노인의 건강실천행위가 건강관련 삶의 질에 미치는 영향에서 치아건강도의 매개효과에 관한 융합연구 (A Convergence Study on the Mediating Effects of Dental Health Determinants on Relationship between Health Practice Behaviors and Health-Related Quality of Life of the Elderly)

  • 본 연구는 노인의 건강실천행위가 치아건강도를 매개하여 건강관련 삶의 질(EQ-5D)을 예측할 수 있는지를 알아보기 위해 시행하였다. 연구대상은 2015년도 국민건강영양조사를 이용하여 만 65세 이상 총 1,311명의 노인으로 하였으며, SPSS 24.0 프로그램과 SPSS PROCESS macro를 이용하여 자료를 분석하였다. 그 결과, 노인의 동거유형에 따른 건강실천행위와 EQ-5D의 경우 부부를 포함한 1세대 동거(4.62, 0.90)가 가장 높았으며 독거노인(4.40, 0.86)이 가장 낮게 조사되었다. 또한, PROCESS macro를 통한 매개효과 검증결과, FS-T 지수가 건강실천행위와 EQ-5D의 관계에서 매개효과가 있는 것으로 나타났다(${\beta}=.0011$, BCCI [.003~.0024]). 이러한 결과는 FS-T 지수가 노인의 건강관련 삶의 질에 밀접하게 연관되어 있으며, 향후 노인을 대상으로 하는 건강증진프로그램 개발에 필요한 기초자료로 활용할 수 있으리라 생각된다.

  • Objectives: This study intended to determine significant factors that influence the health-related quality of life ("HRQoL"; EuroQol 5 Dimension health-related quality of life (EQ_5D) & EuroQol visual analogue scale (EQ_VAS)) of the elderly in Korea. Methods: This study was based on 3,903 subjects aged 65 years or more who participated in the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2012. The HRQoL was analyzed by various factors (general characteristics, health habits, mental health, chronic diseases, nutrient intakes). SPSS statistics for complex samples (Windows ver. 21.0) was used. Results: The HRQoL was higher in the males, those with higher educational level or higher income level while it was lower in those belong to single households. In particular, the EQ_5D was significantly higher in the group who reported walking practice, moderate physical activity (male), and the group who reported no vigorous physical activity (female). The EQ_VAS was significantly higher in the group who reported walking practice. Both EQ_5D and EQ_VAS were significantly lower in the group with stress, melancholy, suicidal thinking, and osteoarthritis. EQ_5D was significantly lower in the group with < 75% Estimated Energy Requirements (EER) in energy intake, and with < Estimated Average Requirements (EAR) in iron or niacin intake. A stepwise regression analysis revealed that i) higher educational level (male), and good self-rated health status significantly increased the EQ_5D, ii) age, alcohol intake (male), melancholy (female), suicidal thinking, osteoarthritis, and niacin intake deficiency (male) significantly decreased the EQ_5D, iii) higher income level (male) and good self-rated health status significantly increased the EQ_VAS, and iv) age (male), stress, suicidal thinking (female) and osteoarthritis significantly decreased the EQ_VAS. Conclusions: This study suggested that general characteristics, mental health, osteoarthritis, and niacin intake were associated with the HRQoL. Prospective research of long-term control is needed to establish the causal relationship between factors and the HRQoL.

일 지역 노인의 건강관련 삶의 질과 영향요인 (Health-Related Quality of Life and Its Affecting Factors among the Aged in one Korean Community)

  • The purpose of this study was to investigate the characteristics of health-related quality of life among the aged who have been living in one urban community. The number of participants was 167 of people who were 60 and over. health-related quality of life was measured by EQ-5D in Korean Version. The order of frequency of 5 domain in EQ-5D were pain/discomfort, mobility, anxiety/depression, usual activity, self care. 32 of profiles were found from the study participants, and '11111' among them was 28.7%. Mean of EQ-5D index was $0.82{\pm}0.14$. Multiple Regression analysis of health-related quality of life explained 31% of variance of health-related quality of life using EQ-5D. In this analysis, age, regular exercise, spouse were found to be predictors to health-related quality of life measured by EQ-5D. The finding of this study will be the basis of well organized model of health-related quality of life for the aged.

지역사회건강조사에서 EQ-5D index의 지역간 변이와 관련 요인: 사회심리적 요인의 중요성 (Regional Variation of EQ-5D Index and Related Factors in Community Health Survey: Major Role of Psychosocial Factors in Korea)

  • 이 연구는 생태학적 연구로서 EQ-5D index로 측정되는 건강수준의 시군구 단위 변이를 파악하고 지역사회건강조사 지표에서 이러한 변이를 설명하는 요인을 탐색하고자 하였다. 2017년도 지역사회건강조사자료에서 산출되는 전국 229개 시군구의 EQ-5D index 변이는 EQ(격차비) 1.1배, CV(변이계수) 1.0으로 지역 간 변이는 연구에 포함된 독립변수들에 비해 상대적으로 작았다. EQ-5D index 값이 전국 하위 25%에 속하는 시군구 비중이 높은 광역시도는 전라북도(9개 시군, 64.3%)와 충청남도(7개 시군, 46.7%)이었다. 다중 선형회귀분석 결과 지역 간 변이의 주요요인으로 우울감 경험률, 스트레스 인지율, 자살 생각률, 관절염진단 경험률 등이 도출되었다. 지역 간 건강수준의 격차를 줄이기 위해서는 사회심리적 요인들을 포함한 상기 요인들의 개선에 우선순위를 두어야 할 것으로 사료된다.

Evaluation of Health-Related Quality of Life among Patients with Cervical Cancer in Indonesia

  • Background: Evaluation of health-related quality of life (HRQOL) in cervical cancer patients is important in order to design the interventions for improving patient outcomes. Reports of HRQOL among cervical cancer patients in Indonesia are limited. Moreover, measurement using EQ-5D-3L is to our best knowledge has hitherto not been performed. This study aimed to examine the HRQOL of cervical cancer patients in Indonesia using EQ-5D-3L. Materials and Methods: A cross-sectional study was conducted by interviewing cervical cancer patients using the EQ-5D-3L questionnaire. Percentages of patients who reported having problems in each dimension of EQ-5D as well as EQ-5D index score (utility) were calculated. Results: Our findings indicated that the most frequent reported problems were pain/discomfort (67.8%) followed by anxiety/depression (57.5%). The mean of EQ-5D VAS was 75.8 (SD=17.0). The mean (SD) utility scores were 0.85 (0.19), 0.76 (0.20), 0.71 (0.21), and 0.77 (0.13) for cervical cancer patients in stage I, II, III, and IV, respectively. Conclusions: Cervical cancer significantly affects patient HRQOL. Efforts should be made to improve the quality of life of cervical cancer patients especially in terms of pain /discomfort and anxiety/depression reduction.

베트남 이주여성의 생활습관이 삶의 질에 미치는 영향 (Effect of Lifestyle Habits of Vietnamese Immigrant Women on Quality of Life)

  • Objectives The purpose of this study was to examine the daily lifestyle of Vietnamese immigrant women and to find out if they could affect health-related quality of life improvement. Methods The subjects of this study were 273 women who immigrated to Korea from Vietnam. This study was conducted at Daejeon and nearby Healthy Family Support Centers from September 2019 to January 2020 with the cooperation of the Daejeon City Healthy Family Support Center. Correlation analysis was performed between lifestyle and quality of life, and ANOVA analysis was performed to confirm the difference in health-related quality of life scores for each subgroup of dietary nutrition, sleep quality, and physical activity. A regression analysis was conducted to investigate the contribution of dietary nutrition, sleep quality, and physical activity to the quality of life of Vietnamese immigrant women. The significance level was p<0.05. Results Physical activity showed significant correlations with Euro Quality of Life -Visual Analogue Scale (EQ VAS), sleep quality with EQ VAS and Euro Quality of Life-5Dimension (EQ-5D) Index, and dietary nutrition with EQ-5D Index. There were significant differences in EQ VAS and EQ-5D Index scores for each sleep quality subgroup, and a significant difference in EQ VAS scores for each activity subgroup. The quality of sleep was found to have the greatest effect on EQ VAS and EQ Index of Vietnamese immigrant women, and physical activity on EQ VAS and dietary nutrition had some influence on EQ-5D Index. Conclusions Quality of sleep is closely related to health-related quality of life, and plays an important role in health-related quality of life for Vietnamese immigrant women.

Exploratory Study of Dimensions of Health-related Quality of Life in the General Population of South Korea

  • Objectives: This study aimed to explore dimensions in addition to the 5 dimensions of the 5-level EQ-5D version (EQ-5D-5L) that could satisfactorily explain variation in health-related quality of life (HRQoL) in the general population of South Korea. Methods: Domains related to HRQoL were searched through a review of existing HRQoL instruments. Among the 28 potential dimensions, the 5 dimensions of the EQ-5D-5L and 7 additional dimensions (vision, hearing, communication, cognitive function, social relationships, vitality, and sleep) were included. A representative sample of 600 subjects was selected for the survey, which was administered through face-to-face interviews. Subjects were asked to report problems in 12 health dimensions at 5 levels, as well as their self-rated health status using the EuroQol visual analogue scale (EQ-VAS) and a 5-point Likert scale. Among subjects who reported no problems for any of the parameters in the EQ-5D-5L, we analyzed the frequencies of problems in the additional dimensions. A linear regression model with the EQ-VAS as the dependent variable was performed to identify additional significant dimensions. Results: Among respondents who reported full health on the EQ-5D-5L (n=365), 32% reported a problem for at least 1 additional dimension, and 14% reported worse than moderate self-rated health. Regression analysis revealed a $R^2$ of 0.228 for the original EQ-5D-5L dimensions, 0.200 for the new dimensions, and 0.263 for the 12 dimensions together. Among the added dimensions, vitality and sleep were significantly associated with EQ-VAS scores. Conclusions: This study identified significant dimensions for assessing self-rated health among members of the general public, in addition to the 5 dimensions of the EQ-5D-5L. These dimensions could be considered for inclusion in a new preference-based instrument or for developing a country-specific HRQoL instrument.

한국 노인의 치과보철 상태에 따른 건강 관련 삶의 질에 관한 연구 (A study of the EQ-5D index according to the dental prostheses status of the elderly in Korea)

  • Purpose: This study aimed to obtain basic data on oral health improvement by investigating the EQ-5D index according to the oral prosthesis status of elderly persons aged 65 years or older in Korea using the Seventh Korea National Health and Nutrition Examination Survey. Methods: In this study, 3,426 elderly persons aged 65 years or older were included to analyze the relationship between the dental prostheses status and EQ-5D index in the collected data. Results: Compared to the "complete denture" in oral prosthesis, EQ in case of no dental prostheses or fixed dental prostheses is comparable to removable prosthetics such as "no dental prostheses," "one fixed bridge," and "two or more fixed bridges." Removable partial denture and complete denture required for the maxillary and mandibular EQ-5D index were significantly higher in "not necessary" than in "complete denture necessary" in all the elderly persons. Conclusion: The condition of the dental prosthesis and the need for a removable partial denture and complete denture for the elderly's EQ-5D index were confirmed, and it was found that the fixed dental prostheses were more effective than the removable dental prostheses. Therefore, oral health education programs for the elderly should be developed to promote dental function and oral health maintenance.

외상성으로 발생한 급성 요배통 환자에 대한 도침치료 3례 : 증례보고 (The Effect of Acupotomy on Traumatic Acute Low back pain : Case Report)

  • 이 증례보고의 목적은 도침 시술이 외상성으로 유발된 급성 요배통 환자의 통증, 기능, 삶의 질에 미치는 영향을 보고하기 위함이다. 1주일 이내의 외상성 계기로 인해 요배통을 호소하는 총 3명의 환자에게 도침 시술을 시행하여 NRS, ROM, RMDQ, EQ VAS에서 유의한 호전이 있었으며 EQ-5D의 경우 증례 1,3에서 유의한 호전이 있었다. 향후 보다 많은 증례보고 및 연구가 필요할 것으로 사료된다.

농촌지역 노인의 영양소 적정섭취 수준과 건강관련 삶의 질 (Quality of Nutrient Adequacy and Health-related Quality of life of the Rural Elderly)

  • Objectives: In Korea, the percentage of elderly is increasing at an unprecedented rate, and is expected to account for 40% of the population by 2060. This massive demographic change stresses the importance of research on aging as it is necessary to improve the quality of life (QoL) of this population. This study aimed to examine the health-related quality of life (HRQoL) of the rural elderly and to clarify its association with the nutrient adequacy ratio (NAR). Methods: A cross-sectional study was performed in S-gun, Chonbuk, a critical agricultural area. The elderly people without abnormal physical functioning composed our study population and the data were collected by personal visits to 336 elderly people aged over 65 years (110 males and 226 females). Subjects were interviewed with questionnaires pertaining to general characteristics and EuroQol (EQ-5D). Nutrient intakes were assessed two days by 24-hours recall method. Subjects were defined as high QOL group if EQ-5D index with Nam's model was above the median. Results: Generally, EQ-5D index was lower in women than in man, and lower in older subjects than in younger subjects. The percentages of people below the median were 42% (low QoL group) and 58% (high QoL group) were found to be the above the median. The high QoL group had higher NAR, especially for vitamin C, vitamin B1, vitamin B2 and folate. All dimensions in the EQ-5D were affected by NAR of some nutrients and especially anxiety/depression dimension was significantly correlated with NAR of 5 nutrients (protein, calcium, iron, vitamin C and vitamin B1) and EQ-5D scores. Conclusions: HRQol was significantly reduced in elderly with increasing age and this was more pronounced in women than in man. The NAR of some nutrients were associated with the EQ-5D index, especially anxiety/depression dimension, among rural elderly.
