Survey of Hospital Information Systems and e-Hospital Strategy of Large-sized Hospitals (국내 대형 병원 정보화 현황 조사 및 e-Hospital 전략)

  • IT investments of hospitals are growing exponentially and various information systems such as OCS (Order Communication System), EMR (Electronic Medical Record), PACS (Picture Archiving and Communication System), ERP (Enterprise Resource Planning) have been implemented at a number of hospitals in Korea. We have surveyed and analyzed the current status of hospital information systems of major hospitals in Korea, and proposed an information strategy for e-Hospital implementation. Firstly, implementation status of major subsystems of HIS such as OCS, PACS, and EMR has been surveyed and types of IT personnel management have been examined. Based on the field survey result, an information strategy for e-Hospital implementation has been proposed, that can be referenced by hospitals to build their own information strategy. We expect that the study result can contribute to understanding the present status and issues of HIS and information strategy planning of hospitals.

You can increase the stability of the patient by checking the image to show the real tester, we expect the quality of care would be improved.

  • The development of the internet and information management has enabled new applications which include: Electronic medical record (EMR), intelligent transportation, environmental monitoring, etc. In this paper, we design and implement the Continuity Care Record(CCR) Data Stream management server that compiled with DSMS and DBMS in EMR system for processing, monitoring the incoming CCR data stream and storing the processed result with high-efficiency. The proposed system enables users not only to query stored CCR information from DBMS, but also enables to execute continue query for the real-time CCR Data Stream. By using of CCR Viewer Application users can view or update their personal health records even compare self health care records with standard health care records in order to monitor the healthy status, and the on line updating information would be minimized and medical error.

  • With the development of IT. convergence systems are applied and operated in many different fields. A representative field among them is medical service, which develops in diverse types in combination with nano-technology and bio technology. However, there is a lack of technical innovation in terms of medical data operation and management. For example, data and documents are saved and integrated separately depending on their forms when electronic health records or data like SAM files are transmitted or kept. In other cases, such records and data are still kept after being recorded in paper. This study tries to design and implement the EMR system that makes it possible to capsulize forms of data and documents and to digitalize documents in work process as they are in terms of operation and storage. The system is expected to support efficient operation of electronic documents in the aspects of work and management.

Factors influencing Video Display Terminal Syndrome in Clinical Nurses (임상간호사의 영상표시단말기 증후군 및 영향요인)

  • Purpose: The present research is a descriptive study aimed at understanding clinical nurses' Video display terminal (VDT) syndrome and identifying the factors that affect their VDT syndrome. Methods: Data were collected from 239 clinical nurses working in two metropolitan cities. Research tools included subject's VDT syndrome assessing musculoskeletal, ophthalmic, dermal, psychiatric, and whole body syndromes. The data were analyzed using frequency analysis, average and standard deviation, t-test, One-way ANOVA, and multiple regression analysis with the SPSS/WIN 20.0 program. Results: The subjects' VDT syndrome score was 1.34 out of 5. There were significant differences in participating subjects' VDT syndrome, hospital's size, working unit, health status, diagnosis of illness, having an Order Communication System (OCS), having an Electronic Medical Record (EMR) System, continuous VDT working for more than one hour, break time during VDT use, VDT use time, comfort of the chair, adjustability of the height of the chair, size of the VDT's desk, distance between the monitor and the user's eyes, resolution of the monitor, and frequency of eye exercise during VDT use. According to the research, influencing factors on VDT syndrome in clinical nurses included size of the VDT's desk, frequency of eye exercise during VDT use, having an EMR system, break time during VDT use, diagnosis illness, and having an OCS' system. Conclusion: The findings from this study can be used as a basis for future VDT syndrome prevention education and programs for clinical nurses.

Electronic Medical Record Modification Prevention Protocol (전자의무기록 변경 방지 프로토콜)

  • Medical records are very important records and should not be modified after creation. The current medical records are liable to improper modification. With the development of information technology, electronic medical records (EMR) are used widely. For the EMR, cryptographic primitives may be used to develop techniques to prevent medical record modofication. In this research, a technique to prevent improper medical record prevention is proposed. It uses crytographic primitives such as linked hash, digital signature, and electronic notarization. A prototype system is also developed for performance analysis. The proposed method makes the medical record modification impossible with a small amount of additional cost.

  • 최근 유비쿼터스 시대에는 사용자의 편의성과 보안을 위주로 한 시스템들이 등장하고 있다. 병원에서는 컴퓨터를 이용하여 차팅을 하는 EMR 시스템을 도입하여 병원 내에서의 사용자 정보의 공유가 쉽게 이루어지고 있다. 사용자 중심의 전자ID지갑은 하나의 ID로 여러 사이트의 가입과 정보공유를 할 수 있다는 편리성과 개인정보보호 서버로 인한 피싱과 파밍으로부터 안전하다는 보안성을 지니고 있다. 이에 사용자는 전자ID지갑의 기능을 EMR시스템과 연동하여, 사용하는 측과 사용 받는 측 모두 효율적인 관리를 할 수 있는 기술이 필요하다. 본 논문에서는 이러한 전자ID지갑과 U-healthcare 시스템의 기능을 이용하여 사용자가 효율적인 서비스를 받기 위한 시스템과 보안관련 사항을 제안한다.

  • The existing EMR method placing computer servers in hospitals could expose patients' personal information to hospital officers and people for wrong purposes. In addition, if medical malpractice occurs, the possibility of distorting medical records might be higher because patients' medical records are stored in hospitals. This study provides an electronic medical record with a security system to solve patients' information disclosure. The electronic medical record system could be utilized as an important information when medical malpractice occurs. This system can provide higher security services certifying patients safely and efficiently as well as protecting patients' personal information.

  • This study explored the Eye Movement, the horizontal and vertical flow of vision, and genetic, cultural and empirical schema, and the differentiation of the visual system of the cerebrum through the engineering psychological or the cognitive neuroscientific experiments measuring Eye Movement to maximize the efficiency of the Public Sign System in the urban environment. Accordingly, the study derived an algorithm that can bring out the most efficient visual design measurement by using EMR test and bitmap counting method. The scientific test data in this study will show the existence of the momentum of differentiation and visual sampling by the Schema and Sensory Qualia of the human.

Development of a Delirium Occurrence Screening Model for Patients in Medical Intensive Care Units (내과계 중환자 섬망발생 선별모형 개발)

  • Purpose: The purpose of this study was to investigate risk factors related to delirium and to develop screening model on delirium occurrence in MICU (Medical Intensive Care Unit) patients. Methods: For developing a preliminary tool for delirium, the data of 166 patients were collected and analyzed. In order to estimate the accuracy and discriminating power for the developed screening model, 98 patients were enrolled. The data used in this study were collected by EMR (Electronic Medical Record) review from January to September in 2012. The collected data were analyzed using SPSS/PC Win 18.0 program. Results: Screening model on delirium in MICU patients was developed using the results of logistic regression. The total score of screening model was 24 point and measuring point was 10 point. When the measuring point is over 10 point, it means that the risk of delirium occurrence is high. The discriminating power and the validity of screening model showed AUC .908 (p <.001) and .935 (p <.001) respectively. This result showed that the screening model on delirium which developed in this study was an appropriate model for screening the delirium risk group in MICU. The sensitivity of the screening model was 83%, specificity 89% and accuracy 84%. Conclusion: The developed screening model on delirium occurrence in MICU should be combined with EMR for screening and preventing delirium in a high risk group.
