  • 본 논문은 인공지능 플랫폼 기반의 요로결석 진단을 위한 CT 영상 데이터 자동판독 시스템에 대해 기술하고자 한다. 제안한 시스템은 웹 기반의 플랫폼을 기반으로 하며, 인공지능 기반의 진단 알고리즘을 장착하여 빠르게 요로결석 환자의 스크리닝에 목적을 두고 있다. 병원정보시스템의 PACS와 EMR과 연계와 Deep learning 진단 알고리즘을 적용한 요로결석 자동판독 시스템을 개발하였다. 특히, 기 구축된 인공지능 플랫폼을 통해 추출한 데이터셋을 기반으로 진단 알고리즘 개발 방법과 수행 결과를 보인다. 제안한 시스템은 요로결석 진단과 수술여부에 의사결정지원 시스템으로 임상에서 활용될 것으로 기대하고 있다.

  • The rise of the Internet of Things (IoT) devices have greatly influenced many industries and one of them is healthcare where wearable devices started to track all your daily activities for better health monitoring accuracy and even down to tracking daily food intake in some cases. With the amounts of data that are being tracked and shared between from these devices, questions were raised on how to uphold user's data privacy when data is shared between these IoT devices and third party. With the blockchain platforms started to mature since its inception, the technology can be implemented according to a variety of use case scenarios. In this paper, we present a system architecture based on the healthcare system and IoT network by leveraging on multiple blockchain networks as the medium in between that should enable users to have direct authority on data accessibility of their shared data. We provide proof of concept implementation and highlight the results from our testing to show how the efficiency and scalability of the healthcare system improved without having a significant impact on the performance of the Electronic Medical Record (EMR) that mostly affected by the previous solution since these solutions directly connected to a public blockchain network and which resulted in significant delays and high cost of operation when a large amount of data or complicated functions are involved.

  • According to a rapid development of medical technology and increasing requirements of the users on prevention and control of diseases, whole healthcare services are changing into user-oriented services. There are many attempts and studies on integrating users' medical information but it is so difficult to implement a true user-oriented medical services because carrying the information from each medical facility to the integrated medical information system involves many conflicts of interests and authentication problems. This paper presents integrated medical information system which provides real-time medical services, allowing the users to be a critical player who can receive the medical information they want from any medical facility on their mobile devices without any change in the form of documents in relation to those issued by that facility and give that information themselves to the system through the mobile devices.

  • Bian-zheng(辨證) of Korean Medicine(KM) is based on four examinations(四診) of Korean medical doctor. The interrogation or questioning(問診) provides the most information of four examinations. The symptom obtained from the interrogation or questioning is the main basis of the Bian-zheng. KM is understood in the whole state of the body of a specified time without seeing the disease exist. So the observable symptom is disease itself. Symptom in KM is used as an important basis for the diagnosis. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. In this paper, we propose daily living symptom record system as a method that can be applied to the health care according to the importance of collecting the symptom in the KM. Daily living symptom record system can record the symptom in the individual to awaken daily. The system stores the symptom in structure and provides an open shared services. So it can be used as a symptom of other systems, such as PHR, EMR, CDSS. In addition, Doctor may be able to help in the treatment determined by reference to shared symptom.

  • Purpose: Once the decision has been made to adopt an integrated medical information system (IMIS), there are a number of issues to overcome. Users need to be aware of the impact the change will make on end users and be prepared to address issues that arise before they become problems. The purpose of this study is to investigate the benefits and unexpected problems encountered in the implementation of IMIS and to determine a useful framework for IMIS. Materials and Methods: The Yonsei University Dental Hospital is steadily constructing an IMIS. The vendor's PACS software, Piview STAR, supports transactions between workstations that are approved to integrating the healthcare enterprise (IHE) with security function. It is necessary to develop an excellent framework that is good for the patient, healthcare provider and information system vendors, in an expert, efficient, and cost-effective manner. Results : The problems encountered with IMIS implementation were high initial investments, delay of EMR enforcement, underdevelopment of digital radiographic appliances and software and insufficient educational training for users. Conclusions: The clinical environments of dental IMIS is some different from the medical situation. The best way to overcome these differences is to establish a gold standard of dental IMIS integration, which estimates the cost payback. The IHE and its technical framework are good for the patient, the health care provider and all information systems vendors.

A Trusted Sharing Model for Patient Records based on Permissioned Blockchain

  • As there has been growing interests in PHR-based personalized health management project, various institutions recently explore safe methods of recording personal medical and health information. In particular, innovative medical solution can be realized when medical researchers and medical service institutes can generally get access to patient data. As EMR data is extremely sensitive, there has been no progress in clinical information exchange. Moreover, patients cannot get access to their own health data and exchange it with researchers or service institutions. It can be operated in terms of technology, yet policy environment are affected by state laws as well as Privacy and Security Policy. Blockchain technology-independent, in transaction, and under test-is introduced in the medical industry in order to settle these problems. In other words, medical organizations can grant preliminary approval on patient information exchange by using the safely encrypted and distributed Blockchain ledger and can be managed independently and completely by individuals. More apparently, medical researchers can gain access to information, thereby contributing to the scientific advance in rare diseases or minor groups in the world. In this paper, we focused on how to manage personal medical information and its protective use and proposes medical treatment exchange system for patients based on a permissioned Blockchain network for the safe PHR operation. Trusted Model for Sharing Medical Data (TMSMD), that is proposed model, is based on exchanging information as patients rely on hospitals as well as among hospitals. And introduce medical treatment exchange system for patients based on a permissioned Blockchain network. This system is a model that encrypts and records patients' medical information by using this permissioned Blockchain and further enhances the security due to its restricted counterfeit. This provides service to share medical information uploaded on the permissioned Blockchain to approved users through role-based access control. In addition, this paper presents methods with smart contracts if medical institutions request patient information complying with domestic laws by using the distributed Blockchain ledger and eventually granting preliminary approval for sharing information. This service will provide an independent information transaction and the Blockchain technology under test will be adopted in the medical industry.

A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization (한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

A Retrospective Study of Facial Paralysis Sequelae for Korean Medical Treatment (말초성 안면마비 후유증에 대한 한방 치료를 위한 후향적 차트리뷰 연구)

  • Objectives : The purpose of this study is to confirm and to suggest the Korean medical treatment is effective treatments to patients with facial paralysis sequelae. Methods : We conducted a survey on patients with facial paralysis sequelae who visited the facial paralysis center from August 2017 to November 2018. We then evaluated House-Brackmann Grading System(HBGS), Sunnybrook Scale(S-Scale), Visual analog scale(VAS) against those who agreed and analyzed the information through Electronic Medical Record(EMR) and Order Communication System(OCS). Results : Clinically, Korean medical treatment such as embedding therapy are effective for the facial paralysis sequelae. For the purpose of raising the level of evidence against this, research will be needed to confirm the treatment effects by comparing accurate assessment indicators that are conducted before and after the treatment. Conclusions : In order to confirm the progress of treatment of facial paralysis sequelae, evaluation indicators such as House-Brackmann Grading System and Sunnybrook Scale should be performed by experts. And after 3-4 weeks, if patients have any sequelae symptoms, it may be helpful to take treatments such as pharmacopuncture treatment and embedding therapy at intervals of 1-2 weeks depending on the symptoms or areas.

  • Hospitals these days are trying to introduce the a practice has recently been generalized in the test or diagnosis process, where test results and images from different test labs are interlinked together. This process is identical to that of physical aspect in EMR process, which computerizes the paper results within the hospital. One of the prerequisites for the process of computerizing test results is the interface between clinical test devices in the test labs. However, due to the variety of prescription inputs, disparity of test result papers, complexity of job in test labs and diversify of interfaces among the different devices, interconnection with the hospital information system is a complicated job. A universal control of clinical test devices which have independent communication protocols has become possible by connecting them with an interface workstation. As for the patients, waiting time for test has been reduced, and, thanks to the synchronized result retrieval system, it has become possible to check the test results on the very day of the test. As a result, the length of hospitalization has been reduced, too. In terms of workflow, as the transfer of charts and transfer of result papers are separated, the embarrassing job of collecting result papers has disappeared. As patients' test appointment and the results processing can be made on-line, extra work for doctors have disappeared. And, thanks to the computerization of test results information management, the job of statistical processing has become convenient.

  • The need of Clinical Decision Support System (CDSS) in healthcare setup is increasing day by day. EHR Meaningful Use advocates CDSS as an important component of EHR/EMR systems. CDSS can be ranged from a simple to a very sophisticated system. The more complex CDSS systems need more attention to develop because of many reasons including its Knowledge Base (KB) structure/maintenance/evolution, inference capabilities and usability. Above all the KB maintenance and evolution is very crucial and important from the perspective of useful decision capabilities. Also the richness of the KB is important to cover the decision gaps handling a particular situation in the course of patient care. It cannot be expected from the clinicians to remember everything in regard to patient diagnosis and treatment. Similarly, it is also crucial for clinicians to keep themselves updated with the new research in the area. That is the reason they frequently require accessing to the online knowledge resources. Literature proved that online knowledge resources are capable providing answers to questions that might not be answered rely only on clinician wisdom and experience. This paper provides the theme of meaningful utilization of online knowledge resources in the context of diagnosis and treatment process for cancer patients more specifically Head and Neck cancer.
