives: The aims of this study were to set up benzene exposure matrices according to industry and process and to assess the risk of those occupational exposure to benzene. Methods: The benzene exposure matrices were assembled depending on industry and process, based on an exposure database provided by KOSHA(the Korean Occupational Safety and Health Agency), which was gathered from a workplace hazards evaluation program in Korea. These exposure matrices were assessed by Hallmark Risk Assessment tool. Results: The benzene was treated 412 industries sector(36%), 2,747 business places, and 471 industrial processes according to database. The arithmetic mean of past decade 8 hours time-weighted average of airborne benzene concentrations in the workplace was 0.10722 ppm. 1.07% of the total sample were greater than OEL, and 59.8% were showed less than the limit of detection. The highest risk values(Danger Value) were seen 36 industries including manufacture of general paints and similar product and 12 processes, such as other painting of manufacture of metal fabricated members. Exposure matrices based on employee exposure data base may provide exposure histories and can be used in epidemiological studies. Conclusions: It was found that more attentions should be paid to 36 among 412 industries and 12 of 471 processes, with a higher risk value.

자살사건에의 노출과 자살생각의 관련성 : 자살노출, 자살 외 사망노출, 사망 비노출 집단 간 비교 (Association between Exposure to Suicide Events and Suicidal Ideation : Comparison Among Groups with Exposure to Suicidal Death, Non-Suicidal Death, and No Death)

  • Objective : This study aims to examine the association between exposure to suicide events and suicide ideation by analyzing the levels of suicide ideation among the groups with exposure to suicide death, non-suicide death, and no death in their social relationships. Methods : Data were derived from Wave I of the Longitudinal Study of Suicide Survivors' Mental Health. 1,998 adults nationwide selected using a stratified sampling method based on the Korean Census Data, were categorized into 3 groups with exposure to suicidal death, non-suicidal death, and no death. The levels of depression (Brief CES-D), subjective health status, and suicidal ideation (SSI) were measured. To examine the association between exposure to suicide and the level of suicide ideation, multiple regression analysis was used after controlling the socio-demographic and clinical factors including subjective health status and depression. Results : 32% reported their exposure to suicide. Compared to the other groups, the suicide-exposed group's level of depression and suicide ideation were significantly higher but the subjective health status was lower. Multiple regression model revealed that suicide exposure had a statistically significant association with suicidal ideation at p=0.000 even after controlling the clinical characteristics. Conclusion : HThe findings suggest that exposure to suicide is a risk factor for suicidal ideation. In the clinical field, it is necessary to consider patients' experience in exposure to suicide while treating and intervening in suicide-related cases. At the policy level, a mental health system for suicide prevention should consider this risk factor for those exposed to suicide in their family and social relationships.

The Effect of Exposure to Mixed Organic Solvents on Lipid Peroxidation in Ship Building Painters

  • In the last several years, studies on the association of oxidative stress damage with exposure in the work place have been conducted. Xenobiotics create an imbalance of the homeostasis between oxidant molecules and antioxidant defense. By monitoring oxidative stress biomarkers, information was obtained on damages induced by oxidative stress and the toxicity of xenobiotics. In the present study, a Job Exposure Matrix (JEM) was constructed using the data from the Working Environment Measurement (WEM) of painters in the shipyard industry from the past 3 years to assess the exposure status. Additionally, by measuring the concentration of urinary malondialdehyde (MDA), the effect of lipid peroxidation was examined. The subjects consisted of 68 workers who were exposed to mixed organic solvents in the painting process and 25 non-exposure controls. The exposure indices of the exposure groups were significantly different (sprayer: 0.83, touchup: 0.54, assistant: 0.13, P<0.05). The urinary MDA concentration of the exposure group was 48.60${\pm}$ 39.23 ${\mu}mol$/mol creatinine, which was significantly higher than 18.03${\pm}$16.33 ${\mu}mol$/mol creatinine of the control group (P<0.05). From the multiple regression analysis of urinary MDA, the regression coefficient for exposure grade was statistically significant. In future studies, evaluation of the antioxidant levels of subjects should be performed simultaneously with quantitative exposure measurements.

  • 본 연구는 자동노출제어장치(Automatic Exposure Control, AEC)와 수동노출 이용 시 입사표면선량(Entrance Surface Dose, ESD)과 Entropy를 분석하여 자동노출제어장치의 유용성에 대해 알아보고자 하였다. 실험방법은 Skull, Chest, Abdomen, Pelvis 부위에 대하여 란도팬텀(Rando Phantom)에 반도체 선량계를 위치시켜 선량을 측정하였고, 동시에 획득한 DICOM(Digital Imaging and Communications in Medicine) 파일을 Matlab으로 Entropy 분석을 하였다. 그 결과 자동노출제어장치 이용 시 모든 부위의 입사표면선량이 수동노출보다 낮았고 Entropy 수치는 높았으며, paired t-test는 p<0.05로 유의한 차이가 있음을 알 수 있었다. 결론적으로 자동노출제어장치의 사용은 X선 검사 시 발생할 수 있는 불필요한 방사선량과 정보의 손실량을 줄여서 피폭선량과 영상 화질의 최적화에 기여할 수 있는 유용한 방법이 될 수 있다.

슬라임(액체괴물) 중 붕소 함량과 어린이의 붕소 노출량 추정 (Estimation of Exposure to Boron from Children's Slime Use in Korea)

  • Objectives: This study was conducted to investigate the boron concentrations in 30 slime products and estimate children's dermal exposure to boron. Methods: Thirty slime products from online and offline stores were purchased for boron analysis. The boron content of each slime sample was analyzed using ICP-OES. A nationwide survey was performed to determine use patterns of slimes by a home-visit survey of 10,000 children divided into three age groups: 0-2, 3-6, and 7-12 years. The dermal exposure to boron was calculated using an exposure algorithm for child slime users. Results: The proportion of the child population using slime was 45.8%, and the mean use frequency was $2.5{\pm}3.8$ per week (range: 0.08-35). Mean time spent playing with slime was $19.6{\pm}11.3min$ (range: 1-100). Twenty-five of the thirty slimes contained a boron concentration exceeding the EU limit for toys made with sticky material of 300 mg/kg. Dermal exposure ranged from $0.0008-13.78{\mu}g/kg/day$ with the maximum weight fractions of boron in the 30 products. The dermal exposure estimate was the highest in 7-12 years old group. Conclusion: Slime use among Korean children may cause high exposure to boron. Regulation is needed to minimize boron exposure from slime products.

납에 대한 국내 직업적 노출 문헌 자료 고찰을 통한 노출 특성 평가 (Evaluation of Lead Exposure Characteristics Using Domestic Occupational Exposure Literature Data)

  • Objectives: The purpose of this study is to evaluate exposure characteristics of lead using data from the domestic occupational exposure literature. Methods: Occupational airborne exposure data on lead reported in the domestic literature from 1981 to 2018 were collected and re-analyzed. The exposure levels in the data were expressed as an estimated arithmetic mean and a weighted arithmetic mean (WAM) of the number of samples. Lead exposure characteristics were analyzed by industry, process, and year. Results: From a total of 14 documents, 8,305 airborne lead measurements for 17 industries were identified, and the WAM concentration in eight industries exceeded the occupational exposure limit of 50 ㎍/m3. Three industries (battery manufacturing, lead smelting, and litharge manufacturing) accounted for 95% of the total data, and exposure trends could be confirmed over 10 years. Exposure levels continue to decrease in all three industries. Conclusions: Considering the distribution outlook of lead and lead compounds, the main management targets are lead storage battery manufacturing and secondary smelting for lead regeneration.
